REPEATABILITY OF TRAPEZIUS MUSCLE TONE ASSESSMENT BY A MYOMETRIC METHOD

The goal of this paper is to examine the trapezius muscle tone by measuring simultaneously using Myoton-2 myometer i.e., the natural oscillation frequency, stiffness and the elasticity of the trapezius muscle. With this method, the mechanical response of the muscle, to a short applied mechanical impulse, is registered by an acceleration probe. From the acquired damped natural oscillation waveform, the frequency (Hz), the stiffness (N/m) and the logarithmic decrement of damping (characterizing tissue's elasticity) are calculated, quantifying the functional state of the muscle. The trapezius muscle on both sides of the body was tested in twenty adult women by two investigators using the Myoton-2 myometer. During the measurements, the subjects were in a relaxed sitting position. The Bland and Altman graphical test, comparing the differences of the measurements of two investigators, was used for assessing the inter-observer repeatability. The registered values for the trapezius muscle tension, stiffness and elasticity are varying between the tested subjects, but the intra-class correlation coefficient (ICC) was near 1 for three muscular properties, showing that the variation within the subject (due to the investigator) is negligible, compared with the variation between the subjects.

Two aspects of body image (body perception and body acceptance), muscle tone at test, and disposition to perceive positively connoted stimuli (tickle) were studied with 35 female subjects. Our hypothesized relation of muscle tension and body perception was confirmed by an inverse correlation between these variables. Also evident was the link of the good acceptance of the body with the disposition to perceive pleasurable stimuli (tickle perception) longer and the link of level of body awareness and resistance to change (latency to tickle). Also a direct relationship between body perception and body acceptance emerged.

AbstractAcceleromyography at the adductor pollicis located in a distal part of the body may not reflect the degree of neuromuscular blockade (NMB) at the proximally located muscles manipulated during lumbar surgery. We investigated the usefulness and characteristics of acceleromyographic monitoring at the trapezius for providing moderate NMB during lumbar surgery. Fifty patients were randomized to maintain a train-of-four count 1–3 using acceleromyography at the adductor pollicis (group A; n = 25) or the trapezius (group T; n = 25). Total rocuronium dose administered intraoperatively [mean ± SD, 106.4 ± 31.3 vs. 74.1 ± 17.6 mg; P < 0.001] and surgical satisfaction (median [IQR], 7 [5–8] vs. 5 [4–5]; P < 0.001) were significantly higher in group T than group A. Lumbar retractor pressure (88.9 ± 12.0 vs. 98.0 ± 7.8 mmHg; P = 0.003) and lumbar muscle tone in group T were significantly lower than group A. Time to maximum block with an intubating dose was significantly shorter in group T than group A (44 [37–54] vs. 60 [55–65] sec; P < 0.001). Other outcomes were comparable. Acceleromyography at the trapezius muscle during lumbar surgery required a higher rocuronium dose for moderate NMB than the adductor pollicis muscle, thereby the consequent deeper NMB provided better surgical conditions. Trapezius as proximal muscle may better reflect surgical conditions of spine muscle.

Aim: Capturing body images through digital photography method for calculation of quadriceps angles and pelvic position in clinical assessment is an inexpensive method. The aim of this study is to evaluate the reliability of measuring Q-angle and pelvic tilt angle with digital camera and Microdicom software. Place and duration: University of Lahore and Tehran University of medical sciences, from June 2020 to June 2021 Methods: Bilateral lower limbs of fifteen healthy asymptomatic subjects were considered for calculations of desired angles. A 13-megapixel camera was used to capture images of quadriceps angle (q-angle) measurement in an anterior view and for pelvic angle measurement image in lateral view along with specific landmark markers on the body were captured. Images were transferred to Microdicom software for calculating desire angles. Three trails of procedures and Intra-class Correlation Coefficient (ICC) with 95% Confidence Interval (CI), Munro’s classification for reliability coefficients were used for the description of degrees. Standard error of measurement (SEM) was also calculated. Results: The Intra-class correlation coefficient values for right side Q- angle was 0.949(0.878-.981) and for left side it was 0.924(0.819-0.972) and <0.005 significant p-value. The ICC results for measuring pelvic title angle in the right side .990 (.975-.996) and left legs were .991 (.974-.997). Showing that the results were highly reliable and significant (p<0.005). Conclusions: The q-angle and pelvic angle measured with digital image have excellent consistency and reliability. Keywords: angle, asymptomatic, Images, pelvic, quadriceps

