Paraneoplastic myelopathies are uncommon, but maintenance of a high index of clinical suspicion is critical, as the neurological syndrome often predates discovery of an underlying malignancy and can facilitate early detection and potentially improve prognosis. Most paraneoplastic myelopathies are associated with additional neurological symptomatology, such as encephalitis, peripheral neuropathy, or cerebellar ataxia, due to the specific, respective auto-antibodies involved. Anti-Hu antibody, identified in our patient, appears to be the most common antibody, followed by amphiphysin, and collapsing response mediator protein 5, among others. The therapeutic approach to patients with paraneoplastic myelopathies focuses primarily on treatment of the underlying cancer and, thereafter, subsequent trials of immunotherapies, which have demonstrated variable response.

Objective. Examine the effectiveness of the 128 Hz tuning fork, two monofilaments, and Norfolk Quality of Life Diabetic Neuropathy (QOL-DN) questionnaire as tools for the early detection of diabetic peripheral neuropathy (DPN) in overweight, obese, and inactive (OOI) adults or those who have prediabetes (PD) or type 2 diabetes (T2D). Research Design and Methods. Thirty-four adults (mean age 58.4 years ± 12.1) were divided by glycemia (10 OOI normoglycemic, 13 PD, and 11 T2D). Sural nerves were tested bilaterally with the NC-stat DPNCheck to determine sural nerve amplitude potential (SNAP) and sural nerve conduction velocity (SNCV). All other testing results were compared to SNAP and SNCV. Results. Total 1 g monofilament scores significantly correlated with SNAP values and yielded the highest sensitivity and specificity combinations of tested measures. Total QOL-DN scores negatively correlated with SNAP values, as did QOL-DN symptoms. QOL-DN activities of daily living correlated with the right SNAP, and the QOL-DN small fiber subscore correlated with SNCV. Conclusions. The 1 g monofilament and total QOL-DN are effective, low-cost tools for the early detection of DPN in OOI, PD, and T2D adults. The 128 Hz tuning fork and 10 g monofilament may assist DPN screening as a tandem, but not primary, early DPN detection screening tools.

Aspergillus can cause devastating opportunistic infections in immunocompromised patients. Rarely does this fungus invade the heart, and when it does, survival is especially poor despite optimal medical and surgical treatment. We report a case of cardiac aspergilloma with involvement of the tricuspid valve and both the right atrium and ventricle found on a transthoracic echocardiogram in an immunocompromised patient after developing atrial fibrillation with rapid ventricular rate. The findings from this case suggest that early clinical suspicion is critical in early diagnosis and thus early treatment.

A de Garengeot hernia is defined as an incarcerated femoral hernia containing the vermiform appendix. We describe the case of a patient with a type 4 appendiceal diverticulum within a de Garengeot hernia and delineate valuable learning points.A 76-year-old woman presented with a 2-week history of a non-reducible painless femoral mass. Outpatient ultrasonography demonstrated a 36mm × 20mm smooth walled, multiloculated, partially cystic lesion anterior to the right inguinal ligament in keeping with an incarcerated femoral hernia. Intraoperatively, the appendix was found to be incarcerated in the sac of the femoral hernia and appendicectomy was performed. Histopathology demonstrated no evidence of inflammation in the appendix. However, an incidental appendiceal diverticulum was identified.It is widely recognised that a de Garengeot hernia may present with concomitant appendicitis, secondary to raised intraluminal pressure in the incarcerated appendix. Appendiceal diverticulosis is also believed to develop in response to raised pressure in the appendix and may therefore develop secondary to incarceration in a de Garengeot hernia. To our knowledge, only one such case has been described in the literature. A de Garengeot hernia is a rare entity, which poses significant diagnostic challenges. A high index of clinical suspicion is necessary as these hernias are at particularly high risk of perforation and so prompt surgical management is paramount.

Melioidosis, a disease with protean clinical manifestations, is prevalent in many parts of India, with established endemic hotspots on the southern coast of the country. However, it is still underdiagnosed in many resource-poor regions of the country. We report what is, to the best of our knowledge, the first case of melioidosis diagnosed and treated in Bihar, an economically underdeveloped state in East India. The patient, a 52-year-old diabetic male, presented to the outpatient department with a fever of insidious onset along with pain and restriction of movement in the right shoulder joint and right knee joint, and swelling and tenderness of bilateral ankle joints. Radiological features were suggestive of multiple joint and organ abscesses. A diagnosis of disseminated septicaemic melioidosis was confirmed microbiologically. The patient improved clinically following aggressive treatment with meropenem and cotrimoxazole. The case highlights the need for increased clinical suspicion of melioidosis and adequate diagnostic facilities, as well as the need for early institution of appropriate empirical antibiotics in suspected cases of melioidosis in this region of the world.

