Prevalence of Telehealth in Nursing: Implications for Regulation and Education in the Era of Value-Based Care

Value-based care theoretically catalyzes the business case for telehealth. Hence, the purpose of this study was to define the proportion of a statewide nursing workforce who self-reported telehealth or telephonic nursing as their primary work setting in a U.S. state undergoing rapid transitions to value-based care. We conducted a secondary analysis of a 2017 statewide nurse relicensure survey ( n = 10,851), overall response rate 99%. The focus of the analysis was registered nurses who reported that they were currently working in Vermont or serving residents of the state ( n = 8,457). Analysis was limited to descriptive statistics. We found that 18.4% of respondents ( n = 1,556) reported their employment status as “telehealth/ working as a telephonic nurse.” Responding to a different question, 17.2% ( n = 1,458) defined “telehealth/telephonic” as their primary work setting. Thus, nearly one fifth of nurses practicing in the state were employed in telehealth, a role for which there is scant preparation in nursing education. The multistate practice of roughly one third of these nurses highlights the importance of the Enhanced Nurse Licensure Compact and raises questions about global telenurse practice. Taken as a whole, these findings have profound implications for health care policy development and implementation, ongoing workforce development and analyses, nursing regulation, education, and continuing education. New and renewed skills are needed to provide safe, effective, culturally relevant telehealth, and virtual care.

This article discusses the politics of social policy development in Hong Kong following the Asian financial crisis. It examines the cause, mode and political significance of social policy reform in an Asian late industrialiser that has been experiencing the twin pressures of economic globalisation and socio-economic change. Financial austerity has prompted the state to adopt social policy reforms through re-commodification and cost containment, resulting in the retrenchment of the residual welfare state. The state's policy choices are structured by local politics, including the state of political development and the path dependence nature of policy change. The article questions the effectiveness of the social authoritarian approaches adopted by the state in attempting to renegotiate the social pact with its citizens, and contends that progressive development in social policy is inevitably bound to democratisation.

Purpose Within the “wicked” concept of ageing, this paper aims to primarily model an integrated approach to identifying and evaluating opportunities that deliver innovative outcomes in Ageing Well Practice, Health and Economic Policy and Research Actions using a collaborative and entrepreneurial mindset. The strategic focus is on a “Boomer” (user)-driven and facilitated Network – that brings together health professionals, research specialists, technologists, ageing well providers, “encore” career specialists, life-style providers, community groups, wealth creation specialists and industry innovators to streamline the progression of identified concepts to valued users and markets and enhance the economy. Design/methodology/approach Using the unit of analysis for innovation, i.e. the “added-value” as perceived by the user and not simply a product or a technology, the identified “opportunity-outcome” will embed a new service concept or intervention, which embraces and promotes ageing well, independent living or resident-centred care in the community and delivers direct and indirect economic benefits. Findings The authors model a point of differentiation in facilitating existing ageing well policies in the community, through a focus on an integrated and multi-dimensional collaborative framework that can deliver user value and contributes to community and economic benefits. Research limitations/implications Generalising results without a commercial business case from this single strategic viewpoint requires caution. The positive outcomes from this innovation collaborative concept can be used to guide further policy development and business investment in ageing well needs. Practical implications Such an integrated innovation collaborative structure provides the capacity to identify ageing well opportunities, to contract enterprises, both SMEs’ and larger companies, for development of the opportunities into user-valued outcomes, to network venture resources and deliver these outcomes to a sustainable market of ageing well citizens. Social implications The Ageing Well Innovation collaborative framework identifies practical ways to integrate new concepts of ageing participation to be realised by the increasing number of “Boomers”. It provides a self-managing process for linking individuals, public and private parties to maximise information and ideas flow, and engagement of the skilled resources in the Boomer group. Originality/value The innovation collaborative structure proposed is not simply novel but is a targeted focus on entrepreneurship and innovation applied strategically to the needs of ageing boomers and community needs. The added-value is in the demonstrated enhancement to effective innovation outcomes in community ageing and the economy.

Objetivo: Refletir teoricamente sobre a Política Nacional (brasileira) para Prevenção e Controle do Câncer na Rede de Atenção à Saúde das Pessoas com Doenças Crônicas e articulações na formação do enfermeiro e na prática profissional. Metodologia: Reflexão teórica, por meio de questionamentos das pesquisadoras, discussões em grupo e busca na literatura. Resultados: Os trabalhadores que atuam no cuidado ao paciente oncológico possuem papel essencial na efetivação dessa política. Porém, para que sejam implementadas, é necessário que seus princípios estabelecidos façam sentido na formação e nas práticas profissionais. Conclusão: O ensino reflexivo sobre essa política na formação do enfermeiro contribui para melhorias em sua prática, assim como, práticas reflexivas e melhor qualificadas certamente impactam positivamente na formação profissional. Abstract There is a lack of clarity in the literature about what nursing research is or ought to be. There are ample guidelines and texts on how to do nursing research and on syllabus development and countless arguments on why nursing research is synonymous with qualitative approach. This suggests concerns within the profession that nursing research has been developing in a direction of its own that could have had very little influence on health care policy development. Some claim the opposite that nursing practice, in particular in North America, has been evidence-based for the past three decades. In this context, evidence-based is often interpreted as taking part in randomised clinical trials. In this paper, we use historical observations to identify areas of interest which include all issues related to education and training, expertise and working environment for future research.
