Pisa syndrome is known to be a condition in which there is sustained involuntary flexion of the body and head to one side and slight rotation of the trunk so the person appears to lean like the Leaning Tower of Pisa.The development of Pisa syndrome is most commonly associated with prolonged treatment with antipsychotics. Although less frequently, Pisa syndrome has been reported, in patients who are receiving other medications (such as cholinesterase inhibitors and antiemetics), in those not receiving medication (idiopathic Pisa syndrome) and also patients with neurodegenerative disorders like Alzheimer's disease and multiple system atrophy.We report a case of a 67 year- old male diagnosed with Schizophrenia for 20 years. He has been following a treatment with Clozapine 400 mg/day for 4 years. Amisulpiride was added to the established regimen of antipsychotic and increased during the last month reaching the doses of 600 mg/day. Three weeks later he was observed walking with a tilt toward the right. A first physical examination revealed sustained tonic flexion of the trunk to the right side. No deficits or mental status changes during neurological exploration were shown. We prescribed biperidene hydrochloride therapy. After 24 hour side effects disappeared.As far as we know, no many cases of amisulpride-induced Pisa syndrome in the literature have been reported. This abstract presents a case of amisulpride induced Pisa syndrome.

The number of children swallowing magnetic foreign bodies has been a significantly high for the past decades, increasingly needing endoscopic or surgical interventions. Case report. In our observation, a 12-year-old girl swallowed magnetic balls from childrens designer 10 days prior to hospital admission. Foreign bodies (5 balls) were found during X-ray examination in the projection of the cecum. Conservative therapy carried out for 4 days had no success, thus colonoscopy was performed to remove foreign bodies. Foreign bodies were fixed to the intestinal wall, and attempts to separate them were unsuccessful. When a medical magnet was placed outside the body in the right iliac region, a chain of magnetic balls detached from the intestinal wall and made it possible to be captured in a trap loop and be removed. The girl avoided a laparotomy with an opening of the colon. Conclusion. External use of a medical magnet is effective for navigation and assistance during colonoscopic extraction when magnetic foreign bodies are found in the colon.

Herein we present a case of crossed renal ectopia with an unusual type of fusion, discovered incidentally in a 11-year-old girl presented with recurrent urinary tract infections. Both kidneys were located on the right side of the body fused in their upper poles only, forming an inverted U shape. After reviewing the published data on this topic, we found that most of the described anomalies were within the six well-known types of fusion anomalies. This child had an unusual clinical presentation of severe hydronephrosis of the orthotopic kidney. A unique surgical technique to correct the pathology to be able to preserve the residual mass of that kidney was performed.

A three year old female adult albino Python bivittatus weighing 12 kilograms was presented with a chief complaint of right spectacle growth and whitening. The snake also had a respiratory infection which started about 2 weeks prior to being presented. Physical examination showed bubbly nostrils, inflammation on the right eye, inflammation on the right palatum of the oral cavity, and crackling sound during auscultation when exhaling and hissing. The snake was diagnosed with subspectacular abscess and respiratory infection. Spectaculotomy was performed under general and local anesthesia. All abscesses were removed using a sterile sexing probe while continuously flushing the eye. Topical antiseptic was diluted and flushed into the eye before topical antibiotic eye ointment was applied. Post-operative treatment included daily flushing using an intravenous catheter, eye ointment application, and administration of systemic antibiotic and NSAID to treat the co-related respiratory infection. Three weeks after the surgery the snake went through ecdysis, after which the snake regained its appetite and recovery was thereafter significantly improved.

Spermatic vein thrombosis is a rare event that mostly affects the left vein thrombosis, but, in our report, it had developed on the right one that requires a meticulous physical examination for diagnosis. The purpose of this case report is to introduce an adult patient with right spermatic vein thrombosis in a 30-year-old man admitted to the operating room for hernia surgery. Spermatic vein thrombosis is an unexpected finding in the differential diagnosis of acute testicular pain.

CONTEXT: Supernumerary breast tissue may be affected by the same diseases and alterations that compromise topical breast tissue. Nevertheless, reports of fibroadenoma in supernumerary breast tissue in the axillae are rare. OBJECTIVE: To describe a case of fibroadenoma in an axillary supernumerary breast. DESIGN: Case report. CASE REPORT: A 39-year-old woman was referred to the gynecology and obstetrics outpatient clinic at Hospital Estadual Sumaré, complaining of bilateral axillary masses. The patient reported cosmetic problems and local pain and discomfort. On physical examination, alterations compatible with bilateral axillary accessory breasts, without palpable nodules, were observed. Supplementary examinations (mammography and ultrasonography) revealed a 1.1 cm mass in the right axillary breast. The patient underwent resection of the supernumerary breasts and histopathological examination revealed fibroadenoma of the right axillary breast tissue.

