Breast-fed infants in Bangladeshi villages were weighed at 1, 2, 6, 9 and 12 months. The concentrations of zinc and copper in the breast milk were measured and the daily intake of these elements calculated. Breast milk Zn concentration decreased over the year but was comparable with that found in developed countries. The calculated daily intake decreased from 17·7 to 8·0 μmol (10–30% of recommended dietary allowances (RDA); National Academy of Sciences, 1980). Breast milk Cu concentration also fell over the year and was lower than that reported from developed countries. Calculated daily Cu intake was 1·95–2·63 μmol (RDA 7·81–15·63 μmol). Deficiencies of trace elements may therefore be a problem in poorly nourished communities where breast feeding is continued for several years with only small amounts of additional food. Breast milk may not be adequate as the only source of infant nutrition after the first few months of life in Bangladesh.

I read with interest the report by the Provisional Committee for Quality Improvement and Subcommittee on Hyperbilirubinemia (Pediatrics 1994;94:558-565) entitled, "Practice Parameter: Management of Hyperbilirubinemia in the Healthy Term Newborn." I wish to make a couple of comments on jaundice and the breast-fed newborn. It was gratifying to see recommendations discouraging the interruption of breast-feeding and eliminating the use of supplemental water or dextrose and water in this situation. Many jaundiced breast-fed newborns simply need more breast milk, ie, more feedings and a check to see that the newborn is swallowing milk at breast.

Dr. Bland found an increase in otitis media in bottle-fed versus breast-fed infants. He speculates that this may be due to transfer of IgA in breast milk. Another possibility is that the increased incidence of otitis is due to positional differences between bottle- and breast-feeding. Bottle babies are often fed in the recumbent position which promotes entry of milk into the eustachian tubes and the middle ear, whereas the anatomy of the maternal breast requires that the infant's head be vertical during feeding. This concept of positional otitis has been recently reviewed.

Four cases of extensive seborrheic dermatitis or Leiner's disease are presented which dramatically improved after the nursing mother was given injections of biotin. The relationship between seborrheic dermatitis in infants and biotin deficiency is discussed and the literature reviewed. From this review it appears that breast milk is deficient in biotin in comparison to cow's milk. The deficiency is further aggravated by poor maternal nutrition. Infection and diarrhea in the infant may also contribute to low blood levels of biotin. On the basis of this limited experience, injections of biotin to the nursing mother appear to be a useful treatment for the breast-fed infant with extensive seborrheic dermatitis.

To the Editor.— I read the article, "Infant Acceptance of Postexercise Breast Milk" (Pediatrics 1992;89:1245-1247). Although it may be noteworthy that postexercise breast milk contains higher levels of lactic acid which change its taste, does this observation necessitate the recommendation to give a breast-fed baby supplemental feedings after the mother exercises? If the mother feeds the baby before exercising and the baby requires another feeding after the exercise, it is highly unlikely that he will suffer from caloric deprivation or malnutrition if he does not take a full feeding at that time.

The case records of 11 infants with prolonged hyperbilirubinemia have been presented. The elevation of indirect reacting bilirubin in these infants may be due to interference with the normal conjugating mechanism by a substance present in breast milk. Introduction of cow's milk formulas should be carried out as part of any extensive investigation of prolonged hyperbilirubinemia of the indirect type in otherwise well infants who have been wholly breast fed from birth.

