Effect of Background Case Characteristics on Decisions in the Delivery Room

Background. The authors investigated whether obstetricians make different decisions about a medical test case depending on the characteristics of background cases that preceded the test case. Methods. Five hypothetical cases were sent to 1247 obstetricians. The outcome of interest was the proportion of physicians who elect to perform a cesarean on a borderline test case, presented with 4 background cases. Participants were randomly assigned to 1 of 3 conditions: 1) pathological background, in which the test case was preceded by abnormal cases, typically requiring cesarean; 2) physiological background, where the test case was preceded by relatively uncomplicated cases, often suggesting a less invasive treatment; and 3) control, where the test case appeared first. Results. A significantly higher proportion of respondents chose a cesarean when the test case was preceded by physiological cases (75.4%) than when it was preceded by pathological cases (52.2%). This tendency was observed among those actively and not actively involved in obstetrics and in physicians with different levels of training. Conclusions. A patient’s chances of undergoing cesarean section can be influenced by the immediately prior experience of the physician. This study with hypothetical vignettes found that background cases can influence physicians’ decisions. The test case was apparently perceived as more grave when it followed uncomplicated cases as compared to when it was preceded by abnormal cases. Such inconsistencies in decision making are unlikely to be fully resolved by expertise, as suggested by the lack of differences between physicians with different training levels. An understanding of such effects may contribute to more informed consideration of unappreciated influences in making decisions.

In this paper empirical finding from a study conducted at an aerospace company is compared to theory regarding Experience Feedback (EF), Lessons Learned (LL) and Decision Making (DM). The purpose with the study was to examine how EF within the organization was conducted and what problems and possibilities that was seen. A qualitative approach was taken and interviews and a workshop was conducted. The empirical findings show that EF exist on different levels within the organization but current feedback processes are currently leaning more towards archiving and storing than knowledge sharing and learning. Also passive dissemination approaches are mostly used whereas active dissemination within the correct context is needed The aim with this paper is to discuss issues and empirical findings that should be considered when creating work methods and systems that support learning by EF and LL dissemination.

In order to investigate to which extent naval ships can execute their operational scenario after damage, an early stage assessment of the vulnerability of distributed systems needs to be carried out. Such assessments are currently mostly done by evaluating the performance of predefined concepts. However, such an approach does not necessarily lead to the most desirable solution, since solutions outside the scope of the designer’s preconceived ideas or experience are inherently hard to investigate. This paper therefore proposes several steps towards an approach that enables a vulnerability assessment that is independent of predefined concepts. This is done by incorporating several additions to an existing system vulnerability approach developed by the authors, using a Markov chain. With this approach there is no longer a need for modelling individual hits or damage scenarios. Whereas the approach has previously been shown in concept, this paper introduces three improvements that contribute to the applicability of the approach: 1)it is scaled up in order to model a larger number of compartments and distributed systems, 2) the hit probabilities for different compartments can be adjusted, and 3) it is shown how the availability of main ship functions can be derived from the availability of individual connections. A test case that compares two powering concepts (conventional and full electric powering) of a notional Oceangoing Patrol Vessel (OPV) is provided to illustrate the principles behind the improvements. From the results the two main contributions of this paper can be obtained: 1)the possibility to assess the system vulnerability for different levels of required residual capacity at different impact levels, and 2) and the quantitative nature of the results, aiding ship designers and naval staff with understanding the consequences of various concepts on the system vulnerability.

This paper aims to explore the rationale of classroom negotiation - understood as a discussion between all participants in the teaching/learning process to decide on the organization of foreign language learning and teaching. It outlines relevant issues connected with the process syllabus and the benefits that can be expected from involving students in classroom decision making. The article presents results of research conducted in Polish schools among both students and teachers at different levels of education in order to see to what extent the foreign language syllabus is negotiated there.

Research in decision making and metacognition has long investigated the calibration of subjective probabilities. To assess calibration, mean ratings on a percentage scale (e.g., subjective likelihood of recalling an item) are typically compared directly to performance percentages (e.g., actual likelihood of recall). Means that are similar versus discrepant are believed to indicate good versus poor calibration, respectively. This chapter argues that this process is incomplete: it examines only the mapping between the overt scale values and objective performance (mapping 2), while ignoring the process by which the overt scale values are first assigned to different levels of subjective evidence (mapping 1). The chapter demonstrates how ignoring mapping 1 can lead to conclusions about calibration that are misleading. It proposes a signal detection framework that not only provides a powerful method for analyzing calibration data, but also offers a variety of measures of relative metacognitive accuracy (resolution).

Decisions at different levels of the supply chain can no longer be considered independently, since they may influence profitability throughout the supply chain. This paper focuses on the interest of multi-agent paradigm for the collaborative coordination in global distribution supply chain. Multi-agent computational environments are suitable for a broad class of coordination and negotiation issues involving multiple autonomous or semiautonomous problem solving contexts. An agent-based distributed architecture is proposed for better management of rush unexpected orders. This paper proposes a first architecture validated by a real and industrial case.

The issue of sustainability of smart cities is approached in this chapter from two different angles: dimensions sector-specific and city governance. Following a short review of smart city sectors, emphasis is placed on the concept of smart governance of cities. The concept of smart governance is reviewed especially regarding cooperation with city governments of other cities within the region or country and empowering citizens. Different levels are presented with focus on four main conditions for smart governance, namely participation in decision-making, public and social services, transparent governance, and political strategies and perspectives. The chapter presents sustainability of smart governance through a paradigm of sustainability as an effectual model based on sustainability, wellbeing, productivity, and resilience. Finally, the chapter addresses the success criteria of sustainable smart cities regarding different aspects, such open data, agility to new technologies, cross-sectoral harmonization, and careful policy, strategy, and program alignment.

IntroductionA common problem low-income and middle-income countries face is the scarcity of community-based rehabilitation (CBR) resources and low service utilisation among persons with severe mental illness (SMI). Despite this problem, the factors and pathways followed influencing one’s decision on service utilisation in China have not been fully comprehended. This study aims to develop a theory-based model that systematically describes the integrated decision-making process of mental health CBR utilisation among persons with SMI in China.Methods/DesignThis cross-sectional, mixed-methods study involves three main stages and is expected to last 3 years, from January 2018 to December 2020. In stage 1, the Social Exchange Theory is deployed as an analytical framework to comprehensively capture factors associated with tendency to use CBR services in China using semistructured interview methodology involving patients with SMI, their primary caregivers and CBR service providers. In stage 2, interpretive structural modelling will be applied to analyse the relationships between factors in different dimensions, at different levels and with different levels of impact. Stage 3 involves a multiregion survey among at least 300 family decision-makers (either the patient or their caregivers) in six communities in three cities to statistically validate the initial model derived in stage 2 using a further structural equation modelling.Ethics and disseminationEthical approval was granted by the Ethics Committee of Tongji Medical College, Huazhong University of Science and Technology (No 2017S319). All interviewees will be provided with written information about the study, and a signed consent will be retrieved prior to the interview. Rules on confidentiality and anonymity of data will be strictly followed. The findings of this study will be disseminated via international and domestic peer-reviewed journals, reports, conference presentations and symposium discussions. Reports will be submitted to the National Natural Science Foundation of China.
