Objectives To investigate the long-term relationship between intraoperative temperature changes of the palm, treatment effects and compensatory hyperhidrosis (CH). Methods We retrospectively analyzed the data of 41 patients with palmar hyperhidrosis who underwent bilateral endoscopic sympathicotomy 3 to 6 years ago. Before and after the operation, changes in ipsilateral palm temperature were monitored and recorded to evaluate the curative effect of the sympathicotomy. Results All operations were performed successfully. Concerning cure, there was no statistically significant difference between patients with different maximum temperature (Tmax) values (p = 0.455). There was a very weak correlation between postoperative palm temperature (34.309 ± 1.377°C) (p = 0.049; correlation coefficient − 0.218). The T3 + T4 sympathicotomies had a higher Tmax (p = 0.000). The incidence and degree of CH had no relationship with Tmax in the left (p = 0.266 and p = 0.168, respectively) or the right hand (p = 0.640 and p = 0.824, respectively). Conclusions Temperature change has a relationship with surgery, but it cannot directly predict the long-term curative effect of a sympathicotomy or the occurrence of CH. Additional studies are required.

SARS-CoV-2 genetic material has been detected in raw wastewater around the world throughout the COVID-19 pandemic and has served as a useful tool for monitoring community levels of SARS-CoV-2 infections. SARS-CoV-2 genetic material is highly detectable in a patient feces and the household wastewater for several days before and after a positive COVID-19 qPCR test from throat or sputum samples. Here, we characterize genetic material collected from raw wastewater samples and determine recovery efficiency during a concentration process. We find that pasteurization of raw wastewater samples did not reduce SARS-CoV-2 signal if RNA is extracted immediately after pasteurization. On the contrary, we find that signal decreased by approximately half when RNA was extracted 24-36 hours post-pasteurization and ~90% when freeze-thawed prior to concentration. As a matrix control, we use an engineered enveloped RNA virus. Surprisingly, after concentration, the recovery of SARS-CoV-2 signal is consistently higher than the recovery of the control virus leading us to question the nature of the SARS-CoV-2 genetic material detected in wastewater. We see no significant difference in signal after different 24-hour temperature changes; however, treatment with detergent decreases signal ~100-fold. Furthermore, the density of the samples is comparable to enveloped retrovirus particles, yet, interestingly, when raw wastewater samples were used to inoculate cells, no cytopathic effects were seen indicating that wastewater samples do not contain infectious SARS-CoV-2. Together, this suggests that wastewater contains fully intact enveloped particles.

Abstract The body temperature of 10 clinically healthy green iguanas (Iguana iguana) was measured using a thermographic camera (FLIR E6, Flir Systems Sweden) before and after the food was offered. For each animal there were performed a total of 6 measurements (3 before feeding and 3 after the food was offered). The purpose of this experiment was to observe the thermographic pattern of the body before and after the feeding, since herbivore reptiles tend to bask after the feeding to increase the body temperature that will help them afterwards digest the food. The animals were housed in individual vivariums with every animal having a basking spot available. The pictures were taken outside the vivarium in an adjacent room. The animals were handled with gloves and transported in a cardboard box in order to avoid heat transfer between the handler and the iguana that would have produced thermal artefacts. Each individual was placed on a table on a styrofoam slate, again, to avoid the heat transfer between the table and the animal`s body. For each animal a total of 4 pictures were taken (up, front, left and right). The pictures were analysed with the FLIR Tools program that is provided by the manufacturer and 3 temperatures were taken into consideration (the head temperature, body temperature on the right side and body temperature on the left side). The temperatures were compared between them and with the temperature of the vivariums that consisted of the average between the temperature in 3 different spots (basking spot, the feeding bowl site and the coldest spot) measured with an infrared thermometer GM300 (Benetech, China). The temperature of the body was dependent on the vivarium temperature and it was a significant temperature difference between the measurements before the feeding and after the feeding. Also we discovered a significant difference between the head temperature and the body temperature on the left side before the feeding that disappeared after the animals ate. There was also a significant difference between the temperature on right side and on left side of the animals both before and after the feeding. No significant temperature difference was observed between the head and the right side of the body neither before nor after the feeding.

Abstract Traditionally, the daily average air temperature at a weather station is computed by taking the mean of two values, the maximum temperature (Tmax) and the minimum temperature (Tmin), over a 24-h period. These values form the basis for numerous studies of long-term climatologies (e.g., 30-yr normals) and recent temperature trends and changes. However, many first-order weather stations—such as those at airports—also record hourly temperature data. Using an average of the 24 hourly temperature readings to compute daily average temperature has been shown to provide a more precise and representative estimate of a given day’s temperature. This study assesses the spatial variability of the differences in these two methods of daily temperature averaging [i.e., (Tmax + Tmin)/2; average of 24 hourly temperature values] for 215 first-order weather stations across the conterminous United States (CONUS) over the 30-yr period 1981–2010. A statistically significant difference is shown between the two methods, as well as consistent overestimation of temperature by the traditional method [(Tmax + Tmin)/2], particularly in southern and coastal portions of the CONUS. The explanation for the long-term difference between the two methods is the underlying assumption for the twice-daily method that the diurnal curve of temperature is symmetrical. Moreover, this paper demonstrates a spatially coherent pattern in the difference compared to the most recent part of the temperature record (2001–15). The spatial and temporal differences shown have implications for assessments of the physical factors influencing the diurnal temperature curve, as well as the exact magnitude of contemporary climate change.

