Purpose Altitude illness can be a severe to even life-threatening condition for individuals ascending to altitudes higher than 2,500 m without proper acclimatization, a process that requires at least a couple of days. Considering disaster scenarios in high-altitude settings where humanitarian aid workers need to be deployed immediately, altitude illness may pose a relevant risk factor. The paper aims to discuss these issues. Design/methodology/approach Population data on people living in high-altitude risk areas worldwide were matched with data on natural hazards in these areas. Findings Globally, more than 140 million people live in the risk zone for altitude illness and at least 19 cities with a population of more than 100,000 people exist above 2,500 m. These cities are situated in countries that are highly vulnerable to various natural hazards, and most of these cities have already been affected by hazardous events in the past. The relevance of considering altitude illness is underlined by a report on the emergency operation after the 2010 earthquake in Yushu County, China, where at least 200 aid workers had to be withdrawn due to this condition. Originality/value There are possible disaster scenarios where altitude illness can become a serious issue for aid workers, however, the only available preventive measures are not feasible with proper acclimatization requiring a couple of days and chemoprophylaxis not being 100 percent efficient. Therefore, this work intends to raise awareness within the humanitarian community for and to proactively engage with the topic of altitude illness in order to ensure the security and well-being of staff.

In this chapter, we will focus on the importance of caring for the family of a child with a life-limiting condition (LLC) or life-threatening condition as a unit, each of the family members being integral to the well-being and care of the others. We recognize that the family unit itself is embedded within a wider context including the health and social care system, and more broadly within its society and culture. We discuss the concept of family, exploring the impact of having a child with an LLC, and how families adjust to this. We then expand on considerations about how to offer effective and timely support and help. We have included parents’ narratives with the aim of adding depth to the discussion, and in recognition of the truth of families’ own experiences.

High-altitude pulmonary edema potentially is fatal. Adults with unilateral absence of a right pulmonary artery are particularly susceptible to high-altitude pulmonary edema. The occurrence of high-altitude pulmonary edema was documented in a child with congenital absence of the right pulmonary artery. Improvement occurred only upon descent to low altitude. Physicians should be aware of this life-threatening condition in children ascending to high altitude, particularly in individuals with unilateral absence of a pulmonary artery.

Colic is a common and potentially life-threatening condition of horses. Multiple risk factors have been previously identified and it is known that a careful management routine can help reduce colic rates. The British military working horse population represents a unique cohort of horses that are intensively managed with a strict regimen. This retrospective study examined the incidence and mortality rate of colic within this population, as well as the signalment of affected horses, and compared these with the general population. Data for 717 horses over a five-year period (2008–2012) were analysed. Of these, 163 horses (22.7 per cent) experienced 267 colic episodes and 13 horses (1.8 per cent) died because of colic. Recurrent colic was experienced by 35 per cent (57/163) of horses. The incidence of colic was 11.1 episodes per 100 horse-years and of colic-related death was 0.5 deaths per 100 horse-years. Horses purchased from mainland Europe were more likely to suffer from colic (OR 4.6; P<0.001) and from recurrent colic (OR 6.0; P=0.005) than horses purchased from Ireland. Only 3 per cent (8/267) of colic episodes were treated surgically. It was concluded that the incidences of colic and colic-related deaths within the British military working horse population are similar to those of the general horse population.

Sibling relationships are one of the most long-lasting and influential relationships in a human’s life. Living with a child who has a life-threatening condition changes healthy siblings’ experience. This scoping review summarized and mapped research examining healthy siblings’ experience of living with a child with a life-threatening condition to identify knowledge gaps and provide direction for future research. Studies were identified through five electronic databases. Of the 34 included studies, 17 used qualitative methods, four gathered data longitudinally and 24 focused on children with cancer. Four broad themes of sibling experience were identified across studies: family functioning, psychological well-being, social well-being, and coping. Siblings experienced challenges and difficulties over the course of the child’s illness. Future research should incorporate longitudinal designs to better understand the trajectory of siblings’ experiences and focus on a wider variety of life-threatening conditions.

Prerequisites of separate criminalistic methodology (micromethods) development for investigating murders associated with occupancy of human housing are considered. Attention is drawn to the fact that among the factors that lead to the growth of crime in real estate market are social contradictions (market economy development and institution of private property, expansion of the apartment privatization market, ineffective work of law enforcement agencies, decline in citizens' well-being, etc.). The essence and mechanism of this murder type is investigated, and the main signs of their commission are highlighted. It has been determined that the mechanism for committing murders involving the occupancy of housing includes: 1) actions of the directly injured regarding of housing occupancy and provoking murder commission; 2) actions accompanied by violations by officials (housing and operational organizations, notary offices, local governments and law enforcement agencies); 3) offender actions directly related to the murder commission of the person and housing occupancy. Peculiarities of the mechanism of investigated murder committing is that methods of committing in their structure (substantive part) can unclude following actions: 1) of active nature (infliction of bodily harm, using firearms and cold weapon etc.); 2) actions of active-passive nature (leaving in danger, inadequate discharge of responsibilities for health protection, failure to provide assistance to a person in a life-threatening condition, etc.). Structural elements of a separate criminalistic methodology (micromethodics) for investigating murders associated with housing occupancy have been proposed, among which the following should be highlighted: 1) criminalistic description; 2) typical investigative situations; 3) circumstances to be clarified; 4) typical investigative versions; 5)  interaction of the investigator with the operative search units; 6) tactics of investigative (search) actions.

