Community-based dementia day programmes: Common elements and outcome measures

Dementia Day programmes are considered important in supporting the well-being of both people living with dementia and their caregivers. There is, however, limited evidence on the effectiveness of these programmes. This article reports on a study undertaken in New Zealand on the effectiveness of community-based dementia day programmes. The small-scale pilot study was aimed at investigating the elements that make up an effective client-focused dementia day programme and the methods employed by organisations to measure the outcomes of these programmes. A mixed methods approach was employed with multiple stakeholders. The research revealed that effective day programmes comprised five core elements, and that surveys, reporting and auditing processes are routinely used to measure the quality of outcomes of day programmes. Although these findings are reflective of a specific context, it raises concerns about the nature and availability of evidence informing decisions regarding the design and implementation of day programmes internationally.

Abstract Artisanal and small-scale mining (ASM) has long been a vital source of livelihoods for rural populations in the global South. Yet, it has also been linked to a host of social, political and environmental adversities, including violent conflict. As environmental peacebuilding increasingly stresses the importance of livelihood improvement as a means of fostering peace in conflict-affected extractive societies, ASM formalization has been identified as a solution to mitigate the sector's challenges, thereby addressing underlying causes of conflict. This article critically investigates the contribution of ASM formalization to sustainable peace by focusing on its impact on the livelihood dimension of peacebuilding. It analyses the livelihood impact of three formalization interventions in the diamond sectors of two countries: cooperatives in Liberia, and, in Sierra Leone, ethical sourcing schemes and a community-based natural resource management initiative. In line with calls for a paradigm shift from a narrow legalization-centred understanding of formalization to a broader approach that accounts for livelihood quality, the analysis presented here focuses on interventions that were informed by the ideal of improving the well-being of ASM workers and communities. We propose three pathways through which ASM formalization could potentially contribute to livelihood enhancement: income security, working conditions and community benefits. Based on fieldwork, this article highlights the challenges of generating livelihood improvements through formalization. Even when specifically designed to address the needs of ASM communities, during implementation, they risk prioritizing a narrow conceptualization of formalization and thus failing to become a conductor of transformative change.

Parents from individualist cultures (those focused on autonomy of individuals; that is, Australian) may view their autistic children differently compared to parents from collectivist cultures (where community needs are valued over an individual’s, that is, South-East Asian cultures). As most research on autism and parenting has been undertaken in Western individualist cultures, knowledge of parenting beliefs and mental health within collectivist cultures is lacking. We compared the mental health, quality of life, well-being and parenting sense of competency between families raising an autistic child from two groups: 97 Australian parents and 58 parents from South-East Asian backgrounds. Children from both groups were receiving the same community-based early intervention. No group differences were found on the measures of mental health but, when compared to Australian parents, parents from South-East Asian backgrounds reported higher well-being and less impact on their quality of life resulting from their child’s autism-specific difficulties. Furthermore, a positive association between well-being and quality of life was only observed for South-East Asian parents. Hence, the views of, and responses to, disability for South-East Asian parents may act as a protective factor promoting well-being. This novel research indicates that culture plays a role in parenting autistic children and highlights the need to accurately capture cultural background information in research. Lay abstract We know that parents of autistic children experience poorer mental health and lower well-being than parents of non-autistic children. We also know that poorer mental health among parents of autistic children has been observed across different cultures. Most research focuses on Western cultures, so we know little about parental mental health and well-being of parents from different cultural backgrounds; yet, it is likely that cultural background contributes to how parents view their child’s condition and respond to the diagnosis. Here, we compared mental health, quality of life and well-being between families raising an autistic child from Australian backgrounds to families from South-East Asian backgrounds. All children in the current study were receiving the same community-based early intervention. When compared to the general population, parents had poorer mental health overall, but there were no differences between the two groups of parents. However, parents from South-East Asian backgrounds reported higher well-being and fewer difficulties associated with their child’s autism. These findings suggest that cultural background likely influences not only parent’s view of, and response to, their child’s autism, but also their own sense of well-being. As researchers and clinicians working with families of autistic children, we should more explicitly consider family’s cultural background within our work.

