Abstract Background Strain distribution was investigated to assess the occlusal resistance alterations in immature teeth under different occlusal force. Methods In vitro apexification models of teeth with a funnel-shaped immature apex were obturated with mineral trioxide aggregate (MTA; ProRoot MTA) using different combinations of core materials (10/group): group 1, full-length orthograde obturation of MTA; group 2, a 5-mm MTA apical plug with a composite core; group 3, a 5-mm MTA apical plug and back-filling with warm gutta-percha. Teeth with calcium hydroxide (CH)-medicated canals and untreated teeth with normal apices were tested as controls. The teeth were arranged between two adjacent normal-apex teeth, embedded in a resin mold with a simulated periodontal ligament space. Strain data were recorded from the 3-unit teeth assembly under static compressive occlusal forces (50, 100, 200, and 300 N). Measurements were repeated 20 times for each condition, and the data were statistically analyzed. Results The immature teeth showed altered occlusal force resistance, placing increased strain on adjacent teeth. Teeth with CH-medicated canals showed significantly inferior occlusal resistance under all tested forces (P < 0.05). Application of an MTA plug with deep composite resin core resulted in significantly better stress-bearing capacity especially under forces of 50 and 300 N (P < 0.05). Conclusions The pattern of occlusal force distribution in immature teeth differed according to the canal obturation materials used for apexification. Immature teeth with an MTA apical plug showed more favorable occlusal force resistance than those with CH-medicated canals.

ABSTRACT Objective: The purpose of this study was to evaluate fracture resistance of teeth with immature apices treated with coronal placement of mineral trioxide aggregate (MTA), bioaggregate (BA), and Biodentine. Materials and Methods: Forty-one freshly extracted, single-rooted human premolar teeth were used for the study. At first, the root length was standardized to 9 mm. The crown-down technique was used for the preparation of the root canals using the rotary ProTaper system (Dentsply Maillefer, Ballaigues, Switzerland) of F3 (30). Peeso reamer no. 6 was stepped out from the apex to simulate an incompletely formed root. The prepared roots were randomly assigned to one control (n = 5) and three experimental (n = 12) groups, as described below. Group 1: White MTA (Angelus, Londrina, Brazil) was prepared as per the manufacturer's instructions and compacted into the root canal using MAP system (Dentsply Maillefer, Ballaigues, Switzerland) and condensed by pluggers (Angelus, Londrina, Brazil). Group 2: The canals were filled with DiaRoot-BA (DiaDent Group International, Canada). Group 3: Biodentine (Septodont, Saint Maur des Fosses, France) solution was mixed with the capsule powder and condensed using pluggers. Instron was used to determine the maximum horizontal load to fracture the tooth, placing the tip 3 mm incisal to the cementoenamel junction. Mean values of the fracture strength were compared by ANOVA followed by a post hoc test. P < 0.05 was considered statistically significant. Results: No significant difference was observed among the MTA, BA, and biodentine experimental groups. Conclusion: All the three materials tested, may be used as effective strengthening agents for immature teeth.

The purpose of this study was to evaluate the apical seal in root apex treated with different demineralization agents and retrofilled with mineral trioxide aggregate (MTA) using marginal dye leakage. Fifty-six, human single-rooted teeth were instrumented, filled, resected and had retrofilling cavities prepared with ultrasonic tips. Demineralizing agents were applied before the apical cavities were retrofilled with Pro Root MTA. The specimens were assigned to 4 groups (n=14), as follows: group 1 (no demineralizing agent); group 2 (35% phosphoric acid, for 15 s); group 3 (17% EDTA solution, pH 7, for 3 min); and group 4 (24% EDTA gel, pH 7, for 4 min). The extension of dye (2% rhodamine B, at 37°C, for 24 h) penetration was measured in millimeters using a stereomicroscope. Results were statistically analyzed by ANOVA and Tukey's test at 5% significance level. Among the experimental groups, the least extension of dye penetration was observed in group 1 (1.89 mm), followed by groups 2 (2.18 mm), 4 (2.54 mm) and 3 (2.64 mm). No statistically significant differences (p<0.05) were found in marginal microleakage among groups 1, 2 and 4 and groups 2, 3 and 4. Based on the results obtained in this study, it may be concluded that the application of demineralizing agents cannot be recommended when MTA is used in periradicular surgeries.