Purpose The Pacific islands face the highest disaster risk globally in per capita terms. Countries in the region have been affected by several recent catastrophic events, as well as by frequent natural hazards of smaller magnitude. The purpose of this paper is to quantify total disaster risk faced by Pacific island countries (PICs). Design/methodology/approach The paper evaluates the three main sources of data for quantifying risk in the region—the International Emergency Database (EMDAT), DesInventar and the Pacific Catastrophe Risk Assessment and Financing Initiative, evaluating the information available on indirect disaster impacts and their likely impacts on poverty and well-being. Findings The analysis suggests that the three available data sets contain inconsistencies and underestimate disaster risk, especially for atoll nations. It also identifies four trends with respect to changes in natural hazards that result from climate change and are likely to have the greatest long-term impact on Pacific islands. Focusing on Tuvalu, the paper also quantifies the likely consequence of some of the possible interventions that aim to reduce those impacts. Practical implications The paper’s main conclusion is that improving the systematic collection of quantitative data on disaster events should be a basic first step in improving future policy decisions concerning resource allocation and efforts to insure losses from future disasters and climate change. Originality/value While a lot of research explored disaster risk in PICs, comparative analysis of quantitative information on disasters across the diverse countries of the region is limited.

Summary. Iliac vein compression syndrome (May-Thurner syndrome – MTS) is an anatomically variable clinical condition in which the left common iliac vein is compressed between the right common iliac artery and the underlying spine. This anatomic variant results in an increased incidence of left iliac or iliofemoral vein thrombosis. It predominantly affects young women in the second or third decades of life with preponderance during pregnancy or oral contraceptive use. Although MTS is rare, its true prevalence is underestimated but it can be a life-threatening condition due to development of pulmonary embolism (PE). In this case based review the authors present three cases of MTS. All patients had been previously confirmed with PE, but despite they were admitted to hospital, diagnosed and correctly treated for PE and investigated for thrombophilia, the iliac vein compression syndrome was not suspected or investigated. With this presentation the authors would like to emphasize that MTS is mostly underdiagnosed, and it needs to be ruled out in left iliofemoral vein thrombosis in young individuals.

Background: Alveolar hemorrhage is a potentially life-threatening condition which is usually managed by the pulmonologist. When considering its etiology, there is a rare association that sets the disease into the hands of the gastroenterologist. Case presentation: We report the case of a 48 year-old female who was admitted to the intensive care unit for severe anemia and hemoptysis. On imaging, diffuse pulmonary infiltrates suggestive of alveolar hemorrhage were detected and a diagnosis of pulmonary hemosiderosis was made. She received cortisone therapy and hematologic correction of anemia, with slow recovery. In search of an etiology for the pulmonary hemosiderosis, an extensive workup was done, and celiac disease specific serology was found positive. After confirmation of celiac disease by biopsy, a diagnosis of Lane-Hamilton syndrome was established. The patient was recommended a gluten-free diet and at 6 months follow-up, resolution of anemia and pulmonary infiltrates were observed. Conclusion: Although the association is rare, celiac disease should be considered in a patient with idiopathic pulmonary hemosiderosis. In our case, severe anemia and alveolar infiltrates markedly improved with glucocorticoids and gluten-free diet. Acute cholecystitis (AC) is a potentially life-threatening condition. LC was initially considered to be a relative contraindication for laparoscopic cholecystectomy (LC), but with increase in general expertise, early LC was recommended in selected patients1. Aprospective study of LC in grade 1 and 2 AC patients with mild to moderate inflammatory changes in the gallbladder and no significant organ dysfunction, was performed during October 2016 to July 2019. A total of 78 patients, out of 408 cholecystectomies performed during this period, were included in this study. Criteria for diagnosing AC was, recent onset of pain in right hypochondrium, fever, leucocytosis, pericholecystic fluid collections, subserosal oedema on ultrasound, pyocele and other pathological evidence of AC. Patients presented and operated within 4 days of onset of symptoms showed better results as compared to those who could be operated after 4 days and within 14 days. Five patients required conversion to open cholecystectomy because of complex adhesions in 2, critical view of safety was unachievable in 2 and in 1 for troublesome bleeding.

Cystic fibrosis (CF) is a progressive life–shortening disease caused by a mutation in the cystic fibrosis transmembrane conductance regulator (CFTR) gene leading to a dysfunctional CFTR protein. The disease affects over 70,000 patients worldwide and while many mutations are known, the F508del mutation affects 90% of all patients. The absence of CFTR in the plasma membrane leads to a dramatic decrease in chloride efflux, resulting in viscous mucus that causes severe symptoms in vital organs like the lungs and intestines. For CF patients that suffer from the life threatening F508del mutation only palliative treatment exist. PRO–CF–MED addresses the specific challenge of this call by introducing the first disease modifying medication for the treatment of the CF patients with F508del mutation. The PRO–CF–MED project has been designed to assess the potential clinical efficacy of QR–010, an innovative disease modifying oligonucleotide–based treatment for F508del patients. Partners within PRO–CF–MED have generated very promising preclinical evidence for QR–010 which allows for further clinical assessment of QR–010 in clinical trials. PRO–CF–MED will enable the fast translation of QR–010 towards clinical practice and market authorisation. PRO–CF–MED has the potential to transform this life–threatening condition into a manageable one.
