The excitement about immunotherapy is justified. Patients with advanced disease and limited life expectancy before immune checkpoint inhibitors are now having prolonged and sometimes complete responses to treatment; however, most patients do not respond to checkpoint inhibitors. The hope for a meaningful response with only a limited risk of high-grade toxicity generated a prognostic dilemma for patients with advanced cancers and their treating oncologists. Older adults with advanced cancers are at the intersection of multiple biologic and clinical factors that can influence the efficacy of immunotherapy. Treating physicians should take all of these elements into account when considering treatment options for an older adult with advanced disease. Oncologists should have an honest conversation with their patients regarding the uncertainty around the clinical profile of checkpoint inhibitors. Early high-quality goals of care discussions can help manage expectations of older adults with advanced cancer treated with immunotherapy. We review in this paper select clinical characteristics that are important to consider when evaluating an older adult for checkpoint inhibitor therapy. In addition, we discuss strategies to optimize goals of care discussion given the increasing complexity of prognostication in the immunotherapy era.

Thus, a necessity exists to collect and systematize information on all treatment options for rising damp. HUMITECNIC files/catalogue allows the fulfilment of that objective.HUMITECNIC files/catalogue is prepared based on an extensive search of systems, products and treatments in the civil engineering marketplace, patented or not, accompanied when necessary by a suitable bibliography. In this way, the user is presented with different treatment options that communicate the intervention approach, the technology used, installation process, relevant properties, application and limitations. In a way to promote the information to users in a dynamic an interactive way, the catalogue is available from the purpose built HUMITECNIC website.

Pancreatic adenocarcinoma is common among older adults, who may be more frail or have comorbid medical conditions that make treatment more challenging. Pancreatic adenocarcinoma is a lethal malignancy with relatively few effective treatment options. To date, most clinical research has not enrolled large numbers of older adults, despite this being the demographic predominantly affected by this illness. In this article, etiology and treatments for pancreatic adenocarcinoma are reviewed with a particular emphasis on studies that have examined matters relating to treating older adults; future directions for research are also explored.

SummaryUrinary incontinence is a highly prevalent yet largely ignored condition that affects older women. Conservative treatment options are widely under-used and containment methods more widely prescribed. This study sought to review the efficacy of conservative approaches and identify which should be recommended. It was found that there is a strong evidence base in support of pelvic floor muscle and behavioural training with biofeedback as a useful adjunctive treatment. The use of therapeutic electrical stimulation was difficult to evaluate due to the different combinations of treatment available and a paucity of evidence of its use with older women. Lifestyle intervention and complementary therapies both demonstrated some positive outcomes. In conclusion it was felt that there is a place for conservative treatments in the management of urinary incontinence rather than relying on containment products. A combination of different treatment approaches is felt to be the most beneficial.

Any step in cardiac surgery may have potentially catastrophic consequences. All treatment options need to be carefully evaluated because the most aggressive is not always the best one. We present the case of a 63-year-old man who suffered an interventricular septum hematoma causing left ventricular tract obstruction, secondary to a left ventricular vent. The lesion could be repaired by closing the entry point.

Tinnitus is a prevalent and costly chronic condition; no universally effective treatment exists. Only 20% of patients who report tinnitus actually seek treatment, and when treated, most patients commonly receive sound-based and educational (SBE) therapy. Additional treatment options are necessary, however, for nonauditory aspects of tinnitus (e.g., anxiety, depression, and significant interference with daily life) and when SBE therapy is inefficacious or inappropriate. This paper provides a comprehensive review of (1) conventional tinnitus treatments and (2) promising complementary therapies that have demonstrated some benefit for severe dysfunction from tinnitus. While there has been no systematic study of the benefits of an Integrative Medicine approach for severe tinnitus, the current paper reviews emerging evidence suggesting that synergistic combinations of complementary therapies provided within a whole-person framework may augment SBE therapy and empower patients to exert control over their tinnitus symptoms without the use of medications, expensive devices, or extended programs.

IntroductionDespite renewed treatment options for advanced epithelial ovarian cancer, survival remains poor. The Patient Association and the Gynecological Oncology Working Party in the Netherlands have identified a need for a tool to improve shared decision-making. The aim of this study was to develop an evidence-based online decision aid for patients with advanced epithelial ovarian cancer and their medical team.MethodsFirst, we identified the patients’ and clinicians’ needs using surveys and in-depth interviews. Second, we conducted multidisciplinary face-to-face meetings with representatives from all stakeholders (clinicians and patient representatives) to determine the content of the decision aid. Third, we developed the decision aid using standardized criteria and national guidelines. Finally, we tested the usability of the tool with patients and clinicians who participated in the needs assessment.ResultsPatients and clinicians indicated the need for more sources of reliable information that include all treatment options available in the Netherlands. Although most interviewees were satisfied with the level of information available at the time of their own treatment, the majority (90%) of the patients stated that no choice of treatment was offered. We developed a consultation sheet and an online decision aid based on patient interviews and team discussions. The sheet contains a summary of all treatment options and login codes for the decision aid; it will be offered to patients at their first consultation. The decision aid can be used at home and includes information about epithelial ovarian cancer and all available treatment options and questions about quality of life and treatment preferences, delivering a personalized summary for discussion during the following consultation about the primary treatment choices.DiscussionIn cooperation with patients and clinicians, we developed a decision aid for advanced-stage epithelial ovarian cancer patients and their medical team to support shared decision-making, based on a confirmed need for more extensive information sources. The decision aid is currently under assessment in a multicenter implementation trial.

AbstractThe disease caused by the new coronavirus (COVID-19) is characterized by fever and cough, in addition to affecting the lower respiratory tract and being associated with age, comorbidities and a weakened immune system. Lymphopenias occur in severe cases and an excessive production of inflammatory cytokines, which would explain the role of the hyperinflammatory response in the pathogenesis of COVID-19. In the absence of treatment for this virus, there is an urgent need to find alternative methods to control the spread of the disease, so we have conducted an online search for all treatment options related to coronavirus infections, as well as some infections due to viruses, general treatments, specific coronavirus treatments and antiviral treatments should be useful in the fight against COVID-19, the therapeutic agents evaluated included chloroquine/hydroxychloroquine, lopinavir/ritonavir, tocilizumab, ribavirin, interferons, nelfinavir, ivermectin, monoclonal antibodies and convalescent plasma.

Abstract Background This study aimed to determine if International Microsurgery Club (IMC) is an effective online resource for microsurgeons worldwide, in providing an avenue for timely group discussions and advice regarding complicated cases, and an avenue for collaboration and information sharing. Methods All posts on the IMC Facebook group from member 1 to 8,000 were analyzed according to inclusion criteria and categorized into three categories—case discussion, question, and information sharing. Posts were retrospectively analyzed for number of responses, time of responses, number of “likes,” number of treatment options, time of day, and demographics of authors and responders. Results A retrospective analysis of 531 cases showed an average response rate of 75.7% within 1 hour and as membership grew. The response rate stabilized averaging between 72.5 and 78% across all times of the day. An average of 11.8 microsurgeons was involved per case discussion, and 5.7 treatment options were provided per case. Conclusion IMC is shown to be an effective resource to allow microsurgeons to access timely advice from other microsurgeons without time and distance limitation, and to have interactive group discussions on complicated cases.
