AbstractThis study compared macular capillary parameters between healthy black and white subjects using optical coherence tomography angiography (OCTA). We measured vessel density (VD) of superficial (SCP), intermediate (ICP), and deep (DCP) capillary plexuses and choriocapillaris blood flow area (BFA) of the fovea, parafovea and total 3 mm-diameter circular area centered on the fovea, as well as the foveal avascular zone (FAZ) parameters, controlling for axial length. Black subjects had lower foveal and parafoveal VD in the SCP (p = 0.043 and p = 0.014) and the ICP (p = 0.014 and p = 0.002). In the DCP, black subjects had a trend toward lower foveal and parafoveal VD. Black subjects had decreased choriocapillaris BFA in the total 3 mm area (p = 0.011) and the parafovea (p = 0.033), larger FAZ area (p = 0.006) and perimeter (p = 0.014), and a higher capillary density in a 300 μm wide region around the FAZ (FD-300) (p = 0.001). There was no significant difference in FAZ acircularity index. To our knowledge, this is the first report analyzing the three distinct retinal capillary plexuses and identifying differing baseline VD, choriocapillaris and FAZ parameters in healthy young black compared to white subjects. Larger studies are needed to validate these findings and better understand racial differences in vulnerability to ocular diseases.

Abstract Background To quantify and compare retinal microvascular features using optical coherence tomography angiography (OCTA) in constant (XT) and intermittent exotropia (IXT). Methods An observational cross-sectional study was conducted from September 2020 to November 2020 at the Affiliated Eye Hospital of Wenzhou Medical University. OCTA examination was performed to evaluate the macular perfusion density of the deep capillary plexus (DCP), superficial capillary plexus (SCP), and foveal avascular zone (FAZ) parameters in XT and IXT patients, and in age-matched controls. The study parameters were analyzed. Results A total of 76 participants, including 16 (21%) XT patients, 24 (32%) IXT patients, and 36 (47%) controls, were recruited. The exodeviation angle was 39.06 ± 10.38 prism degrees (PD) at near and 43.00 ± 9.74 PD at distance in the XT group, and 27.13 ± 18.28 PD at near and 31.04 ± 18.82 PD at distance in the IXT group. The macular perfusion density of the DCP in 6 × 6-mm scans and the SCP in 3 × 3-mm scans were lower in the deviating eyes than in the fixating eyes of XT patients (p < 0.001, p = 0.032, respectively). The macular perfusion density of the DCP in the deviating eyes of XT and IXT patients was significantly lower than that of the controls. There was no significant difference in the FAZ parameters among the groups. Conclusions In XT patients, OCTA revealed lower macular perfusion density in deviating eyes than in fixating eyes and control eyes. IXT patients showed no difference between the two eyes, but the macular perfusion density of the DCP was lower than that of the controls.

Purpose. To evaluate the effects of the menstrual cycle on the retinal vascular status of healthy women by optical coherence tomography angiography (OCTA). Materials and Methods. Healthy women with regular natural menstrual cycles of 28 to 30 days were recruited for this prospective study. The women’s retinal vascular status was measured by OCTA at 3 time points: the early follicular, ovulatory, and midluteal phases of the menstrual cycle. The main outcome measures were foveal avascular zone (FAZ) parameters, perfusion density (PD) percentage in the superficial retinal capillary plexus (SCP), and PD percentage in the deep retinal capillary plexus (DCP). The mean arterial pressure (MAP), spherical equivalent (SE), best-corrected visual acuity (BCVA), intraocular pressure (IOP), and axial (AL) were also measured in a same menstrual cycle. Results. In total, 62 right eyes of 62 women were included in the study. The mean age was 27.0 ± 1.73 (range, 24 to 31) years, and the mean menstrual cycle was 28.90 ± 0.84 (range, 28 to 30) days. The mean values of the DCP-PD parameters were significantly decreased in the nasal and inferior ETDRS subfields during the ovulatory phase. The mean DCP-PD in the nasal ETDRS subfield in the early follicular, ovulatory, and luteal phases was 54.11 ± 2.85, 56.39 ± 3.03, and 55.70 ± 3.27, respectively. The mean DCP-PD in the inferior ETDRS subfield in the early follicular, ovulatory, and midluteal phases was 52.90 ± 3.30, 54.86 ± 2.51, and 55.21 ± 2.64, respectively. No significant differences were found in MAP, SE, AL, IOP, FAZ area, or other quadrants of PD parameters, and no significant correlation was found between parameters by OCTA and age, MAP,SE, axial length, or IOP. Conclusions. The DCP-PD decreased in the nasal and inferior ETDRS subfields during the ovulatory phase in our study. This may indicate the need to consider the menstrual phase when interpreting DCP-PD parameters by OCTA in healthy women.

