The Effect of Injury Definition and Surveillance Methodology on Measures of Injury Occurrence and Burden in Elite Volleyball

AbstractA time-loss injury definition continues to be the most widely used injury definition despite evidence that it fails to accurately capture overuse injuries. An overuse injury questionnaire, using an “all complaints” definition has been created to address the limitation of a time-loss definition. The main aim of this work was to determine the effect that injury definition and registration methodology has on the collection of knee injuries among elite level volleyball players. To reach this goal, seventy-two volleyball players were prospectively followed over 32-weeks. Time-loss injuries were captured using an individual injury report form (IIRF). Study participants completed an overuse injury questionnaire (mOIQ) via a weekly short message service (SMS). The IIRF captured 15 time-loss knee injuries in 72 study participants (20%). Based on the mOIQ, 84.7% of participants reported having a knee problem and 66.7% sustained a substantial knee problem. All IIRF knee injuries captured were also registered by the mOIQ. Agreement on the specific diagnosis occurred for 66.7% of injuries resulting in a moderate Kappa score of 0.51. In conclusion, an overuse injury questionnaire provided a greater understanding of the magnitude and burden of knee injuries in this population.

Background Little is known about injury risk or the pattern of injuries among competitive snowboarders. Purpose To describe the incidence and pattern of injuries among female and male snowboard athletes at the highest national level. Study Design Descriptive epidemiology study Methods This study consists of 2 parts: prospective registration of injuries at snowboarding national cup events and the national championships during the 2002 season, as well as a retrospective interview of the participants during the national championships in March 2002. All injuries that resulted in missed participation were recorded. Exposure was recorded as the number of runs in all disciplines. Results In the prospective study, the competition incidence was 4.0 ± 0.7 injuries per 1000 runs (n = 32 injuries). Back (22%), knee (16%), and hand/wrist injuries (9%) were the most common. The incidence for the different disciplines was 14.2 ± 5.3 per 1000 runs for big jump, 6.1 ± 1.8 for snowboardcross, 3.1 ± 0.9 for halfpipe, and 1.9 ± 1.9 for giant slalom. The retrospective interview (n = 163 athletes, 83% response) revealed 84 acute time-loss injuries during the season; knee (16%), back (13%), head (13%), and hand/wrist injuries (12%) were the 4 most common injury types. The overall competition incidence was 3.4 ± 0.6 injuries per 1000 runs (6.6 ± 3.0 for big jump, 5.8 ± 1.7 for snowboardcross, 2.1 ± 0.7 for halfpipe, and 6.6 ± 4.7 for giant slalom). No injury was recorded in parallel slalom in either study. Conclusion The incidence of injuries is high among competitive snowboarders at the elite national level. The injury pattern is different from the panorama seen among less experienced athletes, with fewer wrist injuries and more knee injuries.

Background Little is known about the pattern of injury in short track speed skating. Purpose To investigate the incidence and characteristics of injuries in short track speed skating. Study Design Retrospective study. Methods Ninety-five of 150 elite-level skaters (63.3%) were surveyed to collect information on training and competition load as well as on injuries sustained during the 1999—2000 competitive season. Injuries were characterized in terms of anatomic location, type of injury, time loss from training and competition, and circumstance of injury (acute onset during competition, on-ice practice, off-ice training, or insidious onset). Results Sixty-one of the 95 skaters (64.2%) reported sustaining at least one injury. The knee, ankle, spine, leg, and groin were the most commonly reported sites of injury. Skaters were also asked to list previous on-ice injuries. The two most common injuries occurring on-ice before the 1999—2000 season were lacerations from the knee down (11.1%) and ankle fractures (10.2%). Conclusion The results of this study suggest that there is a high incidence of injury in competitive short track speed skating.

