The effect of sit-stand workstations to decrease sedentariness in office work

Sedentary behaviors in office workers has become a major public health concern and several initiatives have been proposed to break up sedentary behavior patterns during the performance of computer-intensive office work (Neuhaus et al., 2014; Toomingas et al., 2012). Among such initiatives, the use of sit-stand workstations has been suggested to be one of the most promising by recent reviews (Commissaris et al., 2015). However, there still is only limited scientific evidence showing how effective sit-stand workstations are, in reducing sedentary behaviors and also documentation of their sustainability of use in studies of regular office work (i.e. as the “newness” of the system wears off, with time since introduction). This study aimed to document user behaviors and compare the use of two sit-stand workstation based interventions among two groups of administrative office workers: an “autonomous” group in which these workstations were introduced following some general ergonomic guidelines, and another “feedback-system” group in which the sit-stand tables were furnished with an automatic reminder system: users were prompted to accept, delay or refuse pre-programmed changes in table position, and if they accepted, the system automatically raised the table to a high (i.e. standing) position. This system was programmed for 10 minutes of standing after every accumulated 50 minutes of the table being in a low (i.e. sitting) position. In addition to documenting average use of the two sit-stand workstations, the sustainability of their use over two continuous months since their introduction was also studied. The results averaged over two months of usage of the two interventions showed that the percentage (%) sitting time was 87.4 (84.9-89.2) and 84.0 (83.5-85.4) on average ( P=0.001), and the frequency of switches between sitting and standing was 0.3 (0.2-0.3) per hour and 0.7 (0.6-0.7) per hour ( P=0.001) in the autonomous and feedback-system groups respectively. These usage behaviors were found to be stable over the entire 2 months of use in both groups. Thus, the sit-stand table system integrated with the automatic reminder system led to more reduction in sitting time and more switches in posture between sitting and standing as compared to the traditional sit-stand table, and behaviors of both groups were seen to be sustained over the 2-month intervention period. In conclusion, a semi-automated sit-stand table may thus offer an effective contribution to making sedentary behavior patterns more variable among office workers, and thus aid in preventing negative health effects associated with prolonged sitting times.

Background: Sedentary behavior is associated with increased risk of poor outcomes in breast cancer survivors, but underlying mechanisms are not well understood. This pilot study explored associations between different aspects of sedentary behaviors (sitting, prolonged sitting, sit-to-stand transitions, and standing) and breast cancer risk-related biomarkers in breast cancer survivors (n = 30). Methods: Sedentary behavior variables were objectively measured with thigh-worn activPALs. Breast cancer risk-related biomarkers assessed were C-reactive protein (CRP), insulin, and homeostatic model assessment of insulin resistance (HOMA-IR) and were measured in fasting plasma samples. Linear regression models were used to investigate associations between sedentary behavior variables and biomarkers (log CRP, insulin, and HOMA-IR). Results: Sit-to-stand transitions were significantly associated with insulin resistance biomarkers (P < .05). Specifically, each 10 additional sit-to-stand transitions per day was associated with a lower fasting insulin concentration (β = −5.52; 95% CI, −9.79 to −1.24) and a lower HOMA-IR value (β = −0.22; 95% CI, −0.42 to −0.03). Sit-to-stand transitions were not significantly associated with CRP concentration (P = .08). Total sitting time, long sitting bouts, and standing time were not significantly associated with CRP, insulin, or HOMA-IR (P > .05). Conclusions: Sit-to-stand transitions may be an intervention target for reducing insulin resistance in breast cancer survivors, which may have favorable downstream effects on cancer prognosis.

