Sexual and reproductive health needs are global health, development and human rights priorities, essential to the wellbeing of individuals, couples and families. Despite widespread recognition of this, young people, including those from culturally diverse backgrounds in wealthy nations such as Australia, remain largely hidden to, and underserved by, sexual and reproductive healthcare services. A scoping review was undertaken to describe the range of research on sexual and reproductive health among culturally diverse young people in Australia, in order to identify gaps in the literature. This comprised a systematic literature search and key informant consultations, followed by descriptive thematic analysis of the included literature. One hundred and twenty papers were deemed eligible for inclusion, and findings are presented as three separate analyses. The first provides an overview of four dimensions identified in the literature: (i) cultural diversity; (ii) the resettlement experiences of migrant communities; (iii) the sexual and reproductive health needs of these communities; and (iv) the practices of health services in this field. The second explores what is known from the literature about culturally diverse young people’s needs for, use of, and engagement with services for sexual and reproductive health, while the third identifies two knowledge gaps: (1) the perspectives of culturally diverse young people regarding sexual and reproductive health and health care; and (2) the engagement of culturally diverse young people with sexual and reproductive health services. New directions for a research agenda on sexual and reproductive health care for culturally diverse young people in Australia are proposed, based on the identified knowledge gaps.

Objective: Internet-based health promotion offers a promising approach to fostering healthy sexual behaviour among adolescents. The objective of this study was to identify the potential of an Internet-based programme to improve sexual and reproductive health (SRH) knowledge among Malaysian young people. Design: A pre-test/post-test design was used to study the potential of the Malaysian Care for Adolescent Project (MyCAP) website for online SRH education. Created by health professionals, the SRH module provides detailed and accurate information about the male and female reproductive systems, the stages of adolescent development, puberty (bodily development, menstruation and wet dreams), teenage pregnancy, sexually transmitted infections (STIs) and social situations such as avoiding premarital sex. Setting: A total of 209 young adolescents aged 12 years were recruited from a school in Lembah Pantai, Kuala Lumpur. Method: Students were randomly assigned to either an Internet-based or a conventional method group. The Internet-based intervention group consisted of 101 young people (50 boys and 51 girls), while the conventional method group consisted of 108 participants (62 boys and 46 girls). Results: Using the Internet-based method, there was an increase of 3.88 in the mean knowledge score of participants from pre- to post-intervention. The Internet-based method had a greater eta-squared score of 0.59 compared to the conventional method, which had an eta-squared score of 0.41 ( p < .001). Conclusion: The results revealed that the Internet-based method significantly improved adolescents’ knowledge of SRH. Since the majority of students have access to the Internet, using online learning could provide an alternative educational method for teaching SRH.

The effectiveness of sexual behaviour change interventions in sexual health clinics is unknown. Risk factors for poor sexual and reproductive health such as depression, violence, alcohol and smoking in sexual health clinics are all common and can be identified easily in sexual health services. Targeting these risk factors could be as effective as traditional sexual health promotion and could have additional benefits. The authors propose a pilot to assess the cost-effectiveness and acceptability of incorporating screening and interventions for these risk factors.

Both significant progress and profound backlash have occurred in the inclusion of sexual and gender diversity across eastern and southern Africa. This includes the decriminalization of homosexuality in Mozambique in 2015 and the introduction of the Anti-Homosexuality Act (later annulled) in Uganda in the preceding year. Simultaneously there is increased pressure on Ministries of Education to engage more robustly with sexual and reproductive health and rights (SRHR) education in education systems across the region. Emerging regional research points to a narrow, heteronormative focus in comprehensive sexuality education; access barriers to sexual and reproductive health services; and pervasive school-related gender-based violence, including homophobic and transphobic violence. Civil society organizations (CSOs) play a key role in developing best practice in advancing the SRHR of sexual and gender minority youth and are therefore a valuable resource for government SRHR policies and programmatic responses. The regional SRHR education policy landscape is underpinned by two policy narratives: that of young people’s SRHR as a public health concern and a focus on young people’s human rights. These policy narratives not only underpin SRHR policy in the region but also in many instances are drawn on in CSO advocacy when positioning the SRHR of lesbian, gay, bisexual, transgender, queer, and intersex (LGBTQI) young people as an important policy concern. These two dominant policy narratives, however, have a narrow focus on young people’s risks and vulnerabilities, may inadvertently perpetuate stigma and marginalization of LGBTQI youth, and may limit youth voice and agency. These narratives also do not sufficiently engage local sociocultural and structural conditions that drive negative SRHR outcomes for young people in the region. Research, advocacy, and policy development toward the full realization of the SRHR of sexual and gender minority youth can address some of the limitations of health and rights-based policy narratives by drawing on a sexual and reproductive justice framework. Such a framework expands the policy focus on health risks and individual rights to include engagement with sociocultural and structural constraints on young people’s ability to exercise their rights. A sexual and reproductive justice framework provides a more robust toolkit when working toward full inclusion of sexual and gender diversity in regional school-based SRHR policy and programs.