Objectives: The purpose of this in vitro quantitative laboratorial study is to compare the ability of three filling techniques to fill simulated lateral canals.Methods: Thirty extracted, single-rooted human teeth were used. After cleaning and shaping, three lateral canals were created, one in each third. The teeth were randomly separated into three groups: continuous wave of condensation (Group 1); thermomechanical compaction (Group 2); and lateral condensation (Group 3). The teeth were cross-sectioned, making the cut through points over the lateral canals; thus, 90 specimens were obtained. Each specimen was immersed in a polyester resin, and the blocks were polished. Images were obtained using a stereoscopic lens (40x). Radiographic analysis was performed, followed by a filling linear measure using the Image Tool 3.0 program (University of Texas). Data were statistically analyzed using SPSS 12.0 for Windows (Kruskal- Wallis test).Results: A greater number of simulated lateral canals were obturated in Groups 1 and 2. Group 2 presented the largest percentage of linear measure of lateral canals filling with gutta-percha and sealer. No statistical differences were found between Group 1 and Group 2 when we analyzed the filling with gutta-percha and sealer or just sealer (P>.05).Conclusions: Thermoplasticized gutta-percha filling techniques (Groups 1 and 2) are better for filling lateral canal with gutta-percha and sealer or with just sealer than lateral condensation (Group 3). ABSTRACTObjectives: The aim of this study was to assess the apical sealing ability of different core materials used with Epiphany™ sealer and to evaluate the effect of Resilon/Epiphany™ system on creating an apical mono-block seal.Methods: Fifty-five extracted human teeth were used. Forty-five were selected and randomly divided into three groups and ten teeth were used as positive and negative control groups. In Group 1, roots were obturated using Resilon™ and Epiphany™, In Group 2, roots were obturated using tapered single-cone gutta-percha and Epiphany™ and in Group 3, roots were obturated using Thermafil obturators and Epiphany™ sealer. The apical leakage was measured using the dye penetration methodology. The teeth were cleared, and the linear extent of dye penetration was measured with a stereomicroscope. Data were analyzed by the one-way ANOVA and Tukey HSD tests.Results: The teeth filled with Thermafil obturators and Epiphany™ sealer displayed the most apical leakage. The least apical leakage was shown with Resilon™ and Epiphany™. Groups 1 and 2 had significantly leaked less than Group 3.Conclusions: Based on the data of this study, although Resilon™ and Epiphany™ sealer showed the least apical leakage, statistical analysis showed no difference between gutta-percha and Epiphany™. Abstract In this study, the aim was to assess the in vitro apical microleakage of a resin-based sealer used with two different adhesives. Thirty nine freshly extracted maxillary incisors were used. The teeth were decoronated at the cemento-enamel junction with a water-cooled fissure bur. Chemo-mechanical debridement of the root canals was accomplished with the step-back technique. The smear layer was removed by 19% ethylenediamine tetra acetic acid (EDTA). The roots were then divided into three experimental groups of thirteen teeth in each. Specimens in group 1 were filled with gutta-percha, AH Plus sealer, and water-based adhesive system (Syntac Single Component). Group 2 specimens were filled with gutta-percha, AH Plus sealer, and acetone-based dentin adhesive (Prime & Bond NT). Specimens of group 3 were filled with only gutta-percha and AH Plus sealer (no adhesive was applied). The teeth were immersed into 2% methylene blue solution. Apical sealing qualities were assessed by measuring the linear dye penetration with a stereomicroscope. Dentin tubule penetration was observed under scanning electron microscopy (SEM). ABSTRACT The aim of this study was to compare and evaluate the coronal microleakage following endodontic treatment of teeth restored with three different intracoronal restorations using calcium-45 radioisotope and autoradiography. A total of 48 extracted human mandibular premolars with single canals were divided into three experimental groups of 16 specimens each. The teeth were sectioned horizontally at the cementoenamel junction and at the middle-third of the root, orifices and canals were enlarged with Gates Glidden bur and finished using K files, and obturated with gutta-percha cones using lateral condensation technique. 3 mm of gutta-percha were removed from the coronal part of the teeth to prepare a clean 3 mm deep cavity for the coronal restorative material. Group 1 consisted of teeth restored with Ketac Molar Easymix; group 2 with Miracle Mix and group 3 with Filtek Z350. The teeth were then thermocycled and tested for microleakage using calcium-45 radioisotope. The results were analyzed using Kruskal-Wallis test and Mann-Whitney test. Filtek Z350 exhibited the least leakage followed by Ketac Molar Easymix and Miracle Mix, and the microleakage among the three restorative materials was statistically significant.

