Abstract Context Time spent in moderate-to-vigorous physical activity (MVPA), but not in sedentary behavior (SB), is related to cardiometabolic risk among non-Hispanic white youth. Objective Examine associations of SB and MVPA with cardiometabolic risk factors among Hispanic/Latino youth. Design Cross-sectional analysis. Setting Four US communities. Participants Hispanic/Latino youth (N = 1,426) ages 8 to 16 years. Measurements Associations of MVPA and SB, measured using 7-day accelerometer data (independent variables), with markers of glucose and lipid metabolism, inflammation, and endothelial function (dependent variables), were assessed in multivariable linear regression models while adjusting for sociodemographic characteristics and accelerometer wear time. Additional models controlled for obesity measures. Results SB comprised a mean (SD) of 75% (13%) of accelerometer wear time; mean (SD) time of MVPA was 35 min/d (22 min/d). Deleterious levels of high-density lipoprotein–cholesterol (HDL-C), triglycerides, insulin resistance, C-reactive protein, and plasminogen activator inhibitor-1 were associated with lower levels of MVPA and higher levels of SB (all P < 0.05). Associations of MVPA with log-transformed triglyceride concentrations (β per 15-min/d increment, −0.039; SE, 0.018; P = 0.037) and SB with HDL-C (β per 30-min/d increment, −0.63; SE, 0.26; P = 0.018), but not those with other markers, remained significant after adjusting for MVPA or SB and further adjustment for body mass index and waist circumference. Higher SB tertiles were associated with lower soluble receptor for advanced glycation end products in fully adjusted models (P for trend = 0.037). Conclusions Physiological precursors of diabetes and cardiovascular disease were associated with MVPA and SB among US Hispanic/Latino youth, a group that bears a disproportionate burden of metabolic disorders.

Introduction: Sedentary behavior (SED) is associated with higher obesity and cardiometabolic risk in youth, independent of physical activity. Studies showing the association between screen time and SED among Hispanics/Latinos, have primarily focused on Mexican-origin Hispanics. Additional research is needed to examine other socio-environmental factors that can influence SED among diverse Hispanics/Latinos. This cross-sectional study examined the home-, neighborhood-, and school- environment to identify factors associated with total sedentary time among youth. Methods: Data from 1,104 youth ages 8-16 years and 728 caregivers (mean age 43.1 ± 8.2 years) from four U.S. cities, who participated in the Study of Latino Youth (2012-2014), were examined. Associations between socio-environmental factors (measured by self-report) and total sedentary time (measured by one-week Actical accelerometry) were examined in linear regression models that included MVPA minutes/day, demographic covariates, and accounted for the complex survey design and sampling weights. Results: Mean sedentary time was 10.1 ± 1.8 hours/day. Home environment factors, such as electronics in the bedroom and parent limit setting, were not associated with total sedentary time. Presence of barriers to physical activity in the neighborhood (e.g., muggings, gangs) was associated with 13.4 more minutes of sedentary time per day. Attending a school that never/rarely compared to sometimes offered after school physical activity opportunities was associated with more sedentary time (B=38.0 minutes/day; 95% Confidence Interval: 13.5-62.4). Conclusions: The study findings highlight the need for future research to investigate other sources of sedentary behavior in the home for interventionist to focus on specific SED-based strategies to decrease sedentary time among youth. Minimizing barriers by identifying safe places to be active in participant’s neighborhoods may also support youth to spend less time indoors where sedentary time is prevalent.

