Abstract Background Early detection of dysplastic changes within oral potentially malignant disorders is the mainstay to prevent oral cancer. Ki-67 is one of the most useful antigens in this purpose. Aims The study aims were to recognize and mutually compare the proliferative status of idiopathic oral leukoplakia (OL) patches, which presented through different forms of dysplasia and carcinoma. Settings and Design In 4 years of observation, cumulatively 140 OL lesions were included for examination. The wholesome Ki-67 labeling scores in each of the subgroups were calculated. Subjects and Methods The World Health Organization recommended histopathological classification was used to categorize the dysplastic and malignant lesions. Paraffin-embedded tissue sections were processed for Ki-67 immunostaining. The labeling indices (LIs) were quantified semiquantitatively at the site of maximal reactive cells on tissue sections. Statistical Analysis The statistical comparison was performed by means of the SPSS software (Version 16.0 SPSS Inc.). A p-value < 0.05 was considered as the benchmark for statistical significance. Results A steady and significant increment in Ki-67 expression was discovered from dysplastic to malignant OL patches compared with normal mucosa. The labeling differences were significant between normal mucosa and mild dysplasia, as well as between mild, moderate, and severe dysplasia. However, the expression did not differ significantly with the severity of oral cancers. Conclusions Ki-67 is a useful molecular marker of carcinogenesis in OL. It also serves worthwhile in separating marginally dysplastic lesions, such as mild dysplasia or verrucous carcinoma from their benign epigones.

The aim of the study was to define the distribution of p53, bcl-2 and Ki-67 proteins in the inflammatory-regenerative and dysplastic lesionsof the colon mucosa. The relationship between the presentation of p53, bcl-2 and Ki-67 proteins and the intensity of the inflammatory-regenerative and dysplastic lesions in the colon flat mucosa was investigated as well. Biopsy specimens from 270 patients were examined: 74 were classified as inflammatory-regenerative and 196 as dysplastic lesions (108 mild, 58 moderate, and 30 severe dysplasia). The expression of all three proteins was assessed on the basis of location, quantity, and intensity of immunostaining, by counting antigen positive cells, in comparison with normal mucosa and adenocarcinoma. p53 protein appears only in sporadic cases (6.6%) of severe dysplasia. Bcl-2 expression appears significantly (p<0.005) more often in cases of mild dysplasia (61.1%) compared to inflammatory-regenerative mucosa (14.8%). In cases of mild dysplasia, bcl-2 positive cells were spreading from the lower third to the middle third of the crypts. Bcl-2 expression was maintained through the stadiums of moderate and severe dysplasia (75.8%), where antigen positive cells were found all along the crypts. A significant increase (p<0.005) in the expression of nuclear protein Ki-67 was noticed in the stadiums of moderate (labelling index =26.3) compared to mild dysplasia (labelling index=16.7), and severe (labelling index=36.7) compared to moderate dysplasia, where the zone of cellular proliferation was widen along the whole crypt length. In the process of the development of epithelial dysplasia in the flat mucosa of colon a degree of the gene p53 alteration is low and appears only in sporadic cases of severe dysplasia. Mutation of the bcl-2 gene is involved in the genesis of the lesion but not in its progression to carcinoma. Increased expressionof Ki-67 protein speaks in favour of an increased cellular proliferation which, together with the above mentioned mechanisms, is involved in the process of occurrence and progression of epithelial dysplasia in the flat mucosa of colon.