Abstract SARS-CoV-2, which causes COVID-19, is a new virus that has spread fast over the world. The severity of COVID-19 at different ages has been a notable and constant observation: severity, the requirement for hospitalization, and mortality all grow sharply with age, although severe disease and death are uncommon in children and young adults.. The majority of children infected with SARS-CoV-2 are asymptomatic or have moderate symptoms, which include fever, cough, pharyngitis, gastrointestinal symptoms, and changes in taste and smell. The question of whether children are less likely to be infected with SARS-CoV-2 is still being debated. Children make up only 1 to 2% of all SARS-CoV-2 cases, according to large epidemiological research. these numbers are heavily, depend on testing criteria, and in many reports, testing was limited to those who were symptomatic or required hospitalization, which is less common in children.. According to certain research, children are just as likely as adults to contract SARS-CoV-2.9. Recent research suggests that children are less likely to become infected after coming into touch with a SARS-CoV-2-positive person.According to some reports, children and adolescents have similar virus loads and are hence just as likely to transmit SARS-CoV-2 as adults. Furthermore, the viral load in asymptomatic and symptomatic people may be identical. Reassuringly, transmission of the virus from children to other children or adults in schools has been infrequent.Children are less likely to be infected with SARS-CoV-2 and have less severe symptoms, which is similar to what has been observed with SARS-CoV-1 and Middle East respiratory disease (MERS)-CoV. Infection with most other respiratory viruses (e.g., respiratory syncytial virus (RSV), metapneumovirus, parainfluenza, or influenza viruses), on the other hand, has a far higher prevalence and severity in youngsters. Dr. Mahmoud Elkazzaz and Dr Amr kamel khalil Ahmed, the lead investigators of this observational study, recently published a preprint that demonstrated Docosahexaenoic acid (DHA) had a high binding affinity and greatest interactions with ACE2 active sites, as well as a moderate binding affinity and moderate interactions with the active sites of IL-6. The Docosahexaenoic acid (DHA) interacts with different active sites of IL6 and ACE2 which are involved in direct or indirect contacts with the ACE2 and IL-6 receptors which might act as potential blockers of functional ACE2 and IL-6 receptor complex.. A study proposed, a clinical benefit of targeting IL-17A signaling and the synergic inflammatory cytokine IL-6 to manage COVID-19 patients, particularly those presenting with cytokine storm syndrome.Hypercytokinemia, caused by notably high pro-inflammatory cytokines such as interleukin (IL)-1B, IL-6, IL-8, and IL-17, is mostly linked to the worsened clinical presentation of COVID-19 patients(14). In PBMCs from individuals with relapsing-remitting multiple sclerosis, a combination of docosahexaenoic acid (DHA) and all-trans-retinoic acid (ATRA) inhibits IL-17 gene expression.ConclusionsDocosahexaenoic acid (DHA) was detected in abundance in breast milk and other algal sources milk supplement used for newborns and children's feeding. As a result, we believe that docosahexaenoic acid (DHA) may protect children and newborns thorough competing with COVID-19 for ACE2 receptors and inhibiting IL-6 activity and may possibly help them avoid a cytokine storm and save their lives through inhibiting IL-6 and preventing SARS- CoV-2 RBD attachment to ACE2. In addition to IL-17 was fond to increase COVID-19 inflammatory complication in this case DHA combined with retinoic acid is expected to be effective in inhibiting IL-6 and IL-17.

The problem discussed in this study is designing a system that can be used as an ASI bank. The Posyandu in Bantarsoka Village is very concerned about the importance of breastfeeding for babies. A lot of babies who are not breast fed are a concern. On the other hand, many breastfeeding mothers also have excess breast milk. The absence of a system that accommodates information about ASI at the Posyandu in Bantarsoka Village makes the Posyandu less optimal in helping its residents overcome the problem of breastfeeding distribution. With current technological developments, the idea emerged to create an information system for ASI management, which later could be a solution to the problems. The ASI Bank Management System aims to provide a forum for mothers who have been excess breast fed to be able to donate their breast milk to babies who do not get their mother's milk intake. This study uses the Waterfall method which is carried out with a structured approach, starting from the system requirement's stages, analysis, design, coding, testing, and maintenance. The result of this research is an information system in the form on the ASI Bank website. With this system, it is hoped that it can help with the problem of breastfeeding distribution. The system created can properly manage and record breastfeeding ownership, who is the donors and recipients. By utilizing Internet technology, information dissemination can be accessed quickly and easily.
