Background. Altered balance is a common and debilitating symptom of fibromyalgia. Previous studies have investigated balance under single-task conditions that do not reflect real-life situations. The present study evaluated the impact of a simultaneous cell-phone conversation on balance performance in a case-control cohort from Spain. Methods. A cross-sectional case-control study was performed in 34 women recruited from local self-help organizations and university facilities (n = 18 fibromyalgia cases; n = 16 healthy, pain-free controls). Participants performed the 30 s postural stability test, as implemented in the Biodex Balance System, under two conditions: (a) as a single task and (b) as a dual task, i.e., while holding a simultaneous cell-phone conversation with a technician. Intergroup differences in dual task costs were calculated. Findings. Compared with controls, women with fibromyalgia showed higher dual-task costs in balance variables, such as overall sway (pp value = 0.039) and anterior/posterior sway (p value = 0.007). In the dual-task condition, overall (p value = 0.004) and anterior/posterior (p value = 0.012) sway indices significantly decreased in women with fibromyalgia but not in controls. Interpretation. In women with fibromyalgia, balance performance was adversely impacted by the holding of a simultaneous cell-phone conversation. The inability to conduct two tasks simultaneously may be related to reduced attention and may increase the risk of falling in this population.

Abstract Objective To investigate the effect of a cognitive task on postural control during treadmill walking. We hypothesized that postural sway acceleration would decrease during dual-task compared to single-task gait. Method Ten (5 females, 5 males) physically active, healthy young adults (Age: 22.50 ± 3.01) with no history of cognitive deficits or head injury were recruited from a University campus for this cross-sectional study. Participants reported to a biomechanics laboratory for a single testing session and completed two, 5-minute walking trials on a treadmill at 90% of maximal gait-speed under single and dual-task conditions while instrumented with a single inertial measurement unit (IMU) on the lower-back. During dual-task gait, participants concurrently performed a Stroop Test displayed on a mounted screen in front of the treadmill during the entire trial. Root mean square (RMS) of acceleration was analyzed in the medial-lateral, anterior-posterior, superior-inferior, and resultant planes during the dual-task trial. Paired t-tests and effect sizes (ES) were calculated for each sway plane between single and dual-tasks. Results No differences were identified between single and dual-task RMS acceleration in the medial-lateral (p = 0.98, ES = 0.01), anterior-posterior (p = 0.49, ES = 0.23) or superior-inferior (p = 0.98, ES = 0.01) planes. However, the RMS acceleration in the resultant plane was lower in the dual-task condition (Mean Difference: 0.031 ± 0.037; p = 0.02, ES = 0.85). Conclusions This novel dual-task gait paradigm utilized constant sub-maximal gait speed to investigate the influence of a sustained cognitive dual-task on postural sway using a single IMU. These preliminary results indicate that this dual-task gait paradigm may affect gross postural sway in healthy adults.

BackgroundDual-task, performing a balance task and a cognitive task simultaneously, has been used as sport-related concussion assessment tool. However, the effect of dual task on balance performance remains controversial. Also, the fluctuation of hormone across menstrual cycle on balance and dual-task performance has not been fully examined.ObjectiveTo examine (1) the effect of dual task on balance performance, (2) the interaction of female sex hormonal level, balance, and dual-task performance.DesignA cohort study.SettingA laboratory of concussion research and services.ParticipantsA group of 49 healthy female college-aged students (age = 21.6 ± 2.0 years).Main outcome measuresThe Stability Evaluation Test (SET) protocol, Balance error scoring system (BESS) test and postural sway velocity (deg/sec) by using VSR SPORT force plate. Three auditory mathematics questions were given for each condition of the SET test. Two single-task conditions were provided, including balance only and math only. Balance test and math questions were given simultaneously during the dual-task condition.ResultsThere was no significantly statistically difference in BESS or sway velocity between single and dual task. However, the performance of math calculation is better in dual-task comparing to the single task (math only), with an 8%–12% higher rate of accuracy, but it didn't reach a statistical significance. The rate of accuracy during the dual task didn't vary among menstrual phases.ConclusionsBalance performance was maintained under dual-task conditions and fluctuation of female sex hormones across the menstrual cycle may not affect working memory and executive function. These findings are potentially clinically applicable to detecting balance deficits and cognitive alterations in female concussed individuals. Future studies are needed to investigate the underlying mechanism of balance maintenance and attention dividend under dual-task conditions.

PURPOSE: Aside from typical symptoms such as dizziness and vertigo, persons with vestibular disorders often have cognitive and motor problems. These symptoms have been assessed in single-task condition. However, dual-tasks assessing cognitive-motor interference might be an added value as they reflect daily life situations better. Therefore, the 2BALANCE protocol was developed. In the current study, the test-retest reliability of this protocol was assessed. METHODS: The 2BALANCE protocol was performed twice in 20 healthy young adults with an in-between test interval of two weeks. Two motor tasks and five different cognitive tasks were performed in single and dual-task condition. Intraclass correlation coefficients (ICC), the standard error of measurement, and the minimal detectable difference were calculated. RESULTS: All cognitive tasks, with the exception of the mental rotation task, had favorable reliability results (0.26≤ICC≤0.91). The dynamic motor task indicated overall substantial reliability values in all conditions (0.67≤ICC≤0.98). Similar results were found for the static motor task during dual-tasking (0.50≤ICC≤0.92), but were slightly lower in single-task condition (–0.26≤ICC≤0.75). CONCLUSIONS: The 2BALANCE protocol was overall consistent across trials. However, the mental rotation task showed lowest reliability values.