Purpose Bitner’s (1992) concept of servicescape has received widespread academic attention, resulting in many conceptual and empirical studies. By scanning the servicescape literature and other relevant concepts, Pizam and Tasci (2019) provided experienscape, an expanded version of servicescape, to be measured from different stakeholders’ perspectives with a multidisciplinary approach. This paper aims to build on Pizam and Tasci’s conceptualization of experienscape and expand its nomological network with other pertinent concepts related to different stakeholders with an interdisciplinary approach. Design/methodology/approach This is a conceptual study analyzing diverse literature related to servicescape, experienscape and other related concepts and theories to provide an integrated and holistic picture of experienscape for more robust theory development. Several new relationships are synthesized for hypothesis development and testing in future research. Findings The review of past research reveals that servicescape literature has mostly focused on outcomes for the benefit of brands and firms and missed outcomes for consumers and other stakeholders. In addition, servicescape literature lacks several critical concepts in the affective, cognitive and behavioral reaction domains, as well as moderator factors. The relevance of some theories such as branding (e.g. brand identity, personality, image, perceived quality, consumer value, brand value and self-congruity), cocreation/coproduction/codestruction, transformation, subjective happiness, subjective well-being and quality of life is completely overlooked. Research limitations/implications Experienscape is a container of complex systems where needs, wants and expectations of multiple stakeholders are entertained, often at the same time through dynamic interactions among multiple stakeholders. Thus, a holistic understanding of experienscape requires dynamic integration of theories explaining the behavior of different stakeholders by cross-fertilizing theories through interdisciplinary research rather than unidisciplinary or multidisciplinary research conducted in separate silos. Originality/value By adopting Pizam and Tasci’s (2019) experienscape concept, this study expanded the relational network of service environment components (i.e. sensory, functional, social, natural and cultural components of experienscape) by incorporating diverse theories and concepts that explain cognitive, affective and conative reactions of different stakeholders to an experience environment. Additionally, the current study recommends attention to human-centric outcomes such as transformation, subjective well-being, subjective happiness and quality of life, which were completely overlooked in previous servicescape research.

Abstract When children are exposed to serious life adversities, Ed Zigler believed that developmental scientists must expediently strive to illuminate the most critical directions for beneficial interventions. In this paper, we present a new study on risk and resilience on adolescents during COVID-19, bookended – in introductory and concluding discussions – by descriptions of programmatic work anchored in lessons learned from Zigler. The new study was conducted during the first two months of the pandemic, using a mixed-methods approach with a sample of over 2,000 students across five high schools. Overall, rates of clinically significant symptoms were generally lower as compared to norms documented in 2019. Multivariate regressions showed that the most robust, unique associations with teens’ distress were with feelings of stress around parents and support received from them. Open ended responses to three questions highlighted concerns about schoolwork and college, but equally, emphasized worries about families’ well-being, and positive outreach from school adults. The findings have recurred across subsequent school assessments, and strongly resonate with contemporary perspectives on resilience in science and policy. If serious distress is to be averted among youth under high stress, interventions must attend not just to the children's mental health but that of salient caregiving adults at home and school. The article concludes with some specific recommendations for community-based initiatives to address mental health through continued uncertainties of the pandemic.

ObjectivesResearch shows that cognitive stimulation therapy (CST) improves cognitive function, quality of life, and well-being of people with mild–moderate dementia. Despite consistent evidence and recommendations, CST is not routinely available in Ireland post-diagnosis. The aim of the current research was to develop and evaluate community-based CST for people with mild–moderate dementia, run by the Alzheimer Society of Ireland across four pilot sites in Ireland.MethodsParticipants with mild–moderate dementia attended once weekly CST sessions for 14 weeks. Baseline and post-intervention assessments were completed by CST participants, carers, and CST facilitators. Primary outcomes of interest for CST participants included quality of life (Quality of Life in Alzheimer Disease Scale), cognitive function (Montreal Cognitive Assessment), and subjective cognitive function (Memory Awareness Rating Scale-Functioning Subscale). Secondary outcomes included well-being, cognitive ability, satisfaction with cognitive performance, and engagement and confidence of CST participants; well-being of carers; and job satisfaction of facilitators. Post-intervention interviews supplemented quantitative analyses.ResultsIn total, 20 CST participants, 17 carers, and six CST facilitators completed evaluation assessments. Results showed that CST improved participants’ satisfaction with cognitive performance (p=0.002), level of engagement (p=0.046), level of confidence (p=0.026). Improvements on subjective cognitive function just fell short of significance (p=0.055). Qualitative analysis of interview data identified consistent themes of cognitive and overall benefits of CST; and provided support for quantitative data.ConclusionsCommunity-based CST positively impacted the lives of people with dementia and their families. This study supports prior recommendations that CST should be made routinely available to people with mild–moderate dementia, particularly in light of the lack of post-diagnostic interventions currently offered in Ireland.