The aim of this research is to establish by immunohistochemistry if there is a change in the expression of collagen type IV, as a substitute of basement membrane, in development of epithelial dysplasia in chronically inflamed colon mucosa.Methods. Biopsy specimens from 270 patients were examined: 74 were classified as inflammatory-regenerative and 196 as dysplastic lesions. There were 108 cases of mild dysplasia, 58 cases of moderate and 30 cases severe dysplasia, respectively. Visualisation of collagen IV and its way of expression within basement membrane of glandular crypts was performed by immunohistochemistry and then compared with findings in normal colon mucosa and colon adenocarcinoma tissue.Results. Changes in the expression of collagen IV comprised of its focal irregularities, diffuse thinning and/or thickening, focal interruptions or its complete absence. Significant changes in the expression of collagen IV in relation to normal mucosa already occur in inflammatory-regenerative mucosa. In mild dysplasia, these changes are more intensive in relation to those in inflammatory altered mucosa as well as at severe dysplasia in relation to moderate dysplasia. Changes in the expression of collagen IV in severe dysplasia are significantly more serious than in moderate dysplasia but are identical to those in colon adenocarcinoma tissue.Conclusion. These findings suggest that change in the expression of collagen IV is in correlation to a degree of epithelial dysplasia that developed in flat chronically inflamed colon mucosa.

Introduction: There is a strong relationship between malnutrition and increased length of hospitalization and morbidity and mortality. Studies have shown that malnourished patients can have up to twenty times more complications than eutrophic ones. In critically ill patients, there is a tendency to catabolism, resulting in the loss of lean body mass, which when it reaches 40% is usually lethal. Methods: A quantitative, descriptive study was conducted on adults from both genders, admitted to the intensive care unit (ICU) of a university hospital, from March to December 2018. The following variables were collected from the evaluation and nutritional records: length of hospitalization in the ICU, date of discharge or death, nutritional risk through specific screening, height, weight and arm circumference (AC). For the screening, the Nutric score was used. For the nutritional evaluation, the body mass index (BMI) and AC indicators and the classifications recommended by the World Health Organization (2004) and Blackburn and Thornton (1979) were used. After collecting the data, they were analyzed in the Statistical Package for Social Sciences (SPSS) 13.0 and for the association of the variables the Chi-square test was used, considering statistical difference when the p value <0.05. Results: The sample consisted of 116 patients, mostly female (53.4%) whose median age was 46 years (interquartile range IQR 31-53). Regarding the frequency of nutritional risk, most patients (61.5%) had a low score. There was an important frequency of malnutrition, according to the AC indicator (73%), although BMI (43.5%) showed eutrophy. Even though most patients had low nutritional risk, those with high nutritional risk (38.5%) had a higher tendency to mortality, however, not statistically confirmed (p> 0.05). There was also a tendency of association between death and malnutrition, although no statistical significance was shown(p> 0.05). Conclusion: Patients at nutritional risk and/or malnutrition appear to be vulnerable to worse clinical outcomes.

The odontogenic keratocyst (OKC) well-known for its aggressiveness and high recurrence rate, comprises approximately 11% of all jaw cysts. Due to its aggressive behavior it was placed into category of tumour in 2005 by the World Health Organization (WHO). Objectives: The purpose of this study was to determine the Ki-67 expression in Odontogenic Keratocysts to predict its proliferative potential. Study Design: Descriptive study. Setting: Department of Morbid Anatomy and Histopathology, UHS. Periods: June 2014- June 2018. Material & Methods: This is a descriptive study comprising of 39 cases of odontogenic cysts. These surgically removed samples were processed at University of Health Sciences (UHS) laboratory. Routine staining with Hematoxylin & Eosin stain along with immunohistochemistry (IHC) with Ki-67 antibody was performed. Immunohisto chemical scoring was done on the basis of percentage of the nuclear staining of Ki-67. Data was entered into SPSS 22 and descriptive statistics were measured in the form of percentage and frequency. Quantitative variables such as age of patient, size of the cyst, and Ki-67 score were also measured. P value <0.05 was taken as significant. Results: The mean age of the patients was 25.08 ±14.5 years. Significant association was observed between histological variables with odontogenic keratocyst such as parakeratinized epithelial lining (p = 0.00), epithelial hyperplasia both typical and atypical (p = 0.02) and focal spongiosis (p = 0.04). Foci having epithelial atypia demonstrated stronger staining intensity compared to adjacent normal epithelium. However, no significant association was observed between the histological variables and Ki-67 expression. Conclusion: OKC expressed low Ki-67 expression in most of the cases, however, foci of strong expression were also observed in few cases.