PurposeTo evaluate swept source optical coherence tomography angiography (SS-OCTA) in patients treated with hydroxychloroquine (HCQ) for more than 5 years and to compare results with the tests currently recommended for screening of HCQ retinopathy.MethodsIn this controlled pilot study, consecutive patients treated with HCQ for more than 5 years underwent SS-OCTA, SS-OCT B-scan and en-face C-scan, fundus autofluorescence (FAF), 10–2 automated visual field (AVF) testing and multifocal electroretinography (mfERG). On SS-OCTA, evaluation of the retinal superficial capillary plexus, middle capillary plexus, and deep capillary plexus (DCP) and the choriocapillaris (CC) was obtained.ResultsWe included 10 patients under HCQ treatment (20 eyes, mean age 38.91±11.73 years) and 18 healthy control patients (36 eyes, mean age 38.87±8.6 years). Mean duration of HCQ treatment was 10.0±3.25 (5–15) years and HCQ cumulative dose/body weight was 15.86±5.56 g/kg. The HCQ group showed a reduction of the vessel density in the 1 mm central, in the nasal and temporal subfields of DCP and in the 1 mm central subfield of CC, an increased foveal avascular zone in the three capillary plexuses, a greater frequency of CC flow voids and a reduced foveal choroidal thickness (p<0.05). Best-corrected visual acuity (BCVA), mfERG, SS-OCT B-scan and C-scan, AVF and FAF were normal in 20/20 eyes (100%).ConclusionsIn patients treated with HCQ for more than 5 years, choroidal thinning and flow abnormalities at SS-OCTA in the retinal capillary plexuses and CC may be observed even if BCVA, FAF, mfERG, AVF and SS-OCT are normal.

Introduction: The structural and vascular changes in the retina and choroid in women in the third trimester of pregnancy were analyzed using optical coherence tomography angiography (OCTA). Methods: Forty women in the third trimester of uncomplicated pregnancy and 40 age-matched healthy women were included. Vascular density (VD) in the superficial and deep capillary plexuses (SCP/DCP), foveal density (FD), and foveal avascular zone (FAZ) area and perimetry measured with OCTA, as well as OCT measurements of central macular thickness (CMT) and choroidal thickness (CT) were compared between the groups. Correlations between structural OCT parameters and vascular OCTA metrics were analyzed. Results: The mean gestational age was 34 (28–41) weeks. Mean age was comparable in the groups ( p = 0.732). The pregnant women had significantly higher parafoveal DCP-VD ( p = 0.015), FAZ area ( p = 0.044), and FD ( p = 0.002). Mean subfoveal CT was 21 µm higher in pregnant women but was not significant ( p = 0.472). There was no difference in CMT ( p = 0.448). FAZ metrics were positively correlated with CT in pregnants and with CMT in the control group ( p < 0.05). Parafoveal VD was negatively correlated with CT in the control group ( p < 0.05). After adjusting for CT and CMT, the significant difference in VD and FD persisted ( p < 0.05), while the difference in FAZ area lost significance ( p > 0.05). Conclusions: Considering the effects of the probable covariant factors CMT and CT, systemic changes in pregnant women in their third trimester may cause an increase in VD in the macula and parafoveal DCP.