A 39 year old woman, scheduled for elective caesarean section in her second pregnancy, developed thrombocytopenia. Therefore, at the time of surgery, spinal anaesthesia and non-steroidal analgesic drugs were avoided and she was given a standard general anaesthetic procedure including fentanyl 100μg and morphine 10mg. In the early postoperative period she received tramadol 100mg and a further 10mg of morphine. These drugs did not control her pain, but caused side effects - in particular nausea and retching. Acupuncture to LI4 and PC6 on the right side produced dramatic pain relief within minutes.

Lichen planus (LP) is a papulosquamous disorder characterized by violaceous, flat topped papules and plaques seen on the volar aspect of the wrists, lumbar region and around the ankles. It can have a varied presentation. Here we describe a rare variant of LP in a female who presented with unilateral eruptions of violaceous papules over the right side of the body.

AbstractBackgroundSymptoms from disseminated cancer can develop very slowly. This could be very difficult to distinguish those symptoms from chronic disabilities and nuisances in patients with chronic non-malignant pain.ObjectiveIn this report, the case of a woman with both nonmalignant pain and cancer is presented.Case reportA 54 years old woman was referred by a general practitioner to Multidisciplinary Pain Center. The diagnosis was chronic non-malignant neck pain on the basis of degenerative columnar disease. The patient was also suffering from osteoporosis. During the first visit in the Center, the patient complained of shooting pains in the neck and had tingling sensations in the fingers – most of his right hand. Moreover, the patient experienced shooting pains in the hips, lower back and spine. The multidisciplinary treatment with medication, physical therapy, TENS and cognitive behavioral therapy was offered. Paracetamol together with gabapentin was used. The patient experienced relief of pain. The doses of gabapentin was escalated up to 2400 mg daily without significant side effects. Afterwards, the dose was gradually increased to 3600 mg daily and the patient experienced fatigue, mild headache and dizziness. These symptoms were initially interpreted as side effects of gabapentin. However, the tingling sensations in the fingers were almost disappeared. The doses of gabapentin was reduced, but without relief of symptoms. Within 2 weeks, the patient developed partial paresis of the right upper limb and aphasia. The patient was urgently referred to the neurologic inpatient clinic. CT- and MR-scans showed multiple cerebral metastases. Under the diagnostic workup the lung tumor was found. The biopsy showed pulmonary adenocarcinoma.ConclusionsThe symptoms of lung cancer with cerebral metastases can mimic side effects of gabapentin.

Background. Basketball is a popular sport around the world, the number of players is growing, the probability to experience trauma is increasing, too. Studies have shown that women sustain 60% more injuries than men do. Pre- season physical examination helps to establish sports specific changes in the body of the athlete and to determine risk factors for training, competition, which cause injury to the players. Research aim was to analyse risk of non-contact musculoskeletal injuries in elite women basketball players according to the position on the court. Methods. The study involved 100 players in Lithuanian women basketball league who completed the questionnaire about the level of physical fatigue, performed Lower Quarter Y Balance Test™ and Functional Movement Screen ™ tests during pre-season physical examination in 2014. Data were analysed and compared according to player’s position on the court (guards and forwards). Results. The level of physical fatigue during pre-season period was low in both groups. Dynamic stability of the right leg was better in guards (composite score – 105.2 to 101.8 in forwards) while dynamic stability of the left leg remained the same in both groups. Guards had higher total scores in Functional Movement Screen™ tests pointing to the better quality of basic functional movement patterns. Conclusions. The guard’s playing efficiency during the season was higher than that of forwards and it was associated with better dynamic stability and higher level of physical fatigue at pre-season. Indirect indicators of non-contact musculoskeletal sport injuries showed that guards should have lower risk of non-contact sport-related injuries in women basketball.

Tietze’s Syndrome: A Case Report Tietze syndrome (TS) is a rare inflammatory arthropathy characterized by tenderness, pain, and non-purulent swelling of the costosternal, costochondral and sternoclavicular joints. It often involves the costosternal joints and sternoclavicular jointinvolvement is rare. In this case report, we present a 72-year-old female patient suffering from pain and swelling in the right upper chest wall fo rabout 2 years. Physical examination revealed tenderness in the right sternoclavicular joint and an approximately 2x2 cm moderate swelling. Ultrasonic imaging of the swelling area revealed increased echogenicity and edema in the right sternoclavicular joint area. Laboratory findings were normal. The patient was diagnosed with TS and a local injection of a corticosteroid and local anesthetic mixture was administered. The patient’s complaints decreased significantly after the infiltration. In this case report, we present a TS case with sternoclavicular joint involvement that we treated with local injection. Keywords: Tietze’s syndrome, sternoclavicularjoint, costochondritis
