Effort of NGO in Promoting Comprehensive Sexuality Education to Improve Quality of Life among Local and Refugee Communities

Federation of Reproductive Health Association, Malaysia (FRHAM) is a pioneer Non-governmental Organization (NGO) in disseminating the knowledge and services of sexual reproductive health in Malaysia. A qualitative case study research design was employed to explore the roles of FRHAM in promoting Comprehensive Sexuality Education (CSE) for the well-being of locals and refugees’ communities. In-depth interviews were conducted with the staffs of FRHAM to inquire about their effort in delivering their services. The interviews were recorded, transcribed, coded and emergent themes were identified. FRHAM had played a part as educator, service provider, advocate and actively involved in improving the quality of life among the refugee community. The organization had developed modules for CSE and conducted programs for dissemination of knowledge. They also collaborated with local governmental agencies and other NGOs to promote awareness and utilization of services among the communities. Nevertheless, promoting CSE had experienced many challenges in Malaysia. The perceptions of Malaysian viewing sexuality education as taboos, cultural factors, the reluctance of educators in delivering, and the low visibility of FRHAM are the barriers encountered by FRHAM in promoting CSE. FRHAM recommended including parents, teachers and other stakeholders in the promotion of CSE and acknowledging CSE as an official subject to be implemented in schools.  The information from this study may encourage more partnerships between the stakeholders, policy makers and NGOs for providing high quality CSE and sexual reproductive health services.

The Adolescent Friendly Health Clinic (AFHCs), a key component of the Government of India’s National Adolescent Health Programme a.k.a. Rashtriya Kishor Swasthya Karyakram (RKSK), aims to increase the accessibility and utilization of sexual-reproductive health services by adolescents and youth. However, low quality of care provided at AFHCs by counsellors calls for attention. We, thus, explore both the clients’ and providers’ perspectives using the World Health Organization’s (WHO) global standards for quality health-care services for adolescents to assess the quality of the sexual reproductive health service delivery at AFHCs in Rajasthan, India. We conducted a qualitative study, comprising observation of the service delivery using mystery clients (MCs) (n = 12) and in-depth interviews with the counsellors (n = 4) in four AFHCs. Interviews were transcribed in local language and were translated in English. The transcripts were coded thematically. Our study, using five of the eight WHO global standards for quality health-care services for adolescents highlighted several gaps in the quality-of-service delivery at AFHCs. We unearth various intricacies related to the quality of the services provided at the AFHCs by referring to the relevant input, process, and the output criteria of WHO global standards I, III, IV, V and VI. Our study calls for efforts to improve- (i) the counsellors’ competencies to increase adolescents’ health literacy on sensitive topics, (ii) the facilities at the clinic to ensure privacy, comfort and confidentiality of the adolescents seeking services, (iii) the referrals to improve appropriate package of services, and (iv) an overall environment to ensure an equity and non-discrimination for all the adolescents. Our findings unearth the barriers that both the service providers and the adolescents face at the AFHCs and underscore the need for regular monitoring and evaluation of the AFHCs to strengthen the facility-based intervention of the RKSK programme.

We compared sexual/reproductive health services and sexuality education topics provided in Texas alternative high schools (AHSs) with the prevalence of sexual risk behaviors among students in AHS. Using cross-sectional data from convenience samples of 14 principals, 14 lead health educators, and 515 students, we calculated descriptive statistics for 20 services and 15 sexuality education topics provided by AHSs and seven sexual risk behaviors among students in AHS. AHSs provided few sexual/reproductive health services and limited educational content, despite high levels of sexual risk taking among students. For example, no AHSs taught students about proper condom use, yet 84% of students have had sex. Findings provide preliminary evidence of unmet needs for school-based sexual/reproductive health services and comprehensive sexuality education in AHS settings. Future investigation with larger, representative samples is needed to assess the provision of sexual/reproductive health services and sexuality education in AHSs and monitor sexual risk behaviors in the AHS population.

Parents who lost their only child in the 12 May 2018 Wenchuan earthquake came to be known as the “shidu” (loss of an only child) parents. After the earthquake, they were beneficiaries of free reproductive health services, and most had another child. This study focuses on the psychosocial well-being of those children born to the shidu parents, and explores factors associated with mothers’ quality of life (QoL) and their involvement with their children. A cross-sectional survey was conducted in two primary schools in Wenchuan County. A sample of 192 families was analyzed (147 non-shidu and 45 shidu). The statistical analyses indicated that the children of shidu families had poorer peer relationships than children of non-shidu families. Moreover, shidu mothers’ expectations of their children’s achievements were significantly lower than for non-shidu mothers, but there was no statistically significant difference in the QoL between them. Additionally, a higher home-based involvement of mothers was found to be moderately associated with peer relationship problems of children in shidu families. Therefore, we suggest a future research focus on developing joint activities for parents and children that help to increase emotional communication for the psychosocial development of children in shidu families after disasters in China.