Purpose: To emphasize the importance of optical coherence tomography angiography (OCTA) in the diagnosis and follow-up of diabetic individuals with and without retinopathy. Methods: Retrospective, cross-sectional, observational study of healthy persons and diabetic individuals with and without retinopathy. Area of the foveal avascular zone (FAZ) (mm²), non-perfused areas in the superficial capillary plexus (SCP), mean vessel density (VD) in the SCP, and deep capillary plexus (DCP) were calculated. In eyes with diabetic retinopathy (DR), measurements at baseline, 3rd, 6th, 9th, and 12th months were evaluated. Results: This study conducted on 39 eyes of 23 patients with DR (group 1), 59 eyes of 30 diabetic individuals without DR (group 2), and 51 eyes of 27 healthy persons (group 3). When the mean area of the FAZ at baseline was compared between groups, the difference among groups 1 and 3 ( p < 0.001) and the difference among groups 2 and 3 ( p = 0.001) were statistically significant. There was no significant difference among the measurements of mean area of the FAZ, mean non-perfused area in the SCP, mean VD in SCP, and DCP at baseline, 3rd, 6th, 9th, and 12th months in group 1. Conclusion: OCTA is a developing technology that can detect early microvascular changes in diabetic patients.

Background and Objective: The purpose of this study was to quantify the geometric dimension of the foveal avascular zone (FAZ) area and its index measurement of morphology as a circle (acircularity index) in healthy participants according to age and sex, using optical coherence tomography angiography (OCTA). Study Design/Materials and Method: This is a cross-sectional descriptive study conducted at LV Prasad Eye Institute, India. Written informed consent was obtained from all participants before the study. Deep range imaging-optical coherence tomography (DRI-OCT; Topcon) was performed on right eyes of all participants in this study. The OCTA scans were analyzed and processed; FAZ dimensions and the acircularity index were calculated for both superficial retinal capillary plexus (SCP) and deep retinal capillary plexus (DCP). Results: A total of 88 normal right eyes from 88 participants were included (48 males, 40 females; mean 42 ± 13 years of age). Mean FAZ area for SCP and DCP was 3.37 ± 1.28 mm2 and 2.31 ± 1.12 mm2, respectively. Mean acircularity index for SCP and DCP was 1.76 ± 1.58 and 1.60 ± 0.90, respectively. In the SCP, male sex was correlated with lower FAZ values; however, there was no correlation with age or refractive error. For the DCP, male sex and increasing age correlated with lower FAZ values, but there was no correlation with refractive error. Foveal avascular zone areas decreased 0.0225 mm2 per year in the DCP. The acircularity index of both the SCP and DCP was not correlated with sex, age, or refractive error. Conclusions: Male sex and increasing age were correlated with lower FAZ values in the DCP, while only male sex was correlated with lower FAZ values in the SCP. There was no correlation between sex, age, or refractive error with the acircularity index for both the SCP and DCP.

Objective: To investigate the effect of chronic smoking on the macula and optic nerve integrity using optical coherence tomography angiography (OCTA). Methods: This prospective, comparative study involved the right eyes of 160 healthy subjects (80 smokers group, and 80 control group). OCTA scanning was performed for all subjects. All macular subfields thickness, peripapillary retinal nerve fiber layer (RNFL) thickness, macular ganglion cell complex (GCC) thickness, superficial and deep macular vessel density, foveal avascular zone (FAZ) area, and the macular outer retina flow area, were measured and analyzed. Results: The thickness of some macular subfields, inferior GCC, and RNFL was statistically significantly lower in the smokers’ group ( p  < 0.05). The FAZ area was 0.409  ± 0.048 mm2 in the smokers' group and 0.327  ± 0.055 mm2 in the control group ( p  < 0.001). The superficial foveal VD was 31.35  ± 5.79% in the smokers’ group and 33.63  ± 4.78% in the control group ( p  = 0.06). The deep foveal VD was 39.38  ± 4.56% in the smokers’ group and 34.15  ± 6.57% in the control group ( p  ≤ 0.001). The superficial and deep parafoveal VD was 52.09  ± 4.56% and 59.99  ± 2.25% respectively in the smokers’ group, and 54.75  ± 2.30% and 55.58  ± 4.69% respectively in the control group ( p  < 0.05). The macular outer retina flow area was 1.29  ± 0.12 mm2 in the smokers’ group and 1.16  ± 0.11 mm2 in the control group ( p  < 0.001). Conclusion: Chronic smoking affected the integrity of the macula and optic nerve. It decreased the macular VD, enlarged the FAZ, and increased outer retinal flow area.
