The Use of Large Group Assistive Listening Devices With and Without Hearing Aids in an Adult Classroom Setting

During a hearing loss management workshop, 10 listeners with normal hearing sensitivity and 18 listeners with sensorineural hearing loss compared four group assistive listening devices (ALDs)—FM, induction loop, infrared, and soundfield amplification—to no system. Listeners with hearing loss were tested using the ALDs alone and using the ALDs inductively coupled to personal in-the-ear (ITE) hearing aids. Significant improvements in word recognition ability with the FM, induction loop, and infrared systems were noted in listeners with normal hearing and with all ALD systems in listeners with hearing loss, as compared to performance with no system. Listeners with hearing loss performed better and preferred using the FM, induction loop, and infrared systems with headsets, but preferred the soundfield amplification system with their hearing aids. Both groups of listeners preferred the FM system over other systems in terms of performance, comfort, and ease of use.

Purpose A questionnaire survey was conducted to collect information from clinical audiologists about rehabilitation options for adult patients who report significant auditory difficulties despite having normal or near-normal hearing sensitivity. This work aimed to provide more information about what audiologists are currently doing in the clinic to manage auditory difficulties in this patient population and their views on the efficacy of recommended rehabilitation methods. Method A questionnaire survey containing multiple-choice and open-ended questions was developed and disseminated online. Invitations to participate were delivered via e-mail listservs and through business cards provided at annual audiology conferences. All responses were anonymous at the time of data collection. Results Responses were collected from 209 participants. The majority of participants reported seeing at least one normal-hearing patient per month who reported significant communication difficulties. However, few respondents indicated that their location had specific protocols for the treatment of these patients. Counseling was reported as the most frequent rehabilitation method, but results revealed that audiologists across various work settings are also successfully starting to fit patients with mild-gain hearing aids. Responses indicated that patient compliance with computer-based auditory training methods was regarded as low, with patients generally preferring device-based rehabilitation options. Conclusions Results from this questionnaire survey strongly suggest that audiologists frequently see normal-hearing patients who report auditory difficulties, but that few clinicians are equipped with established protocols for diagnosis and management. While many feel that mild-gain hearing aids provide considerable benefit for these patients, very little research has been conducted to date to support the use of hearing aids or other rehabilitation options for this unique patient population. This study reveals the critical need for additional research to establish evidence-based practice guidelines that will empower clinicians to provide a high level of clinical care and effective rehabilitation strategies to these patients.

The loudness growth in 1/2-octave bands (LGOB) procedure has been shown previously to provide valid estimates of loudness growth for adults with normal hearing and those with hearing loss (Allen, Hall, & Jeng, 1990), and it has been widely incorporated into fitting strategies for adult hearing aid users by a hearing aid manufacturer. Here, we applied a simple modification of LGOB to children and adults with normal hearing and then compared the loudness growth functions (as obtained from end-point data) between the two age groups. In addition, reliability data obtained within a single session and between test sessions were compared between the two groups. Large differences were observed in the means between the two groups for the lower boundary values, the upper boundary values, and the range between boundaries both within and across all frequencies. The data obtained from children also had greater variance than the adult data. In addition, there was more variability in the data across test sessions for children. Many test-retest differences for children exceeded 10 dB. Adult test-retest differences were generally less than 10 dB. Although the LGOB with the modifications used in this study may be used to measure loudness growth in children, its poor reliability with this age group may limit its clinical use for children with hearing loss. Additional work is needed to explore whether loudness growth measures can be adapted successfully to children and whether these measures contribute worthwhile information for fitting hearing aids to children.

The auditory system allows the estimation of the distance to sound-emitting objects using multiple spatial cues. In virtual acoustics over headphones, a prerequisite to render auditory distance impression is sound externalization, which denotes the perception of synthesized stimuli outside of the head. Prior studies have found that listeners with mild-to-moderate hearing loss are able to perceive auditory distance and are sensitive to externalization. However, this ability may be degraded by certain factors, such as non-linear amplification in hearing aids or the use of a remote wireless microphone. In this study, 10 normal-hearing and 20 moderate-to-profound hearing-impaired listeners were instructed to estimate the distance of stimuli processed with different methods yielding various perceived auditory distances in the vicinity of the listeners. Two different configurations of non-linear amplification were implemented, and a novel feature aiming to restore a sense of distance in wireless microphone systems was tested. The results showed that the hearing-impaired listeners, even those with a profound hearing loss, were able to discriminate nearby and far sounds that were equalized in level. Their perception of auditory distance was however more contracted than in normal-hearing listeners. Non-linear amplification was found to distort the original spatial cues, but no adverse effect on the ratings of auditory distance was evident. Finally, it was shown that the novel feature was successful in allowing the hearing-impaired participants to perceive externalized sounds with wireless microphone systems.

