Abstract Study Objectives This field study (a) assessed sleep quality of sailors on the U.S. Navy (USN) ships while underway, (b) investigated whether the Pittsburgh Sleep Quality Index (PSQI) scores were affected by occupational factors and sleep attributes, and (c) assessed whether the PSQI could predict impaired psychomotor vigilance performance. Methods Longitudinal field assessment of fit-for-duty USN sailors performing their underway duties (N = 944, 79.0% males, median age 26 years). Participants completed questionnaires, wore actigraphs, completed logs, and performed the wrist-worn 3-min Psychomotor Vigilance Task (PVT). Results Sailors slept on average 6.60 ± 1.01 h/day with 86.9% splitting their sleep into more than one episode/day. The median PSQI Global score was 8 (interquartile range [IQR] = 5); 80.4% of the population were classified as “poor sleepers” with PSQI scores >5. PSQI scores were affected by sailor occupational group, rank, daily sleep duration, and number of sleep episodes/day. Sleep quality showed a U-shape association with daily sleep duration due to the confounding effect of split sleep. Sailors with PSQI scores >9 had 21.1% slower reaction times (p < 0.001) and 32.8%–61.5% more lapses combined with false starts (all p < 0.001) than sailors with PSQI scores ≤9. Compared to males and officers, females and enlisted personnel had 86% and 23% higher risk, respectively, of having PSQI scores >9. Sailors in the PSQI > 9 group had more pronounced split sleep. Conclusions Working on Navy ships is associated with elevated PSQI scores, a high incidence of poor sleep, and degraded psychomotor vigilance performance. The widely used PSQI score>5 criterion should be further validated in active-duty service member populations.

Background: Driving performance is strongly vulnerable to drowsiness and vigilance fluctuations. Excessive sleepiness may alter concentration, alertness, and reaction times. As people age, sleep undergoes some changes, becoming fragmented and less deep. However, the effects of these modifications on daily life have not been sufficiently investigated. Recently, the assessment of sleepiness became mandatory in Europe for people at risk who need the driving license release. Moreover, considering the expectation that people around the world are rapidly aging, it is necessary to investigate the relationships between senescence sleep changes, vigilance levels, and driving-related cognitive skills. Method: 80 healthy subjects (40 young adults and 40 elders) participated in the study. Sleep quality, sleepiness, and vigilance levels were assessed through the Pittsburgh Sleep Quality Index, the Karolinska Sleepiness Scale, the Epworth Sleepiness Scale, and the Psychomotor Vigilance Task (PVT). Driving-related cognitive abilities were assessed through Vienna Test System TRAFFIC, investigating selective attention, tachistoscopic perception, and risk assumption. Results: 2 × 2 between-subject ANOVAs showed less habitual sleep efficiency and worse performances in PVT in the older group. Unexpectedly, younger subjects show higher self-rated sleepiness. Moreover, older adults have lower performance in attention and perception tests, but they appear to be more cautious in situations involving traffic. Finally, the multiple regressions show age to be the only robust predictor of cognitive driving-related abilities. Conclusions: This is the first study that investigates the relationships among sleepiness/vigilance and specific driving-related cognitive skills on a sufficiently large sample. Nevertheless, the study should be considered preliminary and does not allow us to understand how specific changes in sleep architecture impact performances in the elders’ everyday life and, specifically, on driving skills.

This naturalistic study of 52 sailors on the USS Jason Dunham (DDG-109) compared actigraphic sleep and psychomotor vigilance performance of crewmembers working a 3hours-on/9hours-off (“3/9”) with the performance of their counterparts who worked a 6hours-on/6hours-off (“6/6”) watchstanding schedule. Crewmembers on the 3/9 slept more (6.46±0.77 hours) and reported less daytime sleepiness (Epworth Sleepiness Scale (ESS) score = 10.7±3.70) than their peers on the 6/6 schedule (sleep 5.89±0.87 hours; ESS = 13.1±3.45). The length of the workday for participants on the 6/6 schedule was 15 hours compared to 12.2 hours for crewmembers on the 3/9. Based on 11 of 13 Psychomotor Vigilance Task metrics, performance of crewmembers on the 6/6 schedule had significantly greater variability than the 3/9 sailors (p<0.05). Overall, participants evaluated the 6/6 as being worse and the 3/9 as the better schedule. Results showed that the 3/9 outperformed the 6/6 in terms of daily sleep duration, fatigue levels, psychomotor vigilance performance, and acceptance from the participants.

AbstractPreliminary findings suggest that pets may impact the owner’s sleep. By using data from the Swedish CArdioPulmonary bIoimage Study (SCAPIS) cohort, we aimed to investigate the association of pet ownership with the following self-reported sleep outcomes in 3788 to 4574 participants: (i) achieving the recommended daily sleep duration for adults (i.e., at least 7 h per day); (ii) sleep quality as measured by the Pittsburgh Sleep Quality Index (a score of > 5 indicating poor sleep quality); and (iii) difficulty falling or staying asleep. Sleep metrics were not associated with pet ownership, dog ownership, and dog walking when controlling the logistic regression for possible confounders (e.g., shift work, lack of social interaction, and chronic stress). In contrast, cat ownership was associated with a higher odds ratio of failing to achieve the recommended duration of 7 h of sleep per day (adjusted odds ratio [95% CI]:1.18 [1.02, 1.37] versus non-cat owners). Our findings suggest that certain pet groups might have a more significant impact on the owner’s sleep than others. As the observed association between cat ownership and short sleep duration might be a chance finding, this observation should be seen as hypothesis-generating only.