Objective: Data on the association between body mass index (BMI) and dental caries in children with orofacial clefts are sparse. Therefore, studies on the impact of BMI on caries frequency in children with cleft lip and/or palate (CL/P) are of importance. The aim of the current study was to investigate the association between BMI and frequency of dental caries in children with and without CL/P. Height, weight, and BMI in children with CL/P were also compared to controls. Design: This study used a cross-sectional case-control design. Participants: One hundred and thirty-nine 5- and 10-year-old children with CL/P and 299 age-matched controls. Main Outcome Measures: Caries was recorded according to the International Caries Detection and Assessment System. Height and weight were recorded, and BMI was calculated as weight/height2. Results: There was no correlation between BMI and caries frequency. Weight, height, and BMI were significantly lower in all children with CL/P compared to controls. After adjustment for international adoption, only BMI was significantly lower in CL/P children compared to controls. Non-adopted children with CL/P were significantly heavier and longer than adopted children with CL/P. Conclusions: Five- and 10-year-old children with corrected CL/P seemed to have a lower BMI than controls, but there was no association between BMI and caries frequency. Internationally adopted children with CL/P were lighter and shorter than non-adopted CL/P children and controls.

Abstract Impairments in attentional, working memory and sensorimotor processing have been consistently reported in schizophrenia. However, the interaction between cognitive and sensorimotor impairments and the underlying neural mechanisms remains largely uncharted. We hypothesized that altered attentional processing in patients with schizophrenia, probed through saccadic inhibition, would partly explain impaired sensorimotor control and would be reflected as altered task-dependent modulation of cortical excitability and inhibition. Twenty-five stabilized patients with schizophrenia, 17 unaffected siblings and 25 healthy control subjects were recruited. Subjects performed visuomotor grip force-tracking alone (single-task condition) and with increased cognitive load (dual-task condition). In the dual-task condition, two types of trials were randomly presented: trials with visual distractors (requiring inhibition of saccades) or trials with addition of numbers (requiring saccades and addition). Both dual-task trial types required divided visual attention to the force-tracking target and to the distractor or number. Gaze was measured during force-tracking tasks, and task-dependent modulation of cortical excitability and inhibition were assessed using transcranial magnetic stimulation. In the single-task, patients with schizophrenia showed increased force-tracking error. In dual-task distraction trials, force-tracking error increased further in patients, but not in the other two groups. Patients inhibited fewer saccades to distractors, and the capacity to inhibit saccades explained group differences in force-tracking performance. Cortical excitability at rest was not different between groups and increased for all groups during single-task force-tracking, although, to a greater extent in patients (80%) compared to controls (40%). Compared to single-task force-tracking, the dual-task increased cortical excitability in control subjects, whereas patients showed decreased excitability. Again, the group differences in cortical excitability were no longer significant when failure to inhibit saccades was included as a covariate. Cortical inhibition was reduced in patients in all conditions, and only healthy controls increased inhibition in the dual-task. Siblings had similar force-tracking and gaze performance as controls but showed altered task-related modulation of cortical excitability and inhibition in dual-task conditions. In patients, neuropsychological scores of attention correlated with visuomotor performance and with task-dependant modulation of cortical excitability. Disorganization symptoms were greatest in patients with weakest task-dependent modulation of cortical excitability. This study provides insights into neurobiological mechanisms of impaired sensorimotor control in schizophrenia showing that deficient divided visual attention contributes to impaired visuomotor performance and is reflected in impaired modulation of cortical excitability and inhibition. In siblings, altered modulation of cortical excitability and inhibition is consistent with a genetic risk for cortical abnormality.

Falls during stair negotiation have become one of the leading causes of accidental death. The effects of a concurrent cognitive or manual dual-task paradigm on dynamic gait stability remain uncertain. How much dynamic gait stability is influenced by gait velocity is also not clear. A total of 16 healthy young females descended a staircase under three different walking conditions: descend stairs only (single task), descend stairs while performing subtraction (cognitive dual-task), and descend stairs while carrying a glass of water (manual dual-task). An eight-camera Vicon motion analysis system and a Kistler force plate embedded into the third step of the staircase were used synchronously to collect kinematic and kinetic data. Gait velocity decreased and dynamic gait stability increased with both cognitive and manual dual-task conditions. The center of mass–center of pressure inclination angle increased with gait velocity but decreased with the manual dual-task condition compared to the single-task condition. Changes in gait velocity caused by the dual-task paradigm can partially explain the effects of dual-task dynamic gait stability. The influence of gait velocity should be considered in the assessment of dual-task effects.