The Ministerial Commitment on Comprehensive Sexuality Education and Sexual and Reproductive Health Services for Adolescents and Young People in Eastern and Southern Africa (ESA), or the ESA Commitment, was affirmed December 7, 2013, by 21 countries located across this region during the 17th International Conference on AIDS and Sexually Transmitted Infections in Africa. The ESA Commitment speaks to the numerous practices and challenges of school age populations stemming from interplay among education, health, and contextual issues varying by country. Analysis of this policy is approached using methodology drawn from Bartlett and Vavrus (2014, 2017) and using a lens of policy borrowing, particularly focused on incorporating agency, process, impact, and timing (Steiner-Khamsi, 2000, 2010). This analysis seeks to understand the ESA Commitment and national curriculum subsequently implemented in Zambia by situating these actions among broader international, regional, and national discourse in the area of sexual and reproductive health and education for young people between 1994 and 2016. Through analysis considering its effectiveness in terms of implementation, scalability, and sustainability, its ability to enable progress towards improving the lives of young people, especially through increased knowledge of HIV/AIDS prevention, is examined and recommendations are presented.

Неудовлетворительное состояние здоровья населения страны в целом, ухудшающиеся показатели репродуктивного здоровья женщин лежат в основе тяжелой демографической ситуации. Такое положение в значительной мере обусловлено снижением доступности и качества медицинской помощи. В статье рассматривается деятельность родильного дома с помощью методологии IDEF0, IDEF3, дерева целей, а также проведена оценка его показателей и составлена таблица занятости медицинского персонала. Уровень оказываемой роддомом медицинской помощи напрямую влияет на здоровье рожениц и новорожденных, поэтому строгое наблюдение за его деятельностью и за соответствие всех показателей нормам необходимо. Репродуктивное здоровье женщин имеет высокую демографическую значимость, так как рождающееся потомство определяет здоровье популяции в целом, и социальную обусловленность - зависимость внутриутробного и раннего постнатального развития ребенка от качества жизни и состояния здоровья матери, обеспечивающей благополучие младенца после рождения. Для визуализации структуры организации и понятия целей ее деятельности использовались организационная структура и такой метод системного анализа как дерево целей. Для наглядного представления процессов, протекающих в роддоме, выявления взаимосвязей основных и вспомогательных элементов системы, управления и главных видов ресурсов использовалась общая модель IDEF0 и декомпозиция ее основных блоков. Диаграмма IDEF3 обеспечивает дискретность моделирования процесса, что позволяет контролировать ход выполнения работ. Основной целью моделирования деятельности родильного дома является анализ его показателей и выявление проблем, существующих в его процессах The unsatisfactory state of health of the population of the country as a whole, the deteriorating indicators of the reproductive health of women underlie the difficult demographic situation. The level of medical care provided by the maternity hospital directly affects the health of women in labor and newborns, therefore, strict monitoring of its activities and the compliance of all indicators with standards is necessary. The reproductive health of women has a high demographic significance, since the offspring being born determines the health of the population as a whole, and social conditioning is the dependence of the intrauterine and early postnatal development of the child on the quality of life and the state of health of the mother, which ensures the well-being of the baby after birth. To visualize the structure of the organization and the concept of the goals of its activities, the organizational structure and such a method of system analysis as the tree of goals were used. To visualize the processes taking place in the maternity hospital, to identify the interrelationships of the main and auxiliary elements of the system, control and main types of resources, the general IDEF0 model and the decomposition of its main blocks were used. The IDEF3 diagram provides discrete process modeling, which allows you to monitor the progress of work. The main purpose of modeling the activities of a maternity hospital is to analyze its indicators and identify problems that exist in its processes. Abstract. To date, few studies have focused on perceived overprotection from the perspective of people with dementia (PwD). In the present examination, the association of perceived overprotection in PwD is examined as an autonomy-restricting factor and thus negative for their mental well-being. Cross-sectional data from the prospective DYADEM study of 82 patient/partner dyads (mean age = 74.26) were used to investigate the association between overprotection, perceived stress, depression, and quality of life (QoL). The analyses show that an overprotective contact style with PwD has a significant positive association with stress and depression, and has a negative association with QoL. The results emphasize the importance of avoiding an overprotective care style and supporting patient autonomy.