Abstract Introduction Poor sleep quality is common during pregnancy and can increase the risk of adverse obstetric and fetal outcomes. Existing research on the association between prenatal sleep and infant sleep is scarce and has focused on other aspects of prenatal sleep such as sleep duration, chronotype, and insomnia symptoms. To our knowledge, no studies have examined the association between prenatal sleep quality and infant sleep outcomes. Thus, this study aimed to investigate whether maternal sleep quality during pregnancy was prospectively associated with infant sleep dimensions, independent of relevant covariates. Methods Participants were a subset of 272 mother-infant dyads enrolled in an ongoing cohort study. Maternal prenatal sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI) in early to mid- (M gestational age = 15.12 ± 3.56 weeks) and late- (M gestational age = 32.44 ± 0.99 weeks) pregnancy. Mothers completed the Brief Infant Sleep Questionnaire (BISQ) at 3, 6, and 12 months postpartum. The following infant sleep parameters were assessed: sleep duration (day, night, 24-hour), number of night awakenings, and wake after sleep onset. Prenatal depressive symptoms were assessed with the Edinburgh Postnatal Depression Scale (EPDS) at both pregnancy time points. Other covariates included maternal age at enrollment, infant age, parity, and co-sleeping status. Results Generalized estimating equations (GEE) models revealed that poorer maternal sleep quality during early-to-mid pregnancy did not significantly predict infant sleep parameters after adjustment for covariates (p &gt; .05). However, in late pregnancy, poorer maternal sleep quality significantly predicted shorter 24-hour sleep duration and longer wake after sleep onset, but not daytime sleep duration, nighttime sleep duration, and number of night awakenings (p &lt; .05). Conclusion Study findings advance our understanding of the prospective link between maternal prenatal sleep quality and infant sleep. Results indicate that maternal sleep quality during late gestation may play a role in the development of infant sleep patterns. These findings have important implications for intervention efforts targeting maternal sleep quality during pregnancy. Future research should use objective measures of sleep, such as actigraphy, to better elucidate the effects of prenatal sleep quality on infant sleep outcomes. The main purpose of the present study was to explore the associations of sleep duration and sleep quality with self-rated health. In this cross-sectional study, participants were 894 elderly individuals. Self-rated health, sleep duration, and sleep quality were self-reported. The associations were examined using multiple logistic regression analyses.After adjusting for sex, physical activity, smoking consumption, alcohol consumption, psychological distress, socioeconomic status, and chronic disease/s, sleeping <6 h (OR (Odds ratio) = 3.21; 95% CI (95 percent confident interval) 1.61 to 6.39), 6–7 h (OR = 2.47; 95% CI 1.40 to 4.36), 8–9 h (OR = 3.26; 95% CI 1.82 to 5.83), and >9 h (OR = 3.62; 95% CI 1.57 to 8.34) and having ‘poor’ sleep quality (≥5 points; OR = 2.33; 95% CI 1.46 to 3.73) were associated with ‘poor’ self-rated health. When sleep duration and sleep quality were entered simultaneously into the model, the same associations remained. Our findings provide evidence that both ‘short’ and ‘long’ sleep and ‘poor’ sleep quality are associated with ‘poor’ self-rated health. Thus, interventions that promote healthy sleep hygiene in the elderly are warranted.

Abstract Background : To assess the association of sleep duration and quality with the risk of preterm birth. Methods : Relevant studies were retrieved from the PubMed and Web of Science databases up to September 30, 2018. The reference lists of the retrieved articles were reviewed. Random effects models were applied to estimate summarized relative risks (RRs) and 95% confidence intervals (CIs). Results : Ten identified studies (nine cohort studies and one case-controlled study) examined the associations of sleep duration and quality with the risk of preterm birth. As compared with women with the longest sleep duration, the summary RR was 1.23 (95% CI = 1.01–1.50) for women with the shortest sleep duration, with moderate between-study heterogeneity ( I 2 = 57.4%). Additionally, as compared with women with good sleep quality, the summary RR was 1.54 (95% CI = 1.18–2.01) for women with poor sleep quality (Pittsburgh Sleep Quality Index > 5), with high between-study heterogeneity ( I 2 = 76.7%). Funnel plots as well as the Egger’s and Begg’s tests revealed no evidence of publication bias. Conclusions : This systematic review and meta-analysis revealed that short sleep duration and poor sleep quality may be associated with an increased risk of preterm birth. Further subgroup analyses are warranted to test the robustness of these findings as well as to identify potential sources of heterogeneity.

