Excessive daytime sleepiness and alcohol consumption among commercial drivers

Abstract Background Commercial drivers suffering from excessive daytime sleepiness (EDS) have been identified as a major cause of road traffic accidents. Alcohol usage directly affects sleep, adversely affecting next-day alertness and performance. Aims To examine the relationship between alcohol consumption and EDS among commercial truck drivers in Japan and the implications of this on public health. Methods All participants in this cross-sectional study were commercial motor vehicle drivers from Tokyo and Niigata Prefecture. Participants completed a self-administered questionnaire with details of their age, body mass index, alcohol consumption, Epworth Sleepiness Scale (ESS) score and tobacco usage. Participants’ oxygen desaturation index was determined by a pulse oximetry device that participants took home. Results A total of 1422 males registered with the Japan Trucking Association and aged 20–69 years participated. The multivariate-adjusted odds ratio (OR) of EDS among participants aged <43 years was 0.81 (95% confidence interval (CI) 0.47–1.40) for light drinkers, 0.93 (95% CI 0.51–1.70) for moderate drinkers and 0.61 (95% CI 0.21–1.79) for heavy drinkers, compared to non-drinkers. The multivariate-adjusted OR among participants aged ≥43 years was 1.42 (95% CI 0.59–3.45) for light drinkers, 1.53 (95% CI 0.63–3.75) for moderate drinkers and 3.37 (95% CI 1.14–9.96) for heavy drinkers (P for interaction = 0.05). Conclusion We found that the association between ESS and alcohol intake was more evident among those aged ≥43 years, who reported higher levels of EDS with increased alcohol consumption.

BACKGROUND: Commercial drivers have been identified as eliciting behaviours that promote non- communicable diseases and road traffic accidents. The aim of the study is to determine the prevalence and pattern of alcohol use, smoking and physical inactivity among commercial taxi drivers in Buffalo City Metropolitan Municipality, South Africa. METHODS: A cross-sectional study was conducted among 403 commercial drivers using the face-to-face interviews method. The WHO STEPwise questionnaire was used to obtain the demographic data, self-reported rate of alcohol consumption, tobacco use and physical inactivity. RESULTS: The participants&rsquo; mean age was 43.3 &plusmn; 12.5 years. About 30% of the participants were daily smokers, 37% consumed alcohol regularly and only 18% were physically active, whilst 82% were physically inactive. CONCLUSION: The prevalence of alcohol use, smoking and physical inactivity is high among commercial drivers in East London. Workplace health education on the health effects of these lifestyles&rsquo; risky behaviours on individuals and the general public should be given to the drivers.

This study aimed to investigate the prevalence of excessive daytime sleepiness (EDS) and explore the association between free sugars intake and EDS. In this cross-sectional study, a total of 1517 middle school students (808 boys and 707 girls) aged 12~14 years were recruited. The study was conducted in Changsha city, China. Adolescents completed an online questionnaire, including the Epworth Sleepiness Scale (ESS), sleep characteristics, a 12-item Food Frequency Questionnaire (FFQ), and other self-reported information. The ESS score ≥ ten was defined as EDS. The anthropometric indices, including height, weight, and waist circumference, were measured and recorded by uniformly trained assistants. Statistical analyses included the Chi-square test and binary logistic regression model. The mean ESS score and free sugars consumption were 6.8 ± 3.9 points and 53.1 ± 44.7 g/d, respectively. The prevalence of EDS among adolescents was 22.5%, and more girls than boys had EDS (26.1% vs. 19.4%, p < 0.05). An exceeded free sugars intake was positively associated with EDS, with the adjusted Odds Ratio (OR) with its 95% Confident Interval (95% CI) of 1.366 (1.060~1.761, p < 0.05). EDS and excessive consumption of free sugars are commonly found among Chinese adolescents. Further studies are needed to confirm whether free sugars restriction can be meaningful to improve daytime drowsiness in those with EDS.

Abstract Background Sarcopenia is emerging as an important public health problem, and evidences have determined that poor sleep is associated with muscle strength, but the potential effects of excessive daytime sleepiness (EDS), snoring and witnessed apnea on handgrip strength have not been evaluated. Aim We aimed to examine the association between EDS, snoring, witnessed apnea and muscle strength in an adult population. Design Cross-sectional study. Methods This cross-sectional study comprised 19 434 adults. Handgrip strength was measured using a handheld digital dynamometer. EDS was assessed by Epworth Sleepiness Scale, snoring and witnessed apnea during sleep were reported through simple yes/no questions. Analysis of covariance was carried out to determine the association between EDS with snoring or witnessed apnea and muscle strength. Results The means (95% confidence interval) for average handgrip strength/body weight (kg/kg) across symptoms categories were 0.396 (0.333–0.472), 0.393 (0.330–0.467), 0.396 (0.333–0.471) and 0.386 (0.325–0.460) (P < 0.0001), respectively. Similar results were observed with maximal handgrip strength/body weight (kg/kg). Conclusions Self-reported EDS accompanied with snoring or apnea is associated with lowest handgrip strength, independently of confounding factors. Whether improvement of EDS, snoring and apnea, can ameliorate age-associated decline in muscle strength warrants further studies.

Road traffic accidents are the leading cause of death among the Indian population. Motorcycle accidents are the most prominent type of injuries among road traffic accidents in India. In this prospective cross-sectional study, all the patients attending three centers in north Bangalore, with facial injuries occurring from a motorcycle accident, were included. The subjects were analyzed for the type of collision, helmet use, type of helmet use, and fastening status of the helmets. A total of 311 motorcyclists were included in this study for a period of 18 months (December 2015 to June 2017). There were 79.7% males and 20.3% females. The most prominent age group was 21 to 30 years. The percentage of riders sustaining facial injuries was significantly more in the non-helmeted group. The most common injuries in open face helmets were in the middle and lower third of the face, whereas in closed face helmets it was in the middle third of the face. The numbers of injuries were significantly higher in the nonfastened helmet group as compared with fastened helmet group. Helmet fixation is an important characteristic along with helmet type for the better effectiveness and safety of the helmets for the motorcyclists.

Sleepiness is a major public health problem associated with motor vehicle crashes, occupational accidents, decreased productivity, and interpersonal problems. It can be influenced by many factors, including the individual's circadian rhythm. The objective of this study is to assess the correlation between the morningness/eveningness dimension and daytime sleepiness in medical school students. This is a cross-sectional study; 310 subjects (123 women and 187 men, M age 20.5 ± 1.9 yr.) completed a questionnaire on use of drugs, diagnostic diseases, sleeping habits, the Morningness/Eveningness Questionnaire and the Epworth Sleepiness Scale. The only variable related to daytime sleepiness was the morningness/eveningness dimension ( r = –.18, p = .002). Questions with higher coefficients of discrimination between the morning and the evening chronotype were “Lying down to rest in the afternoon when circumstances permit” (.61), “Sitting and reading” (.53), and “As a passenger in a car for an hour without a break” (.53). The implications of the findings are discussed.