Preference for non-use of hearing aids among older adults who are candidates for amplification remains to be explained. Clinical studies have examined the contribution of consumer attitudes, behaviors, and life circumstances to this phenomenon. The present study extends the interests of earlier investigators in that it examines psychological control tendencies in combination with hearing loss and demographic variables among older adults who elected to accept (adherents) or ignore (nonadherents) advice from hearing professionals to acquire and use hearing aids. One hundred thirty-one individuals participated by completing measures of hearing, hearing handicap, psychological control, depression, and ego strength. Participants were asked to provide demographic information and personal opinions regarding hearing aid use. Adherence group and gender differences were noted on measures of hearing sensitivity, psychological control, and demographic factors. Female adherents demonstrated greater hearing loss and poorer word recognition ability but less hearing handicap, higher internal locus of control, higher ego strength, and fewer depressive tendencies than female nonadherents. They reported demographic advantages. Female adherents assumed responsibility for effective communication. Although male adherents and nonadherents did not differ significantly demographically, male adherents were more accepting of their hearing loss, took responsibility for communication problems, and found hearing aids less stigmatizing. Implications for clinical practice and future clinical investigations are identified and discussed. Results are expected to be of interest to clinicians, clinical investigators, and health care policymakers.

Objectives -to determine the normative data of sentence speech intelligibility in a free sound field and to estimate the applicability of the Russian Matrix Sentence test (RuMatrix) for assessment of the hearing aid fitting benefit. Material and methods. 10 people with normal hearing and 28 users of hearing aids with moderate to severe sensorineural hearing loss were involved in the study. RuMatrix test both in quiet and in noise was performed in a free sound field. All patients filled in the COSI questionnaire. Results. The hearing impaired patients were divided into two subgroups: the 1st with high and the 2nd with low hearing aid benefit, according to the COSI questionnaire. In the 1st subgroup, the threshold for the sentence intelligibility in quiet was 34.9 ± 6.4 dB SPL, and in noise -3.3 ± 1.4 dB SNR, in the 2nd subgroup 41.7 ± 11.5 dB SPL and 0.15 ± 3.45 dB SNR, respectively. The present study investigated two measures of spatial acoustic perception in children and adolescents with sensorineural hearing loss (SNHL) tested without their hearing aids and compared it to age-matched controls. Auditory localization was quantified by means of a sound source identification task and auditory spatial discrimination acuity by measuring minimum audible angles (MAA). Both low- and high-frequency noise bursts were employed in the tests to separately address spatial auditory processing based on interaural time and intensity differences. In SNHL children, localization (hit accuracy) was significantly reduced compared to normal-hearing children and intraindividual variability (dispersion) considerably increased. Given the respective impairments, the performance based on interaural time differences (low frequencies) was still better than that based on intensity differences (high frequencies). For MAA, age-matched comparisons yielded not only increased MAA values in SNHL children, but also no decrease with increasing age compared to normal-hearing children. Deficits in MAA were most apparent in the frontal azimuth. Thus, children with SNHL do not seem to benefit from frontal positions of the sound sources as do normal-hearing children. The results give an indication that the processing of spatial cues in SNHL children is restricted, which could also imply problems regarding speech understanding in challenging hearing situations.

A 23-month-old female was referred for hearing aid fitting after failing newborn hearing screening and being diagnosed with significant hearing loss through subsequent diagnostic testing. Auditory brainstem response (ABR) and behavioral testing revealed a moderate-to-severe bilateral mixed hearing loss. Prior to the hearing aid evaluation, tympanostomy tubes had been placed bilaterally with little or no apparent change in hearing sensitivity. Initial testing during the hearing aid fitting confirmed earlier findings, but abnormal middle ear results were observed, requiring referral for additional otologic management. Following medical clearance, binaural digital programmable hearing aids were fit using Desired Sensation Level parameters. Behavioral testing and probe microphone measures showed significant improvements in audibility. Decrease in hearing sensitivity was observed six months following hearing aid fitting. Radiological studies, ordered due to the mixed component and decreased hearing sensitivity, revealed large vestibular aqueduct syndrome (LVAS). Based on the diagnosis of LVAS, a cochlear implant was placed on the right ear; almost immediate speech-language gains were observed.