The aim of this study was to systematically examine the literature on physical activity and sleep in non-clinical and population-based settings. The inclusion criteria were original studies testing the association between physical activity (as exposure) and sleep (as outcome) in representative samples of the general population, workers, or undergraduate students. Sleep health included sleep duration, sleep quality and insomnia. Studies evaluating samples including only individuals with some disease or a health condition were excluded. A search was performed in the PubMed, Scopus, Lilacs, CINAHL, and SPORTdiscus databases in March 2018. Data extraction was performed using the following items: year, author, country, population, age group, sample size, study design, sleep measurement/definition, physical activity measurement/definition, adjustment and main results. A total of 57 studies were selected, which markedly used heterogeneous instruments to measure physical activity and sleep. The majority were conducted in high-income countries and with cross-sectional design. Physical activity was associated with lower odds of insomnia (observed in 10 of 17 studies), poor sleep quality (observed in 12 of 19 studies) and long sleep duration (observed in 7 of 11 studies). The results about short sleep or continuous sleep duration remain unclear. Physical activity seems to be associated with sleep quality and insomnia, especially among adult and elderly populations in which these outcomes are more usually measured. The short- and long-term effects of physical activity intensities and dose-response on sleep should be better evaluated.

Abstract Objective To examine the association of sleep quality/duration with chronic health conditions, psychological distress, and quality of life (QOL) in older adults with chronic traumatic brain injury (TBI). Methods 120 older adults (x-age = 64.2 ± 8.3) 1 or more years (med = 9.8, range = 1.1–45.6) post moderate–severe TBI reported on history of chronic health conditions and current sleep duration and quality. Participants were categorized by sleep duration (&lt; 6, 6–8, &gt;8 hours) and whether or not they felt well-rested. Outcome measures were QOL (Quality of Life after Brain Injury questionnaire) and psychological distress (Brief Symptom Inventory-18). Results 65% of individuals reported receiving 6–8 hours of sleep; 78% reported feeling well-rested. 17.5% reported no health conditions, 47.5% one condition, and 35% reported two or more. High blood pressure, high cholesterol, and diabetes were the most common. Number of health conditions was not related to sleep quality χ2(2,N = 120) =0.83, p = 0.66, or quantity, χ2(4,N = 120) =7.4, p = 0.12. MANCOVA controlling for age, chronicity, and injury severity revealed a significant association between poor sleep quality and decreased QOL across multiple life domains, V = 0.30, F(6,105) = 4.6, p &lt; 0.001, ηp2 = 0.21. Sleep duration was also associated with QOL, Λ = 0.80, F(12,208) = 2.1, p &lt; 0.05, ηp2 = 0.108. In ANCOVAs, poor sleep quality was related to increased psychological distress, F(1,110) = 18.3, p &lt; 0.001, ηp2 = 0.142, but sleep duration was not, F(2,109) = 2.2, p = 0.12, ηp2 = 0.038. Conclusion Although most participants received the recommended amount of sleep, poor sleep quality/quantity were associated with poorer QOL and sleep quality was additionally associated with psychological distress. Chronic health conditions were prevalent in the sample, but not related to self-reported sleep quality/duration.

Introduction: Research shows that compared to non-Hispanic whites, African Americans (AAs) have poorer sleep quality, lower mean sleep duration, and a higher prevalence of sleep-disordered breathing. AAs also report more frequent exposures to certain stressors over the life course, which may impact physiological processes that may impair sleep. Goal-striving-stress (GSS), the discrepancy between aspiration and achievement, weighted by the subjective probability of success, and the level of disappointment experienced if goals are not reached, may be an important stressor among AA’s that may influence sleep; however this has yet to be explored. The objective of this study was to assess the relationship between GSS and sleep duration and sleep quality in AAs. Hypothesis: We assessed the hypothesis that high (versus low) GSS would be associated with short or long sleep duration and poor sleep quality. Methods: We utilized data from the baseline exam of the Jackson Heart Study (JHS; n=5306), an AA sample of women and men, 35-84 years old. There were a total of 5082 participants in the sample; 63.34% female with a mean age of 55.30 (± 12.75) and mean sleep duration of 6.43 hours (±1.51). The sample was categorized into GSS tertiles: low (n=2121), moderate (n=1716), high (n=1296). Participants self-reported sleep duration (hours) and rated their sleep quality. Sleep duration was categorized as short ( < 6 hours), normal (7 or 8 hours) and long ( > 9 hours). Sleep quality was categorized as high (good/very good/excellent) and low (fair/poor). Logistic regression models were used to obtain odds ratios (OR, 95% confidence interval-CI) to assess the associations of GSS levels with sleep duration and sleep quality categories. Models were adjusted for sex, age, socioeconomic status, health behaviors, discrimination, and health outcomes. Results: Significant results showed that participants who reported high (versus low) GSS had a 29% increased odds [1.29 (1.10, 1.52)] of short (versus normal) sleep after full adjustment. Participants who reported high (versus low) GSS had a 42% increased odds [1.42 (1.20, 1.67)] of low (versus high) sleep quality after full adjustment. Conclusion In conclusion, the deficit between goal aspiration and achievement is associated with short sleep duration and poor sleep quality. Potential interventions should consider the extent to which GSS may contribute to the development of short sleep duration and poor sleep quality.
