SummaryThe thrombolytic effect of pig plasmin was tested in a double blind trial on patients with deep venous thrombosis in the lower limb. Only patients with not more than three days old thrombi were selected for this study. The diagnosis of deep vein thrombosis was made clinically and confirmed by phlebography. Lysofibrin Novo (porcine plasmin) or placebo (porcine plasminogen) was administered intravenously to the patients. The enzyme and the placebo were delivered as lyophilized powder in labelled bottles - the contents of the bottles were unknown to the doctor in charge of the clinical administration of the trial. An initial dose of plasmin/plasminogen of 30 unit per kg body weight given slowly intravenously (1-1% hours infusion) was followed by a maintenance dosis of 15 per cent the initial dose per hour for the following 5-7 hours. In most cases a similar maintenance dosis was given the next day. In all patients heparin was administered after ending the plasmin/plasminogen infusion. The results of the treatment was evaluated clinically as well as by control phlebo- grams the following days.A statistically significant improvement was found in the plasmin treated group compared with the placebo (plasminogen) treated group. Thrombolysis was obtained clinically and phlebographically in 65 per cent of the plasmin treated group, but only in 15 per cent of the control patients were improvements found.This study has thus demonstrated that plasmin treatment according to a standard scheme was able to induce thrombolysis. There were only a few and insignificant side effects. Allergic reactions have not been seen and only very simple tests are required.

Objectives Percutaneous pharmacomechanical thrombectomy is an emerging therapy for acute deep vein thrombosis and may reduce long-term incidence of post-thrombotic syndrome. This study investigates the proportion of patients presenting with lower limb deep vein thrombosis who are potentially suitable for percutaneous pharmacomechanical thrombectomy. Methods A retrospective review of all duplex ultrasound scans for lower limb deep vein thrombosis over two-year period at a regional vascular unit was conducted. All acute occlusive iliofemoral deep vein thrombosis were screened for percutaneous pharmacomechanical thrombectomy suitability according to predefined criteria. Results There were 2513 duplex ultrasound scans for suspected lower limb deep vein thrombosis in the two-year period. There were 120 cases of acute occlusive iliofemoral deep vein thrombosis. After application of inclusion and exclusion criteria 48 out of 120 (40%) patients were identified as potential candidates for percutaneous pharmacomechanical thrombectomy. Conclusions This indicates that a large randomised trial of percutaneous pharmacomechanical thrombectomy is feasible given expected recruitment rates in a multicentre study.

This study assessed the performance of a pocket-sized ultrasound system for the diagnosis of proximal lower limb acute deep vein thrombosis (DVT) compared to a full-sized ultrasound system. Patients who needed urgent lower limb sonograms for acute DVT were invited for the study. In each examination, the investigator scanned the patient using the pocket-sized system and then repeated the scan using the full-sized system. The sensitivity, specificity, and accuracy of the pocket-sized system were determined in reference to the full-sized system. The venous segments that failed to be visualized using the two systems were compared. One hundred lower limbs comprising 500 venous segments were examined. There were four venous segments, including two mid and two lower femoral veins in two patients who failed to be visualized using both systems. The sensitivity, specificity, and accuracy for diagnosing proximal lower limb acute DVT were 100% (95% confidence interval [CI], 94.94%–100%), 100% (95% CI, 99.05%–100%), and 100% (95% CI, 99.19%–100%), respectively. The pocket-sized ultrasound system and the full sized-ultrasound system demonstrated a comparable performance in detecting acute DVT in the leg.

Drugs known to affect platelet function have been shown to be effective in the prevention of venous thrombosis after surgery. A double-blind randomized trial of aspirin plus dipyridamole was carried out to investigate the prophylactic value of the drug combination in the prevention of venous thrombosis detected by 125 Iodine-labelled fibrinogen leg-scanning after elective hip surgery. Aspirin 900 mg plus dipyridamole 100 mg were given once pre-operatively and twice daily for 10 days postoperatively, or until the patient was fully ambulant. 43 patients were entered into the trial, 22 in the control group and 21 in the treated group. Both groups were similar with respect to age, sex and factors known to predispose towards venous thrombosis. 8 patients (36.7%) in the control group and 8 patients (38.1% ) in the treated group developed leg scan-detected deep vein thrombosis. The thrombi were detected between the 1st and 17th post-operative day and were found as frequently in the operated limbs as in the non-operated limbs in both groups. It is concluded that prophylaxis with aspirin 900 mg and dipyridamole 100 mg twice daily does not influence the incidence of leg scan-detected deep vein thrombosis after elective hip surgery.

SummaryTreatment with streptokinase (‘Kabikinase’) was given to 26 patients with venographically confirmed deep vein thrombosis extending into the popliteal vein or above. Treatment was continued for 4 days and the patients were allocated randomly to oral anticoagulant therapy or a course of treatment with ancrod (‘Arvin’) for 6 days followed by oral anticoagulant therapy. The degree of thrombolysis as judged by further venographic examination at 10 days was not significantly different between the 2 groups. The majority of patients showed clinical improvement but there was no appreciable difference between the groups at 3 and 6 months. Haemorrhagic complications were a more serious problem during the period of treatment with ancrod than during the equivalent period in the control group.