Abstract Objective: To compare the sealing ability of three single-cone obturation systems using a glucose leakage model. Materials and Methods: A total of 90 freshly extracted human mandibular incisors with oval canals were selected and their crowns were cut. The root canal of each sample was instrumented using a rotary crown down technique and then divided into four experimental (n = 20 each) and two control groups (n = 5 each). Samples in the experimental groups were filled as follows: Group 1, cold lateral condensation using Gutta-percha/AH Plus; Group 2, single-cone C-Points/Smart-Paste Bio Sealer; Group 3, single cone Bio-Ceramic Impregnated Gutta-Percha/Endo-Sequence Bio-Ceramic Sealer; Group 4, single-cone Resilon/RealSeal SE. After 7 days, the sealing ability of root canal fillings was tested at different time intervals using glucose leakage model. Glucose leakage values were measured using a spectrophotometer and statistically analyzed. Results: The four experimental groups presented significantly different glucose leakage values at all test periods (P < 0.05). At the end of the observation period, the cumulative glucose leakage values of Groups 2 and 3 were significantly lower than those of Groups 1 and 4 (P < 0.05). Conclusion: C-Points/Smart-Paste Bio and Bio-Ceramic Impregnated Gutta-Percha/Endo-Sequence Bio-Ceramic Sealer combinations provided the superior sealing ability over the lateral condensation technique.

Background:Systemic lupus erythematosus (SLE) is an autoimmune disease mainly affecting women of childbearing age. The interplay between genetic and environmental factors may contribute to disease pathogenesis1. At today, no robust data are available about the possible contribute of diet in SLE. Caffeine, one of the most widely consumed products in the world, seems to interact with multiple components of the immune system by acting as a non-specific phosphodiesterase inhibitor2.In vitrodose-dependent treatment with caffeine seems to down-regulate mRNA levels of key inflammation-related genes and similarly reduce levels of different pro-inflammatory cytokines3.Objectives:We evaluated the impact of caffeine consumption on SLE-related disease phenotype and activity, in terms of clinimetric assessment and cytokines levels.Methods:We performed a cross-sectional study, enrolling consecutive patients and reporting their clinical and laboratory data. Disease activity was assessed by SLE Disease Activity Index 2000 (SLEDAI-2k)4. Caffeine intake was evaluated by a 7-day food frequency questionnaire, including all the main sources of caffeine. As previously reported, patients were divided in four groups according to the daily caffeine intake: <29.1 mg/day (group 1), 29.2-153.7 mg/day (group 2), 153.8-376.5 mg/day (group 3) and >376.6 mg/day (group 4)5. At the end of questionnaire filling, blood samples were collected from each patient to assess cytokines levels. These were assessed by using a panel by Bio-Plex assays to measure the levels of IL-6, IL-10, IL-17, IL-27, IFN-γ, IFN-α and Blys.Results:We enrolled 89 SLE patients (F/M 87/2, median age 46 years, IQR 14; median disease duration 144 months, IQR 150). The median intake of caffeine was 195 mg/day (IQR 160.5). At the time of the enrollment, 8 patients (8.9%) referred a caffeine intake < 29.1 mg/day (group 1), 27 patients (30.3%) between 29.2 and 153.7 mg/day (group 2), 45 patients (51%) between 153.8 and 376.5 mg/day (group 3) and 9 patients (10.1%) >376.6 mg/day (group 4). A negative correlation between the levels of caffeine and disease activity, evaluated with SLEDAI-2K, was observed (p=0.01, r=-0.26). By comparing the four groups, a significant higher prevalence of lupus nephritis, neuropsychiatric involvement, haematological manifestations, hypocomplementemia and anti-dsDNA positivity was observed in patients with less intake of caffeine (figure 1 A-E). Furthermore, patients