This chapter provides an overview of the first family therapy session. It describes how to introduce families to the PFIT program and to develop a collaborative environment for establishing treatment goals. It describes psychoeducation about the role of the family in child OCD treatment, including family responses and expectations that may undermine success. It places particular emphasis on helping families to understand patterns of symptom accommodation that may be a barrier to treatment success, and it describes broader family dynamics that may interfere with efforts to change accommodation. The chapter also outlines steps for assessing current family functioning, including strengths and weakness, and for evaluating the family’s current strategies for managing OCD. Initial skills training begins with exercises designed to promote positivity in the home environment.

The authors compare the effectiveness of multifamily group therapy (MFGT) with traditional family therapy with abusive and neglectful caregivers and their children. Positive changes in the family functioning of the MFGT group occurred in areas critical to the reduction of child abuse and neglect, whereas changes in the family functioning of those in traditional family therapy occurred only in the area of parental support. The MFGT group children became significantly more assertive and less submissive according to their self-report and, according to their caregivers, showed significantly fewer overall behavior problems and greater social competence at the end of treatment. Changes in the children in the comparison group were not self-reported and did not occur in the area of social competence. These positive findings argue that agencies should consider adopting this modality in the treatment of this population and that it should be included as a treatment option in family-centered child welfare services.

The work of Virginia Satir, a pioneer family therapist, is examined as a transpersonal approach to family therapy. Ways in which transpersonal perspectives may be applied in family therapy are explored in Satir’s notions of grounding and centering, the evolving and transcending concept of congruence using the Self/I AM concept in the Satir iceberg model, as well as the Satir conceptualization of the therapist’s use of self. Aspects of transpersonal psychotherapy relevant to the practice of family therapy are examined, including the creation of a transpersonal space of trust in order to strengthen the therapeutic alliance, going beyond meaning in working with the family system to apply transpersonal (e.g. nondual psychotherapeutic) approaches to the therapist’s use of self in therapy. A discussion of intersubjectivity and the role of the beingness of the family therapist in promoting transcendence, awareness, and healing for the family is included. Satir family therapy is consistent with transpersonal psychotherapeutic perspectives and is therefore recommended as a prospective family therapy modality for the transpersonally-oriented psychotherapist. Purpose: To investigate the role that rehabilitation professionals play in the rehabilitation of children with disabilities in the rural and under-resourced community of Giyani in South Africa.Method: A qualitative, exploratory and descriptive approach was used. Semi-structured face-to-face interviews were used to collect data from a convenient sample of eight rehabilitation professionals. Data were transcribed verbatim by two trained students and verified by the main researcher. An inductive approach to qualitative data analysis was used. In vivo and open coding were used to generate codes.Results: Analysis of data resulted in 21 codes, 9 subcategories, 5 categories and 1 theme. The role of rehabilitation professionals was described in terms of the five categories which are to examine newborn babies and children at risk, support caregivers of children with disabilities, impart skills training for caregivers of children with disabilities, rehabilitate children with disabilities and conduct follow-ups in communities where the children with disabilities reside.Conclusion: The role that rehabilitation professionals play in the rural and under-resourced community of Giyani in South Africa is similar to the role played in high-income countries. The role that rehabilitation professionals play is not only focused on the child but also on the family.

Despite being the longest relationships across the lifespan, the sibling dyad is the most under-studied relationship in the family system. Researchers have documented the harmful and beneficial effects of sibling relationships and family dynamics on individual well-being. Extending this research to middle childhood, the current study examined family functioning, sibling relational dynamics, and self-reported adjustment and internalizing symptoms among 8- to 11-year-old children. Path analyses revealed significant direct effects between conflictual family functioning and children’s psychological well-being. Significant interactions between family functioning and sibling dynamics suggested that congruent relational dynamics at multiple levels of the family system had a cumulative impact on the child’s well-being. Findings from the current study highlight the role of family functioning and the unique contributions of sibling relationships to children’s functioning, suggesting that consideration of sibling relational dynamics may improve treatment planning by pointing out additional targets for intervention and/or potential sources of support.

Objective Current competencies required for fellowship of the RANZCP require psychiatry registrars to have experience in working with clients across all age groups, as well as working with families and the client’s wider network, however gaining this experience is not always easy for trainees. This paper reports on the experience of participating in Single Session Family Therapy (SSFT) during registrar training as a different modality for learning. Method: An online survey was conducted with fourteen registrars who had participated in SSFT during their child and adolescent rotation. Qualitative and simple quantitative data were collected and analysed. Results: Participating in SSFT during training was initially daunting, but had a positive effect on trainees, including influencing some towards focussing their future sub-specialisation in the child and youth area. Experience came through learning by doing, and seeing change. Registrars learnt about: understanding the role of the family; teamwork; technical skills; and gained confidence. Conclusions: Opportunities for trainees to participate in SSFT enables powerful learning beyond what can be taught in the classroom. Such opportunities may enhance registrars’ perceptions of family work, and may positively influence decision about future sub-specialisation.

Objective: The presence of posttraumatic stress disorder (PTSD) in trauma survivors has been linked with family dysfunction and symptoms in their children, including lower selfesteem, higher disorder rates and symptoms resembling those of the traumatized parent. This study aims to examine the phenomenon of intergenerational transfer of PTSD in an Australian context. Method: 50 children (aged 16–30) of 50 male Vietnam veterans, subgrouped according to their fathers’ PTSD status, were compared with an age-matched group of 33 civilian peers. Participants completed questionnaires with measures of self-esteem, PTSD symptomatology and family functioning. Results: Contrary to expectations, no significant differences were found between the selfesteem and PTSD symptomatology scores for any offspring groups. Unhealthy family functioning is the area in which the effect of the veteran's PTSD appears to manifest itself, particularly the inability of the family both to experience appropriate emotional responses and to solve problems effectively within and outside the family unit. Conclusion: Methodological refinements and further focus on the role of wives/mothers in buffering the impact of veterans’ PTSD symptomatology on their children are indicated. Further effort to support families of Veterans with PTSD is also indicated.

The problem of undernutrition is a threat to the development of a child. The role of family is critical in chalking the developmental trajectory of an undernourished child. This study explores whether the interpersonal relations among parents and home environment predicts the undernutrition in children. Findings reveal that interpersonal relationships, socioeconomic status, and partner violence significantly predicted child’s nutrition (R2=.403). The undernourished children were less closely attached to their mothers and their mothers were significantly less satisfied in their marriages when compared to the healthy children. The study has implications for designing and implementation of nutrition-sensitive intervention programs for malnutrition.

This pilot study compared the efficacy of two models of family therapy plus medication in the treatment of ten schizophrenic patients previously considered poor responders to neuroleptics alone. Focal Family Therapy (FFT), a limited psychodynamic model, was compared with Supportive Management Counselling (SMC), an educative, problem-solving approach. Family treatment was begun during admission to hospital and continued for up to six months post-discharge. Patient and family measures were administered on assessment, termination, and at three, six and 12 month follow-up interviews. Our data suggest that patients in both groups improved significantly following treatment on measures of social functioning and community tenure. The average increase in amount of time out of hospital was a full year, compared to previous rates. Patients receiving FFT demonstrated significantly greater improvement in symptoms, compared to patients in the SMC group. On average, families scored in the normal range on the family functioning measure at assessment and upon termination of treatment.