ContextParticipation in high school athletics carries a substantial risk of musculoskeletal injury, particularly to the knee. However, limited information is available on the care being provided by athletic trainers (ATs) for athletes with these injuries.DesignDescriptive epidemiology study.SettingData on athletic training facility visits and AT services were collected from 147 high schools in 26 states.Patients or Other ParticipantsHigh school student-athletes who participated in 13 boys' sports and 14 girls' sports and sustained a diagnosed knee injury during the 2011–2012 through 2013–2014 academic years. The ATs documented 6797 knee injuries, with 4242 sustained in boys' sports and 2555 sustained in girls' sports.Main Outcome Measure(s)The number of athletic training facility visits and individual AT services as well as the average, median, and interquartile range of athletic training facility visits (per injury) and AT services (per injury) were calculated by sport and for time-loss (TL; participation restricted for ≥24 hours) and non–time-loss (NTL; participation restricted for <24 hours) injuries.ResultsIn total, 28 788 athletic training facility visits were reported, with an overall average of 4 athletic training facility visits per knee injury over the 3-year period. Most athletic training facility visits (72.6%) were associated with NTL injuries, but the majority of AT services (68.6%) were associated with TL injuries. A total of 81 245 AT services were provided for all knee injuries. Therapeutic activities or exercise were the most common type of AT service (52.5%). Overall, an average of 12 AT services were reported per knee injury. Compared with NTL injuries, TL injuries had a larger average number of AT services per injury (34 versus 9; P < .001).ConclusionsKnee injuries at the high school level are a considerable health care burden. This report demonstrates a high proportion of AT attention to the evaluation and treatment of these injuries. This study confirms the recommended management of knee injuries, with neuromuscular and therapeutic activities being the primary services after knee injury.

Context: Injuries are a major adverse event in a soccer player's career. Reducing injury incidence requires a thorough knowledge of the epidemiology of soccer injuries. Objective: To investigate the incidence and characteristics of injuries in the Dutch premier soccer league. Design: Cohort study. Setting: The Dutch premier soccer league. Patients or Other Participants: During the 2009–2010 soccer season, a total of 217 professional soccer players from 8 teams were prospectively followed. Main Outcome Measure(s): The medical staff recorded time-loss injuries, including information on injuries (ie, type, body part, duration) and exposure data for training sessions and matches. Results: A total of 286 injuries were recorded, affecting 62.7% of the players. The overall injury incidence was 6.2 injuries per 1000 player-hours, 2.8 in training sessions and 32.8 in matches. Most of the recorded injuries were acute (68.5%). Eight percent of the injuries were classified as recurrent. Injuries were most likely to be located in the lower extremities (82.9%). Injury time loss ranged from 1 to 752 days, with a median of 8 days. Knee injuries had the greatest consequences in terms of days of absence from soccer play (on average, 45 days). The most common diagnosis was muscle/tendon injury of the lower extremities (32.9%). Conclusions: Injury risk in the Dutch premier soccer league is high, especially during matches. Preventive measures should focus on the most common diagnoses, namely, muscle/tendon injuries of the lower extremities.

ObjectivesTo define incidence and injury patterns of International Ski Federation (FIS) World Cup (WC) women ski jumpers over three seasons.MethodsSki jump athletes competing in the Women’s FIS WC were recruited for prospective injury surveillance from 2017–2018 to 2019–2020. Team representatives recruited the athletes annually and prospectively recorded all injuries requiring medical attention. Retrospective end-of-season interviews corroborated injury surveillance. Medical doctors collected and processed the data. The 4-month competitive season was used to calculate the annual incidence of injuries per 100 athletes per season. Injury type, location, severity and aetiology were reported.ResultsAthletes from 19 nations were enrolled equalling 205 athlete-seasons. Mean age was 21.2 years (SD=3.8). Thirty-nine injury events resulted in 54 total injuries (26.3 injuries/100 athletes/season). Injuries were mostly acute (83%) and occurred on the ski jump hill (78%). The most common injury location was the knee (n=18, 33%). Crash landings were the most common cause of injury events (70%). Nearly half of the acute ski jump injury events occurred in snowy, windy or cloudy conditions (44%) and/or during telemark landings (46%), and most jumps (96%) were shorter than hill size. One third of the injuries were severe, and 78% of severe injuries involved the knee.ConclusionAcute injury events occur relatively frequently in elite women ski jumpers, most resulting in time-loss from sport and a significant proportion involving serious knee injuries. Crash landing was the leading cause of injury. This baseline information can be used to guide and evaluate future efforts at injury prevention.

