This is the second of a two-part profile on mental healthcare in Bangladesh. It describes the state of mental health research in the country and presents a set of priorities for addressing improvements to the fundamental gaps in mental healthcare highlighted in part 1. Focus on building infrastructure for public mental health facilities, training skilled mental health professionals, adequate distribution of financial resources and addressing stigma are all priorities that will contribute to significantly improving mental healthcare in Bangladesh.

Diagnostic nomenclatures have been central to mental health research and practice since the turn of the 20th century. In recent years, an increasing number of mental health professionals have proposed that a paradigm shift in diagnosis is inevitable. The Standards and Guidelines for the Development of Diagnostic Nomenclatures and Alternatives in Mental Health Research and Practice are intended to serve as a reference for the development of scientifically sound and ethically principled diagnostic nomenclatures and descriptive alternatives. The Standards and Guidelines are divided into four sections that address the purposes; development; type, content, and structure; and scientific grounding of nomenclatures and alternative systems. They are intended to represent best practice in the classification and description of emotional distress for multidisciplinary mental health professionals.

Abstract For more than a decade, Improving Access to Psychological Therapies (IAPT) has been training a new workforce of psychological therapists. Despite evidence of stress and burnout both in trainee mental health professionals, and qualified IAPT clinicians, little is known about these topics in IAPT trainees. Consequently, this systematic review sought to establish the current state of the literature regarding stress and burnout in IAPT trainees. Electronic databases were searched to identify all published and available unpublished work relating to the topic. On the basis of pre-established eligibility criteria, eight studies (including six unpublished doctoral theses) were identified and assessed for quality. This review identifies that research into the experience of IAPT trainees is under-developed. Existing evidence tentatively suggests that IAPT trainees may experience levels of stress and burnout that are higher than their qualified peers and among the higher end of healthcare professionals more generally. The experience of fulfilling dual roles as mental health professionals and university students concurrently appears to be a significant source of stress for IAPT trainees. More research regarding the levels and sources of stress and burnout in IAPT trainees is urgently needed to confirm and extend these findings. Recommendations for future research in the area are given. Key learning aims (1) To establish the current state of the literature regarding stress and burnout in IAPT trainees. (2) To raise practitioner, service and education-provider awareness regarding the levels and perceived sources of stress and burnout in IAPT trainees. (3) To make recommendations regarding future research on the topic.

Abstract Background: E-mentalhealthcare is the convergence of digital technologies with mental health services. It has beendevelopedto fill a gap in healthcare for people who need mental wellbeing support and may never otherwise receive psychological treatment.This study aimed to apply text mining techniques to analyze the huge data of e-mental health researches and to report on research clusters and trends as well as the co-occurrence of biomedical and the use of information technology in this field.Methods: The e-mentalhealth research data was obtainedfrom 3,663 bibliographicrecords from Web of Science (WoS)and 3,172 full-text articlesfrom PubMed Central (PMC). The text mining techniques utilized for this study includedbibliometric analysis, information extraction, and visualization.Results: The e-mental health research topic trendsprimarily involvede-health care services and medical informatics research. The clusters of research comprise 16 clusters, which refer to mental sickness, ehealth, diseases, IT, and self-management. Based onthe information extraction analysis, in the biomedical domain, a “depression” entity was frequently detected and it pairs with other entities in the network with a betweenness centrality weighted at 0.046869 (eg. depression-online, depression-diabetes, depression-measure, and depression-mobile).The IT entity-relations of “mobile” were the most frequently found(weighted at 0.043466). The top pairs are related to depression, mobile health, and text message.Conclusions: E-mental health research trends focused on disease related-depression and using IT for treatment and prevention, primarily via online and mobile devices. Producing AI and machine learning are also being studied for e-mental healthcare. The results illustrate that physical sickness is likely to cause a mental health problem and identify the IT that was applied to help manage and mitigate mental health impacts.

AbstractApproaching 2 years into a global pandemic, it is timely to reflect on how COVID-19 has impacted the mental health of the global population. With research continuing apace, a clearer picture should crystallise in time. COVID-19 has undoubtedly had some impact on population mental health, although the severity and duration of this impact remain less clear. The exceptional period of COVID-19 has provided a unique prism through which we can observe and consider societal mental health. This is a momentous time to be involved in mental health research as we strive to understand the mental health needs of the population and advocate for adequate resourcing to deliver quality mental healthcare in the post-pandemic period.

BACKGROUND Despite Digital Mental Health’s potential to provide cost-effective mental healthcare, its adoption in clinical settings is limited and little is known about the perspectives and practices of mental health professionals regarding its implementation or the factors influencing such perspectives and practices. OBJECTIVE The current study aimed at characterizing in-depth the perspectives and practices of mental health professionals regarding the implementation of Digital Mental Health and exploring the factors impacting such perspectives and practices. METHODS A qualitative study using in-depth semi-structured interviews with Portuguese mental health professionals (N=13) - psychologists and psychiatrists – was conducted. Transcribed interviews were thematically analysed. RESULTS Mental health professionals deemed important or engaged in the following practices during the implementation of Digital Mental Health: i) Indication evaluation; ii) Therapeutic contract negotiation; iii) Digital psychological assessment; iv) Technology setup and management; and v) Intervention delivery and follow-up. Low threshold accessibility and professionals' perceived duty to provide support to their clients facilitated the implementation of Digital Mental Health. Conversely, the lack of structured intervention frameworks; the unavailability of usable, validated, and affordable technology; and the absence of structured training programmes, inhibited Digital Mental Health’s implementation by mental health professionals. CONCLUSIONS The publication of practice frameworks, the development of evidence-based technology, and the delivery of structured training seem key to expedite implementation and encourage the sustained adoption of Digital Mental Health by mental health professionals.

Abstract Aim: To explore how mental health professionals address spiritual care for outpatients in weekly multidisciplinary care meetings (MDM), and to explore the barriers and facilitators in the ways health professionals address spiritual care in those meetings. Method: Two teams of mental health professionals providing care for psychiatric outpatients are included. Qualitative data were collected from audio recordings of multidisciplinary meetings and from focus-group interviews afterwards. Data were analysed using ‘open coding’. Results: Spiritual care was not frequently addressed mostly due to the requirements of the health insurance reimbursement system. Aspects of spirituality addressed in these meetings pertained mainly to meaningful daily activities. Addressing spiritual care was facilitated, on the other hand, by a holistic focus on health and recovery-oriented care. Conclusion: In ambulatory mental healthcare spirituality is sparsely addressed and, when addressed, few aspects of spirituality come into view. Facilitating healthcare professionals’ awareness of their clinical perspective is an essential step to improve spiritual care for psychiatric outpatients.
