Calcaneocuboid Joint Involvement in Calcaneal Fractures

The charts and radiographs of 48 surgically treated patients who underwent surgery for calcaneal fractures (right in 25 patients, left in 22, and bilateral in 1) between 1987 and 1994 were retrospectively reviewed. Coronal computed tomographic scans alone were obtained in 33 patients, and both coronal and axial computed tomographic scans were obtained in 9 patients. Three fractures exiting close to the calcaneocuboid joint (CCJ), but not involving the joint, were excluded. Nineteen patients (38.7%) had involvement of the CCJ. The extension of the fracture to the CCJ was apparent in anteroposterior or oblique radiographs or both in 18 patients. There was intra-articular fracture displacement of ≤1 mm in 6 patients, and 13 patients had a step or a gap of ≥2 mm with or without angulation. Eleven patients had joint depression type fractures, 6 had tongue type fractures, and 2 had comminuted fractures. Extension of the calcaneal fracture into the CCJ was significantly more common with the joint depression type calcaneal fractures (chi-square test; P = 0.008). The coronal computed tomographic images showed significant lateral subluxation of posterior facet fragments in 8 patients and considerable comminution of the lateral calcaneal wall with or without lateral subluxation of posterior facet fragment in 10 patients. These patients also had CCJ involvement, thus establishing a strong correlation between lateral subluxation of the posterior facet fragment or comminution of the lateral calcaneal wall and CCJ involvement. CCJ involvement is more common with joint depression type fractures. Extension of the fracture line into the CCJ should be suspected in presence of significant lateral column comminution or lateral talar subluxation.

Category: Hindfoot Introduction/Purpose: There has been considerable interest in recent years in performing a combined talonavicular and subtalar arthrodesis instead of a standard triple arthrodesis for a rigid hindfoot deformity. The ”medial double” (diple) avoids fusing the calcaneocuboid joint with its lateral incision, common bone block graft and high rate of complications. Although the results of the “medial double” are generally satisfactory, there are no reports of late midfoot abduction deformity, although it was predicted by Evans in 1975. Methods: A 79 year-old-male with long-standing skin psoriasis with joint involvement presented with severe bilateral flatfoot deformities. The left hindfoot was arthritic, deformed and painful. Despite a conservative course of orthotics and ankle braces, the pain persisted. Because there was a psoriatic plaque over the lateral side of the foot, it was decided to perform a ”medial double” arthrodesis with screw fixation, a plantarflexing 1st tarsometatarsal joint fusion for medial column stabilization, and a heel cord lengthening Results: Skin and bony healing were uneventful. Over the subsequent three years a progressive abduction deformity developed through the naviculocuneiform joints and the unfused calcaneocuboid joint. The patient had only occasional pain in the midfoot and wore wide shoewear to accommodate the deformity. Conclusion: A late abduction deformity can develop through the midfoot in patients who have the ‘medial double” procedure for hindfoot valgus. The cuboid settles proximally against the short distal calcaneus, and the naviculocuneiform joints sublux laterally. In patients with hindfoot valgus with severe lateral column shortening secondary to calcaneocuboid joint remodeling, it may be better to perform an actual triple arthrodesis with a bone graft in the calcaneocuboid joint instead of a “medial double.” Alternatively, one could add a naviculocuneiform arthrodesis to the ”medial double” arthrodesis to prevent this complication.

Objective: The aim of this study is to test the hypothesis that patients with calcaneal fractures who are surgically treated and have Böhler and Gissane angles restored to normal have a faster return to work than patients without restoration of these angles. Calcaneal fractures represent 1-2% of all fractures, and approximately 75% of these are articular, indicating surgical treatment. Joint involvement results in functional limitation and can lead to late complications such as chronic pain, subtalar arthrosis, difficulty walking, and deformities. Methods: Lateral radiographs of the calcaneus of 44 patients who underwent surgical treatment for calcaneal fracture during the period from 2014 to 2016 were analysed to measure the Böhler and Gissane angles and to evaluate the association of their restoration to normal with the time to return to work. Results: Among the patients, 70.2% presented restoration of the Böhler angle and 44.7% presented restoration of the Gissane angle with surgery. The mean time away from work was 8.38 months. A total of 76.6% of patients returned to the same function. Patients who had good fracture reduction had a shorter time to return to work, but this result was not statistically significant. Conclusion: Surgical restoration of angles can positively influence the functional outcomes of patients, but this is not the only variable and thus should not be exclusively used to analyse the functional outcome and time to return to work of patients. Level of Evidence III; Therapeutic Studies; Cases Series.

Statement of the Problem. The clinical significance of condyle-fossa relationships in the temporomandibular joint is a matter of controversy. Different studies have evaluated whether the position of the condyle is a predictor of the presence of temporomandibular disorder.Purpose. The purpose of the present study was to investigate the condylar position according to gender in patients with temporomandibular disorder (TMD) and healthy controls using cone-beam computed tomography.Materials and Methods. CBCT of sixty temporomandibular joints in thirty patients with TMD and sixty joints of thirty subjects without TMJ disorder was evaluated in this study. The condylar position was assessed on the CBCT images. The data were analyzed using Pearson chi-square test.Results. No statistically significant differences were found regarding the condylar position between symptomatic and asymptomatic groups. Posterior condylar position was more frequently observed in women and anterior condylar position was more prevalent in men in the symptomatic group. However, no significant differences in condylar position were found in asymptomatic subjects according to gender.Conclusion. This study showed no apparent association between condylar positioning and clinical findings in TMD patients.

