ObjectivesTo explore the pharmaceutical care needs of, and service provision to, older people with sensory impairment (visual, hearing and dual impairment) on prescribed polypharmacy (≥4 medicines) in Scotland.DesignInterviews were conducted with older people with sensory impairment and community pharmacy personnel, which informed the content of a subsequent national cross-sectional survey of community pharmacists.SettingScotland, 2015–2016.ParticipantsOlder people with sensory impairment and community pharmacy personnel.ResultsInterviews were completed with 23 older people with sensory impairment (dual impairment n=13, visual or hearing impairment n=5 of each) and 30 community pharmacy personnel from eight of 14 Scottish Health Boards. A total of 171 survey responses were received.Older people reported that they did not always disclose their sensory impairment to pharmacy personnel. They also reported that medicines were difficult to identify particularly when their name, shape or colour changed. Pharmacy personnel relied on visible cues such as white canes or guide dogs to identify visual impairment and suggested that hearing loss was less visible and more difficult to identify. Many assistive aids in support of medicine management, such as dosette boxes, seemed inadequate for complex medication regimens. Few community pharmacy personnel reported receiving training in the care of people with sensory impairment.ConclusionsThis is the first comprehensive, multistakeholder, in-depth exploration of the pharmaceutical care needs of older people with sensory impairment. Strategies are needed to enable people with sensory impairment to disclose their impairment to pharmacy personnel (and other healthcare providers). Community pharmacy personnel require training to deliver person-centred pharmaceutical care for older people with sensory impairment particularly regarding communication with individuals in this vulnerable population.

We examined the influencing factors of the undermet care needs of the Chinese disabled oldest old people when their children are both caregivers and are themselves older people. Data were obtained from a cross-sectional survey: the Chinese Longitudinal Healthy Longevity Survey (CLHLS) in 2018. The study participants included 1617 disabled oldest old people whose primary caregiver were their children or children-in-law and were aged 60 years and over. The results showed that the prevalence of undermet needs remained high, with 49.6% disabled oldest old people reporting undermet care needs. Binary logistic regression analysis revealed that living in a rural area (OR = 1.309, 95% CI = 1.133–1.513) and a higher frailty index (OR = 1.103, 95% CI = 1.075–1.131) were significantly positively associated with higher odds for undermet care needs, while a higher annual household income (OR = 0.856, 95% CI = 0.795–0.923), more financial support from children (OR = 0.969, 95% CI = 0.941–0.997), higher care expenditures (OR = 1.044, 95% CI = 1.002–1.088), better caregiver’s performance (OR = 0.282, 95% CI = 0.196–0.407) and sufficient income to pay for daily expenses (OR = 0.710, 95% CI = 0.519–0.973) were significantly inversely associated with higher odds for undermet care needs. This evidence suggests the importance of policies to establish a community-based socialized long-term care system and supporting family caregivers of the disabled oldest old people.

Objective: To calculate the return on investment (ROI) for a targeted medication intervention program developed by corporate management of a community pharmacy. Design: Retrospective analysis and cross-sectional survey. Setting: Regional community pharmacy chain in North Carolina. Participants: Targeted medication interventions completed from February 1, 2010, to July 31, 2010, were included in the retrospective analysis. Community pharmacists employed by the pharmacy chain that completed the questionnaire were included in the cross-sectional analysis. Intervention: Targeted medication intervention services were provided to the patient and documented by the pharmacist. Main Outcome Measure: The ROI for a community pharmacist-provided targeted medication intervention program. Results: Of the 180 pharmacists, 69 completed the questionnaire (38% response rate). The average time to complete one targeted medication intervention was calculated to be 22.63 minutes. The total cost for providing a targeted medication intervention program during the study time frame was $15 760.86. Total revenue was $15 216.00; therefore, the program resulted in an ROI to the pharmacy chain of negative 3%. Conclusion: This 6-month study resulted in an ROI to the pharmacy chain of negative 3%. Under the current reimbursement model, for this program to break even, the average time to complete one targeted medication intervention must equal 21.85 minutes or less.

