Convergence Mental Health

The world is in the throes of a global health, economic, and mental health crisis with severe physical, societal, and economic ramifications. Modern mental health problems are characterized by their complexity, multisystemic nature, and broad societal impact, making them poorly suited to siloed approaches of thinking and innovation. To solve the unprecedented complexities and challenges associated with the current global crisis, a paradigm shift is needed. Convergence science integrates knowledge, tools, and thought strategies from various fields and is the focal point where novel insights arise. In the context of mental health, convergence involves integration of scientists, clinicians, bioinformaticists, global health experts, engineers, technology entrepreneurs, medical educators, caregivers, and patients; synergy between government, academia, and industry is also vital. A convergence mental health approach will lead to improved outcomes for patients and healthcare systems. Predicate examples of convergence science in adjacent fields to mental health provide a model for the path forward. Further, within the field of mental health, there are examples of convergence science currently in action that include early-stage companies, neuroscience initiatives, public health projects, and unconventional funding mechanisms. The world has a historic opportunity to leverage convergence science to lead to a new era of innovation and progress in global mental health. Contributions for this book come from authors affiliated with the Milken Institute, Asia Pacific Economic Cooperation, Organization for Economic Co-operation and Development, the National Academies of Science, Medicine and Engineering, Stanford University, and Harvard University. This book is written for practitioners and leaders in mental health innovation, including clinicians, researchers, policymakers, investors, entrepreneurs, and philanthropists.

Background: In response to the potentially concurrent mental health crisis due to the COVID-19 outbreak, there have been ongoing mental health policies put in place in China. This review aims to systematically synthesize the implemented national-level mental health policies released by the Chinese government during the COVID-19 outbreak, and summarize the implementation of those mental health policies.Methods: Six databases and two websites were systematically searched, including published studies and gray literature published between December 1, 2019 and October 29, 2020.Results: A total of 40 studies were included. Among them, 19 were national-level policies on mental health released by the Chinese government, and 21 studies reported data on the implementation of those mental health policies. Mental health policies were issued for COVID-19 patients, suspected cases, medical staff, the general population, patients with mental illness, and mental institutions. In the early stage of the COVID-19 epidemic, attention was paid to psychological crisis intervention. In the later stage of the epidemic, the government focused mainly on psychological rehabilitation. During the COVID-19 outbreak, more than 500 psychiatrists from all over China were sent to Wuhan, about 625 hotlines were notified in 31 provinces, several online psychological consultation platforms were established, social software such as TikTok, Weibo, and WeChat were used for psychological education, and many books on mental health were published. Responding quickly, maximizing the use of resources, and emphasizing the importance of policy evaluation and implementation quality were characteristics of the mental health policies developed during the COVID-19 outbreak. Challenges facing China include a low rate of mental health service utilization, a lack of evaluation data on policy effects, and no existing national-level emergency response system and designated workforce to provide psychological crisis interventions during a national emergency or disaster.Conclusions: This review suggests that China has responded quickly and comprehensively to a possible mental health crisis during the COVID-19 outbreak, appropriate mental health policies were released for different members of the population. As the epidemic situation continues to change, the focus of mental health policies has been adjusted accordingly. However, we should note that there has been a lack of separate policies for specific mental health issues during the COVID-19 outbreak.

Mental health has always been stigmatised and overlooked. Since, anxiety and stress are natural responses while facing a threat of new diseases. Hence, this pandemic has shown us the importance of mental health. During this time there have been many triggers that have caused repercussions in people’s mental health. Mental health crisis is on the rise. So, it is important to be able to tackle mental health problems as effectively as physical health problems, not only now but in the future as well, once and for all.

The outbreak of COVID-19 pandemic is constantly posing warning and creating mental health crisis among people without any discrimination. Therefore, the current study purpose is to explore mental health upheaval and transition in future perspectives due to COVID-19. Using exploratory study design, semi structure interviews were conducted. Interviews were recorded, transcribed and analyzed by using Thematic Content Analysis. The major themes which were identified showed that COVID-19 is constantly eliciting panic and mental health issues such as anxiety, stress, and depression, fears of death, xenophobia, OCD and uncertainty about future among general population. Results also indicated the major transition in public future perspectives and perception. Concern related to adjustability in life after pandemic is major emerging future concern among Pakistani People. During pandemic outbreak, people are becoming vulnerable to different mental health problems. To overcome these issues successfully, role of mental health professionals cannot be denied. There is strong need to devise activities and strategies which help people to increase resilience and build strong relationships during the period of social distancing. This paper gives a deep insight into mental health problems among general population due to COVID-19 and it also implicates need of psychological services to overcome these issues.

BACKGROUND Mental health problems are the leading cause of disability worldwide. Despite the prevalence and cost of mental illness, there are insufficient health services to meet this demand. Crisis hotlines have a number of advantages for addressing mental health challenges and reducing barriers to support. Further, mental health crisis services have recently expanded beyond telephone hotlines to include other communication modalities such as crisis chat and text messaging services, largely in response to the increased use of mobile phones and text messaging for social communication. OBJECTIVE Despite the high uptake of crisis text line services (CTLs) and rising mental health problems worldwide, CTLs remain understudied. Thus the current study aimed to address an urgent need to evaluate user experiences with text-based crisis services. METHODS The current study explored user experiences of CTLs by accessing users’ publicly available Twitter posts that describe personal use and experience with CTLs. Data were qualitatively analysed using inductive thematic analysis. RESULTS Six main themes were identified from 776 tweets: (1) approval of CTLs, (2) helpful counselling, (3) invalidating or unhelpful counselling, (4) problems with how the service is delivered, (5) features of the service that facilitate accessibility, and (6) indication that the service suits multiple needs. CONCLUSIONS Overall, there was strong evidence to support the value of text-based crisis support, as many users reported positive experiences of effective counselling that provided helpful coping skills, de-escalation, and reduction of harm. Results also identified areas in need of improvement, particularly ensuring more timely service delivery and effective communication of empathy and support. Text-based services may require targeted training to apply methods that effectively convey empathy in this medium. Moving forward, CTL services will require systematic attention in the clinical research literature to ensure their continued success and popularity among users.

