School-based obesity interventions in the metropolitan area of Rio De Janeiro, Brazil: pooled analysis from five randomised studies

Abstract School-based studies, despite the large number of studies conducted, have reported inconclusive results on obesity prevention. The sample size is a major constraint in such studies by requiring large samples. This pooled analysis overcomes this problem by analysing 5926 students (mean age 11·5 years) from five randomised school-based interventions. These studies focused on encouraging students to change their drinking and eating habits, and physical activities over the one school year, with monthly 1-h sessions in the classroom; culinary class aimed at developing cooking skills to increase healthy eating and attempts to family engagement. Pooled intention-to-treat analysis using linear mixed models accounted for school clusters. Control and intervention groups were balanced at baseline. The overall result was a non-significant change in BMI after one school year of positive changes in behaviours associated with obesity. Estimated mean BMI changed from 19·02 to 19·22 kg/m2 in the control group and from 19·08 to 19·32 kg/m2 in the intervention group (P value of change over time = 0·09). Subgroup analyses among those overweight or with obesity at baseline also did not show differences between intervention and control groups. The percentage of fat measured by bioimpedance indicated a small reduction in the control compared with intervention (P = 0·05). This large pooled analysis showed no effect on obesity measures, although promising results were observed about modifying behaviours associated with obesity.

Objective: The COVID-19 pandemic has had a major impact on teachers professional and personal lives. Our primary aim was to assess the effect of a blended Inquiry-Based Stress Reduction (IBSR), an emerging mindfulness and cognitive reframing intervention on teacher’s well-being. Our secondary aims were to assess the effect of IBSR on resilience, burnout, mindfulness, and stress among teachers during the COVID-19 pandemic. Methods: The study was a prospective controlled trial with an intervention group (N = 35) and a comparison control group (N = 32). The intervention took place in the Jerusalem District throughout the school year from November 2019 to May 2020. The sessions were conducted in blended learning that included traditional learning (face-to-face) and online learning. Data was analyzed on an intention-to-treat basis. Results: IBSR blended intervention enhanced the resilience and improved the subjective and psychological well-being of teachers in spite of the breakout of the COVID-19 pandemic and the first lockdown in Israel. Simultaneously the control group suffered from enhanced burnout levels and a decline in psychological and subjective well-being. Conclusions: Implementation of IBSR blended intervention during the school year may benefit teachers’ well-being and ability to flourish, even during stressful events such as the COVID-19 pandemic.

Abstract Background We redesigned the Schisto and Ladders health educational board game and evaluated its potential to encourage compliance to school-based mass drug administration with praziquantel. Methods Two hundred and seventy-five children from six schools who rejected praziquantel treatment were divided into intervention and control groups. Before the intervention, preassessment interviews were conducted on their knowledge about praziquantel treatment and schistosomiasis. The Schisto and Ladders version 2 game as an intervention, and the Snakes and Ladders game as a control, were played for 6 mo. Postassessment interviews, including focus group discussions, were conducted. Results At preassessment, 0/98 (0.0%) children in the intervention group had heard of praziquantel compared with 2/177 (1.1%) in the control group. Similarly, 0/98 (0.0%) children in the intervention group did not know that praziquantel does not kill compared with 4/177 (2.3%) in the control group. The postassessment showed that 53/78 (67.9%) in the intervention group were aware of praziquantel compared with 2/177 (1.1%) in the control group (p=0.000). Similarly, 53 (69.7%) in the intervention group knew about the safety of praziquantel compared with 0/177 (0.0%) in the control group (p=0.000). Sixty-four children (65.3%) from the intervention group sought praziquantel treatment after the trial. Conclusions Schisto and Ladders version 2 is a useful sensitisation tool with which to encourage compliance to praziquantel treatment in schools.

Abstract OBJECTIVE The aim of this study was to investigate the effect of radiology nursing intervention in abdominal examination at 3-T MRI. METHOD 60 patients with abdominal diseases were divided into two groups randomly: MR nursing intervention group and control group. All the patients underwent abdominal MR examination at 3-T. The MR nursing interventions were performed in nursing intervention group. The outcomes, including one-time success rate, the ratio of diagnosable MR images and the points of image quality, were compared between these two groups. RESULTS The one-time success rates in control group and MR nursing intervention group were 66.67% and 96.67% with significant difference ( χ2 =9.017, P<0.05). The ratios of diagnosable images in the two groups were 76.67% and 96.67% with significant difference (χ2 =5.192, P<0.05). The points of MR image quality in the two groups were 1.87±0.86 and 2.33±0.55, respectively. There was significant difference between these two groups (t=-2.508, P<0.05). CONCLUSION The effective nursing intervention can make the patients cooperation better in abdominal MR examination and improve the image quality significantly.