AbstractThe objective of this study was to describe current obstetric, neonatal, and long-term neurodevelopmental outcomes of higher order multifetal gestations (≥ 3 fetuses) in the 1990s. We also intended to identify a target gestational age at which neonatal and neurodevelopmental morbidities are low. Records from all multifetal pregnancies (≥ 3 viable fetuses ≥ 20 weeks gestation) delivered at the two perinatal centers in Toronto, Ontario, Canada during the study period (January 1, 1990–December 31, 1996) were reviewed. Data were collected on obstetric, neonatal, and long-term neurodevelopmental outcomes. Follow up data were gathered regarding the presence of a severe deficit in four categories (vision, hearing, cognition, and motor skills). Statistical analysis was performed to determine a gestational age at which a significant decrease in deficit occurred. During the study period 165 multifetal pregnancies were delivered. This resulted in 511 fetuses, of which 496 were live births. Of these 496 infants, 453 survived to discharge. Follow up data were obtained on 332 (73.3 per cent) infants. Infant survival increased with gestational age, and was approximately 90 per cent or greater at 26 weeks or more. Of all infants followed, the proportion of those without deficit increased with increasing gestational age, such that the per cent without deficit was 96.9 at 31 weeks or greater. Of all infants followed, 301 (90.7 per cent) had no deficit. Statistical analysis revealed a significant difference in long-term neurodevelopmental outcome between infants born before and after 28 weeks gestation. The incidence of a major deficit was 44.1 per cent for those born earlier than and 5.4 per cent for those born later than this gestational age (p = 0.001). In our cohort, survival figures were high. Even in lower gestational groupings, survival was high, but not without serious concerns about severe morbidity. This information is useful when counseling parents of higher order multifetal pregnancies.

Background: Diabetic retinopathy (DR) is a visual impairment-related eye disease developed by long-term hyperglycemic status. Diabetic condition in DR patients leads to diabetic organopathies (e.g., renal failure). Albuminuria, as a hallmark of renal failure, can be correlated with visual indicators in DR patients. Objectives: This study aimed to investigate the role of albuminuria status in visual acuity (VA) and bevacizumab therapy outcomes in DR patients. Methods: In this retrospective study, 48 DR patients were admitted to the Ophthalmology Center of Ayatollah Rouhani Hospital, affiliated with Babol University of Medical Sciences, Babol, Iran. The retinopathy status and VA were identified before and after treatment through 45 days of bevacizumab therapy. In addition, fast blood sugar, hemoglobin A1c, urine albumin, and urine creatinine were evaluated using standard laboratory methods. Results: The VA value before treatment in microalbuminuric DR patients (0.106 ± 0.036) was significantly lower than non-microalbuminuric DR patients (0.347 ± 0.286; P < 0.001). Furthermore, VA value after treatment in microalbuminuric DR patients (0.115 ± 0.071) was significantly lower than non-microalbuminuric DR patients (0.355 ± 0.272; P < 0.001). There was no significant difference in the percentage of VA increase between microalbuminuric and non-microalbuminuric patients. Moreover, the albumin-to-creatinine ratio (ACR) was correlated with a lower VA level before and after treatment (P < 0.001 for both). There was no correlation between the percentage of VA increase with ACR, albumin, and creatinine. Conclusions: The current study results showed that different VA before and after bevacizumab therapy status was correlated with microalbuminuria status. Additionally, microalbuminuria status did not affect the percentage of VA increase in the treatment of DR patients.

Abstract Background: The objective of our study was to compare clinical outcome and postoperative complications between patients with thoracic myelopathy caused by ossification of the ligamentum flavum (OLF) treated with and without intraoperative methylprednisolone (MP).Methods: This retrospective study enrolled 101 patients who underwent posterior approach surgery for OLF and were followed up at least 1 year. Patients were divided into two groups according to MP use in the operation: MP group (n=47) and non-MP group (n=54). Clinical outcomes and complications were evaluated before and after operation and at the last follow-up. Results: Significant differences were found in modified Japanese Orthopedics Association (mJOA) scores and proportion of Frankel grade (A-C) between the two groups immediately after surgery and at 2-week follow-up. No significant differences were found between the two groups in mJOA score before operation and at the final follow-up. Moreover, no significant differences were observed in recovery rate according to mJOA score at any time points, and there was no significant difference in the proportion of Frankel grade (A-C) between the two groups at final follow-up. There were 13 documented infections: 10 in the MP group and 3 in the non-MP group (P=0.034).Conclusion: Management therapy with intraoperative 500 mg MP showed better recovery of nerve function within 2 weeks in patients with thoracic myelopathy caused by OLF compared with those did not receive MP. However, long-term follow-up results showed that there was no significant difference in neurological recovery between patients with intraoperative MP or not. Moreover, intraoperative MP increased the rate of wound infection.

