Preliminary investigation of interdisciplinary trainee’s improvement in knowledge about autism spectrum disorder

Purpose Individuals with autism spectrum disorder (ASD) often receive services from a variety of professionals. However, not all providers receive adequate training in ASD. The Leadership Education in Neurodevelopmental and Related Disabilities (LEND) program includes a core competency of increasing knowledge about neurodevelopmental and related disabilities. This study attempted to assess trainees’ ASD knowledge and self-reported confidence in working with individuals with ASD and sought to understand if training through the LEND program increases these competencies. Additionally, the purpose of this study is to determine factors that predict ASD knowledge and self-reported confidence in providing services to this population, specifically in an interdisciplinary trainee sample. Design/methodology/approach Participants were 170 interdisciplinary LEND trainees during the 2017–2018 academic year. Participants across the USA completed online pre- and posttraining surveys. The survey included demographics, ASD knowledge, questions assessing training experiences, perceived ASD knowledge and self-reported confidence. Findings A one-way analysis of variance determined that there was a statistically significant difference in measured ASD knowledge across disciplines F(7, 148) = 5.151, p < .001. Clinical trainees (e.g. psychology, pediatrics and speech) exhibited more measured ASD knowledge than nonclinical trainees (e.g. neuroscience, legal). Additionally, training experiences, self-reported confidence and perceived ASD knowledge were predictors of measured ASD knowledge. Moreover, trainees increased their measured ASD knowledge, self-reported confidence and had more experiences with individuals who have ASD at the end of the training year. Originality/value These findings suggest that the LEND program may assist in the preparation of professionals to work with individuals with ASD. Training opportunities, including educational and practical experience, to train interdisciplinary providers who will work with individuals with ASD are advised.

Purpose This paper aims to explain, first of all, signal modeling steps using Poincaré, and then considering the occurred events, concept of information applying Poincaré section and information approach, the brain pattern variations in autism spectrum disorder (ASD) cases will be diagnosed. A kind of representation of electroencephalogram (EEG) signal, namely, complementary plot, in which the main characteristic is special attention to asymmetry and symmetry coexist in natural and human processes, is introduced. In this paper, a new model is provided whose variations of patterns are similar to EEG’s when the transformation parameter is changed. A significant difference between ASD and healthy cases was also observed, which could be used to distinguish between various types of systems. Design/methodology/approach Complementary plot method is one of the most proper representations for Poincaré section of complex dynamics, because, as it was said about its characteristics, it has a qualitative approach toward signal (Sabelli, 2000, 2001, 2003, 2008, 2005, Sabelli et al., 2011). Considering the special conditions of this representation, here, intersection with a circle y2 + x2 = r2 will be used; the important fact is, on the contrary to previous representations in which circular section had energy concept, here circular section considers phases. For finding trajectory intersection points, after calculating the sin and cosine of each term of EEG, plotting them in XY plane and drawing a chord between successive points of presentation transitions, then its intersections with the assumed circle are determined. But considering the sampling frequency, chords and Poincaré section, in this space, a minimum error – as the threshold – should be assumed in the program. Findings Natural and human processes are biotic (life-like) and creative (Sabelli and Galilei), and studying coexisting opposites by calculating the sine and cosine of each term in heartbeat intervals, weather variables and integer biotic series or random walk reveals an astonishingly regular mandala pattern; these patterns are not generated by random, periodic or chaotic series (Sabelli, 2005). This paper shows that in EEG of ASD children, mandala-like patterns of concentric rings are emergent in all situations (baseline – watching animation with voice and without voice) and electrode site (C3 and C4), but not in healthy individuals. The authors take the relation between sine and cosine functions as a mathematical model for complementary opposition, because it involves reciprocity and orthogonality sine and cosine are natural models for information. In fact, trigonometric analyses of empirical data to be described in this paper suggest expanding the concept of co-creative opposition to include uncorrelated opposites and partial opposites, i.e. partial agonists and partial antagonists that are neither linear nor orthogonal. Using Poincaré sections, it is shown that the difference in information and creativity of the data is the distinctive characteristic in ASD and healthy cases. Creation is the generation of novelty, diversity and complexity in complex systems. Originality/value This paper is an original paper based on cybernetic approaches for studying the variations of ASD children.

Abstract Background The association of stereotypies and tics is not rare in children with severe autism spectrum disorder (ASD). The differential diagnosis between stereotypies and tics in this patient population can be difficult; however, it could be clinically relevant because of treatment implications. Methods A total of 108 video recordings of repetitive behaviors in young patients with stereotypies in the context of ASD were reviewed by a movement disorders expert and a trainee, in order to assess the prevalence of possible co-morbid tics. The Modified Rush Videotape Rating Scale (MRVS) was used to rate tic frequency and severity. Results Out of 27 patients with stereotypies (24 males; mean age 14 years), 18 (67%) reported possible tics. The most frequently observed tics were eye blinking, shoulder shrugging, neck bending, staring, and throat clearing. The mean MRVS score was 5, indicating mild tic severity. The only significant difference between patients with tics and patients without tics was the total number of stereotypies, which was higher in the subgroup of patients without tics (p = 0.01). Conclusions Expert review of video-recordings of repetitive behaviors in young patients with ASD and stereotypies suggests the possibility of a relatively high rate of co-morbid tics. These findings need to be integrated with a comprehensive clinical assessment focusing on the diagnostic re-evaluation of heterogeneous motor manifestations.