Introduction: Sedentary behavior is associated with cardiometabolic risk independent of physical activity, but the underlying mechanisms are not fully understood. Hypothesis: Sedentary behavior may influence serum metabolite profiles which are associated with cardiometabolic risk factors. Methods: The analyses included 2711 participants (43% men) of the HCHS/SOL aged 18 to 74 years, free of CVD and other major chronic diseases. Serum metabolomics was performed using an untargeted approach. Physical activity and sedentary behavior were measured by accelerometer (Actical) for a 7-day period. Survey linear regressions were used to examine associations between sedentary behavior and metabolites. Metabolite scores were calculated by summing up z-scores of significant metabolites within the same sub-pathway. Results: Of 624 known metabolites, 95 were significantly associated with sedentary behavior ( P <0.05 after FDR correction), adjusting for socioeconomic and lifestyle factors including moderate to vigorous physical activity ( Figure 1A ). The majority of significant metabolites were clustered into 7 sub-pathways. Three sub-pathway scores (branched-chain amino acids [BCAA], acylcarnitine and medium-chain fatty acid metabolism) were positively associated with sedentary behavior, while the other 4 sub-pathway scores (acylcholine, plasmalogen, nicotinate, and arginine) were inversely associated with sedentary behavior. Among these scores, the BCAA score showed unfavorable correlations with all cardiometabolic traits including obesity measures, glycemic traits, blood lipids and blood pressure. The acylcholine score showed favorable correlations with obesity measures and glycemic traits, though positive correlations with blood lipids were observed ( Figure 1B) . Conclusions: Our study identified multiple serum metabolites associated with sedentary behavior independent of physical activity, suggesting potential mechanistic links between sedentary behavior and cardiometabolic diseases.

Purpose: 1) To compare group-level physical activity calculated from a single versus multiple non-consecutive, one-week accelerometer monitoring periods in individuals with medial-compartment knee osteoarthritis and asymptomatic controls; and 2) to examine agreement among these estimates of physical activity at the individual-level. Methods: Accelerometer data from 38 individuals with knee osteoarthritis and 47 asymptomatic individuals was collected during three non-consecutive monitoring periods over one year. General linear models examined the effects of number of sessions averaged (one, two, or three) and group on light and moderate-to-vigorous intensity physical activity, step count, and sedentary behavior. Bland Altman analyses examined agreement between one-, two-, and/or three-session averages. Results: There were no sessions by group interactions. There was a main effect of sessions for sedentary behavior that was borderline significant when expressed as percent wear time. Limits of agreement indicated that two-session average versus single-session metrics could differ by ±50 minutes for light physical activity, ±20 minutes for moderate-to-vigorous physical activity, and ±2100 steps per day. Conclusions: These data suggest that objective physical activity monitoring practices might differ between clinical research, where group data are compared, and clinical decision making, where individual data are compared. Good estimates of group level differences in step count, light, and moderate-to-vigorous physical activity were found using a single session of accelerometer data, but a single session of sedentary behavior data should take wear time into account. The large limits of agreement indicate that multiple sessions may be needed to compare these metrics among or within individuals.

Abstract Background Whether physical activity can reduce cardiometabolic risk particularly in understudied populations such as US Hispanics/Latinos is of public health interest. We prospectively examined the association of physical activity and cardiometabolic biomarkers in n = 8049 participants of the Hispanic Community Health Study/Study of Latinos, a community-based cohort study of 16,415 adults aged 18–74 yr who self-identified as Hispanic/Latino from four US urban centers. Methods We assessed physical activity using accelerometry in 2008–2011 at visit 1. We assessed cardiometabolic biomarkers twice: once at visit 1 and collected a second measure in 2014–2017 at visit 2. We used survey linear regression models with changes in cardiometabolic markers as the dependent variables and quartiles of sedentary behavior or whether adults met guidelines for moderate-to-vigorous physical activity as the independent variables. Results In normoglycemic adults without cardiovascular disease, but not in adults with evidence of cardiometabolic disease, those who were in the lowest quartile for sedentary behavior (< 10.08 h/day) had a significant decline in mean LDL-cholesterol of − 3.94 mg/dL (95% CI: − 6.37, − 1.52) compared to adults in the highest quartile (≥13.0 h/day) who exhibited a significant increase in LDL-cholesterol of 0.14 mg/dL (95% CI, − 2.15,2.42) over the six year period (P < 0.02 in fully adjusted models.) There was also a trend toward lower mean increase in HbA1c comparing the lowest with the highest quartile of sedentary behavior. Overall regardless of glycemic level or evidence of cardiometabolic disease, adults who met guidelines for moderate-to-vigorous physical activity at visit 1, had significantly lower mean increases in level of fasting glucose compared to adults not meeting guidelines in fully adjusted models. Conclusions In this cohort of Hispanics/Latinos, being free of cardiometabolic disease and having low levels of sedentary behavior were associated with health benefits. Among all adults regardless of cardiometabolic disease, meeting guidelines for moderate-to-vigorous physical activity was associated with health benefits. Overall these data suggest that an active lifestyle may blunt the association of advancing age with worsening cardiometabolic risk factors.

