In order to establish whether maternal thyroid hormones cross the placenta and produce surfactant in the fetus, possibly reducing the incidence of respiratory distress syndrome (RDS), a retrospective analysis of low-birthweight infants was undertaken. Results indicate that maternal thyroid hormones play a negligible role and that any thyroid mediation would result from fetal thyroid activity.

AbstractObjectivesPrevalence of Maternal and congenital hypothyroidism is on the rise. To present the thyroid stimulating hormone screening results in babies born to hypothyroid mothers and assess the burden, aetiology of hypothyroidism in these babiesMethodsAll antenatal mothers attending our hospital during the study period were enrolled into the study. Group I includes 249 term babies born to hypothyroid mothers and group II comprises 2154 newborns born to mothers who are euthyroid. Heel prick thyroid stimulating hormone was done for all newborns on day 3 for both groups. Confirmatory venous testing was done for all for babies in group I and screen positives belonging to group II. Evaluation and therapy done as per standard guidelines.ResultsThyroid stimulating hormone values in the two groups are presented. There was significant correlation between peak maternal thyroid stimulating hormone and neonatal day 3 heel prick in group I (r=0.7, P<0.05). The prevalence of positive screening test in groups I and II was 3.8 and 1.03% (p<0.05) whereas corresponding values for confirmed disease was 4.3 and 0.6%, respectively (p<0.05). Aetiological evaluation revealed both transient hypothyroidism (33.3%) and permanent hypothyroidism (66.6%).Conclusion4.3% of babies born to hypothyroid mothers develop congenital hypothyroidism; aetiology being both transient and permanent. A venous test by 3 weeks is helpful in these babies to improve case identification.

ABSTRACT Effects of thyroid status on expression of a variety of myocardial genes, such as those encoding contractile proteins, have been reported, as well as interactions between thyroid hormones and developmental and haemodynamic regulation of contractile protein synthesis. In addition, it is clear that developmental and haemodynamic factors regulate expression of specific proto-oncogenes, including c-myc, c-fos and H-ras, in the myocardium but the effect of thyroid status on such proto-oncogene products, which are proposed to play a critical signal-transducing role in the heart, has been previously unexplored. In order to determine whether changes in thyroid status are associated with changes in expression of these putative intracellular signals, we examined the effect of hypothyroidism and tri-iodothyronine (T3) treatment on myocardial levels of c-myc, c-fos and H-ras mRNAs in the rat. The induction of hypothyroidism was associated with a marked increase in myocardial c-myc, c-fos and H-ras mRNAs, changes reversed by 72 h of T3 replacement. Administration of T3 to euthyroid rats had no significant effect on myocardial c-myc or c-fos mRNAs, but inhibition of H-ras mRNA by T3 was evident. These observations demonstrating influences of thyroid status on expression of specific proto-oncogenes suggest that thyroid hormones, as well as exerting direct effects on expression of functionally important myocardial genes, also interact with the cellular transduction pathways mediated by the products of the c-myc, c-fos and H-ras genes. The placental transfer of I131-labeled triiodothyronine has been studied in Rhesus monkeys. The majority of maternal to fetal placental transfer of radioactivity was in the form of iodide and a chemically unrecognized compound on chromatograms. Only traces of triiodothyronine or thyroxine were detected. In the fetal to maternal transfer studies, triiodothyronine was more readily transferred into the maternal circulation with small amounts of iodide and a similar unknown compound appearing. This substance may be sulfate conjugated triiodothyronine. There was a considerably higher concentration gradient used in the fetal to maternal transfers than in the reverse direction. The implications of this gradient difference are discussed. The data from this study add further evidence to the clinical experience that maternal thyroid hormones are not readily available to meet all of the fetal needs during the latter part of pregnancy.

AbstractContextMaternal thyroid hormones during pregnancy play a critical role in fetal development. However, whether maternal heavy metal exposure affects their thyroid hormones and the effects on fetal growth are still unclear.ObjectiveTo explore the effect of heavy metal exposure on maternal thyroid hormones and the potential mediation role of thyroid hormones on birth outcomes.MethodsConcentrations of heavy metals in urine samples and thyroid hormones in blood samples of 675 pregnant women were measured during early pregnancy in a cohort study conducted in China. Multivariable linear regressions were applied to explore the associations of maternal urinary heavy metal levels with both maternal thyroid hormones and birth outcomes. Mediation analyses were performed to assess the mediation role of thyroid hormones in these associations.ResultsMaternal urinary vanadium (V) exhibited an inverse association with free T3 (FT3) and FT3/free T4 (FT4) ratio levels. Urinary arsenic (As) and lead (Pb) had inverse relationships with FT3. We also observed the positive associations of maternal FT3 and FT3/FT4 ratio with birthweight. The mediation analyses suggested that 5.33% to 30.57% of the associations among V, As, and Pb levels and birth size might be mediated by maternal FT3 or FT3/FT4 ratio.ConclusionsWe have shown that maternal exposures to V, As, and Pb at early pregnancy were associated with decreased maternal FT3 or FT3/FT4 ratio, which might contribute to reduced birthweight. Mediation analyses indicated that maternal thyroid hormone was a possible mediator of the association between urinary heavy metals and birth size.
