Fenofibrate improves endothelial function in the brachial artery and forearm resistance arterioles of statin-treated Type 2 diabetic patients

Dyslipidaemia contributes to endothelial dysfunction and CVD (cardiovascular disease) in Type 2 diabetes mellitus. While statin therapy reduces CVD in these patients, residual risk remains high. Fenofibrate corrects atherogenic dyslipidaemia, but it is unclear whether adding fenofibrate to statin therapy lowers CVD risk. We investigated whether fenofibrate improves endothelial dysfunction in statin-treated Type 2 diabetic patients. In a cross-over study, 15 statin-treated Type 2 diabetic patients, with LDL (low-density lipoprotein)-cholesterol <2.6 mmol/l and endothelial dysfunction [brachial artery FMD (flow-mediated dilatation) <6.0%] were randomized, double-blind, to fenofibrate 145 mg/day or matching placebo for 12 weeks, with 4 weeks washout between treatment periods. Brachial artery FMD and endothelium-independent NMD (nitrate-mediated dilatation) were measured by ultrasonography at the start and end of each treatment period. PIFBF (post-ischaemic forearm blood flow), a measure of microcirculatory endothelial function, and serum lipids, lipoproteins and apo (apolipoprotein) concentrations were also measured. Compared with placebo, fenofibrate increased FMD (mean absolute 2.1±0.6 compared with −0.3±0.6%, P=0.04), but did not alter NMD (P=0.75). Fenofibrate also increased maximal PIFBF {median 3.5 [IQR (interquartile range) 5.8] compared with 0.3 (2.1) ml/100 ml/min, P=0.001} and flow debt repayment [median 1.0 (IQR 3.5) compared with −1.5 (3.0) ml/100 ml, P=0.01]. Fenofibrate lowered serum cholesterol, triacylgycerols (triglycerides), LDL-cholesterol, apoB-100 and apoC-III (P≤0.03), but did not alter HDL (high-density lipoprotein)-cholesterol or apoA-I. Improvement in FMD was inversely associated with on-treatment LDL-cholesterol (r=−0.61, P=0.02) and apoB-100 (r=−0.54, P=0.04) concentrations. Fenofibrate improves endothelial dysfunction in statin-treated Type 2 diabetic patients. This may relate partly to enhanced reduction in LDL-cholesterol and apoB-100 concentrations.

Context: Racial and sex differences are documented globally regarding patterns of serum lipids among diabetic patients. Differences of lipid profile of normal and diabetic subjects of the different ethnic groups of Bangladesh may differ.Objectives: To find out if there were any differences in the serum lipid and lipoprotein levels in the type 2 diabetic patients of two different races namely, Bangalee and Chakma of Bangladesh. Materials and Methods: Study population consisted of 76 (Chakma / Bangalee: 35 / 41) patients aged between 40-60 years with type 2 diabetes mellitus who were on follow-up at the Rangamati Diabetes Hospital, Chittagong Hill Tract, Bangladesh and forty (Chakma / Bangalee: 20 / 20) non-diabetic subjects of same age were randomly selected as control. Biochemical parameters analyzed include - Serum total cholesterol and triglyceride (TG), serum high-density lipoprotein (HDL), low-density lipoprotein (LDL) cholesterol and atherogenic index.Results:  Dyslipidemia was found in the diabetic patients of both races and the patients had higher serum TG), atherogenic index and lower serum HDL than their normal counterpart. The Bangalee patients were found to have higher TG and lower atherogenic index compared with the Chakma patients although no racial differences found regarding total, HDL and LDL cholesterol. Also no differences were found between the lipid profiles of normal control subjects of these two races. The purpose of the study was to assess the endothelial dysfunction (ED) in type 2 diabetic patients ultrasonographically and estimate the correlation of ED with glycemia and other cardio-metabolic risk factors. 171 patient (age 60.0 + 8.5 years) with diagnosed type 2 diabetes and coronary artery disease (CAD) were randomly included in a cross sectional study. B-mode ultrasound system with a linear transducer of 7.5 MHz was used for evaluation of flow-mediated vasodilation in brachial artery (FMV). FMV was presented as a change of brachial artery diameter at rest and after limb ischemia, previously provoked by cuff inflation. Peripheral ED was found in 77.2% (132 patients). Multivariate logistic regression model defined: age (OR 1.071, 95% CI 1.003 1,143) and plasma cholesterol (OR 4.083 95% CI 1.080 17,017) as determinants for ED. Linear multivariate analysis presented duration of diabetes (Beta 0.173, Sig 0.024), and glycemia (Beta 0.132, Sig 0.044) to be associated independently with FMV value. Estimated factors influencing FMV, might be potential therapeutic targets for presented endothelial dysfunction in type 2 diabetic patients with coronary artery disease.