Background: Childhood under-nutrition is a major global health problem. Although the rate of under-nutrition in southern Ethiopia has substantially declined in the last decade, but it still remains the major causes of morbidity and mortality of children under-five years. Unfortunately, there was no study in this study area with respect to this topic of interest and therefore, this study was carried out to assess the magnitude of underweight and associated factors among children aged 6-59 months. Methods: A community based cross sectional study was conducted in Wonsho Woreda, Southern Ethiopia. A total of 595 randomly selected child mother pairs were selected using cluster sampling method. Data were collected using a face-to-face interview and children anthropometric measurements. Child Dietary Diversity Score (DDS) was determined. World Health Organization Anthro software was used to convert anthropometric measurements into Z-scores. The data was analyzed using Epidata version 3.1 and SPSS version 20. Bivariate and multivariable logistic regression model was used. A statistical significance was declared at p-value less than 0.05. Result: The overall prevalence of underweight was 122(20.5%) (95% CI, 17.3-23.8%), meanwhile, the prevalence of severe and moderate underweight was 7.1% and 13.4% respectively. Male children were 1.78 times more likely to be underweight than female children (AOR=1.78; 95%CI=1.17, 2.70). Unimmunized, children were 2.45 times more likely to be underweight (AOR=2.45; 95%CI=1.41, 4.24). Conclusion: Prevalence of nutritional underweight was high in the study area. Driving factors of underweight were investigated and therefore, strong stakeholders’ collaboration is compulsory to address the future public health burden.

Abstract IntroductionThe unprecedented global increase in non-communicable diseases (NCDs) death incidences resulted in a joint initiative by the United Nations (UN) and the World Health Organization (WHO) to reduce NCD-related mortality by 25% by the year 2025. In this study, we ascertained the prevalence of NCDs in South Africa and its associated factors. MethodWe used 5-panel waves secondary datasets conducted by national income dynamic survey (NIDS) from 2008 to 2017 among 64,735 South Africans. We calculated the prevalence of all selected NCDs separately and together for each year at the descriptive level. This was followed by the distribution of selected NCDs across the socio-demographic and behavioural characteristics of the respondents. Finally, we used binary logistic regression to assess NCDs' likelihood by the respondents’ socio-demographic and behavioural characteristics. The results were presented as crude odds ratios (cOR) and adjusted odds ratios (aOR) with their corresponding 95% confidence intervals, signifying precision. Statistical significance was set at p-value < 0.05.ResultsWe found that, the prevalence of NCDs in South Africa was highest in the year 2012 (23.8%) and lowest in the year 2015 (15.2%). Over the 10-year period, the NCDs with highest and lowest prevalence were high blood pressure and cancer, respectively. Females [cOR = 1.88, 95% CI=1.79-1.96], smokers [cOR=2.01, 95% CI=1.85-2.19] and those very satisfied with life [cOR=1.10, 95% CI=1.04-1.17] were more likely to have atleast one NCDs. This persisted after adjusting for significant covariates: [aOR = 1.84, 95% CI=1.74-1.94], [aOR=1.49, 95% CI=1.35-1.65] and [aOR = 1.01, 95% CI=0.95-1.09] respectively.ConclusionSouth Africa’s department of health and other health agencies need to strengthen existing policies and develop new interventional frameworks that will deliberately consider various significant factors contributing to the high prevalence of NCDs identified in this study. This will reduce the NCDs prevalence and reduce the morbidity and mortality levels attributable to NCDs in the country.