Summary Objective: To evaluate the effectiveness of video resources in increasing patient knowledge during preoperative orientation for cardiac surgery compared to standard orientation. Method: Randomized clinical trial. Patients in the intervention group (IG) received bedside orientation with the aid of a short video and a slide presentation on the day prior to surgery. Patients in the control group (CG) received standard orientation. Allocation and analysis of knowledge were blinded. Analysis was performed according to the intention to treat principle. Results: We included 90 patients, 45 in each group; 27.8% had incomplete primary education. There were no differences between groups in the baseline. After orientation, IG scored 7.20 (±1.56) and scored CG 2.71 (±1.96, p<0,001). The topic "surgical room" had the lowest proportion of correct answers in CG (6.7%), compared to IG (68.9%). The question about "postoperative unit" was the one with the highest proportion of correct answers in IG (93.3%), while in CG the proportion was 22.2%. "Ventilatory support" had a low proportion of correct answers in IG (60%), but still much higher than the correct answers in CG (17.8%). Conclusion: Orientation performed with the aid of video resources is more effective for knowledge retention in preoperative patients, compared to verbal orientation alone.

BACKGROUND Besides the beneficial health effects of being active, running is associated with a risk of sustaining injuries. Runners need to change their behavior to increase the use of effective measures and subsequently reduce the number of running-related injuries. OBJECTIVE The RunFitCheck intervention was developed according to an evidence- and practice-based approach to stimulate injury-preventive behavior among novice runners. This paper describes the study design in detail. METHODS A randomized controlled trial with a follow-up period of 5 months will be conducted. The participants will be novice runners. At enrollment, participants will be asked to report injury-preventive measures they usually take during their running activities. After completing the enrollment questionnaire, participants will be randomized to intervention and control groups. The intervention group will have access to the RunFitCheck intervention; the control group will perform their running activities as usual. Participants will be asked to report retrospectively in detail what they have done regarding injury prevention during their running activities at 1, 3, and 5 months after enrollment. Descriptive analyses will be conducted for different baseline variables in the intervention and control group. Relative risks and 95% CIs will be used to analyze behavioral changes according to the intention-to-treat principle. RESULTS The project was funded in 2016 and enrollment was completed in 2017. Data analysis is currently under way and the results are expected to be submitted for publication in 2019. CONCLUSIONS To nullify the negative side effects of running, prevention of training errors is desirable. As the use of injury prevention measures is not compulsory in running, a behavioral change is necessary to increase the use of effective injury-preventive measures and to prevent running-related injuries. Schools are an important arena to curb the decline in physical activity (PA) in youth. School-based interventions with accelerometer-measured PA are warranted. This study aimed to increase accelerometer-measured PA in adolescents following a 12-month school-based intervention. Two school-classes of 16–18-year-old Swedish students were allocated to intervention group and control group. Accelerometer-measured PA was gathered at baseline, 6- and 12-month follow-up. Mixed-effects linear regression was used to investigate between-group and within-group differences in mean minutes per day (min/day) of moderate to vigorous PA (MVPA), light PA (LPA) and sedentary time (ST). Fifty-seven students participated (intervention group = 31, control group = 26). At 12-month follow-up, the intervention group performed 5.9 (95% CI: −4.3, 16.2) min/day more in MVPA, 1.8 (95% CI: −17.9, 14.2) min/day less in LPA, and 4.1 (95% CI: −27.3, 19.2) min/day less in ST compared to the control group. Within the intervention group, there was no significant change in PA. Within the control group, LPA decreased (95% CI: −19.6, −0.2; p = 0.044) and ST increased (95% CI: 1.8, 30.8; p = 0.028). Although no between-group differences in PA were statistically significant, the within-group changes may suggest a preventive impact on the decline in PA during adolescence.