Introduction. Modern sports places high demands on the functional systems of the body at all stages of the training and competitive processes. High loads create signifi cant background for the occurrence of musculoskeletal diseases, among which the myofascial syndrome (MFS) holds the leading position. MFS negatively affects the functional readiness of the sportsman′s body and is a risk factor for injuries of the musculoskeletal system. In this regard, timely treatment of MFS is important. It creates the conditions for the improvement of sports achievements, for health maintenance and sports longevity. Treatment of patients with MFS should include methods that normalize trophism and muscle tone. Given the problem of doping, the use of non-drug methods, including osteopathy, is preferable. The goal of research — to develop a system of monitoring and osteopathic correction of myofascial disorders (MFD) and to introduce it into the practice of medical and biological support for sportsmen of different categories and levels during the annual training process.Materials and methods. 93 sportsmen (45 women and 48 men), mean age 22,1±1,1 years, representing diffe rent sports were examined. The essence of the proposed system is regular clinical and electroneurophysiological monitoring of the body condition and preventive correction of myofascial disorders in order to exclude their infl uence on the success of sports activities.Results. It was established that the proposed system of monitoring and osteopathic correction led to a signifi cant reduction in clinical and electroneurophysiological manifestations both in local and in extensive forms of myofascial disorders, which allowed to maintain the optimal functional readiness of sportsmen throughout the entire annual training cycle.Conclusion. The system of monitoring and correction of MFS with the use of osteopathic treatment can be one of the elements of medical and biological support for sportsmen during the annual training process.

AbstractThis paper deals with the mathematical modeling of atherosclerosis based on a novel hypothesis proposed by a surgeon, Prof. Dr. Axel Haverich (Circulation 135(3):205–207, 2017). Atherosclerosis is referred as the thickening of the artery walls. Currently, there are two schools of thoughts for explaining the root of such phenomenon: thickening due to substance deposition and thickening as a result of inflammatory overgrowth. The hypothesis favored here is the second paradigm stating that the atherosclerosis is nothing else than the inflammatory response of of the wall tissues as a result of disruption in wall nourishment. It is known that a network of capillaries called vasa vasorum (VV) accounts for the nourishment of the wall in addition to the natural diffusion of nutrient from the blood passing through the lumen. Disruption of nutrient flow to the wall tissues may take place due to the occlusion of vasa vasorums with viruses, bacteria and very fine dust particles such as air pollutants referred to as PM 2.5. They can enter the body through the respiratory system at the first place and then reach the circulatory system. Hence in the new hypothesis, the root of atherosclerotic vessel is perceived as the malfunction of microvessels that nourish the vessel. A large number of clinical observation support this hypothesis. Recently and highly related to this work, and after the COVID-19 pandemic, one of the most prevalent disease in the lungs are attributed to the atherosclerotic pulmonary arteries, see Boyle and Haverich (Eur J Cardio Thorac Surg 58(6):1109–1110, 2020). In this work, a general framework is developed based on a multiphysics mathematical model to capture the wall deformation, nutrient availability and the inflammatory response. For the mechanical response an anisotropic constitutive relation is invoked in order to account for the presence of collagen fibers in the artery wall. A diffusion–reaction equation governs the transport of the nutrient within the wall. The inflammation (overgrowth) is described using a phase-field type equation with a double well potential which captures a sharp interface between two regions of the tissues, namely the healthy and the overgrowing part. The kinematics of the growth is treated by classical multiplicative decomposition of the gradient deformation. The inflammation is represented by means of a phase-field variable. A novel driving mechanism for the phase field is proposed for modeling the progression of the pathology. The model is 3D and fully based on the continuum description of the problem. The numerical implementation is carried out using FEM. Predictions of the model are compared with the clinical observations. The versatility and applicability of the model and the numerical tool allow.