Background: Hearing loss and age interfere with the auditory system's ability to process temporal changes in the acoustic signal. A key unresolved question is whether high-frequency sensorineural hearing loss (HFSNHL) affects temporal processing in the low-frequency region where hearing loss is minimal or nonexistent. A second unresolved question is whether changes in hearing occur in middle-aged subjects in the absence of HFSNHL. Purpose: The purpose of this study was twofold: (1) to examine the influence of HFSNHL and aging on the auditory temporal processing abilities of low-frequency auditory channels with normal hearing sensitivity and (2) to examine the relations among gap detection measures, self-assessment reports of understanding speech, and functional measures of speech perception in middle-aged individuals with and without HFSNHL. Research Design: The subject groups were matched for either age (middle age) or pure-tone sensitivity (with or without hearing loss) to study the effects of age and HFSNHL on behavioral and functional measures of temporal processing and word recognition performance. These effects were analyzed by individual repeated-measures analyses of variance. Post hoc analyses were performed for each significant main effect and interaction. The relationships among the measures were analyzed with Pearson correlations. Study Sample: Eleven normal-hearing young adults (YNH), eight normal-hearing middle-aged adults (MANH), and nine middle-aged adults with HFSNHL were recruited for this study. Normal hearing sensitivity was defined as pure-tone thresholds ≤25 dB HL for octave frequencies from 250 to 8000 Hz. HFSNHL was defined as pure-tone thresholds ≤25 dB HL from 250 to 2000 Hz and ≥35 dB HL from 3000 to 8000 Hz. Data Collection and Analysis: Gap detection thresholds (GDTs) were measured under within-channel and between-channel conditions with the stimulus spectrum limited to regions of normal hearing sensitivity for the HFSNHL group (i.e., <2000 Hz). Self-perceived hearing problems were measured by a questionnaire (Abbreviated Profile of Hearing Aid Benefit), and word recognition performance was assessed under four conditions: quiet and babble, with and without low-pass filtering (cutoff frequency = 2000 Hz). Results: The effects of HFSNHL and age were found for gap detection, self-perceived hearing problems, and word recognition in noise. The presence of HFSNHL significantly increased GDTs for stimuli presented in regions of normal pure-tone sensitivity. In addition, middle-aged subjects with normal hearing sensitivity reported significantly more problems hearing in background noise than the young normal-hearing subjects. Significant relationships between self-report measures of hearing ability in background noise and word recognition in babble were found. Conclusions: The conclusions from the present study are twofold: (1) HFSNHL may have an off-channel impact on auditory temporal processing, and (2) presenescent changes in the auditory system of MANH subjects increased self-perceived problems hearing in background noise and decreased functional performance in background noise compared with YNH subjects.

A cross sectional study was done in the department of otolaryngology of Central Police Hospital. Rajarbagh, Dhaka., during the period of January 2005 to December 2007 to find out noise induced hearing loss of traffic polices and to raise awareness for early diagnosis and treatment and also preventive measures of noise induced hearing loss. 'Thirty patients who were diagnosed as a case of hearing impairment by detailed history, clinical examination and related investigations were included in the study who were exposed to noise 0-20 years. Common chief complaints of the patients were impairment of hearing (100%), Minims (50%), dizziness (20%), aural discharge (6.66)%. Medical history of 11w patients were diabetes (6.66%) and hypertension (26.66 %). 40 % patients were smoker and 60 % patients were non smoker. A ssoc jar ed other disease like chronic supporative otitis media (CSOM) t vas present in 13.33 %. Pure tone audiometry revealed normal hearing 3.3 %, conductive hearing loss 10 %, sensorineural hearing loss 73.33 %, mixed hearing loss 13.33 % and normal hearing 6.66 %, mild hearing loss 50 To, moderate type of hearing loss 36.66 % and moderately severe 3.33 %, severe hearing loss were 3.33 %. Treatment of the patients were given by vitamin 13,, Lli, B6, cochlear vasodilators such as vinpocetine and hearing aids. Patients were advised to Hume to other department of police service. those traffic police persons are vulnerable of noise induced hearing loss determined by patients complain, clinical examination and investigations like PTA, were advised to be changed to other department of police service.

Aging, consensually assumed as a process that affects and alters significantly the cognitive and behavioral functions of the human being , encompassing multiple effects on a number of clinically significant variables , the most important being those that directly relate to the movement, ability to balance the effectiveness of mental functioning and sensory functions such as taste, smell , sight and hearing . Hearing loss in the elderly, scientifically called presbiacustia emerges quietly and is increased progressively with advancing age. This study had as its aim to contribute to the achievement of the best quality of life for elderly individuals with hearing loss (presbycusis). Thus, the objectives targeted were: trying to understand the motivations of the elderly to the use of hearing aids; determine the degree of satisfaction of users of these prostheses; evaluate how Preabycusis affects the quality of life of the elderly. The method used was qualitative oriented once interviewed the elderly users of the home. The sample was composed of 110 participants, with presbycusis, 69 female and 41 male (institutionalized seniors).
