Abstract. Background: Research has already acknowledged the importance of the Internet in suicide prevention as search engines such as Google are increasingly used in seeking both helpful and harmful suicide-related information. Aims: We aimed to assess the impact of a highly publicized suicide by a Hollywood actor on suicide-related online information seeking. Method: We tested the impact of the highly publicized suicide of Robin Williams on volumes of suicide-related search queries. Results: Both harmful and helpful search terms increased immediately after the actor's suicide, with a substantial jump of harmful queries. Limitations: The study has limitations (e.g., possible validity threats of the query share measure, use of ambiguous search terms). Conclusion: Online suicide prevention efforts should try to increase online users' awareness of and motivation to seek help, for which Google's own helpline box could play an even more crucial role in the future.

Abstract: In recent years, the internet has become a popular medium for pregnant women seeking pregnancy-related information (Gao et al., 2012; Lagan, Sinclair & Kernohan, 2010; Romano, 2007; Song, West, Lundy & Smith-Dahmen, 2012). This study which utilized a qualitative approach explores Israeli women’s online information seeking during pregnancy. Israel is an especially interesting and important case study for exploring pregnancy-related internet use, because Israel has the highest rate of prenatal testing in the world (Lavi, 2010). The findings of this study indicated that the most prominent type of information sought by Israeli pregnant women online was focused on prenatal tests. The internet as a source of pregnancy-related information seems to function not just as a means to fulfill personal informational needs, but also to represent a broader social phenomenon of a highly medicalized pregnancy that seem to intensify uncertainty and therefore, to increase the need to rely on the internet during pregnancy.

BACKGROUND People engage in health information-seeking online when experiencing unusual or unfamiliar bodily changes. It is not well understood how people consult the internet for health information after the onset of unfamiliar symptoms and before receiving a potential diagnosis and how online information-seeking can help people appraise their symptoms. This lack of evidence may be partly due to methodological limitations in capturing in real time the online information-seeking process. OBJECTIVE We explored women’s symptom attribution and online health information-seeking in response to a hypothetical and unfamiliar breast change suggestive of cancer (nipple rash). We also aimed to establish the feasibility of capturing in real time the online information-seeking process with a tool designed to track participant online searches and visited websites, the Vizzata browser tracker. METHODS An online survey was completed by 56 cancer-free women (mean age 60.34 [SD 7.73] years) responding to a scenario asking them to imagine noticing a red scaly rash on the nipple. Participants were asked to make symptom attributions when presented with the scenario (T1) and again after seeking information online (T2). The online tracking tool, embedded in the survey, was used to capture in real time participant search terms and accessed websites. RESULTS The tracking tool captured the search terms and accessed websites of most of the participants (46/56, 82%). For the rest (10/56, 18%), there was evidence of engagement in online information-seeking (eg, medical terminology and cancer attribution at T2) despite their searching activity not being recorded. A total of 25 participants considered cancer as a potential cause for the nipple rash at T1, yet only one of these used cancer as a search term. Most participants (40/46, 87%) used rash-related search terms, particularly nipple rash and rash on nipple. The majority (41/46, 89%) accessed websites containing breast cancer information, with the National Health Service webpage “Paget disease of the nipple” being the most visited one. At T2, after engaging in the internet search task, more participants attributed the nipple rash to breast cancer than at T1 (37/46, 66% vs 25/46, 45%), although a small number of participants (6/46) changed from making a cancer attribution at T1 to a noncancer one at T2. CONCLUSIONS Making a cancer attribution for an unfamiliar breast change did not necessarily translate into cancer-termed searches. Equally, not all internet searches led to a cancer attribution. The findings suggest that online information-seeking may not necessarily help women who experience unfamiliar breast cancer symptoms understand their condition. Despite some technical issues, this study showed that it is feasible to use an online browser tracking tool to capture in real time information-seeking about unfamiliar symptoms.

