Safety attitudes, perceived organizational culture and knowledge of the physiologically deteriorating patient among mental health nurses: Cross‐sectional, correlational study

ABSTRACT Objective: To evaluate the culture of patient safety in a mental health service. Method: Cross-sectional study conducted with health professionals in a mental hospital, by applying the Safety Attitudes Questionnaire (SAQ). Descriptive and inferential analyses were performed. Results: One-hundred and three professionals participated in the study, with female predominance (64.1%) and time of performance equal to or greater than 21 years (32.4%). The nursing professionals of technical level and with statutory work regime were the most participative, 54.4% and 52% respectively. The total score was 69 points. The domain that reached the highest score was Job satisfaction (80 points) and the lowest was Working conditions (57 points). The statutory professionals and those with longer professional experience obtained better scores in the perception of safety culture. Conclusion: The result of safety culture was below the recommended, indicating the need for strengthening this construct in mental health hospitals.

Mental health nurses, together with psychiatrists, are the healthcare professionals who display the highest levels of empathy and the best attitudes towards patients with mental disorders. However, burnout is a common problem among these professionals. The aim of our study is to describe the association between empathy, burnout, and attitudes towards patients with mental disorders among mental health nurses in Spain. A descriptive cross-sectional design was used involving a sample of 750 specialist nurses working in mental health facilities in Spain. An intentional, non-probability, non-discriminative, exponential snowball sampling method was used. The Jefferson Scale of Empathy, the Maslach Burnout Inventory, and the Community Attitudes towards Mental Illness Inventory were used to measure the study variables. A positive correlation was observed between empathy and all the study variables, with the exception of the personal accomplishment dimension of burnout and the social restrictiveness and authoritarianism dimensions of attitudes towards mental illness, where a negative relation was observed. Our findings suggest that empathy is associated with an increase in positive attitudes towards patients with mental disorders, decreasing associated stigma, but did not act as a protective factor against burnout in the study sample.

Several studies report that psychiatric nursing is a highly stressful occupation. The ways that nurses use in order to deal with stressful situations have a serious effect on their psychological mood and their health status. The purpose of this study was to investigate the coping strategies in predicting of depression and anxiety among mental health nurses working in public psychiatric hospitals. A descriptive, cross-sectional study was conducted in Athens, Greece from April to May 2017. A questionnaire consisting of the socio-demographic and work-related characteristics, the Patient Health Questionnaire-2, the Generalized Anxiety Disorder-2, and the 38-items Ways of Coping Questionnaire - Greek version, was completed by a total of 110 mental health nurses. Univariate and multivariate analyses were performed using the logistic regression model. Coping strategies focused on the problem (positive re-evaluation, positive approach, problem solving, and seeking social support) were the most commonly used by the mental health nurses. Strategies focused on emotion (prayer/daydream, prayer, avoidance/escape, resignation, and denial) were positively associated with depression and anxiety outcomes. Also, the coping skills of problem solving (Adjusted Odds Ratio, AOR=0.402), and seeking social support (AOR=3.719) were significant predictors only for elevated anxiety symptoms. The results from the study demonstrated the importance of coping behaviours in mental health problems of psychiatric nurses.

Objective: The goal of this study was to analyse the attitudes and perceptions of emergency and mental health nurses through the validation of the SM-GIBED scale in specialised care in Spain on alcoholics and other drug-dependent patients. Design and Setting: This cross-sectional study was developed using the Spanish hospital version of the Seaman-Mannello scale to denominate the SM-GIBED scale. Participants: 170 Emergency and Mental Health Nursing from five Spanish Hospitals. Intervention: Self-administered questionnaire to analiyse the perceptions and attitudes about the drug addict and the alcoholic. Primary and Secondary Outcome Measures: A descriptive and inferential analysis of the study variables was carried out. A psychometric analysis was performed to validate the scale. Results: A total of 170 questionnaires were collected from 257 healthcare workers. Overall, 99.1% of the participants had contact with drug-dependent patients during their professional experience. Nearly 75% had difficulties in treating them. The psychometric analysis of the SM-GIBED scale in the Spanish context obtained values of KMO = 0.655 and Bartlett's test p < 0.000. Cronbach's alpha of 0.738 was obtained from the reliability analysis. A reliability analysis of each of the SM-GIBED questions found no case with an alpha lower than 0.71. In conclusion, positive aspects include an ingratiating attitude and subject-to-subject communication when nurses self-define as empathic and non-paternalistic. Among the negative aspects, there is a lack of communication skills and assertiveness with these patients. This highlights a certain degree of resignation and dissatisfaction when working with drug addicts.

This study was conducted to assess prevalence of occupational hazards and illnesses among mental health Nurses in Federal Neuropsychiatric Hospital, Calabar, Cross River State. Three research questions were raised and one hypothesis formulated to direct the course of the study. The study adopted cross-sectional descriptive survey design to study 87 mental health nurses who were selected from the Hospital using stratified random sampling technique. Instrument for data collection was a self-structured questionnaire designed in consonance with the study’s objectives. The validity of the instrument was ensured and high reliability coefficient was obtained for the respective variables of the study. Data collected were analyzed using descriptive statistics while hypothesis was tested using Chi-square statistics significant at 0.05. Findings of the study revealed that excessive workload, assaults and violence were the major hazards nurses were exposed to, while occupational illnesses including body pains, stress/psychosocial problems, and back pain were reported among the nurses. Nurses were found to exhibit good practice of prevention of occupational hazard, and there was a statistically significant relationship between years of work experience and practice of prevention of occupational hazard among Nurses in the Hospital. Prominent factors found associated with occupational hazard were; insufficient safety gadgets and equipment in the Hospital, long working hours, lack of adequate safety training, excessive workload, and inadequate staff strength in the Hospital. Based on these findings, it was recommended that government should embark on recruitment of more Nurses to reduce work load and enhance compliance to infection control practices in the Hospital.

Purpose The purpose of this study was to assess burnout, resilience and the association with safety culture in nurses working in mental health institutions in Jazan government Hospital. Design/methodology/approach A cross-sectional survey using convenience sampling, with 119 nurses from the Jazan hospital between June and August 2018 was used. Findings The results of this study showed that 45.6% of the participants experienced a high level of emotional exhaustion, 36.5% reported a high level of depersonalisation and 15.9% reported high personal achievement. The high level of burnout and its dimensions have a negative effect on patient safety and resilience. Originality/value It is particularly important to assess burnout among mental health nurses, resilience and its association with safety culture. Although this study will add to a small body of knowledge, it will also be able to provide policymakers with evidence as how best to reduce burnout among nurses delivering mental health care in Saudi Arabia.