Purpose To examine the vaccine-related beliefs and behaviors associated with likely hesitancy toward vaccination against coronavirus disease 2019 (COVID-19) among nonelderly adults. Methods A cross-sectional survey was conducted in June 2020. Responses were sought from Tennessee adults 18 to 64 years of age who were not healthcare providers. The survey instrument focused on vaccine-related beliefs, prior and planned influenza and pneumococcal vaccine use, and attitudes toward receiving a COVID-19 vaccination. Inferential statistics assessed survey responses, and logistic regression determined predictors of the likelihood of COVID-19 vaccination. Results A total of 1,000 completed responses were analyzed (a 62.9% response rate), and respondents were mostly White (80.1%), insured (79.6%), and/or actively working (64.2%); the sample was well balanced by gender, age, income, and political leaning. Approximately one-third (34.4%) of respondents indicated some historical vaccine hesitancy, and only 21.4% indicated always getting a seasonal influenza vaccination. More than half (54.1%) indicated at least some hesitancy toward vaccination against COVID-19, with 32.1% citing lack of evidence of vaccine effectiveness as the leading reason. COVID-19 vaccine hesitancy was more likely among those with more moderate (odds ratio [OR], 2.51; 95% confidence interval [CI], 1.749-3.607) or conservative (OR, 3.01; 95% CI, 2.048-4.421) political leanings, Black Americans (OR, 1.80; 95% CI, 1.182-2.742), and residents of nonmetropolitan areas (OR, 1.99; 95% CI, 1.386-2.865). Conclusion Subgroups of the population may prove more challenging to vaccinate against COVID-19, requiring targeted approaches to addressing hesitancy to ensure more-vulnerable populations are adequately covered.

INTRODUCTION: Community pharmacists play an important role in increasing patient understanding of medication use. Lack of resources to facilitate communication with non-English speaking (NES) patients may be a communication barrier. AIM: To identify obstacles and coping strategies of community pharmacists when counselling NES patients in Auckland’s North Shore and West Auckland. METHODS: A cross-sectional survey of 46 community pharmacies in West Auckland and the northern Auckland region was carried out in February 2009. RESULTS: Community pharmacists frequently counsel NES patients (65% reported at least once a week). Use of bilingual staff was the most commonly employed strategy (78% of respondents) to communicate with these customers. Pharmacies that reported serving NES clients at least daily all had bilingual staff, compared with 70% of pharmacies with less frequent NES contact (p=0.017). No pharmacists reported using professional interpreting services. In our sample, telephone interpreting was the most preferred (63% of respondents) method of communicating with such patients, assuming that further services were made available. DISCUSSION: Community pharmacists frequently serve NES patients, with limited access to interpreting services or translated resources. Although pharmacists have, in some way, adapted to the needs of their patients, our survey suggests that accessible professional interpreting services would further improve pharmacist/NES client interaction. Introduction: Expansion of telemedicine enabled healthcare access during the COVID-19 pandemic. In response to in-person visit restrictions, our institution trained >1,000 clinicians in telemedicine. Little is known about telemedicine-naïve pediatric healthcare provider's perceptions as they adopted telemedicine practice.Methods: We conducted a cross-sectional survey of clinicians after expanding telemedicine practice at an independent children's hospital. The survey assessed experience with, concerns about, and intentions to continue telemedicine. Outpatient providers were included if they were first trained for telemedicine in response to COVID-19 and conducted at least one video visit, 3/21/2020–6/30/2020. Descriptive statistics were calculated; perceptions were compared across telemedicine activity level quartiles (based on proportions of visits delivered by video in June 2020) using Fisher's exact tests.Results: Of 609 survey responses, 305 (50.1%) met inclusion criteria, representing various roles and disciplines. Over half (54.1%) conducted >20 video visits 3/21/2020–6/30/2020. More than 75% of providers found telemedicine easy to learn. Providers with greater proportions of video visits in a typical week in June reported greater ease of incorporating telemedicine into clinical practice and greater intention to continue telemedicine practice in 6 months. Nearly all providers endorsed concerns. Patient care experiences reinforced technology-related concerns and alleviated liability and privacy concerns. Payer reimbursement was the leading influencer of anticipated future use of telemedicine.Discussion: Providers who conducted more telemedicine encounters reported greater ease of incorporating telemedicine into practice. Provider concerns were influenced by patient care experiences. Targeted training and quality improvement strategies are needed to sustain a robust post-pandemic telemedicine program.

