The purpose of this study was to provide a univariate and multivariate analysis of genomic microbial data and salivary mass-spectrometry proteomic profiles for dental caries outcomes. In order to determine potential useful biomarkers for dental caries, a multivariate classification analysis was employed to build predictive models capable of classifying microbial and salivary sample profiles with generalization performance. We used high-throughput methodologies including multiplexed microbial arrays and SELDI-TOF-MS profiling to characterize the oral flora and salivary proteome in 204 children aged 1–8 years (n=118caries-free,n=86caries-active). The population received little dental care and was deemed at high risk for childhood caries. Findings of the study indicate that models incorporating both microbial and proteomic data are superior to models of only microbial or salivary data alone. Comparison of results for the combined and independent data suggests that the combination of proteomic and microbial sources is beneficial for the classification accuracy and that combined data lead to improved predictive models for caries-active and caries-free patients. The best predictive model had a 6% test error, >92% sensitivity, and >95% specificity. These findings suggest that further characterization of the oral microflora and the salivary proteome associated with health and caries may provide clinically useful biomarkers to better predict future caries experience.

The relationship between dental caries and mutans streptococci (ms) in children and their mothers is a key factor in the prevention of early childhood caries. Changes in early childhood caries over time in Mexican children have not been well documented. Objective: to compare the dental caries indices and presence of ms in 3- to 5-year-old children and their mothers in two surveys, the first one carried out in 1996 and the second one in 2007. Study design: cross-sectional study. Results: The first survey included 102 mother-child dyads and the second 103. Dental caries index dmfs was 7.17 (± 10.9) and 4.58 (± 7.1) in the first and second survey, respectively (p<0.05). Ms levels were higher in 1996 than in 2007 (p<0.001). The prevalence of Severe Early Childhood Caries (SECC) was 42.2% and 34.95% in first and second surveys, respectively,(p>0.05). Children with SECC showed higher counts of ms than children without this condition. A high caries index in mothers was associated with higher caries experience in their children. Conclusion: the compassions of the surveys indicated a decline in dental caries indices over an 11-year period. High caries experience in the mother had a negative impact on the oral condition of the child.

The aim of this study was to analyze the association between BMP2 (rs1884302) and DLX3 (rs2278163) gene polymorphisms and dental caries in primary and permanent dentitions. A total of 914 subjects were genotyped by the TaqMan methods: 176 caries-free children (with Decayed/Missing/Filled Teeth, DMFT = 0), 542 patients with dental caries in permanent dentition (DMFT ≥1), 83 caries-free children with primary teeth (with decayed/missing/filled teeth, dmft = 0), and 113 children with early childhood caries (ECC, dmft ≥1). There were no significant differences in allele/genotype frequencies between patients with caries in permanent dentition/ECC and caries-free children or between patients with very low (DMFT = 0-2), low (DMFT = 3-5), moderate (DMFT = 6-8), or high (DMFT ≥9) caries experience. Variability in BMP2 and DLX3 was not associated with caries in the Czech population.

Abstract The prevalence of early childhood caries and its level varies. The present study was to establish the trends in dental caries and the impact of behavioural changes on the prevalence of caries in three-yearolds in Poland within a fifteen-year period. The results of a cross sectional survey carried out on 3439 three-year-olds in 2002, 2009, and 2017 using WHO criteria for dental caries (dmft, dmft=0, dmft≥4) and the results of a questionnaire filled by their parents with data on sociodemographics, oral hygiene and dietary habits, especially their sugar intake, were assessed. The dmft/dmfs index is applied to the primary dentition and is expressed as the total number of teeth/surfaces that are decayed, missing, or filled. The Cochran-Armitage test for trend was used to assess the fraction changes in time. The Pearson correlation coefficient was used to assess the changes in dmft trends and the correlations between behavioural changes, awareness levels, and the prevalence of caries. Within the fifteen-year period minimal changes in the prevalence of early childhood caries (15% down), dmft≥4 (11.4% down) and a lower dmft (36% down) were accompanied by a better parent awareness about the causes of caries and better oral hygiene routines. Sugary beverages were no longer drank at least once a day, however sweetened milk, cake, doughnuts, and sweet rolls were consumed more often. Being female, living in an urban area, having parents more aware about caries, consuming sugary beverages less frequently, brushing teeth twice a day, and using a fluoride toothpaste promoted lower early childhood caries. Gradually healthier teeth are linked to an increased awareness of the parents and healthier routines. A too frequent exposure to sugar promotes early childhood caries. Should the changes of dietary habits be insufficient, brushing teeth with fluoride toothpaste becomes crucial.

Abstract Introduction Sugar ingestion is the most important isolated factor related to dental caries. Contact with sugary foods at an early age may represent a risk to the oral health of preschool children. Objective To evaluate the timing of the introduction of sugar in the diet of preschoolers and its association with early childhood caries. Material and method Cross-sectional population-based study with 888 five-year-old preschoolers from public and private schools. Parents answered a questionnaire on sociodemographic data, eating habits and oral health. The variables timing of sugar introduction (>1 year and ≤1 year) and dental caries experience were dichotomized and data were analyzed using descriptive statistics and Poisson regression (p<0.05). Result The introduction of sugar in the diet occurred during the first twelve months of life for 73.8% of preschoolers. There was no association between the timing of sugar introduction and early childhood caries (p>0.05). The interruption of exclusive breastfeeding and low family income increased the prevalence of preschoolers having early contact with sugar by 32% and 23%, respectively (p<0.05). The prevalence of early childhood caries was 42.3% and was associated with a lower level of maternal education, low income, increased frequency of ingestion of candy and the use of nonfluoridated toothpaste (p<0.05). Conclusion The introduction of sugar in the diet occurred during the first year of life in most preschoolers and there was no association with early childhood caries.