The potential of smartphone apps to improve quality and increase access to mental health care is increasingly clear. Yet even in the current global mental health crisis, real-world uptake of smartphone apps by clinics or consumers remains low. To understand this dichotomy, this paper reviews current challenges surrounding user engagement with mental health smartphone apps. While smartphone engagement metrics and reporting remains heterogeneous in the literature, focusing on themes offers a framework to identify underlying trends. These themes suggest that apps are not designed with service users in mind, do not solve problems users care most about, do not respect privacy, are not seen as trustworthy and are unhelpful in emergencies. Respecting these current issues surrounding mental health app engagement, we propose several solutions and highlight successful examples of mental health apps with high engagement. Further research is necessary to better characterise engagement with mental health apps and identify best practices for design, testing and implementation.

Background: The COVID-19 pandemic is a global health crisis associated with unprecedented levels of morbidity and mortality worldwide. The COVID-19 pandemic has been suggested to contribute to a great burden on global mental health. We assumed that individuals in quarantine outside their home country would be more vulnerable to developing mental health disorders during the current pandemic and might face difficulties in accessing mental health services. Aim: To explore the degree of association between the COVID-19 pandemic and mental health status of Saudi citizens living abroad. Objectives: (1) To measure the prevalence and risk factors of mental health problems among Saudi citizens studying and living abroad during the COVID-19 pandemic; (2) to assess the correlation between the COVID-19 pandemic and mental health status of Saudi citizens living abroad; and (3) to explore the level of anxiety/depression during the COVID-19 pandemic. Methods: A cross-sectional survey was conducted from August 2020 to September 2020 using a self-administrated questionnaire composed of sociodemographic, (GAD-7) and (PHQ-9) scales. Results: A total of 64% of participants experienced psychiatric symptoms during the pandemic, and 34% and 30% met the diagnostic criteria for symptoms of depression and anxiety, respectively. The risk of psychological symptoms was more likely experienced by females, young, single, or divorced, or those who were living alone. In addition, those who lived in the UK and Ireland were more likely to develop depressive and anxiety symptoms. More than 80% appreciated the response of the Saudi government and embassy to meet the MH needs of students undergoing quarantine abroad and in Saudi Arabia. Conclusions: The COVID-19 pandemic represents an unprecedented threat to global mental health. Two-thirds of study participants who were in foreign countries during the COVID-19 pandemic reported anxiety or depressive symptoms. Living away from family and friends was significantly associated with increased loneliness and psychological distress. These and other findings highlight the need to remove barriers preventing easily accessible online mental health services, social and family support, and timely provision of resources.

College students are one of the most affected groups by self-quarantine due to COVID-19, as they may live in loneliness and anxiety, increasing their risk of mental health crisis. This study aimed to identify risk factors for poor mental health and stress coping strategies among healthcare college students during the COVID-19 pandemic in Japan. A cross-sectional survey was conducted over 7 consecutive days starting on 28 April 2020 using a web-based questionnaire. The survey assessed socioeconomic characteristics and the General Health Questionnaire-12 score, self-reported health status, anxiety, and satisfaction with daily life, work, leisure, and new activities. Approximately 70% of 223 respondents had poor mental health. Less communication with friends was the main risk factor for mental health problems. Good health status and satisfaction with leisure and new activities were associated with reduced risk of mental health problems. Students with poor mental health tended to seek social support as a stress coping strategy. This study showed that the mental health of students declined during self-quarantine, and loneliness could be the major reason. There is a need for a new form of communication and learning that deals with the isolation and loneliness of students, especially for students living alone.

Covid-19 is poised to exacerbate the global mental health crisis as social isolation, unemployment, and economic recession are risk factors for negative mental health consequences. Since the need for social distancing can make face-to-face services less accessible, people may turn to mental health apps as an accessible and inexpensive solution. But are the claims about the efficacy of these apps supported by evidence? We identified 19 published articles on PubMed of peer-reviewed randomized clinical trials of mental health apps focused on stress, anxiety, and depression. Despite some evidence for the effectiveness of mental health apps, it remains unclear how effective these apps are compared to standard of care. Populations studied so far also lack diversity, making it difficult to generalize any benefits to racial minorities and low-income individuals—the very people who have been most negatively impacted by the Covid-19 pandemic and traditionally have higher barriers to mental health services. While apps are not a substitute for face-to-face therapy, further evidence is warranted given their potential for delivering scalable, low-cost care to patients. We call for regulatory oversight as a means to ensure mental health apps demonstrate safety and effectiveness prior to marketing.

Aims and MethodTo describe Drayton Park, the first women-only residential mental health crisis facility in the UK and to investigate whether it is succeeding in its remit of providing a viable alternative to hospital admission. We randomly selected case files from 100 women admitted to Drayton Park since its opening and examined variables including demographic details, the reasons for referral, diagnosis and the source of referral.ResultsOur findings show that the service is able to respond quickly to referrals and appears to be functioning safely. The women admitted have a relatively short length of stay, half suffer from depressive episodes and one-third have a relapse of schizophrenia or bipolar disorder.Clinical ImplicationsThis project appears to be succeeding in providing a safe alternative to hospital admission for women with severe and enduring mental health problems.