Abstract This study aimed to examine the associations of sedentary time and cardiometabolic risk markers among working adults in Eastern Ethiopia. A cross-sectional study was conducted among 1,200 participants. Data were collected using the World Health Organization NCD STEPS survey instrument, and the sedentary behavior questionnaire. The biochemical parameters were analyzed by using the Mindray BS-200 chemistry analyzer. STATA version 16.1 software was used for analysis. The associations between sedentary time and cardiometabolic risk markers controlling confounders were examined using linear regression models. An adjusted coefficient (β) with the 95% confidence interval (CI) was used to report the results. P-value < 0.05 was considered for statistical significance. One hour per day increases in total sedentary time increases the average body mass index (β = 0.61kg/m2: 95% CI: 0.49, 0.71), waist circumference (β = 1.48cm: 95% CI:1.14-1.82), diastolic blood pressure (β = 0.87mmHg: 95% CI: 0.56-1.18), systolic blood pressure (β = 0.95mmHg: 95% CI: 0.45, 1.48), triglycerides (β = 7.07mg/dl: 95% CI: 4.01-10.14), total cholesterol (β = 3.52mg/dl: 95% CI: 2.02-5.02), fasting plasma glucose (β = 4.15mg/dl: 95% CI: 5.31-4.98) and low-density lipoprotein cholesterol (β = 2.14mg/dl: 95% CI: 0.96-3.33). Long sedentary time is significantly associated with cardiometabolic risk markers. Interventions to reduce sedentary time to decreasing the risk of cardiovascular diseases among working adults.

Objectives: The aim of the article was to evaluate Ki-67 expression in oral leukoplakia in snuff users and non-users. Study Design: Descriptive Cross sectional study. Setting: At different hospitals of Khyber Pakhtunkhwa province. Period: From August 2016 –March 2017. Material & Methods: Clinically diagnosed and histopathologically confirmed cases of oral leukoplakia in snuff users and non-users 30 cases each were immunohistochemically examined and percentage expression of nuclear Ki-67 was evaluated by a semi quantitative method in basal, parabasal and suprabasal layers of the lining epithelium. Results: The relationship of expression of Ki-67 in parabasal and suprabasal layers of affected epithelium in snuff users was found to be statistically significant with a p value of 0.007 and 0.002 respectively. Conclusion: The present study concludes that the proliferative index is high in snuff users as compared to non-users which can be a factor for malignant transformation.

Background Scientists across the world are working on innovating a successful vaccine that will save lives and end COVID-19 pandemic. World Health Organization (WHO) is working to make sure COVID-19 vaccines can be safely delivered to all those who need them. Indeed, the successful deployment and a sufficient uptake of vaccines is equally important. Acceptance and accessibility of such vaccine is a key indicator of vaccination coverage. Objective This study aimed to assess the determinants of intention to receive COVID-19 vaccine among school teachers in Gondar City. Methods An institution based cross-sectional study was conducted from December, 2020 to January, 2021. A total of 301 school teachers selected using stratified simple random sampling were included. Descriptive analysis such as medians, means, proportions, standard deviations and frequencies were computed. Linear regression analysis was done to identify factors associated with intention to receive COVID-19 vaccine. A p-value of less than 0.05 was used to declare statistical significance. Results The median intention to receive COVID-19 vaccine was 3.33 with interquartile range of 2.67–4.0. Of the participants 54.8% had scored above the median of intention to receive COVID-19 vaccine score. 54% variance in intention to receive COVID-19 vaccine was explained by the independent variables. Being affiliated with other category of religion, bachelor degree educational status, perceived susceptibility, perceived benefit, perceived barrier, and cues to action were significantly associated with the intention to receive COVID-19 vaccine. Conclusion The median score of intention to receive COVID-19 vaccine was 3.33. Socio-demographic and health beliefs influenced the intention to receive the COVID-19 vaccine in the study participant. Policy makers and stakeholders should focus on strong health promotion about risks of the pandemic, benefit, safety, and efficacy of vaccination.