Recently, sleeping status has attracted attention for its relationship with oral health. In the present study, we have investigated the association between early childhood caries and sleeping status. A multicentre observational cross-sectional study was conducted among 332 preschoolers (aged 3–6 years) and their parents in Chitose, Japan. Dental caries and sleeping status were assessed in the children and the sleep quality and health literacy of the parents were also assessed. Univariate and multivariate regression analyses were used in order to investigate the effect of the sleeping status of the parents and their children on dental caries. Among the children, the prevalence of dental caries experience was 12.7%. The children without caries slept significantly longer and their parents had a better Pittsburgh Sleep Quality Index (PSQI) score than those with caries experience. The sleeping status and the numbers of caries in the children were significantly correlated. Health literacy was better in those without caries experience. Parents’ PSQI was significantly positively correlated with the numbers of caries in the children (r = 0.19, p = 0.0004). The children’s sleep durations, screen time, and parental smoking status were independently associated with early childhood caries. Poor sleeping status of children and their parents is related to dental caries among preschoolers.

Introduction: Malnutrition in children is one of the most prevalent global health challenges, and malnourished children have a higher risk of death from childhood diseases. Early childhood caries (ECC) is the most common chronic disease of childhood. Complications from ECC such as pain, loss of tooth/teeth, and infection can undermine a child’s nutrition and growth. Aim: This study aims to evaluate the severity of decay, missing, and filled tooth (dmft) by nutritional status using the z scores of the anthropometric measurements: height for age (HFA), weight for age (WFA), weight for height (WFH), and body mass index for age (BMIA) among children with ECC in Nigeria. Study Design: This is a cross-sectional study conducted in 5 local government areas (LGAs) in Lagos State, Nigeria. A multistage sampling technique was used. Results: A total of 273 cases of ECC were included in the analyses (mean age 4.19 ± 0.96 y). Overall, the mean dmft was 3.04 ± 2.28, and most (96%) were accounted for by untreated decay. The distribution of dmft within the different z score categories of BMIA (<–3 = severely wasted, –2 to –3 = wasted, –2 to +2 = normal, +2 to +3 = overweight and >+3 = obese) showed the highest dmft scores among the combined severely wasted and wasted groups, lowest among children with normal z scores, and intermediate in the overweight and obese groups. There was a significant negative correlation between BMIA z score, WFH z score, and dmft ( r = −0.181, P < 0.05 and r = −0.143, P < 0.05, respectively). However, the correlations between HFA z score, WFA z score, and dmft were positive but not significant ( r = 0.048, P = 0.44 and r = 0.022, P = 0.77, respectively). Conclusion: Our study showed an increased severity of dental caries among severely wasted or wasted children with ECC compared to those of normal or overweight. Knowledge Transfer Statement: The results from this study will raise awareness among clinicians and policy makers on the need for a primary prevention program for early childhood caries in countries with high burden of malnutrition and limited resources. Also, it will help draw the attention of clinicians to the caries status of malnourished children that can be managed to improve the nutritional outcomes.

Objective: Dental caries is both an infectious and transmissible disease. Maternal transfer of Mutans Streptococci occurs at an early age and is important in the initiation of dental caries in children. The aim of this study was to identify certain strains of Streptococcus mutans in mother-child pairs, of children with early childhood caries. Study design: Sixty mother-child pairs of healthy children aged 18–36 months were selected. Mothers with high levels of Streptococcus mutans in their saliva and only children with ECC were included. Dental plaque samples were collected from mother-child pairs. The plaque samples were stored, transferred to the laboratory and analyzed for Streptococcus mutans strains c, f, e and k, present in mother-child pairs using Real time Polymerase Chain Reaction (PCR) technique. Data obtained was subjected to statistical analysis for level of similarity in Streptococcus mutans strains present in mother-child pairs. Results: A similar distribution of Streptococcus mutans strains c, f and k was identified in 28 mother-child pairs. Streptococcus mutans strain e was seen in 18 pairs. Conclusion: Less than 50% of mother-child pairs showed similarity in distribution of Streptococcus mutans strains.

ABSTRACT Introduction Early childhood caries (ECC) is a virulent form of dental caries that can destroy the primary dentition of toddlers and preschool children. The aim was to determine the relationship of feeding methods and oral Streptococcus mutans count in 3- to 5-year-old children with ECC. Materials and methods A cross-sectional study was conducted in children aged 3 to 5 years. Participating mothers were interviewed regarding child's demographic profile, educational level and socioeconomic status of parents, past medical and dental history of the mother and child, child's feeding habits, and dietary habits and oral hygiene practices of mother and child. Clinical examination for dental caries was done using the World Health Organization criteria (1997). Salivary samples of mother–child pair were collected to determine the pH, flow rate, and S. mutans count. Statistical tests, such as Student's t-test, analysis of variance, and Pearson's correlation were applied. Results Out of 150 mother–child pair, statistically significant difference in the caries experience was found between mothers and children with high and low S. mutans count. Moderate but statistically significant negative correlation was found between mean decayed, missing, and filled teeth of mothers and mean decayed, extracted and filled teeth (deft) of children with high S. mutans count. Regarding deft, there was no statistically significant difference between children who were exclusively breast fed (7.85 ± 2.94), exclusively bottle-fed (8.67 ± 3.98), and both breast and bottle-fed (7.77 ± 2.91). Conclusion The mean caries experience of mothers and children was 2.66 ± 2.01 and 7.82 ± 2.94 respectively, with decayed component being maximum. Moderate and significant correlation (r = 0.5) was found between S. mutans of mothers and children in saliva. Significant negative correlation was found between mothers and children with high S. mutans count (r = –0.0284; p = 0.046).