BACKGROUND Digital communication technologies play an important role in governments’ and public health authorities’ health communication strategies during the COVID-19 pandemic. The internet and social media have become important sources of health-related information on the coronavirus and on protective behaviours. In addition, the COVID-19 infodemic spreads faster than the coronavirus itself, which interferes with governmental health-related communication efforts. This puts national public health containment strategies in jeopardy. Therefore, digital health literacy is a key competence to navigate coronavirus-related information and service environments. OBJECTIVE This study aimed to investigate university students’ digital health literacy and online information seeking behaviours during the early stages of the coronavirus pandemic in Germany. METHODS A cross-sectional study among N=14,916 university students aged ≥18 from 130 universities across all sixteen federal states of Germany was conducted using an online survey. Along with sociodemographic characteristics (sex, age, subjective social status) measures included five subscales from the Digital Health Literacy Instrument (DHLI), which was adapted to the specific coronavirus context. Online information seeking behaviour was investigated by examining the online sources used by university students and the topics that students search for in connection with the coronavirus. Data were analysed using univariate and bivariate analyses. RESULTS Across digital health literacy dimensions, the greatest difficulties could be found for assessing the reliability of health-related information (42.3%) and the ability to determine whether the information was written with commercial interest (38.9%). Moreover, respondents also indicated that they most frequently have problems finding the information they are looking for (30.4%). When stratified according to sociodemographic characteristics, significant differences were found with female university students reporting a lower DHLI for the dimensions of ‘information searching’ and of ‘evaluating reliability’. Search engines, news portals and public bodies’ websites were most often used by the respondents as sources to search for information on COVID-19 and related issues. Female students were found to use social media and health portals more frequently, while male students used Wikipedia and other online encyclopaedias as well as YouTube more often. The use of social media was associated with a low ability to critically evaluate information, while opposite differences were observed for the use of public websites. CONCLUSIONS Although digital health literacy is, in summary, well developed in university students, a significant proportion of students still face difficulties with certain abilities to deal with information. There is need to strengthen the digital health literacy capacities of university students using tailored interventions. Improving the quality of health-related information on the internet is also key. CLINICALTRIAL

Years of clinical trials have proven the maturity and safety of certain treatments, however, some of these mature treatments may not be highly effective. Several treatments have emerged through technological innovations, but their long-term safety, efficacy, and adverse effects remain unknown. At present, many patients seek information related to their treatments on the Internet, which may impact their attitudes towards different treatments and their trust in physicians. In this study, a research model was developed to examine how patients’ trust in their physicians is influenced by related online information on mature or emerging treatments. The hypotheses were tested using confirmatory factor analysis (CFA) and structural equation modelling (SEM). A total of 336 valid responses were collected through an online survey. Mature treatments related health information was found to significantly improve patients’ trust. Thus, physicians should pay more attention to mature treatments, and encourage their patients to seek related information online. Moreover, the quality of online information should be developed further to increase patients’ satisfaction. Physicians should also consider their patients’ psychological safety in communication with patients to strengthen their trust.

Background The prevalence of diabetes and the use of electronic health (eHealth) are increasing. Lifestyle changes in a positive direction may reduce morbidity and mortality in patients with diabetes. The main objective of this study was to test the association between lifestyle changes based on online information seeking and discussing the information with a doctor. Methods In this cross-sectional study we used e-mail survey data from 1250 members of The Norwegian Diabetes Association, collected in 2018. Included in the analyses were 847 men and women aged 18 to 89 years diagnosed with diabetes and who reported use of eHealth within the previous year. We used descriptive statistics to estimate lifestyle changes based on information from the internet. Logistic regressions were used to estimate the associations between lifestyle changes after online information seeking and discussing the information with a doctor. Analyses were adjusted for gender, age, education, and self-rated health. Results Lifestyle changes accomplished after online information seeking was reported by 46.9% (397/847) of the participants. The odds of changing lifestyle were more than doubled for those who had discussed information from the internet with a doctor (odds ratio 2.54, confidence interval 1.90–3.40). The odds of lifestyle changes were lower in the age groups 30–39 years and 60 years and over, compared to those aged 18–29 years (the reference group). Lifestyle changes were not associated with gender, education, or self-rated health. Conclusions Our findings indicate that health-care professionals can play an important role in lifestyle changes additional to health-advice found on the internet. This study underlines the importance of easily accessible high-quality online information, as well as the importance of making room for discussing lifestyle in the clinical encounter.

Background Online information seeking on medical topics by patients can have beneficial effects by helping them decide on treatment options and fostering better relationships with doctors. The quality of websites and processes of seeking information online have mostly been studied, with a focus on the accuracy and reliability of websites; however, few studies have examined the relationship between other aspects of quality and the processes of seeking medical information online. Objective This exploratory study aimed to shed light on the quality of websites used for information seeking from the perspective of understanding medical information in combination with seeking it online. Methods The study participants were 15 Japanese university students with no problem using the internet. A questionnaire survey about health literacy (47 items on a 4-point Likert scale) and information navigation skills on the internet (8 items on a 5-point Likert scale) was conducted before participants engaged in online information seeking and qualitative interviews. The students searched for information on a disease and its treatment. The websites viewed were gathered from search behavior recorded by software and browser logs. Follow-up interviews were conducted to elicit explanations from the participants about the assignments and their views of online information seeking. The explanations were evaluated by 55 health care professionals on a 3-point Likert scale and then assessed based on their comments and the participant interviews. Results The mean age of the participants was 20.6 years (median 21; SD 1.06). All participants were able to access reliable websites with information relevant to the assignments. The mean ratings of the students’ explanations were 108.6 (median 109; range=83-134) for the disease and 105.6 (median 104; range=87-117) for its treatment. The inter-rater reliability were 0.84 (95% CI 0.77-0.90) and 0.95 (95% CI 0.93-0.97), indicating good and excellent, respectively. The mean of the sum of the health literacy skills was 115.1 (median 115; range=80-166) and the mean for information navigation skills was 25.9 (median 26; range=17-36), respectively. Health literacy and information navigation skills were moderately correlated (r=0.54; 95% CI 0.033-0.822; P=.04). Among the four stages of health literacy, understanding and appraising (r=0.53; 95% CI 0.025-0.820; P=.04) were moderately correlated with information navigation skills (r=0.52; 95% CI 0.013-0.816; P=.046). The participants had no difficulties operating and browsing the internet and considered medical and public institution websites to be reliable; however, due to unfamiliarity with medical terms, they had difficulties choosing a site from the results obtained and comparing and synthesizing information provided by different sites. They also looked for sites providing orderly information in plain language but provided explanations from sites that gave inadequate interpretations of information. Conclusions This study revealed interactions between searching the internet for, and understanding, medical information by analyzing the processes of information seeking online, physicians’ evaluations and comments about the participants’ explanations, and the participants’ perceptions.

