Much has been written about the relationship between Iran and the Gulf states. This relationship, while extremely complex, historic as well as deep rooted, needs to be revisited, especially in the light of the growing discords. This article investigates the contest over the balance of power in the Middle East which is impacted by state interests, foreign policy, ideology, sectarianism, and geography. There are three questions that need to be asked: (a) What role does Iran play in the Gulf region? (b) What is its relationship with the Gulf Cooperation Council (GCC)? and (c) Is there room for the two regional powers (Iran and Saudi Arabia) in the Gulf region? Iran’s role in the Middle East has expanded exponentially by both diplomatic means as well as by proxy and direct interventions. It has become apparent that while once Saudi Arabia controlled the GCC, due to recent events, the regional group has become trivialized. As Saudi Arabia and Iran vie for power, the Gulf is tension-filled and fraught with the possibility of misperceptions and miscalculations.

Dust storms deteriorated air quality over the Gulf Region, Iraq, Iran, and Pakistan during the last decade. The purpose of this study is to investigate the changes in aerosol optical and radiative properties during a dust episode over the various locations in the Middle East and Southwest Asia using data from the MODerate resolution Imaging Spectroradiometer (MODIS) and the Aerosol Robotic Network (AERONET) during March, 2012. Maximum aerosol optical depth (AOD) values were found to be 2.18, 1.30, 4.33 and 1.80 over Lahore, Kanpur, Kaust, and Mezaira, respectively. The Volume Size Distributions, Single Scattering Albedo, Refractive Index, and Asymmetry parameter indicated that coarse mode aerosols were predominant relative to fine mode aerosols during the dust event. The average shortwave aerosol radiative forcing (ARF) values at the earth’s surface were found to be -96±45 W m-2, -86±22 W m-2, -77±51 W m-2, and -75±40 W m-2, over Lahore, Kanpur, Kaust and Mezaira, respectively. Likewise, the averaged ARF values over Lahore, Kanpur, Kaust and Mezaira at the top of the atmosphere (TOA) were found to be -45±25 W m-2, -27±9 W m-2, -41±29 W m-2, and -75±40 W m-2, respectively. The large differences between surface and TOA forcing produced significant heating within the atmosphere.

Background and methods Stroke incidence and mortality are reported to have increased in the Middle-East and North African (MENA) countries during the last decade. This was a prospective observational study to examine the baseline characteristics of stroke patients in the MENA region and to compare the MENA vs. the non-MENA stroke cohort in the Safe Implementation of Treatments in Stroke (SITS) International Registry. Results Of the 13,822 patients with ischemic and hemorrhagic stroke enrolled in the SITS-All Patients Protocol between June 2014 and May 2016, 5897 patients (43%) were recruited in MENA. The median onset-to-door time was 5 h (IQR: 2:20–13:00), National Institutes of Health Stroke Scale (NIHSS) score was 8 (4–13) and age was 65 years (56–76). Hypertension (66%) and diabetes (38%) were the prevailing risk factors; large artery stenosis > 50% (25.3%) and lacunar strokes (24.1%) were the most common ischemic stroke etiologies. In comparison, non-MENA countries displayed an onset-to-door time of 5:50 h (2:00–18:45), a median of NIHSS 6 (3–14), and a median age of 66 (56–76), with other large vessel disease and cardiac embolism as the main ischemic stroke etiologies. Hemorrhagic strokes (10%) were less common compared to non-MENA countries (13.9%). In MENA, only a low proportion of patients (21%) was admitted to stroke units. Conclusions MENA patients are slightly younger, have a higher prevalence of diabetes and slightly more severe ischemic strokes, commonly of atherosclerotic or microvascular etiology. Admission into stroke units and long-term follow-up need to be improved. It is suspected that cardiac embolism and atrial fibrillation are currently underdiagnosed in MENA countries.

Objective We aimed to describe the baseline clinical characteristics and fracture history of patients taking teriparatide in routine clinical practice in the Middle East (ME) subregional cohort of the Asia and Latin America Fracture Observational Study (ALAFOS). Methods Herein, we report baseline clinical characteristics of patients who were prescribed teriparatide (20 µg/day, subcutaneous injection) in four participant ME countries (Saudi Arabia, United Arab Emirates, Kuwait, and Lebanon). Results The ME cohort included 707 patients mean (SD) age 69.3 (11.6) years. Mean (SD) bone mineral density (BMD) T-scores at baseline were −3.13 (1.28) for lumbar spine, −2.88 (0.94) for total hip, and −2.65 (1.02) for femoral neck. Osteoporotic fractures after age 40 years were reported in 45.8% (vertebral fracture 14.4%, hip fracture 18.4%) and comorbidities in 57.4% of patients. Before starting teriparatide, 19.9% of patients took other osteoporosis medications. The median (Q1; Q3) EuroQoL 5-Dimension 5-Level visual analog scale score for perceived overall health status was 70 (50; 80). Mean (SD) worst back pain in the previous 24 hours was 4.0 (3.2) using a 10-point numeric rating scale. Conclusion This analysis indicated that in ME countries, teriparatide is usually prescribed to patients with low BMD and high comorbidities, with prior fractures.
