Abstract Background Dental implants are sometimes initially placed in a wrong position leading to esthetic damage, which is difficult to solve with prosthetics. Moreover, implants placed in the anterior sector, like ankylosed teeth, are frequently found in a wrong position over time with infraocclusion because of continuous anterior alveolar growth. Different treatments have been proposed to manage the consequences of malpositioned dental implants. Case presentation This paper describes a surgical and orthodontic new procedure that can be used to relocate an implant in a wrong position: the Orthodontic Bone Stretching technique (OBS), which involves deep partial osteotomies combined with heavy orthodontic forces. The applied force facilitates esthetic rehabilitation with a movement towards the occlusal plane and can modify the implant axis and the gingival line alignment. This relocation is made possible thanks to a bone stretching phenomenon in the surgical area without immediate mobilization or repositioning of an alveolar segment. Three cases with the need for implant repositioning are presented here and were treated with the OBS technique. Conclusion In the three cases presented, implant relocation was successfully performed with the OBS technique and the prosthetic crown was modified to improve esthetic results.

AbstractObjectivesDescribe a case with axonal Charcot-Marie-Tooth (CMT) type 2W, a neurological disease characterized by peripheral neuropathy typically involving the lower limbs and causing gait alterations and distal sensory-motor impairment.Case presentationWe report this case, where the application of massive genetic sequencing (NGS) with clinical exome in a molecular genetics laboratory enabled to detect the presence of candidate variants of the clinic of the patient.ConclusionsThe variant detected in HARS gene suggests that this variant could be causative of the symptoms of the patient, who went undiagnosed for 20 years and experienced an exacerbation of symptoms over time.

Background. The surface properties of implants are effective factors for increasing the osseointegration and activity of osteoprogenitor cells. This study compared the stability of dental implants with sandblasted and acid-etched (SLA) and modified surfaces (SLActive) using the resonance frequency analysis (RFA). Methods. In a split-mouth design, 50 dental implants with either SLA surface properties (n=25) or modified (SLActive) surface properties (n=25) were placed in the mandibles of 12 patients with a bilateral posterior edentulous area. Implant stability was measured using RFA (Osstell) at implant placement time and every week for 1, 2, and 3 months before the conventional loading time. Results. One week following the implantation, implant stability increased from 70 to 77.67 for SLA and from 71.67 to 79 for SLActive (P<0.05). Stability improved each week except in the 4th week in SLActive surface measurements. No significant differences were observed between the groups at 2 and 3 months (P>0.05). Conclusions. For both implant surfaces, increased stability was observed over time, with no significant differences between the groups.

ABSTRACT Objective: The aim of the present study was to clarify the biomechanics of en-masse retraction of the upper anterior teeth and attempt to quantify the different forces and moments generated using mini-implants and to calculate the amount of applied force optimal for en-masse intrusion and retraction using mini-implants. Methods: The optimum force required for en-masse intrusion and retraction can be calculated by using simple mathematical formulae. Depending on the position of the mini-implant and the relationship of the attachment to the center of resistance of the anterior segment, different clinical outcomes are encountered. Using certain mathematical formulae, accurate measurements of the magnitude of force and moment generated on the teeth can be calculated for each clinical outcome. Results: Optimum force for en-masse intrusion and retraction of maxillary anterior teeth is 212 grams per side. Force applied at an angle of 5o to 16o from the occlusal plane produce intrusive and retraction force components that are within the physiologic limit. Conclusion: Different clinical outcomes are encountered depending on the position of the mini-implant and the length of the attachment. It is possible to calculate the forces and moments generated for any given magnitude of applied force. The orthodontist can apply the basic biomechanical principles mentioned in this study to calculate the forces and moments for different hypothetical clinical scenarios.

Abstract The purpose was to predict the crown-to-implant ratio variation in the edentulous posterior mandibles rehabilitated with short dental implants. Hence, vertical and horizontal dimensions of dentulous posterior mandibles in a sample of 18- to 25-year-olds were measured, and correlations of these dimensions with sex and site were investigated. Mandibular computed tomography scans from 100 subjects were considered. Vertical and horizontal bone and tooth measurements were taken at the sites of the second premolar (PM), and the mesial and distal roots of the first and second molars (M1m, M1d, M2m and M2d, respectively). A hypothetical crown-to-implant ratio (C/I R) was calculated assuming the insertion of short and extra short implants (5, 6 or 7 mm), at 1.5 mm from the inferior alveolar canal, maintaining the position of the existing occlusal plane. All vertical bone dimensions decreased from the PM to the M2d. Width measurements increased from the mesial (PM) to the distal sites (M1m, M1d, M2m and M2d). Males had significantly greater vertical and horizontal measurements than females at all sites. The mean C/I R was higher than 2 for all sizes of implant. The C/I R was lower for the second molar than for the second premolar, while it was similar for the first molar and the second premolar. Males had a higher C/I R than females. Computed tomography can be used to study the anatomical features of alveolar bone, and to predict some clinical aspects of prosthetic rehabilitation with implants, such as the crown-to-implant ratio in conditions of serious bone atrophy.