Case report of a 95-year-old female patient that was admitted to the emergency room with a sudden weakness on the right who underwent propaedeutic imaging with cerebral perfusion study by CT using artificial intelligence (AI) software for clinical suspicion of acute stroke. The case illustrates a frequent and specific imaging finding for stroke and its disappearance in the control exam even without optimized treatment.

We describe a case of a 52-year-old male presenting with severe mucopurulent conjunctivitis of the right eye. Corneal ulceration and associated anterior chamber activity was noted later in the course of the disease. Neisseria gonorrhoeae was positive on polymerase chain reaction (PCR) testing earlier than traditional microscopy and culture. He was successfully treated with ceftriaxone 500 mg intravenously and azithromycin 1 g orally as single doses in addition to ofloxacin ophthalmic solution 0.3% hourly to the right eye. This case highlights the need to consider the possibility of gonococcus in cases of suspected bacterial conjunctivitis, careful monitoring for corneal involvement and the importance of early detection with PCR.

Introduction: Proptosis and enophthalmos are cardinal signs of many orbito-ocular and systemic diseases. The need for an imaging parameter that will aid its early detection is necessary, as visual compromise is a major consequence if they are not diagnosed and managed early. Methodology: This prospective study was performed in a tertiary center. Data were collected over the period of January to April 2021 with total of 300 normal ocular globes. The measurement was done at the level of lens (midglobe section) on T2-weighted axial image. Results: The distance between the anterior margin and inter zygomatic line of the right ocular globe was 16.95 ± 1.48 mm (ranged 14.2 - 20.5 mm) and that of left was 16.86 ± 1.38 mm (ranged 14.2 - 20.3 mm). The position of the globes showed no statistically significant differences among gender groups in our study and the position of the right globe within the orbit was significantly different from that of the left orbit. Conclusion: The position of the globes showed higher values in males than in female. However, a statistically significant difference in globe position was observed between right and left orbits. The distance between the posterior margin of the globe and the inter zygomatic line was found to be lower among the Nepalese population compared with other population.

Research in agriculture is increasing quality and quantity, but pest reduces it. To prevent the effect of these pests, insecticides are used. But excessive use of pesticides is very harmful to production and environment. So initially pest detection is necessary. We work on nocturnal pests because that can be easily attracting using night trapping tools. The purpose of this review article is to analyse the popular techniques and find the right technique for the initial diagnosis and early detection of major nocturnal flying pests like Pink Bollworm, White Grub, Helicoverpa and Spodoptera. The importance of early detection can be in identifying and classifying the pests in a digital view. We have concluded our results with the various methods and the prospects of future research.

Abstract. The aim of this study was to determine the effect of metal-tipped, plastic-tipped, and electronic ear tags on the likelihood of infection using infrared thermography and to evaluate the effect on welfare by examining the behavioral responses of the lambs. Ninety lambs with an average age of 6 months were used in this study. Before the application of the ear tags, infrared images of the right and left ears of the lambs were taken with the thermal camera and the infrared temperatures were recorded. Measurements were repeated on days 1, 4, 10, and 30 after the ear tag application. The mean of infrared ear temperatures on days 0, 1, 4, 10, and 30 were found to be 14.60, 23.20, 23.77, 20.36, and 17.61 °C for the metal-tipped ear tag group; 15.69, 21.50, 20.42, 18.00, and 16.39 °C for the plastic-tipped ear tag group; and 15.17, 29.47, 27.49, 25.93, and 20.61 °C for the electronic ear tag group. The infrared temperatures on days 1, 4, and 10 in the control and experimental groups were significantly different from each other (P < 0.001). The plastic-tipped ear tags caused fewer problems and are preferred to the traditional ear tags with metal tips. The electronic ear tags caused more inflammatory reactions compared to metal- or plastic-tipped ear tags. The infrared thermography method may be used as a useful tool in the early detection of the inflammatory reactions and infections caused by the application of ear tags.