with less intake of caffeine showed a significant more frequent use of glucocorticoids [group 4: 22.2%,versusgroup 1 (50.0%, p=0.0001), group 2 (55.5%, p=0.0001), group 3 (40.0%, p=0.009)]. Moving on cytokines analysis, a negative correlation between daily caffeine consumption and serum level of IFNγ was found (p=0.03, r=-0.2) (figure 2A); furthermore, patients with more caffeine intake showed significant lower levels of IFNα (p=0.02, figure 2B), IL-17 (p=0.01, figure 2C) and IL-6 (p=0.003, figure 2D).Conclusion:This is the first report demonstrating the impact of caffeine on SLE disease activity status, as demonstrated by the inverse correlation between its intake and both SLEDAI-2k values and cytokines levels. Moreover, in our cohort, patients with less caffeine consumption seems to have a more severe disease phenotype, especially in terms of renal and neuropsychiatric involvement. Our results seem to suggest a possible immunoregulatory dose-dependent effect of caffeine, through the modulation of serum cytokine levels, as already suggested byin vitroanalysis.References:[1]Kaul et alNat. Aim. To compare the microleakage in class II composite restorations without a liner/with resin modified glass ionomer and flowable composite liner.Method. Forty standardized MO cavities were prepared on human permanent mandibular molars extracted for periodontal reasons and then divided into 4 groups of ten specimens. The cavity preparations were etched, rinsed, blot dried, and light cured and Adper Single Bond 2 is applied. Group 1 is restored with Filtek P60 packable composite in 2 mm oblique increments. Group 2 is precure group where 1 mm Filtek Z350 flowable liner is applied and light cured for 20 sec. Group 3 is the same as Group 2, but the liner was cocured with packable composite. In Group 4, 1 mm RMGIC, Fuji Lining LC is applied and cured for 20 sec. All the teeth were restored as in Group 1. The specimens were coated with nail varnish leaving 1 mm around the restoration, subjected to thermocycling, basic fuchsin dye penetration, sectioned mesiodistally, and observed under a stereomicroscope.Results. The mean leakage scores of the individual study groups were Group 1 (33.40), Group 2 (7.85), Group 3 (16.40), and Group 4 (24.35). Group 1 without a liner showed maximum leakage. Flowable composite liner precured was the best.

The purpose of this in vitro study was to evaluate the ability of a silicon-based root canal sealer, compared to zinc oxide and eugenol and an epoxy resin-based sealers, for filling of simulated lateral canals. Thirty extracted single-rooted human teeth were selected, conventional access was made and the working length was established 1 mm from the apical foramen. Three simulated lateral canals, one in each root third (coronal, middle and apical) were prepared in both the mesial and distal surfaces of each tooth using a size 15 reamer adapted to a low-speed handpiece. Each root canal was instrumented using ProTaper rotary files up to file F3 at the working length, and then irrigated with 2.5% NaOCl followed by EDTA. The teeth were assigned to 3 groups (n=10), according to the root canal sealer: Roeko Seal (Group 1), Sealer 26 (group 2) and Grossman's sealer (Group 3). Gutta-percha cold lateral condensation technique was performed in all groups. Postoperative radiographs were taken and the images were projected for evaluation of the quality of lateral canal filling. Data were submitted to statistical analysis by Kruskal Wallis test at 5% significance level. The results showed that Grossman's sealer filled a larger number of lateral canals than Roeko Seal (p<0.05) and Sealer 26 (p<0.01). It may be concluded that Roeko Seal silicone-based root canal sealer was not as effective as the Grossman's sealer for filling of simulated lateral canals. The lateral canals localized in the apical third of the root were more difficult to be filled.