Abstract Background Although approximately 23% of anorexia nervosa (AN) patients have concomitant autism spectrum disorder (ASD), it is clinically difficult to determine ASD coexistence in patients with eating disorders. Restrictive AN is more common in younger patients and self-induced vomiting usually appears during adolescence/young adulthood, in order to prevent gaining weight caused by overeating. However, some patients are tolerant of weight gain even if they start overeating. It is important to understand the essential difference between those who vomit and those who do not vomit. In this study, we hypothesised that the absence of self-induced vomiting may be associated with the presence of ASD and aimed to assess the presence of ASD traits in each eating disorder (EDs). Clarifying this association helps to consider the coexistence of ASD in the clinical setting and can lead to the next detailed ASD evaluation, and as a result, helps to determine the appropriate treatment and support individually. Methods We retrospectively evaluated 43 females aged 15–45 years who attended Chiba University Hospital between 2012 and 2016 using the Eating Disorder Examination Questionnaire (EDE-Q) and Autism-Spectrum Quotient (AQ) to quantify the severity of the EDs and to identify whether ASD traits were present. Results There was no difference in the AQ score between bingeing-purging type AN and restricting type AN. However, there was significant difference in the AQ score between bulimia nervosa and binge EDs (BED). Of the 4 ED subtypes, BED had the highest ASD traits. The non-vomiting group with illness duration < 4 years had a significantly higher AQ communication score than the vomiting group with illness duration ≥4 years. Conclusions There was a difference in the AQ score by the presence or absence of self-induced vomiting. The results of this study suggest an association between high scores on AQ and non-vomiting. Thus, evaluation of patients for the absence of self-induced vomiting while assessing them for EDs may help us to understand the association with ASD traits.

Purpose Although individuals with an autism spectrum disorder (ASD) represent a small proportion of forensic psychiatric patients as a group they present with specific difficulties and needs. There is also evidence that if detained individuals with an ASD experience particular difficulties within custodial environments as a result of a mismatch between the difficulties associated with their ASD and the environmental demands. The purpose of this paper is to explore the experience of individuals with an ASD admitted to a high secure psychiatric care (HSPC) hospital. Design/methodology/approach Using both a semi-structured interview and a quality of life self-report measure (the Lancashire Quality of Life Profile) the experiences and views of seven patients with an ASD detained in one HSPC hospital were qualitatively explored. Findings Whilst a diverse range of negative and positive aspects of being within HSPC were identified by patients interviewed, those with prison experience thought HSPC was a less stressful environment with more therapeutic opportunities. As a group, patients with an ASD reported a similar or significantly better quality of life in many domains (global, leisure, financial and living situation) compared to other detained forensic patient groups. Practical implications Although most patients with an ASD interviewed reported positive experiences, there are a number of practical improvements that could be made within the hospital to reduce experienced stress levels and perhaps improve therapeutic outcomes. Originality/value Within the context of the Department of Health's autism strategy (2010) and subsequent update think autism (2014), the survey highlights continued ASD awareness training for staff as important. In responding to the risks and needs of individuals with an ASD in HSPC there is further support for the development of an ASD specialist service.

Purpose Patients with autism spectrum disorder (ASD) present with specific assessment, specific difficulties, needs and therapeutic issues and therefore are a challenging group for forensic services. Given the challenge that individuals with ASD present to forensic services, the suggested increase in the number of this group within this setting and the relatively little amount of research which suggests they face a number of difficulties within the prison environment, the purpose of this paper is to identify and review all the studies which have been carried out investigating any aspect of ASD in relation to secure hospital settings. Design/methodology/approach Seven internet-based bibliographic databases were used for the present review. The review followed the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. Findings A total of 12 studies were included in this review; 3 looked at the prevalence of ASD in secure psychiatric hospitals. One study evaluated the clinical utility of the AQ screening tool to assess self-reported autistic traits in secure psychiatric settings. Three explored any type of characteristics of patients with ASD detained in secure psychiatric hospitals. One study investigated the experiences or quality of life of patients with an ASD detained in secure psychiatric care. Two studies investigated awareness, knowledge and/or views regarding patients with ASD held by staff working within secure psychiatric hospitals. Lastly, three studies (one of which was also included in the prevalence category above) looked at the effectiveness of interventions or treatment of patients with ASD in secure psychiatric hospitals. Clinical recommendations and future research directions are discussed. Originality/value To the author’s knowledge, this is the first review to explore what research has been carried out looking specifically at patients with ASD in relation to secure forensic settings.