The current study evaluated the effectiveness of a novel three-hour Guided Mindfulness Program on participants’ management of depression, anxiety and stress. The intervention included an educational component emphasizing the psychological value of mindfulness practice combined with a practical training component in mindfulness techniques. The Depression Anxiety Stress Scale was administered at three points in time before and after participation in the program. Additionally, semi-structured interviews were conducted to explore perceived quality of life, happiness, and stress 4 to 8 weeks following the program. All participants reported improvements in overall well-being and reported significant decreases in negative affect when comparing pre-program scores (baseline) to follow-up scores. Notably, qualitative interview results indicated that participants attributed most positive post-program results to informal mindfulness practice.

The article presents findings from a large-scale qualitative research study conducted as part of a three-year Erasmus+ project entitled Old Guys Say Yes to Community, which included partners from Slovenia, Portugal, Poland and Estonia. The project explored how inactive ageing affects the quality of life, health and well-being of men aged 60 years or more, and how (self-)exclusion from the community can lead to social and psychological ‘death’. The article highlights four interconnected themes which are inadequately, insufficiently, or simply not addressed by national institutions and often also the non-governmental sector in the researched countries. The themes – the pluralisation of transitions to retirement and ageing; absent bodies and invisible lives; hegemonic masculinity and gendered experiences; and community-based learning, action and spaces – are supported by well-defined issues and obstacles preventing men from integrating into the community and are completed with suggestions and recommendations to implement much-needed changes. In addition to these four themes, the article touches upon a series of subtopics and questions that should be addressed by further scientific research in the observed countries.

The article analyses various forms of care and social protection that forced-migrant transnational families exchange despite their individual members living in different countries. It presents outcomes of a small-scale empirical study of the family practices of mobile individuals from Syria and Afghanistan who arrived in Germany during and after the "long summer of 2015". Building on social protection research and transnational care studies, the article introduces the concept of care and protection assemblages, which highlights the heterogeneity, processuality and multi-scalar quality of migrant families’ efforts to improve well-being. It includes an empirical analysis that illustrates key elements of the proposed concept and shows the significance of cross-border circulation of remittances, the selectivity in the cross-border circulation of emotions and limitations on the cross-border circulation of hands-on and practical care. These findings are framed by an analysis of solidarity organizations at the meso-level and (multiscalar) securitized asylum policies at the macro-level in the German context. The proposed conceptual framework takes into consideration migrant families’ simultaneity of solidarity and inequality experiences by locating the examination of family-making at the micro-, meso-, and macro-levels of analysis.

Abstract Colombia’s Recreovía program offers community-based free physical activity (PA) classes in parks. We evaluated built and social environmental factors influencing Recreovía local park environments, and facilitated a consensus-building and advocacy process among community members, policymakers and academic researchers aimed at improving uptake and impact of the Recreovía program. We used a mixed-methods approach, with individual and contextual PA measurements and a resident-enabled participatory approach (the Our Voice citizen science engagement model). Recreovía participants were likely to be women meeting PA recommendations, and highly satisfied with the Recreovía classes. Reported facilitators of the Recreovía included its role in enhancing social and individual well-being through PA classes. Reported barriers to usage were related to park maintenance, cleanliness and safety. The Our Voice process elicited community reflection, empowerment, advocacy and action. Our Voice facilitated the interplay among stakeholders and community members to optimize the Recreovía program as a facilitator of active living, and to make park environments more welcoming.

Cities have a wide variety of green infrastructure types, such as parks and gardens. These structures can provide important ecosystem services (ES) with a major impact on human well-being. With respect to urban planning, special consideration must be given to such green infrastructure types when implementing measures to maintain and enhance the quality of life. Therefore, generating knowledge on the urban ES of differently scaled green infrastructure types is important. This systematic literature review provides an overview of existing studies which have explicitly investigated the urban ES of differently spatial-scaled green infrastructure types. By reviewing 76 publications, we confirm rising academic interest in this topic. The most frequently assessed urban ES belong to the category Regulating and Maintenance. Only a few have considered individual small structures such as green roofs or single gardens; green spaces are often aggregated into one, mostly city-wide, object of investigation, with resulting oversimplifications. Moreover, generalizing methods are mostly applied. Simultaneously, many studies have applied methods to evaluate location-specific primary data. More research is needed on small-scale structures, in particular to consider site-, and thus location-specific, parameters in order to successfully implement the ES concept into urban planning and to obtain realistic results for ES assessments.
