Accuracy of Online Pictorial Resources for Common Hand Surgery Procedures

Background: Online medical platforms can provide patients with easily accessible information and greater opportunities to self-advocate. However, the lack of quality control and presence of inaccurate information can lead to miscommunications between the physician and the patient. The objective of this study was to examine the quality and accuracy of online pictorial information regarding common hand conditions. Methods: Medical image information was searched on the search engine Google ( http://www.google.com ) using the terms “de Quervain’s tenosynovitis,” “carpal tunnel syndrome,” and “trigger finger.” The first 20 illustrations to appear on the search were recorded along with the type of source. The images were then examined for veracity of information conveyed. Results: Sixty images were collected from 48 different Web sources. Nonacademic/private medical institutions were the most common image source (35%). The rate of erroneous images was 40%. Web sites of academic hospitals were most frequently the source of incorrect images. Of the conditions, trigger finger had the highest rate of errors (55%), most commonly occurring in positioning of the annular pulleys. The search results did not contain any Web sites from professional hand societies. Conclusion: The quality of online medical information is a significant but often overlooked aspect of health professional and patient education. This study demonstrates the prevalence of incorrect information online and the misunderstandings that patients can have about common conditions. Higher quality online resources are needed to improve patient education and patient-physician interactions. Avenues for improvement are to provide greater accessibility of educational resources offered by professional hand surgery organizations.

During recent years, the Internet has evolved into the largest computer network in the world, allowing access to vast amounts of information and services. Medical information is increasingly available to both patients and professionals, and ever more biomedical resources are becoming available on-line to assist in research, clinical medicine, and education. The Internet has always provided useful resources to otolaryngologists, implemented at various academic departments and national organizations or by specialists or specific medical web sites offering technical, scientific, and biomedical information. The purpose of this article is to provide as complete a list as possible of the World Wide Web sites accessible through the Internet that are of interest to otolaryngologists. It summarizes different types of resources available, including educational material, audiology sites, outcomes research, discussion lists, research laboratories, publications, medical libraries, news and medical conferences, organizations, academic departments, otolaryngological and medical resources, medical and surgical equipment and suppliers, and miscellaneous other sites of interest to otolaryngologists. This review is intended to present the wealth of the accessible information on the Internet and to promote the use of the network to otolaryngologists who do not have extensive experience in computers or telecommunications.

Abstract Introduction The objective of this study was to assess the usefulness of Instagram posts for users seeking information about common hand conditions. Materials and Methods An internet-based study was performed of the top 200 trending posts on Instagram for each of the tags #carpaltunnel and #triggerfinger. The trending position (1 through 200), number of likes (for all posts), and number of views (for video posts only) were recorded. Each post was assessed to determine whether it would be useful for a user seeking medical information. Useful and not useful posts were further classified into thematic categories. Results Useful posts comprised only 21% of #carpaltunnel posts and 12% of #triggerfinger posts. #carpaltunnel had a significantly higher proportion of useful posts than #triggerfinger. For both #carpaltunnel and #trigger finger, useful posts were not associated with trending position or number of likes. Useful posts commonly provided a clinical image or an anatomical diagram. Posts that were not useful were generally irrelevant to the hand condition, but a minority provided information not supported by evidence or were solely advertising material. Conclusion Hand surgery patients can be educated that, in its current form, Instagram is not a source of useful information about common hand conditions.

AbstractStatistics, the science of numerical evaluation, helps in determining the real value of a hand surgical intervention. Clinical research in hand surgery cannot improve without considering the application of the most appropriate statistical procedures. The purpose of the present paper is to approach the basics of data analysis using a database of carpal tunnel syndrome (CTS) to understand the data matrix, the generation of variables, the descriptive statistics, the most appropriate statistical tests based on how data were collected, the parameter estimation (inference statistics) with p-value or confidence interval, and, finally, the important concept of generalized linear models (GLMs) or regression analysis.