Gestures play an important role in communication, but for populations with atypical language development, gestures are often an area of significant difficulty, especially for individuals with Autism Spectrum Disorder (ASD). Research on younger children with ASD show a significant delay in gesture production, as well as a complete absence of certain types of gestures. However, previous work leaves open the question of whether the frequency and variation of gesture production is impaired also in adolescents with ASD. Moreover, very little is known about the semantic integration of speech and gesture in this population. In the present study, 5 adolescents with ASD and 5 typically-developing adolescents completed a narrative task, in which they were asked to retell a story of 4 animated cartoon clips. The results show that while there is no significant difference between the two groups in the frequency of gesture production and the types of gestures they produce, the frequency of gesture production in the individuals with ASD significantly depends on their verbal working memory skills. Moreover, it seems that many impairments that were confirmed by earlier studies for ASD children in early and late childhood seem to be mere delays that get resolved over time, such as the usage of supplementary gestures.

Abstract Background Autism spectrum disorder is a condition related to brain development that affects a person’s perception and socialization, resulting in problems in social interaction and communication. It has no single known cause, yet several different genes appear to be involved in autism. As a genetically complex disease, dysregulation of miRNA expression and miRNA–mRNA interactions might be a feature of autism spectrum disorder. The aim of the current study was to investigate the expression profile of circulating miRNA-128, miRNA-7 and SHANK gene family in ASD patients and to assess the possible influence of miRNA-128 and miRNA-7 on SHANK genes, which might provide an insight into the pathogenic mechanisms of ASD and introduce noninvasive molecular biomarkers for the disease diagnosis and prognosis. Quantitative real-time PCR technique was employed to determine expression levels of miRNA-128, miRNA-7 and SHANK gene family in blood samples of 40 autistic cases along with 30 age- and sex-matched normal volunteer subjects. Results Our study revealed a statistical significant upregulation of miRNA-128 expression levels in ASD cases compared to controls (p value < 0.001). A statistical significant difference in SHANK-3 expression was encountered on comparing cases to controls (p value < 0.001). However, miRNA-7 expression showed no significant difference between the studied groups. Conclusions MiRNA-128 and SHANK-3 gene are emerging players in the field of ASD. They are promising candidates as noninvasive biomarkers in autism. Future studies are needed to emphasize their pivotal role.

PurposeAutism spectrum disorder (ASD) is an exclusion criterion for one of the two attachment disorders in the DSM 5. However, previous findings indicate that ASD and attachment disorder are unrelated conditions and may co-occur. The purpose of this paper is to explore the diagnostic assessment of an adolescent male with ASD, intellectual disability (ID), severe challenging behaviour and a suspected attachment disorder.Design/methodology/approachCase study methodology was chosen because of its suitability in the exploration of complex clinical phenomena where prior knowledge is sparse.FindingsIt was possible to identify symptoms of attachment disorder in a case involving ASD, ID, anxiety and severe challenging behaviour. The Disturbances of Attachment Interview was particularly useful in this assessment, as was assessment of ASD symptoms and developmental history. Differentiating the two attachment disorders proved challenging.Research limitations/implicationsThere is a need for further research in ASD and attachment disorders not limited by current diagnostic categories.Practical implicationsCo-occurring symptoms of attachment disorder may be identified in individuals with ASD and ID, and exploration of these symptoms in assessments of children and adolescents with ASD/ID and challenging behaviour may be beneficial.Originality/valueThe study adds to previous findings on attachment disorder in ASD, demonstrating that identification of attachment disorder is possible even in the presence of a highly complex clinical picture involving severe challenging behaviour. It may also assist other clinicians in identifying and making more accurate assessment of attachment disorder in ASD and ID.

Purpose Students with autism spectrum disorder (ASD) often have challenging behaviours that can create significant teaching challenges in group settings such as in mainstream pre-school inclusive settings. This study aims to identify and understand, from the perspectives of mainstream pre-school teachers in Singapore, the type of common challenging behaviours observed in pre-schoolers (mean age = 4.5 years, SD = 1.1, range = 3-6) with ASD. Design/methodology/approach Participants consisted of 62 teachers (mean age = 35.8 years; SD = 10.8) teaching in mainstream pre-school centres located in Singapore completed an open-ended survey. Twelve codes were generated and served as guidelines for coding the qualitative data. Codes with similar themes were then grouped to form four construct categories: social communication, restricted and repetitive behaviours, attention and learning and others. Findings Results indicated that difficulty with change/transition and social/peer interaction were the most concerning issues across all settings, whereas “lesson time” and “circle time” were the two most common activities/settings in which the concerns were reported by teachers. Originality/value Findings from this paper may add to the limited research looking at the challenging behaviours exhibited by children with ASD in mainstream pre-school settings in Singapore. Understanding the type of challenging behaviours presented in pre-schoolers with ASD and the activities/settings in which these behaviours occur may help the teachers to link the difficulties to specific interventions to further support their inclusion in a mainstream pre-school setting.
