Objective. To investigate the application effect of the standard operating procedure (SOP) in the prevention of venous thromboembolism (VTE). Methods. The clinical data of patients admitted to respiratory, cardiovascular, neurological, and geriatric departments in the hospital (November 2020–May 2021) were retrospectively analyzed, and the patients in line with the inclusion criteria were equally randomized into the observation group (OG) and the control group (CG). The CG was treated with the routine nursing, and the OG received the SOP of VTE prevention additionally. After the record of the incidence of VTE and nursing satisfaction of the two groups, scores of VTE awareness were compared. Results. One hundred and twenty patients were included in this study, and no obvious difference was found in the general data of patients ( P  > 0.05). Compared with the CG, the incidence of VTE of the OG was obviously lower ( P  < 0.05). After nursing, compared with the CG, scores of VTE awareness in the OG were conspicuously higher ( P  < 0.001), and scores of VTE awareness of the nursing staff were conspicuously higher than those before nursing ( P  < 0.001). Compared with the CG, nursing satisfaction of the OG was obviously higher ( P  < 0.001). Conclusion. SOP can reduce the incidence of VTE of patients, improve their disease awareness, and enhance their nursing satisfaction, which should be popularized in practice.

Background and Purpose: Fever in the acute phase of stroke leads to an unfavorable clinical outcome and increased mortality. However, no specific form of effective fever treatment has been established, so far. We analyzed the effectiveness of our in-house standard operating procedure (SOP) of fever treatment.Methods: This SOP was analyzed for a period of 33 weeks. Patients with cerebral ischemia (ischemic stroke, transient ischemic attack) or cerebral hemorrhage (intracerebral, subarachnoid) and body temperature elevation of ≥ 37.5°C within the first 6 days after admission were eligible for inclusion in the analysis. The results of SOP group, who's data have been collected prospectively were then compared with a historical control group that had been treated conventionally 1 year earlier in the same period. The data of control group have been collected in retrospect. The primary endpoint was the total duration of the fever for the first 6 days after admission to the stroke unit.Results: A total of 130 patients (mean age of 78 ± 12) received 370 antipyretic interventions. Sequential application of paracetamol (n = 245), metamizole (n = 53) and calf compress (n = 15) led to significant reduction in body temperature. In patients who did not respond to these applications, normothermia could be achieved after infusion of the cooled saline solution. Normothermia could be achieved within 120 min in more than 90% of the cases treated by the SOP. The SOP reduced the fever duration in the 6 days significantly, from 12.2 ± 2.7 h [95% confidence interval (CI) for mean] in the control group to 3.9 ± 1.0 h (95% CI) in the SOP group (p &lt; 0.001). The SOP was rated to be reasonable and effective.Conclusion: Our in-house SOP is cost-efficient and effective for fever treatment in stroke patients, that can be implemented by local health care professionals.

Latar belakang: Stroke merupakan penyebab kecacatan pertama dan penyebab kematian ketiga setelah penyakit jantung coroner dan kanker. Gejala sisa stroke banyak menyebabkan kecacatan permanen (Hinkle & Cheever, 2014). Lewis, et, al (2011) menyatakan bahwa stroke dapat mengakibatkan hilangnya fungsi motorik. Fungsi motorik tersebut dapat ditingkatkan dengan adanya latihan aktif dari pasien (Smith, 2008), salah satu bentuk latihan ini adalah bridging exercise. Tujuan penelitian ini adalah menganalis pengaruh latihan Bridging Exercise terhadap perubahan kekuatan otot pada pasien Stroke. Metode Penelitian: menggunakan quasi eksperimental design dengan pretest-posttest control group design.  Populasi dalam penelitian ini adalah 30 pasien Stroke di RS Bethesda yang dirawat pada bulan Oktober 2018, terdiri dari kelompok intervensi Bridging Exercise 15 responden dan kelompok kontrol 15 responden. Hasil Penelitian: uji beda two related samples t-test didapatkan p=0,000 , artinyaBridging Exercise memberikan pengaruh yang lebih baik terhadap perubahan kekuatan otot. Latihan Bridging Exersice (BE) yang dilakukan pada pasien stroke 1 kali sehari dengan 8 kali repetisi dilakukan selama satu minggu. Penelitian ini diharapkan dapat pengembangan pelayanan pada pasien stroke di Indonesia dan menjadi dasar dalam penyusunan Standard Operating Procedure (SOP). Motor function can be improved by active exercise from the patient ( Smith, 2008 ), one form of he exercise is bridging exercise. The goal of this research is to analyze the effect of bridging exercise  on changes in muscle strength in stroke patients. Research methods: with quasi eksperimental, pretest-posttest control group design. The population in this research was 30 stroke patients at the Bethesda Hospital who were treated in October 2018, consist of a bridging exercises intervention group of 15 respondents and a control group of 15 respondents. Research result: different test two related samples t-test obtaining p=0,000 , that means bridging exercises give out the better result on changes in muscle strength. Bridging Exercises ( BE ) performed on stroke patients once a day with 8 repetitions performed for a week. This research is expected to be able to develop services in stroke patients in Indonesia and become the basis for the preparation of  Standard Operating Procedure. The aim of this article is to analyze internal control in service company. The analyze used the components of internal control according to COSO: control environment, risk assessment, control activities, information and communication, monitoring. Analysis of internal control used the data obtained through observation, questionnaires, interview, and documentary. Based on this analysis results can be concluded, that the dual positions refer with company SOP (Standard Operating Procedure). Separation of task does not work because that the dual positions. Therefore, the component of control activities applied at company is not in accordance with the component of control activities according to COSO. Nonconformity components still results in very effective internal control because the purpose of the internal control still remains to be achieved even though there are components that are not in accordance with the internal control components according to COSO.