Background: Sedentary behaviors (SB) are activities associated with prolonged time periods of sitting, reclining, or laying down during waking hours. While the relation between SB and physical activity is complex, the common consensus is that SB is not the absence of physical activity and consists of its own determinants posing distinct health outcomes. These behaviors are of significant public health importance as the majority of Americans spend much of their days in SB and due to the increased risks of morbidity and mortality associated with SB. Adverse health outcomes associated with SB include cardiovascular disease, obesity, metabolic syndrome, hypertension and mortality. Television-viewing time and total sitting time have both been used widely to assess time spent in SB and therefore we hypothesize that TV-viewing time and total hours sitting will have high concordance and can be used interchangeably to represent sedentary behaviors. Methods: Using a sample (n = 2858) from the Images of a Healthy Worksite study, a group-randomized control trial involving nutrition and physical activity, the current study assessed how two different tools measured time spent in SB. Tertiles were created based upon the distribution of hours sitting and hours spent TV-vewing. Weighted Kappa statistics were used to measure concordance between hours of TV-viewing and total hours of time spent sitting for the entire sample and for subgroup analyses. Results: Weighted Kappa statistics for tertiles of hours sitting and tv hours were 0.0046, indicating little agreement on the television and the sitting items. Kappa w statistics for BMI categories also showed poor agreement (obese Kappa w = 0.02, overweight Kappa w = 0.002, and healthy subjects Kappa w = 0.006. The Kappa w statistics for males and females were -0.006 and 0.02, respectively. Kappa w statistics for the intervention group (Kappa w = 0.007) and for the control group (Kappa w = 0.0005) also showed little agreement. Conclusions: These results suggest that although commonly used, using television viewing time and total time spent sitting as interchangeable markers of SB, is not a valid assumption. We propose that total time spent sitting and hours spent television-viewing represent different domains within the construct of sedentary behavior. It is important for future researchers to use measures of sedentary behavior that capture the numerous domains involved in measuring SB to allow for the most sensitive measurement of this high-risk behavior.

Background. Sedentary behavior is a growing public health concern and may have particular relevance for the Parkinson disease (PD) population. However, the influence of sedentary time on factors associated with quality of life (QOL) in PD is unknown. The primary purpose of this study was to examine relationships between sedentary behaviors and markers of PD-specific QOL. A secondary purpose was to examine relationships between physical activity behaviors and QOL. Methods. We assessed sedentary and active behaviors using objective and interview measures and examined relationships between these behaviors and a measure of PD-specific QOL in individuals with PD. Results. Results demonstrated that sedentary time was significantly related to several aspects of QOL, including perceived deficits in the domains of mobility, cognitive processing, and communication. Additionally, results showed that time spent watching television was more strongly associated with lower levels of QOL than other more engaging sedentary activities. For physical activity, relationships between objective measures and QOL were weaker and only significantly associated with mobility. Time spent doing housework was associated with lower levels of QOL, whereas time spent in recreational activity was associated with lower levels of discomfort. Discussion. These results suggest that targeting decreases in sedentary behaviors (eg, reducing time spent watching television, breaking up prolonged bouts of sedentary time) may be effective for improving QOL in individuals with PD.

Background:Sedentary behavior is related to obesity, but measures of sedentary behaviors are lacking for adults. The purpose of this study was to examine the reliability and validity of the Sedentary Behavior Questionnaire (SBQ) among overweight adults.Methods:Participants were 49 adults for the 2 week test-retest reliability study (67% female, 53% white, mean age = 20) and 401 overweight women (mean age = 41, 61% white) and 441 overweight men (mean age = 44, 81% white) for the validity study. The SBQ consisted of reports of time spent in 9 sedentary behaviors. Outcomes for validity included accelerometer measured inactivity, sitting time (International Physical Activity Questionnaire), and BMI. Intraclass correlation coefficients (ICCs) assessed reliability and partial correlations assessed validity.Results:ICCs were acceptable for all items and the total scale (range = .51–.93). For men, there were significant relationships of SBQ items with IPAQ sitting time and BMI. For women, there were relationships between the SBQ and accelerometer inactivity minutes, IPAQ sitting time, and BMI.Conclusions:The SBQ has acceptable measurement properties for use among overweight adults. Specific measures of sedentary behavior should be included in studies and population surveillance.

