Acoustic Reflex Latency Test in the Evaluation of Nontumor Patients with Abnormal Brainstem Latencies

The acoustic reflex latency test (ARLT), an indirect measure of neural conduction time utilizing the electroimpedance method, was used to evaluate 43 highly suspect, nontumor patients who demonstrated abnormal latency results on brainstem electric response audiometry (BERA), false-positive for tumor. Normative values and criteria for tumor-positive ARLT results were established on a control group using a second generation test unit, comprised of dual impedance bridges and a digital signal averager, designed especially to perform the ARLT. When cut-off criteria for both absolute reflex latency and interaural latency difference (ILD) values were applied to this BERA false positive population, the ARLT correctly identified 93% of patients as having end-organ lesions. Differences in ARLT and BERA test results are attributed to differences in 1) method of measurement, 2) characteristics of the eliciting signal, and 3) generation of the response. These factors are discussed as sources of variability. Both ARLT and BERA are recommended as tandem procedures when screening for acoustic or angle tumors.

Central auditory function was assessed in 10 stutterers and 10 nonstutterers. Performance of the two groups was compared for seven audiometric procedures including acoustic reflex threshold, acoustic reflex amplitude function, performance intensity function for monosyllabic phonetically balanced (PB) words, performance intensity function for Synthetic Sentence Identification, Synthetic Sentence Identification with Ipsilateral Competing Message, Synthetic Sentence Identification with Contralateral Competing Message, and the Staggered Spondaic Word test. Relative to the control group, the performance of the stuttering group was depressed on three procedures—the acoustic reflex amplitude function, Synthetic Identification with Ipsilateral Competing Message, and Staggered Spondaic Word test. As a group, the stutterers presented evidence of a central auditory deficiency. The pattern of test results suggests a disorder at the brainstem level. The subtlety of the deficiency is emphasized.

The research on the sterlet roe artificial insemination using cryopreserved sperm was carried out in the research base of the RAS Southern Scientific Centre (the Rostov region). Reproductive cells (including cryopreserved cells), larvae, sterlet fry ( Acipenser ruthenus Linnaeus, 1758) were taken as an object of research. A half of the roe (1.7 kg) taken from female starlet was inseminated by native sperm (control group); another half was inseminated by defrosted sperm of two males, which was stored in liquid nitrogen at -196ºC during 3 years (pilot group). Incubation lasted 5 days at water temperature 14.5-18.2ºC, with daily fluctuations of temperature 1.9ºC. Roe insemination in the control group made 90%, in the pilot group - 70%. Roe embryonic growth in the control group was faster, but embryogenesis duration in the pilot group met the standard time limits. Hatching prolarvae in the control group started one hour earlier, than in the pilot group; it made 75% and 60% of all incubated roe, correspondingly. Waste during the period of larvae maturing before they pass to mixed feeding was negligible - 2% in the control group and 3.4% in the pilot group. According to the test results, "open field" of reactivity of the central nervous system in the pilot group fry didn’t change from the control group fry, but more active response to stimuli was noted in the pilot group, which is very important for fry adaptation to the conditions in natural water basins. It was established that sterlet offspring obtained with use of defrosted sexual cells does not differ from the offspring obtained using native sperm and has higher morphometric characteristics. The test results prove the possibility and practicability of using sexual cells stored in liquid nitrogen for artificial restoration and formation of sturgeon fish broodstocks.

This study examines the generalization of instruction in foreign language learning. A group of Japanese learners of English served as participants and received special instruction in the structure of genitive relative clauses. The participants were given a pre-test on combining two sentences into one containing a genitive relative clause wherein the relativized noun phrase following the genitive marker "whose" is either the subject, direct object, or object of preposition. Based on the TOEFL and the pre-test results, four equal groups were formed; three of these served as experimental groups, and one as the control group. Each experimental group was given instruction on the formation of only one type of genitive relative clause. The participants were then given two post-tests. The results indicated that the generalization of learning begins from structures that are typologically more marked genitive relative clauses to those structures that are typologically less marked, and not vice versa.

AbstractIn Bell’s palsy, electrodiagnosis by electroneurography (ENoG) is widely used to predict a patient’s prognosis. The therapeutic options for patients with poor prognostic results remain controversial. Here, we investigated whether early intervention with intratympanic steroid therapy (ITST) is an effective treatment for Bell’s palsy patients with poor electrodiagnostic test results (≤ 10% electroneurography value). Patients in the concurrent ITST group (n = 8) received the standard systemic dose of prednisolone (410 mg total) and intratympanic dexamethasone (16.5 mg total) and those in the control group (n = 21) received systemic prednisolone at the standard dose or higher (average dose, 605 ± 27 mg). A year after onset, the recovery rate was higher in the ITST group than in the control group (88% vs 43%, P = 0.044). The average House-Brackmann grade was better in the concurrent ITST group (1.13 ± 0.13 vs 1.71 ± 0.16, P = 0.035). Concurrent ITST improves the facial nerve outcome in patients with poor electroneurography test results, regardless of whether equivalent or lower glucocorticoid doses were administered. This may be ascribed to a neuroprotective effect of ITST due to a higher dose of steroid reaching the lesion due to dexamethasone transfer in the facial nerve.

Abstract Background Transcranial magnetic stimulation (TMS) is a non-invasive procedure used in a small targeted region of the brain via electromagnetic induction and used diagnostically to measure the connection between the central nervous system (CNS) and skeletal muscle to evaluate the damage that occurs in MS. Objectives The study aims to investigate whether single-pulse TMS measures differ between patients with MS and healthy controls and to consider if these measures are associated with clinical disability. Patients and methods Single-pulse TMS was performed in 26 patients with MS who hand an Expanded Disability Status Scale (EDSS) score between 0 and 9.5 and in 26 normal subjects. Different TMS parameters from upper and lower limbs were investigated. Results TMS disclosed no difference in all MEP parameters between the right and left side of the upper and lower limbs in patients with MS and controls. In all patients, TMS parameters were different from the control group. Upper limb central motor conduction time (CMCT) was prolonged in MS patients with pyramidal signs. Upper and lower limb CMCT and CMCT-f wave (CMCT-f) were prolonged in patients with ataxia. Moreover, CMCT and CMCT-f were prolonged in MS patients with EDSS of 5–9.5 as compared to those with a score of 0–4.5. EDSS correlated with upper and lower limb cortical latency (CL), CMCT, and CMCT-f whereas motor evoked potential (MEP) amplitude not. Conclusion TMS yields objective data to evaluate clinical disability and its parameters correlated well with EDSS.

Multi-arm clinical trials assessing multiple experimental treatments against a shared control group can offer efficiency advantages over independent trials through assessing an increased number of hypotheses. Published opinion is divided on the requirement for multiple testing adjustment to control the family-wise type-I error rate (FWER). The probability of a false positive error in multi-arm trials compared to equivalent independent trials is affected by the correlation between comparisons due to sharing control data. We demonstrate that this correlation in fact leads to a reduction in the FWER, therefore FWER adjustment is not recommended solely due to sharing control data. In contrast, the correlation increases the probability of multiple false positive outcomes across the hypotheses, although standard FWER adjustment methods do not control for this. A stringent critical value adjustment is proposed to maintain equivalent evidence of superiority in two correlated comparisons to that obtained within independent trials. FWER adjustment is only required if there is an increased chance of making a single claim of effectiveness by testing multiple hypotheses, not due to sharing control data. For competing experimental therapies, the correlation between comparisons can be advantageous as it eliminates bias due to the experimental therapies being compared to different control populations.