The World Health Organization is promoting community-based services to end HIV/AIDS by 2030. In Nigeria, studies on community pharmacists’ involvement in HIV testing services (HTS) and antiretroviral (ARV) medication therapy management (MTM) are scarce, and no study had evaluated the clients’ opinion on community pharmacy HTS. We assessed the community pharmacists’ involvement in HIV/AIDS services and clients’ opinions of community pharmacy HTS. Semi-structured questionnaires on a five-point Likert scale were administered to 701 selected community pharmacists and 5840 clients in southwest Nigeria in 2019. Data were analyzed with descriptive and inferential statistics. Response rates were 68.6% and 69.8% for community pharmacists and clients, respectively. Only 18.5% of community pharmacists offered rapid HTS and 30% refills of ARV. The total score of their involvement on the HTS was 20.94 ± 17.521 (range 8–40; midpoint 24) and on ARV/MTM was 22.98 ± 19.61 (range 9–45; midpoint 27), while 91% were willing to participate in training. Barriers to integrating services into practice were lack of clinical tools (46.8%), lack of collaboration with other healthcare professionals (39.1%), and lack of training on HIV/AIDS services (36.2%) among others. Also, 77% of the clients were willing to participate in community pharmacy HTS, and about 83% of them responded that knowing their HIV status will help them to take necessary precautionary actions. Community pharmacists’ involvement in HIV/AIDS services was low. However, they showed willingness to participate in training to improve services. Also, clients were willing to receive community pharmacies HTS to know their status.

Government regulations regulate pharmaceutical care standards at pharmacies to ensure the quality of pharmaceutical care to patient. The impact of not implementing a good pharmaceutical service activity is that medication errors can occur in the pharmaceutical service process. The study aimed to determine if there is any correlation between customer satisfaction and the application of the standards of pharmaceutical care in Rembang. This is a cross sectional survey research with purposive sampling technique. Data was obtained by distributing questionnaires filled by community pharmacists and patients attending community pharmacies. The study was conducted at 4 (four) community pharmacies in Rembang. Data were analyzed using Pearson correlation. Questionnaires were filled by 4 pharmacists’ measuring aspects of facilities, infrastructure and services. The level of satisfaction of 94 community pharmacies’ patients were measured in 5 dimensions i.e. reliability, assurance, tangibles, empathy and responsiveness. The quality of pharmaceutical care scoring and patients’ satisfaction showed no relationship p (95%; 2 tailed) = 0.503.

Background: Pharmaceutical care (PC) is the value of a practice that includes recognizing and solving medication therapy issues to improve patient outcomes. Studies regarding this issue in Yemen are very limited. Objectives: The study aims to assess the perception of community pharmacists to the concept of Pharmaceutical care and the obstacles that limit the provision of pharmaceutical care. Method: A descriptive cross-sectional study of community pharmacy dispensers (CPDs) was conducted among pharmacists and pharmacy technicians working in community pharmacies in different areas of South Yemen from September 2019 until December 2019. The structured self-administered questionnaires consist of four sections dealing with the demographics of participants and their understanding of the concept of pharmaceutical care and their attitudes and obstacles to the implementation of pharmaceutical care in Yemen. The collected data was analyzed using Statistical Package for Social Sciences (SPSS) version 25.0 Results: A total of Two-hundred pharmacists were included in this study. The result revealed that there were a reasonable number of experts (over 5 years' experience) and educated pharmacists( Diploma and bachelor’s degree) available for counseling patients about specific issues in the community pharmacies. Interestingly, all the respondents had good knowledge of pharmaceutical care and the majority of the pharmacists (87.5%) had a favorable attitude toward pharmaceutical care and their role in the health care system. The overall results relating to the perceived frequency of community pharmacy services provision was lower than (50%) in most statements. The result indicated a gap in the community pharmacists' communication with the patients. There are several barriers impedes the active and effective involvement of community pharmacists such as; insufficient communication with physicians, lack of physical space for pharmaceutical care, the slow introduction of pharmacists’ law, and others. Conclusion: The results revealed that pharmacists' understanding of the pharmaceutical care concept is good despite that there was no real implementation of pharmaceutical care.