Abstract Background. Some types of sedentary behaviors tend to cluster in individuals or groups of older adults. Insight into how these different types of sedentary behavior cluster is needed, as recent research suggests that not all types of sedentary behavior may have the same negative effects on physical and mental health. Therefore, the aim of this study was to identify gender-specific typologies of older adults’ sedentary behavior, and to examine their associations with health-related and socio-demographic factors. Methods. Cross-sectional data were collected as part of the BEPAS Seniors, and the Busschaert study among 696 Flemish older adults (60+). Typologies of self-reported sedentary behavior were identified using latent profile analysis, and associations with health-related and sociodemographic factors were examined using analyses of variances. Results. Five distinct typologies were identified from seven sedentary behaviors (television time, computer time, transport-related sitting time, sitting for reading, sitting for hobbies, sitting for socializing and sitting for meals) in men, and three typologies were identified from six sedentary behaviors (television time, transport-related sitting time, sitting for reading, sitting for hobbies, sitting for socializing and sitting for meals) in women. Typologies that are characterized by high television time seem to be related to more negative health outcomes, like a higher BMI, less grip strength, and a lower physical and mental health-related quality-of-life. Typologies that are represented by high computer time and motorized transport seem to be related to more positive health outcomes, such as a lower body mass index, more grip strength and a higher physical and mental health-related quality-of-life.Conclusions. Although causal direction between identified typologies and health outcomes remains uncertain, our results suggests that future interventions should better focus on specific types of sedentary behavior (e.g. television time), or patterns of sedentary behavior, rather than on total sedentary behavior.

Sedentary behavior is highly prevalent despite growing evidence of adverse effects on the cardiovascular and metabolic system that are independent of the level of recreational physical activity (PA). We present results for the association between sitting time and cardiovascular disease (CVD) from selected cohort and cross-sectional studies published in or after the year 2010 according to the domains where sitting time is accumulated during the day. These include TV viewing, occupational sitting, and sitting during transportation as well as overall sitting. The outcomes considered in this review are total CVD, coronary heart disease, and stroke as well as CVD risk factors—namely, hypertension, hypercholesterolemia, and type 2 diabetes and their associated biomarkers. Finally, several current issues with regard to studying the effects of sitting time on CVD are discussed, including how sedentary behavior is assessed, isotemporal substitution modeling, examination of joint associations for sitting and PA, and benefits of breaks in sitting time. Overall, the scientific evidence supports public health recommendations that encourage adults to limit their sedentary time in order to improve their cardiovascular health.

Abstract Background Sitting time is associated with adverse physical and mental health outcomes, and premature mortality. Office workers sit for prolonged periods, so are at particular risk. Scientific advances in public health threats are predominantly communicated to the public through media reports. Aims This study aimed to examine office workers’ impromptu responses to media coverage of scientific evidence related to the health risks of sedentary behaviour. Methods Semi-structured interviews were run with 26 office workers (mean age 35 years), recruited from four organizations in southern England. Within the interview, each participant provided a ‘think-aloud’ narrative as they read three real-world news reports relating to sedentary behaviour. Thematic analysis was conducted on verbatim transcripts. Results Three themes were extracted from the data: gauging the personal relevance of the news reports; questioning their trustworthiness and challenging the feasibility of proposed sitting-reduction strategies. Participants voiced scepticism about the applicability of the reports to their personal circumstances, and the validity of the reports and the scientific evidence underpinning them. Conclusions Researchers, press officers and journalists should emphasise the ways in which participants in research studies represent the broader population of office workers, and offer greater transparency in reporting study methods, when reporting scientific advances in sedentary behaviour.

Background:The purpose of this study was to determine whether an alert to get up once per hour while at work would reduce sitting time, increase sit-to-stand transitions, and improve perceived wellness in women with sedentary jobs.Methods:Female university staff and administrators (48 ± 10 years) were randomly assigned to control-control (CC) (n = 22) or control-intervention (CI) (n = 22) groups. Both used a thigh-worn postural-based activity monitor for 2 weeks. The CC group maintained normal behaviors, whereas the CI group maintained behaviors during control week, but received hourly alerts on their computer during work hours in the intervention week. Time sitting and sit-to-stand transitions during an 8.5-hour workday were examined. A perceived wellness survey was completed at baseline and after the control and intervention weeks.Results:Among all participants (N = 44) during the control week, 68% of the workday was spent sitting and 41 sit-to-stand transitions occurred. An analysis of variance revealed no statistically significant differences in variables over time (P > .05). There was a significant increase in perceived wellness from baseline in both groups (P ≤ .05). Perceived wellness showed no statistically significant difference between groups.Conclusions:The intervention had no statistically significant effect on sitting time or sit-to-stand transitions. Participation improved perceived wellness in the absence of behavior change.