Background: In health and medicine, people heavily use the Internet to search for information about symptoms, diseases, and treatments. As such, the Internet information can simulate expert medical doctors, pharmacists, and other health care providers. Aim: This article aims to evaluate a dataset of search terms to determine whether search queries and terms can be used to reliably predict skin disease breakouts. Furthermore, the authors propose and evaluate a model to decide when to declare a particular month as Epidemic at the US national level. Methods: A Model was designed to distinguish a breakout in skin diseases based on the number of monthly discovered cases. To apply this model, the authors correlated Google Trends of popular search terms with monthly reported Rubella and Measles cases from Centers for Disease Control and Prevention (CDC). Regressions and decision trees were used to determine the impact of different terms to trigger the occurrence of epidemic classes. Results: Results showed that the volume of search keywords for Rubella and Measles rises when the volume of those reported diseases rises. Results also implied that the overall process was successful and should be repeated with other diseases. Such process can trigger different actions or activities to be taken when a certain month is declared as “Epidemic.” Furthermore, this research has shown great interest for vaccination against Measles and Rubella. Conclusions: The findings suggest that the search queries and keyword trends can be truly reliable to be used for the prediction of disease outbreaks and some other related knowledge extraction applications. Also search-term surveillance can provide an additional tool for infectious disease surveillance. Future research needs to re-apply the model used in this article, and researchers need to question whether characterizing the epidemiology of Coronavirus Disease 2019 (COVID-19) pandemic waves in United States can be done through search queries and keyword trends.

BACKGROUND Cyberchondria is considered “the anxiety-amplifying effects of online health-related searches”, and associated with health anxiety and online health-related information seeking behavior. However, data on the prevalence and influencing variables of cyberchondria are still scarce. Until now, there have been few studies on cyberchondria in residents in China, especially in the context of COVID-19 outbreaks in China. OBJECTIVE To investigate the prevalence and influencing factors of cyberchondria in residents in China during the epidemic period of coronavirus disease 2019. METHODS The participants were 674 community residents of a Chinese city surveyed from February 1 to 15, 2020. We administered online measures, including the Chinese Short Form of the Cyberchondria Severity Scale (CSCS), Short Health Anxiety Inventory (SHAI), eHealth Literacy Scale (eHEALS), Patient Health Questionnaire-15 (PHQ-15), and COVID-19-related online information seeking behavior questionnaire. RESULTS In our study, the average CSCS total score of residents was 30.65±11.53 during the virus epidemic; 79.4% of participants had a moderate level of cyberchondria, while 11.1% experienced a higher level of cyberchondria. Gender, age, monthly income, education level, personal illness with Helicobacter pylori infection, relatives’ illness with chronic bronchitis, COVID-19-related online information seeking frequency and duration were all significantly associated with the CSCS total score (p<.05), SHAI total score (β=0.598>0, P<.05), and eHEALS score (β=0.162>0, P<.05). Searching for information on diagnosing COVID-19 (β=2.28>0, P<.05) and the use of general search engines (β=1.867>0, P<.05) were independent risk factors for cyberchondria, while searching lasting less than 10 minutes each (β=-2.992<0, P<.05), the use of traditional media digital platforms (β=-1.650<0,P<.05), and the use of professional medical communication platforms (β=-4.189<0,P<.05) were independent protective factors for cyberchondria. CONCLUSIONS Most Chinese residents have a moderate level of cyberchondria, and cyberchondria has a strong positive association with health anxiety in residents in China during the virus epidemic. Searching online for less than 10 minutes for COVID-19-related information is an adoptable suggestion, and choosing a traditional media digital platform and a professional medical communication platform are recommended and helpful for alleviating cyberchondria.