Radiological anatomy is evaluated in three dimensions for preoperative and paroperative sur-gical planning, which helps the surgeon to identify his or her limits of dissection during sur-gery and avoid complications. Patients attending OPD at Institute of Otorhinolaryngology and Head & Neck Surgery, Kol-kata, India with features of sinonasal disease were advised CT scan depending upon the symptoms and course of the disease. 100 study subjects (200 sides) were selected during the study period between 01/01/2019 to 30/06/2020. Septa related directly related to ICA in 31%(n=31) cases of whom 12 patients (12%), 8 pa-tients (8%) and 11 patients (11%) had septa related to right ICA, left ICA and bilaterally ICA respectively. Septa attachment to optic nerve is seen in 27%(n=27) patients, of whom 10 patients (10%), 11 patients (11%) and 6 patients (6%) had attachment related to right optic nerve, left optic nerve and both optic nerves, respectively. On the right side ICA protrusion and pneumatization of ACP were both present in 8(8%) patients and on the left side in 7(7%) patients. Chi-Square test indicated a statistically significant association (P < 0.001). Pneumatized ACP and ON protrusion consistently en-countered on the right and left side in 7 patients (7%) and 8 patients (8%) respectively. Chi-Square test indicated a significant association between ACP pneumatization and ON protru-sion (P<0.001). This resolution helped in better demonstration of complicated sphenoid sinus anatomy well. Hence, we support 1 mm triplaner CT Scans for preoperative imaging assessment for all sinus surgeries.

Breast feeding is giving milk to be drunk to the baby from the breast. Uterus involution is a process how the uterus return to the condition back, before pregnanting after bearing. At the moment of suckling, happens a stimulus and brings the hormones out, such as oksitosin uses not only to stimulate some muscles constraction but also to stimulate the uterus, so that the process of uterus involution happens foster. According to the explanations above, the research aimed to know about the correlation between breast feeding and uterus involution. This research’s design was, cross sectional by the population of all childbirth mothers approximately 50 persons / month. By using accidental sampling technique we got 21 sample respondents. The place of research in RSIA Aura Syifa in Kediri Regency on 16th until 22nd of June 2009. From this research’s result, we got 14 persons (66,67%) with normal uterus involution, suckled in a good way, one person (4,67%) with normal uterus involution, suckled in a wrong way, 2 persons (9,52%) with abnormal uterus involution, suckled in a good way and 4 persons (19,05%) with abnormal uterus involution, suckled in a wrong way. Statistic test which used chi-square test, counted the probability frequency in advance, from that we got 3 columns with the score, less than 5, so that chi-square can not be continued and by doing exact fisher test, the score was 0,001. Because P with the grade mistake 0,05 smaller, so the conclusion was “Ho” is rejected, it meant “there was correlation between suckling and uterus involution”. Key Word : Breast feeding, uterus involution

<p class="Normal1"><em>Commitment is the attitude or behavior of likes or dislikes shown by someone against the organization at work. Indonesia is recorded as a nurse who has a low organizational commitment (76%) so it harms services. This phenomenon in the hospital can be seen from the indicator of the high turnover rate of nurses which reaches 30.9% which exceeds the standard &lt;5%. Many factors affect organizational commitment. The purpose of this study was to analyze the Personal Factors Associated with Organizational Commitment of Hospital Nurses. This study is a cross-sectional analytic study and purposive sampling technique with a sample of 59 nurses. Chi-Square test results obtained personal factors that have a relationship with organizational commitment Nurse Hospital is age with p = 0,000 and education with p = 0,000, personal factors not related to organizational commitment are gender p = 1,053 and years of service with p = 1,147. Hospitals should conduct research/surveys on Organizational Commitment periodically as an evaluation material in the context of the organization's development and development in the future</em>.</p>

ABSTRACT : The House index and Container Index in the buffer area of ​​the working area of ​​Balikpapan Sepinggan Airport is still above 1%, so the potential for the spread of dengue disease. Mobilization of people, goods and transportation equipment will increasingly affect the transmission of disease in ports and airports, especially for vector-borne diseases. This study aims to analyze the risk factors associated with larvae density of Aedes aegypti and describe the larvae index in the buffer zone of the Sepinggan Balikpapan Airport This study was a descriptive study with a cross sectional design. The sample in this study was 121 houses with a proportionate stratified random sampling, the research location was in the buffer zone of Sepinggan Balikpapan Airport in November 2018. The variables studied were houses with positive larvae containers, breeding sites and PSN behavior and larvasidation. The data was analyzed using the chi square test. There was a relationship between houses with larvae positive Aedes aegypti, behavior of Mosquito Nest Eradication (PSN) and larvasidation with larvae density of Aedes aegypti but not for breeding sites (p = 0.00 and 95% CI = 0.64), and   (p = 0.00 and 95% CI = 0.34). The description of several Aedes aegypti larvae index, namely House Index (HI) = 57.02%, Container Index (CI) = 24.36%, Bruteau Index (BI) = 148.76, and Flick Free Numbers (ABJ) = 42.98 %. Houses with larvae of Aedes aegypti larvae and PSN and larvasidation behavior were associated with larvae density of Aedes aegypti. The index of HI, CI and BI larvae is of high value so there is a risk of DBD transmission