Background. Microscopic analysis of stained blood smear is the most suitable method of malaria diagnosis. However, gaps were observed among clinical laboratory professionals in microscopic diagnosis of malaria. Methods. A cross-sectional study was conducted in December 2015 among 46 laboratory professionals. Data was collected via on-site assessment and panel testing. The slide panel testing was composed of positive and negative slides. The kappa score was used to estimate the agreement between participants and reference reader. Results. The overall agreement between the study participants and the reference reader in malaria detection was 79% (kappa = 0.62). Participating in refresher training on malaria microscopy (Adjusted Odds Ratio (AOR = 7, CI = 1.5–36.3)) and malaria epidemic investigation (AOR = 4.1 CI = 1.1–14.5) had statistical significant association with detection rate of malaria parasites. Conclusion. Laboratory professionals showed low performance in malaria microscopy. Most of the study participants were graded “in-training” in laboratory diagnosis of malaria.

BACKGROUND Traditional methods for recruiting and maintaining contact with participants in cohort studies include print-based correspondence, which can be unidirectional, labor intensive, and slow. Leveraging technology can substantially enhance communication, maintain engagement of study participants in cohort studies, and facilitate data collection on a range of outcomes. OBJECTIVE This paper provides an overview of the development process and design of a cohort management platform (CMP) used in the Newborn Epigenetic STudy (NEST), a large longitudinal birth cohort study. METHODS The platform uses short message service (SMS) text messaging to facilitate interactive communication with participants; it also semiautomatically performs many recruitment and retention procedures typically completed by research assistants over the course of multiple study follow-up visits. RESULTS Since February 2016, 302 participants have consented to enrollment in the platform and 162 have enrolled with active engagement in the system. Daily reminders are being used to help improve adherence to the study’s accelerometer wear protocol. At the time of this report, 213 participants in our follow-up study who were also registered to use the CMP were eligible for the accelerometer protocol. Preliminary data show that texters (138/213, 64.8%), when compared to nontexters (75/213, 35.2%), had significantly longer average accelerometer-wearing hours (165.6 hours, SD 56.5, vs 145.3 hours, SD 58.5, P=.01) when instructed to wear the devices for 1 full week. CONCLUSIONS This platform can serve as a model for enhancing communication and engagement with longitudinal study cohorts, especially those involved in studies assessing environmental exposures.

Abstract Background Acute and overuse injuries affect millions of high school athletes annually and a better understanding of differences between these injuries is needed to help guide prevention, treatment, and rehabilitation strategies. This study compares acute and overuse injuries using a nationally representative sample of high school athletes. Methods Injuries among United States high school athletes participating in 5 boys’ sports (football, soccer, basketball, wrestling, baseball) and 4 girls’ sports (soccer, volleyball, basketball, softball) reported in the High School RIO™ surveillance system during the 2006-07 through 2018-19 school years were classified as acute or overuse. National estimates and injury rates were calculated. Results Of 17 434 646 estimated injuries, 92.0 % were acute and 8.0 % were overuse. The acute injury rate was higher than the overuse injury rate among both male (Rate Ratio [RR] 16.38, 95 % CI: 15.70–17.10) and female (RR 8.14, 95 % CI: 7.71–8.60) athletes. The overuse injury rate per 10,000 athlete exposures among female athletes (1.8) was slightly higher than among males (1.4). The rate of acute injury compared with the rate of overuse injury was higher during competition (RR 32.00, 95 % CI: 29.93–34.22) than practice (RR 7.19, 95 % CI: 6.91–7.47). Boys’ football contributed the most acute (42.1 %) and overuse (23.7 %) injuries among the 9 sports. Among female sports, girls’ soccer contributed the most acute (15.6 % of all acute injuries) and overuse (19.4 % of all overuse injuries) injuries. The lower extremity was most commonly injured in acute (48.9 %) and overuse (65.9 %) injuries. Ligament sprain (31.7 %) and concussion (21.0 %) were the most common acute injury diagnoses, while muscle strain (23.3 %) and tendonitis (23.2 %) were the most common overuse injury diagnoses. Compared with acute injuries, overuse injuries were more likely to result in time loss from sports participation of < 1 week among both boys and girls and across most sports. Acute injuries were more likely than overuse injuries to cause a time loss of 1–3 weeks or medical disqualification from sports participation. Conclusions Acute and overuse injuries display many differences that provide opportunities for data-informed athlete preparation, treatment, and rehabilitation, which may reduce injuries and improve injury outcomes in high school athletics.