Abstract Background: Mental health of adolescents have been exposing to risk factors associated with stress from a variety of sources. Previous studies identified that green space exposure might be effective for promoting mental health and reduction of stress. However, previous literature focused on adults, and for those which studied children and adolescents, the effects of planting and similar green activities remain to be explored. The aim of this study was to investigate the effects of a pioneer school-based program on green space use and satisfaction, and mental wellbeing indicators, among early adolescent students. Methods: This was a feasibility quasi-experiment in which intervention and control groups were studied sequentially. Three secondary schools and 55 grade 7-8 students were sampled conveniently. In the intervention school, the green space program consists of learning hydroponic planting and group practice in creating green areas at school. In the two control schools, the activities consisted of a standardized stress management program including relaxation exercise such as stretching, extension and massage. Both groups learned green diet, tasting and had green living activity, stress awareness games and learned stress coping. The activity schedule for each group (intervention and control) consisted of three one-hour sessions, each was conducted two to three weeks apart. Key outcomes were assessed with questionnaires at baseline and one month after the completion of the activity period as for each school. Data were fit with mixed model applying GEE. Results: At baseline, students were not sure whether they know how to plant herbs or prepare herbal tea. Control group at posttest had lower frequencies of using green facilities in the community and using school gardening facilities in the previous four weeks compared with intervention group and pretest. Females at posttest were associated with higher confidence in planting, and higher level of enjoyment from green activities. Other moderate to small but non-significant effects were also identified. Conclusions: In this study, the school-based green space programme was found feasible for early adolescent students with statistically supported results. Limitations have also been identified and future study with larger scale, more frequent intervention, longer follow-up and RCT design are required.

We conducted a randomized controlled trial using mobile health technology in an ethnically diverse sample of 137 patients with complicated diabetes. Patients in the intervention group ( n = 72) were trained to measure their blood glucose with a sensor which transmitted the readings to a mobile phone via a Bluetooth wireless link. Clinicians were then able to examine and respond to the readings which were viewed with a web-based application. Patients in the control arm of the study ( n = 65) did not transmit their readings and received care with their usual doctor in the outpatient and/or primary care setting. The mean follow-up period was 9 months in each group. The default rate was higher in the patients in the intervention arm due to technical problems. In an intention-to-treat analysis there were no differences in HbA1c between the intervention and control groups. In a sub-group analysis of the patients who completed the study, the telemonitoring group had a lower HbA1c than those in the control group: 7.76% and 8.40%, respectively ( P = 0.06).

Background:We evaluated the effectiveness of a school-based intervention on the promotion of physical activity among high school students in Brazil: the Saude na Boa project.Methods:A school-based, randomized trial was carried out in 2 Brazilian cities: Recife (northeast) and Florianopolis (south). Ten schools in each city were matched by size and location, and randomized into intervention or control groups. The intervention included environmental/organizational changes, physical activity education, and personnel training and engagement. Students age 15 to 24 years were evaluated at baseline and 9 months later (end of school year).Results:Although similar at baseline, after the intervention, the control group reported significantly fewer d/wk accumulating 60 minutes+ moderate-to-vigorous physical activity (MVPA) in comparison with the intervention group (2.6 versus 3.3, P < .001). The prevalence of inactivity (0 days per week) rose in the control and decreased in the intervention group. The odds ratio for engaging at least once per week in physical activity associated with the intervention was 1.83 (95% CI = 1.24–2.71) in the unadjusted analysis and 1.88 (95% CI = 1.27–2.79) after controlling for gender.

Objective: To observe whether the use of WhatsApp™ can contribute to the improvement and retention of ECG knowledge during medical graduation in a period-time of 4 months. Material and Methods: A controlled, quasi-randomized, intention-to-treat, clinical trial. Medical students of the 2nd and 3rd semester attended a 2-hour class on elementar ECG interpretation. A test with 10 ECG tracings covering subjects was applied with a possible 0-10 score at 4 different times: immediately before the lesson (M1), immediately after the lesson (M2), one month after the lesson (M3) and four months after the lesson (M4). Intervention group, formed by 2nd-semester students, were included, shortly after M2, in a WhatsApp™ group, in which final year medical students and a cardiology resident discussed ECG tracings frequently in the absence of a teacher. Control group, formed by 3rd-semester students, was instructed to study on their own. Results: 13 students were included from the 2nd semester and 11 from the 3rd semester. In M1, the intervention and control group obtained a median of 0.0. In M2, both groups presented a similar increase with a median of 4.0 (IIQ=2.8-5.0) for the intervention group and 4.5 (IIQ= 3.3-5.5) for the control group. In M3, there was a difference between group scores, with a median of 6.0 (IIQ= 3.5-7.0) for the intervention group and 2.0 (IIQ=0-4.0) for the control group (p=0.016). In M4, difference was maintained (4.0 for intervention group [IIQ= 3.0-6.3] vs. 1.0 [IIQ= 1.0-3.0] for control group [p= 0.006]). Conclusion: Early-stage medical students learned and retained more elementar ECG knowledge when participating in WhatsApp™ ECG group discussion with more advanced medical students and medical resident, even without a teacher in this group.