ABSTRACT Aims: Previous studies have shown that diabetes mellitus (DM) increases the risk of cardiovascular diseases in females to a greater extent than in males. In this cross-sectional study, we evaluated the lipid profiles of type 2 diabetic males and females. Materials and Methods: The study included 107 type 2 diabetic patients (41 males and 66 females), and 122 hypertensive type 2 diabetic patients (39 males and 83 females), aged 15 years and older. Total cholesterol (TC), triglycerides (TG), low density lipoprotein-cholesterol (LDL-C), very low density lipoprotein-cholesterol (VLDL-C) and high density lipoprotein-cholesterol (HDL-C) concentrations were assayed for each group using standard biochemical methods. Results: The mean TC, TG, VLDL-C, HDL-C and LDL-C concentrations, TG/HDL and LDL/HDL ratios were higher in type 2 diabetic and hypertensive type 2 diabetic patients compared with non-diabetic, and hypertensive non-diabetic control subjects, although these were not significant (P > 0.05). Hypertensive type 2 diabetic females had significantly higher serum TC (7.42 ± 1.63 mmol/L) than hypertensive non-diabetic males (5.76±1.57 mmol/L; P < 0.05). All the other lipid and lipoprotein parameters except HDL-C were non-significantly higher in females with type 2 DM and those with hypertension and type 2 DM, compared with type 2 diabetic and hypertensive type 2 diabetic males, respectively (P > 0.05). Conclusion: This study demonstrated that dyslipidemia exists in our type 2 diabetic population with greater TC in hypertensive type 2 diabetic females compared with hypertensive type 2 diabetic males. This suggests that hypertensive type 2 diabetic females are exposed more profoundly to risk factors including atherogenic dyslipidemia compared with males.

Introduction: Evidence has long existed regarding the relationship between oxidative stress and diabetes. The present study was conducted to assess the effect of atorvastatin on selected oxidative stress parameters in the form of reduced glutathione (GSH), lipid peroxidation byproduct malondialdehyde (MDA) levels, glutathione –S- transferase (GST) activity and catalase (CAT) activity) and its effect on lipid profile (total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL) and very low density lipoprotein (VLDL) in dyslipidaemic type 2 diabetic patients . Materials and Methods: Fifty nine dyslipidaemic type 2 diabetic patients were included in this study. Full history was taken and general examination of patients was performed. Patients studied were taking glibenclamide (an oral hypoglycaemic drug) during the study as a treatment for their disease. These patients were followed up for 60 days and divided randomly into 2 groups. Group I (n = 31): no drug was given and served as dyslipidaemic diabetic control. Group II (n = 28): received atorvastatin tablets 20 mg once daily at night. Of the 59 Fifty patients, 46 completed the study while 13 patients withdrew. This is due to non compliance of the patients. Blood samples were drawn from the patients at the beginning and after 60 days of follow up between 8:30 & 10:30 am after at least 12-14 hours fast. Fasting blood glucose, lipid profile, selected oxidative stress parameters (GSH, MDA levels, GST and CAT activities) were measured. Renal and hepatic functions were also assessed. Results: This study revealed that: atorvastatin treatment increased serum GSH; reduced MDA levels significantly while did not significantly affect CAT and GST activity. In atorvastatin treatment, TC, TG, LDL and VLDL decreased significantly while HDL increased significantly. Conclusion: There was insignificant correlations between atorvastatin induced changes in the oxidation markers and the observed changes of the lipid profile.
