Abstract Background Individuals who are incarcerated have greater healthcare needs than non-justice-involved individuals, yet incarcerated individuals often report substandard care. There are disproportionate numbers of black, indigenous, and people of color (BIPOC) in prison, who, even in general society face greater obstacles to accessing healthcare and have worse health outcomes due to structural racism. Regardless of race, people with criminal justice involvement often report stigma from the non-carceral healthcare system. Providing sufficient healthcare in carceral settings themselves is complicated by lack of privacy and the inherent dialectic of prisons that restrict freedom and providers focusing on healing and health. Based on these adverse experiences, people who are incarcerated may have decreased distrust in the healthcare system, deterring individuals from getting adequate medical care. Methods In this exploratory study, health care system distrust was evaluated among 200 people who were incarcerated using the Revised Health Care System Distrust scale, a community-validated, 9-item measure comprised of 2 subscales (values and competence distrust). Results Distrust was moderately and positively associated with participant age (rs = 0.150, p = 0.034), with the second-oldest quintile (33 to 42-year-olds) reporting the highest level of overall and competence distrust. Participants identifying as Non-Latinx White reported higher competence distrust compared to Latinx and Non-Latinx/Non-White respondents. Conclusions These preliminary findings suggest that select groups of prisoners may be less likely to trust the healthcare system, highlighting an impediment to receiving adequate care while incarcerated. Further study of this topic is warranted.

Background and purpose: Access to healthcare services is an essential component for ensuring the quality of life. Globally, there is inequity and disparities regarding access to health care. To meet the global healthcare needs, different models of healthcare have been adopted around the world. However, all healthcare models have some strengths and weaknesses. The purpose of this study was to examine the satisfaction among a group of undergraduate students from different countries with their health care models namely, insurance-based model in the United States and “out-of-pocket” model prevalent in low-income countries. Methods and materials: The study utilized a cross-sectional research design. Undergraduate students, representing different nationalities from a private Southeastern College, were administered a researcher-designed 14-item self-reported electronic questionnaire. Independent t-test and χ2 statistics were used to examine the differences between two health care systems and the qualitative responses were analyzed thematically. Results: Satisfaction towards health care system between the United States and low-income countries was found significantly different (p < .05). However, students in both settings experienced an inability toward affording quality healthcare due to economic factors and disparities. Conclusions: There is dissatisfaction with health care both in the United States and low-income developing countries among a sample of undergraduate students representing these countries. Efforts to ensure low-cost affordable health care should be a global goal.

The healthcare industry is a growth driver. However, the health system is facing a crisis, affected by the financial development in Europe. An almost completely regulated market is just as little use, as a largely deregulated market such as in the U.S.A. Both lead to gaps in the sustainable and comprehensive patient care. Based on the German Healthcare System, an analysis is performed. Currently, the German health care system is in a transformation process. Traditional forms of health care services provision and the existing governance system are coming to their limits. The current health care system no longer meets the requirements for ensuring accessible and affordable health care services. As new players on the German hospital market, commercial hospital groups have emerged. To get more informed on the effects at the regional level, a scenario analysis was performed. A trend scenario which shows a clear trend toward a substantial increase of regional imbalances was developed. On one hand, there are highly profitable regions with excellent medical service provision by commercial hospital groups, and on the other hand, there are peripheral regions with a second-rate medicine, which are left to the public sector. The paper derives first ideas about a new structure of the healthcare system for ensuring accessible and affordable health care services for the citizens. The paper shows first ideas about the transformation of healthcare as an opportunity for growth.