ABSTRACT Objective: To describe the clinical and epidemiological characteristics of people diagnosed with carpal tunnel syndrome (CTS) treated at a hand surgery outpatient clinic of a regional referral service. Methods: Interview and specific medical examination of 150 people diagnosed with CTS underwent, and statistical analysis of the results obtained. Results: Women smokers, in the fifth decade of life, married, overweight and educated until the elementary level prevailed in our study. Arterial hypertension and diabetes were the most reported comorbidities, and Durkan’s test was the most prevalent. Conclusion: CTS is prevalent in women in their fifth decade of life, with bilateral involvement and high school. The hypertension and diabetes are the most common diseases in people with CTS. ABSTRACT BACKGROUND: The purpose of this study was to establish a subjective patient experience with wide awake local anaesthesia no tourniquet (WALANT) procedures performed in the institution from May 2019 to March 2020. WALANT surgery was initiated to improve standard operating procedure and to decrease theatre burden METHODS: This prospective, descriptive study included 100 patients with a mean age of 59 years who required either a carpal tunnel or trigger finger release. The patients' pain experience was documented on the visual analogue scale (VAS) for the local anaesthetic injection and the surgical procedure. Overall experience was assessed on the patient's preference to have the procedure performed by the WALANT method or the conventional method RESULTS: One hundred patients were included, of which 67 had medical comorbidities. The mean VAS score was 1.5 (SD±1.6) with pain on injection. The mean VAS pain score during the surgical procedure was 0.2 (SD±0.7). One hundred per cent of patients (100/100) felt they would do the WALANT outpatient procedure again instead of admission to hospital and surgery in the theatre. Two complications occurred related to wound care problems, and were successfully managed. None of the patients required reoperations for incomplete release of the carpal tunnel or trigger finger surgery CONCLUSION: The results of this study suggest that minor hand surgery using the WALANT protocol can be performed effectively and with high patient satisfaction rates in the orthopaedic outpatient clinic, and is a useful tool in the skillset of a hand surgeon Level of evidence: Level 4 Keywords: WALANT, hand surgery, trigger finger, carpal tunnel release

AbstractTrigger finger (TF) and carpal tunnel syndrome (CTS) are two common non-traumatic hand disorders that frequently co-occur. By identifying TF and CTS cases in UK Biobank (UKB), we confirmed a highly significant phenotypic association between the diseases. To investigate the genetic basis for this association we performed a genome-wide association study (GWAS) including 2,908 TF cases and 436,579 European controls in UKB, identifying five independent loci. Colocalization with CTS summary statistics identified a co-localized locus at DIRC3 (lncRNA), which was replicated in FinnGen and fine-mapped to rs62175241. Single-cell and bulk eQTL analysis in fibroblasts from healthy donors (n=79) and tenosynovium samples from CTS patients (n=77) showed that the disease-protective rs62175241 allele was associated with increased DIRC3 and IGFBP5 expression. IGFBP5 is a secreted antagonist of IGF-1 signaling, and elevated IGF-1 levels were associated with CTS and TF in UKB, thereby implicating IGF-1 as a driver of both diseases.

The process of making a medical decision is characterized by a lack of knowledge and inconsistency of the available information, the lack of the possibility of attracting competent medical experts, limited time resources, incomplete or inaccurate information about the patient's condition. These aspects may be the causes of medical errors, which lead to further aggravation of the problem situation. Purpose – it is necessary to define and justify managerial medical decisions and types of medical information in conditions of uncertainty, when each variant of the sets of outcomes of the situation (recommendations) has its own unique set of values. The fundamental difference between this process for medical use is the concept of the "best medical solution", in which the key role is given to the patient's state of health in obtaining and evaluating alternatives, as well as the need to take into account the time, adverse reactions of the body and the costs of implementing this solution. In the medical field, support for medical decision-making can be classified as organizational-managerial and therapeutic-diagnostic, but both are determined by the position of the person making the medical decision and are aimed at effective management of the medical institution as a whole. The article describes the causes and factors of the nature of uncertainty in the tasks of supporting medical decision-making in medical-diagnostic and organizational-managerial areas. The analysis of the features of supporting medical decision-making in conditions of uncertainty is carried out. Approaches and directions in this area, as well as the concept of “solution”, are considered. The essence of the management medical decision is reflected. The classification of management medical decisions is given, the requirements that are imposed on them are highlighted. The features of the development of management medical solutions in the conditions of incompleteness and uncertainty, the problems that arise when they are implemented in information systems are presented. The general scheme of the process of creating a management medical solution is shown. The features of making group and individual decisions are reflected. The algorithm of actions of the person making the medical decision in the conditions of uncertainty, incompleteness and risk in medical subject areas is presented.

The aim of this chapter is to review the way portal technology can assist users seeking medical information. There has been an increase in health Internet usage, and better health-care delivery outcomes are predicted as users are better informed when making medical decisions. At the same time, there is much concern about the need for medical portals to meet community information needs. This chapter discusses what constitutes an intelligent portal, discusses desirable portal components and attributes of intelligent portal features, and how these can be implemented to meet the needs of diverse users. Seven Australian medical Web sites have been analysed according to intelligence features. The results and analysis are presented and discussed, in particular, with respect to their functionality as defined for intelligent portals. The discussion is focused on the extent to which these attributes help users with their information seeking and therefore support their decision-making processes.