Abstract Introduction Transcatheter aortic valve implantation (TAVI) is the method of choice for patient with a significant stenosis (AS) who are contraindicated for cardio-surgical procedure. Medium – term results of TAVI are comparable with results observed for surgical replacements. There is limited knowledge of structural, nonstructural and clinical complications in five years follow-up which could contribute to TAVI indication for implantation in young patients. Goal. The aim of this study is to assess structural and non-structural deterioration of TAVI prosthesis by echocardiography and clinical evaluation at 5 years follow-up. Methods By the end of 2013, 112 patients were implanted TAVI in our center( average age, 80,4 ± 7,2 y, 53(47% ) males. All patients underwent both clinical and echocardiographic examination once a year and underwent native CT after five year. We analysed mortality, structural and non- structural deteriorations, and standard echocardiographic parameters before and after implantation, and at 5 years follow-up. Results 5-years mortality was 58%. There was not a statistically significant difference in age between the patients who survive and who died (80,2 ±7,3 vs 80,6 ± 7,3 p = 0.77) ,they had similar logistic EUROSCORE (17,0 ± 12,4 vs 16,9 ± 15,0, p = 0,96) and EF 53,8 ± 11,1 vs 52,3 ± 11,7,p= 0,51) before the procedure. 48 patients underwent the five years follow –up. Only one patient had to undergo reTAVI due to the earlier prosthetic degeneration. The average EOA was 1,88 ± 0,33 cm2), the average V max. after five years was 1,8 ± 0,4 m/s, PG max 13,6 ± 6,2 mm Hg a PG mean 8 ± 3,6 mm Hg ( no significant increase in time),Post procedural paravalvular regurgitation ≥ 2/4 appeared in 14 % patients. Twenty –three patients underwent CT scan which resulted in Agaston score O on TAVI replacement in 19(83 %) patients. Conclusion After 5 years follow-up, low rates of structural and nonstructural deterioration after TAVI were noted, suggesting good long- term durability.

BACKGROUND: In postmenopausal women there is a rapid destruction of dermal collagen, resulting in accelerated skin ageing, which is manifested by cutaneous atrophy, increased number and depth of wrinkles and sagging. This accelerated catabolism of the collagen is due to estrogen deficiency and increased synthesis of the metalloproteinase-1 enzyme, which degrades the dermal collagen. OBJECTIVES: To assess whether the use of topical estradiol 0.05% cream on photo exposed skin can inhibit the expression of the metalloproteinase-1 enzyme on the dermis and subsequently the rapid loss of collagen in women after menopause. METHODS: We included 40 postmenopausal women without hormone replacement therapy. Information about lifestyle, lipid profile, blood glucose level, thyroid hormones, mammography, Pap smear and transvaginal ultrasound were obtained to rule out associated diseases. Skin biopsy of the right preauricular region was performed before and after treatment with topical estradiol 0.05% for 30 days. The biopsy specimens were subjected to immunohistochemistry to identify the expression of the metalloproteinase-1 enzyme. RESULTS: There was no statistically significant difference on the expression of the metalloproteinase-1 enzyme in keratinocytes, fibroblasts and endothelial cells before and after treatment with topical estradiol for 30 days. CONCLUSION: Treatment with estradiol 0.05% cream, in photo exposed skin for 30 days, does not inhibit the production of metalloproteinase-1.

This study investigated whether hyperthermia within the first 24 h after presentation was associated with long-term neurological outcomes after acute carbon monoxide (CO) poisoning. This retrospective study included 200 patients with acute severe CO poisoning. Hyperthermia (≥ 37.5°C) developed during the first 24 h after presentation in 55 (27.5%) patients, and poor long-term neurological sequelae assessed at 23 months after acute CO poisoning developed in 19.5% of the patients. The incidence of poor long-term neurological outcomes was significantly higher in the hyperthermia group than in the normothermia group. Patients with poor long-term neurological outcomes had higher maximum temperatures than patients with good outcomes. No significant difference was found in the time of hyperthermia onset within the first day according to the neurological outcomes. Hyperthermia (adjusted odds ratio (aOR) 5.009 (95% confidence interval (CI) 1.556–16.126)) and maximum temperature (aOR 2.581 (95% CI 1.098–6.063)) within the first 24 h after presentation to the emergency department were independently associated with poor long-term neurological outcomes. Body temperature measurements, which are easily and noninvasively recorded at the bedside in any facility, help to predict the risk for poor long-term neurological outcomes. This study carefully emphasizes fastidious control of pyrexia, particularly during the early period after acute CO poisoning.
