Focused on colonial South India, this article presents and assesses detailed archival records of public health measures in response to plague outbreaks between 1900 and 1947. Starting in 1897 in the Madras Presidency, the colonial government strictly implemented anti-plague measures and introduced various health schemes and medical policies for plague prevention. However, despite partly vigorous government efforts, plague outbreaks could not be fully controlled. In the twentieth and twenty-first centuries, the plague remains among South Asia’s most feared epidemics, with an outbreak in Surat in 1994 causing major havoc. Neither indigenous knowledge nor Western medical systems provided fully effective remedies regarding causation, cure and prevention of plague epidemics. Since the article gained new relevance in light of current struggles faced by India’s public health system in handling the ongoing COVID-19 pandemic, some lessons from history emerge in the concluding discussion.

The COVID-19 pandemic provides us with a rare opportunity to deeply examine the validity of the construction of modern medicine, which is armed by science, and focus more on technologies than on people’s values and more on new ideas than on conventional wisdom. The world’s responses to the COVID-19 emergency have revealed a badly weakened public health system – one of the three pillars of medicine, the other two being basic medicine and clinical medicine. A 100 years ago, public health was the only effective measure for combating infectious diseases, which were then the main cause of human death. It is still a decisive weapon against COVID-19 and other communicable diseases alike, but was barely recognized and trusted at the beginning of the pandemic by the general public and even some international strategists. However, the epidemic has been effectively contained in China by non-pharmacological public health measures, which saved valuable time for the development of vaccines in the country and probably hundreds of thousands of lives as well. Public health aims to improve the health of the entire population by using societal methods. It is not simply a medical issue, and building a strong public health system requires broad participation from various sections of society.

New coronavirus pneumonia has had a significant impact on people's health and safety since the outbreak in the early 2020. The latest version of the new coronavirus pneumonia virus is still spreading around the world. China's public health system has passed a severe test: under the leadership of the Chinese Communist Party and the joint efforts of people of all ethnic groups, China won a great victory in the anti-epidemic campaign. Nevertheless, the public health system has also identified some challenges that need to be actively addressed in the area of medicine and health during the prevention and control of epidemics. This article deals with these issues.

At the beginning of 2020, the global outbreak of the novel coronavirus COVID-19 posed a huge challenge to the governance capabilities of public health in various countries. In this paper, the SEIR model is used to fit the number of confirmed cases in each province in China, and the reduction rate of the basic reproduction number is used to measure the actual score of the control effect of COVID-19. The potential capacity of prevention and control of epidemics, in theory, is constructed, and we use the difference between theoretical ability and actual score to measure the ability of governance of public health. We found that there were significant differences between actual effect and theoretical ability in various regions, and governance capabilities were an important reason leading to this difference, which was not consistent with the level of economic development. The balance of multiple objectives, the guiding ideology of emphasizing medical treatment over prevention, the fragmentation of the public health system, and the insufficiency of prevention and control ability in primary public health systems seriously affected the government’s ability to respond to public health emergencies.

This chapter discusses the most important features of EU law on infectious diseases. Communicable diseases not only cross borders, they also often require measures that cross different areas of policy because of different vectors for disease transmission. The relevant EU law cannot be attributed to one sectoral policy only, and thus various EU agencies participate in protecting public health. The key agency is the European Centre for Disease Prevention and Control. Other important agencies include the European Environment Agency; European Food Safety Authority; and the Consumers, Health, Agriculture and Food Executive Agency. However, while integration at the EU level has facilitated protection of the public's health, it also has created potential conflicts among the different objectives of the European Union. The internal market promotes the free movement of products, but public health measures can require restrictions of trade. Other conflicts can arise if protective public health measures conflict with individual human rights. The chapter then considers risk assessment and the different tools of risk management used in dealing with the challenges of infectious diseases. It also turns to the external and ethical perspective and the role the European Union takes in global health.

AbstractThe plague has been wreaking havoc on people in Madagascar with the COVID-19 pandemic. Madagascar’s healthcare sector is striving to respond to COVID-19 in the face of a plague outbreak that has created a new strain on the country’s public health system. The goal and activities of the gradual epidemic of plague in Madagascar during COVID-19 are described in this research. In order to contain the plague and the COVID-19 pandemic in this country, we have suggested long-term recommendations that can help to contain the outbreak so that it may spread to non-endemic areas.