Abstract The aim of this case presentation is to outline some of the advantages that an adjunct orthodontic treatment can offer in a comprehensive oral rehabilitation of an adult patient. Adjunctive orthodontic treatment is usually limited to a dental arch or to a group of teeth within an arch. The purpose of dental movements in this case will be to position the teeth in a way that makes the restoration or replacement of damaged or missing teeth as easy as possible and with minimal dental tissue sacrifice. In addition, the improved position of the teeth will create a healthier periodontal environment that is easier to maintain over time.

Introduction. Peri-implantitis is a condition of the tissues adjacent to dental implants, which can occur in patients with implant-prosthetic restorations, following inadequate prophylactic measures. However, these peri-implants, untreated or improperly treated, can lead over time to quite severe complications, which can quickly turn into emergencies of low, medium or increased intensity in the usual dental care. Purpose. Peri-implantitis is not a major dental emergency, but ignoring and/or treating it incorrectly can lead to complications that are considered major in dentistry. That is why we tried through this study to verify the knowledge related to peri-implantitis as well as the complications that may arise from the nontreatment of these diseases, among the practitioners involved in this field. Material and method. The study was conducted with the help of 37 specialists involved in implantprosthetic treatments, aged between 37 and 53 years, using a 7-point questionnaire. Results and discussions. The results show the concern of specialists in the field of oral implantology regarding peri-implantitis, as well as the complications arising from them. Conclusions. Knowing the causes but also the complications arising from peri-implantitis are very important for the creating preventive programs, which with a reduced effort bring the long-term success of implant supported restorations.

Introduction: Acute Generalized Exanthematous Pustulosis (AGEP) is a cutaneous reaction that may appear after using certain medications, such as cephalexin. This disease is characterized by non-follicular sterile pustules, erythematous, urticaria, fever over , and leukocytosis. Cephalexin belongs to the family of β-lactam antibiotics, which are widely used to treat infections. However, cephalexin skin sensitivities have been reported rarely. Herein, in this case, we aimed to report a patient presented with AGEP due to cephalexin usage. Case Presentation: A 12-year-old boy presented with warm skin lesions that gradually appeared on the limbs, trunk, face, and neck after using cephalexin powder on his left leg injury. Because of his symptoms, acetaminophen, fexofenadine hydrochloride, loxoprofen sodium, and ointment, including difluprednate and hydrocortisone, were prescribed. Over time, the patient’s fever subsided, and 8 days later, the symptoms of AGEP, including urticaria, erythematous, and pustules in the neck and trunk, disappeared. Conclusions: Cephalexin is one of the antibiotics that doctors and patients consider when there is a possibility of infection. AGEP is a rare but severe reaction that can manifest as skin rashes in any age and sex following the use of cephalexin, so the patient should be careful when using this antibiotic.

This chapter looks at the causes and methods behind the Jacquerie’s sudden emergence after 28 May. The massacre at Saint-Leu-d’Esserent was a watershed moment that enabled the rapid transformation of latent resentments into large-scale, violent rebellion. Due both to recent military developments and economic dislocation connected with the Black Death, rural commoners in northern France were experiencing a crisis of the ‘moral economy’ severe enough to make some of them undertake previously unimagined action. But immediate mobilization required previous preparation. As sociologists have demonstrated, rebellion is not a process that happens by chance even if it is made possible by opportunity. It appears that the Jacquerie’s leaders were able to take advantage of pre-existing efforts to ready the countryside’s defences, as well as social and professional networks among commoners in the Beauvaisis. By 31 May, the rebellion was sufficiently organized to capture a traitor and transfer him to a local captain elsewhere who carried out a public execution attended by hundreds of witnesses. The story of this ‘traitor’ is indicative of the kinds of relationships that facilitated the revolt’s almost instantaneous mobilization, as well as the individual and accidental trajectories that led people to join or to eschew the Jacquerie, and how their paths might change over time.