Improvement in rider mobility represents an important functional gain for people with disabilities undergoing hippotherapy. However, there is no validated measuring instrument to track and document the rider's progress in riding activities. In this study, we aimed to develop and establish validity evidence for an instrument to assess hippotherapy participants’ mobility on horseback. We report on this development through the stages of: (a) content validation, (b) construct validation, (c) inter- and intra-rater reliability and (d) internal consistency analysis. We evaluated its factor structure with exploratory factor analyses, calculated values for inter- and intra-rater reliability using the intra-class correlation coefficient, and calculated its internal consistency using Cronbach's alpha. We followed recommendations by the Guidelines for Reporting Reliability and Agreement Studies. We found good inter-rater reliability (intra-class correlation coefficient – ICC = 0.991–0.999) and good intra-rater reliability (ICC = 0.997–1.0), and there was excellent internal consistency (Cronbach's α = 0.937–0.999). The instrument’s factor structure grouped its three domains into one factor. As this instrument is theoretically consistent and has been found to be appropriate and reliable for its intended use, it is now available for the measurement of horseback mobility among hippotherapy riders.

A bra use can reduce physiological and physical functions because of clothing pressure, which can be a problem for new senior women starting to lose physical function. The present study presents a bra top design development method for promoting new senior women’s physical activity by identifying problems related to bras’ effects on women’s health and minimizing clothing pressure. The analysis utilized the 3D scan data of 42 adult women (age range: 50s) from the 5th Size Korea Project. Bra top design elements were extracted based on new senior consumers’ needs. We developed an average wireframe reflecting the new senior’s physical characteristics, and a standard body form was developed through surface modeling. To produce a consumer-oriented bra with a body shaping effect and reduced clothing pressure that would not affect physical activities, a three-dimensional pattern was developed applying an optimal reduction rate of 80%. To verify the bra’s adequacy for the body form of new senior women, two market-available bras were selected and fit-compared to the developed product. The developed bra received higher expert appearance evaluation and 3D virtual clothing evaluation scores. This study is significant because by using virtual fitting technology, it provides foundational data to quantify the quality of fashion products.

ABSTRACT. Introduction. Regular physical activity has positive effects on human body composition, even if not combined with a diet. For many people this activity aims to reduce the amount of fat in some areas of the body and targets body shaping. Objectives. The main objective of this study was to learn about the effects of a 12-months regular participation in aerobic training exercises. Methods. This study was voluntarily attended by 89 women with an average age of 31.62, practicing aerobic exercises in gyms, three times a week for a period of 12 months. We carried out anthropometric measurements of subjects at the beginning and at end of the study, respectively, and data was statistically analysed using the SPSS 23.0 software. Results. Data collected was statistically processed, showing that participation in physical training led to reducing body weight by 4.81 kg, decrease of adipose tissue - between the two moments there was a difference of 5 % - fat mass decreased by 4.26 kg, and lean body mass by 0.55 kg. Significant decrease in chest, waist and hip circumference was also noticed. Conclusions. Regular participation in physical training programs involving aerobic exercise for 12 months has had the effect of reducing body weight and the amount of fat in some areas of the body, materialized by decreasing the value of the circumference of the chest, waist and hips, with the intention of reaching the ideal body weight and aiming for body shaping.

Katz and Stirrat devised a hand diagram, which uses subjective information from the patient for the diagnosis of carpal tunnel syndrome (CTS). They reported a good result. We tested the inter-observer and intra-observer reliability of this hand diagram. Twenty five consecutive patients with a diagnosis of CTS, 25 with other common hand and wrist problems but CTS and 25 healthy individuals were prospectively recruited. Each patient filled in a hand diagram. Two experienced hand surgeon scored the diagrams blindly on two different sittings four-week apart. The intra-class correlation coefficient (ICC) and Cohen's Kappa were used for intra and inter-observer reliability respectively. The intra-observer agreement was poor (ICC 95% 0.33–0.65) and inter-observer was fair (Kappa = 0.241). This study does not support the use of Katz and Stirrat Hand Diagram for the diagnosis of carpal tunnel syndrome in place if a thorough clinical examination.