Abstract This study investigated the role of objectively measured moderate–vigorous physical activity (MVPA) and sedentary behavior on cardiometabolic risk factors of young Latino children. We hypothesized that MVPA would be associated with lower cardiometabolic risk when sedentary behavior is low. We studied 86 primarily low-income, Latino children using a cross-sectional study design. The study sample consisted of 51 girls and 35 boys, with mean age 5.6 (SD = .53) years. Physical activity was measured by accelerometry, anthropometric measures obtained, and fasting blood samples were used to measure cardiometabolic risk factors. Greater levels of sedentary behavior were associated with increased waist circumference (rs = .24, p &lt; .05) and metabolic risks. MVPA, however, had significant beneficial associations with all cardiometabolic risk factors (rs-range = −.20 to −.45, p &lt; .05) with the exception of plasma insulin. MVPA predicted latent variables representing anthropometric risk (β = −.57, p &lt; .01), cardiac risk (β = −.74, p &lt; .01), and metabolic risk (β = −.88, p &lt; .01). Sedentary behavior significantly moderated the effect of MVPA on anthropometric (β-interaction = .49, p &lt; .01), cardiac (β-interaction = .45, p &lt; .01), and metabolic risk (β-interaction = .77, p &lt; .01), such that more MVPA was associated with better health outcomes under conditions of lower sedentary behavior. The model explained 13%, 22%, and 45% variance in anthropometric, cardiac, and metabolic risk factors, respectively. Increased MVPA is associated with decreased cardiometabolic risk in young Latino children, particularly when sedentary behavior is low.

Introduction. Sedentary behavior (SB) is highly prevalent among older adults, with more than 25% engaging in 6 hours or more of SB daily. SB has been associated with several cardiometabolic biomarkers in younger adults; however, there is a paucity of research in older populations. This study examined associations between patterns of SB and cardiometabolic biomarkers in community-dwelling adults aged 55 years and older. Methods. Data were drawn from a convenience sample of 54 community-dwelling individuals (12 males, 42 females; mean age = 72.6 ± 6.8 years, range = 56–89 years). Cardiometabolic biomarkers assessed included systolic (SBP) and diastolic blood pressure (DBP), body mass index, waist circumference, and fasting blood glucose and cholesterol parameters. SB was assessed via accelerometry over a 7-day period, and measures included daily time in SB, number and length of sedentary bouts, the number and length of breaks between sedentary bouts, moderate-to-vigorous physical activity (MVPA), and light physical activity (LPA). Associations between the SB measures and each cardiometabolic risk factor were examined using separate stepwise multiple regression models, controlling for sex, MVPA, and accelerometer wear time. Isotemporal substitution models were used to examine the change in cardiometabolic outcomes when SB is replaced by an equal duration of either LPA or MVPA. Results. Adjusted regression analyses showed that daily sedentary time was positively associated with DBP (β = 0.052, ∆R2 = 0.112, p=0.022) and inversely associated with HDL cholesterol (β = −0.111, ∆R2 = 0.121, p=0.039). Sedentary bout length was also associated with DBP and HDL cholesterol (β = 0.575, ∆R2 = 0.152, p = 0.007; β = −1.529, ∆R2 = 0.196, p=0.007, respectively). Replacement of 10 minutes of SB a day with LPA was associated with improved DBP and HDL cholesterol (p≤0.05). No other significant associations (p≤0.05) were found. Conclusion. Sitting for prolonged periods of time without interruption is unfavorably associated with DBP and HDL cholesterol. Prospective studies should identify causal relationships and observe specific changes in cardiometabolic profiles in older populations.

Background:We examined the effects of wear time on a population profile of time-stamped accelerometer outputs using the 2005−2006 National Health and Nutrition Examination Survey (NHANES) data representing 3744 adults ≥ 20 years of age.Methods:Outputs included activity counts, steps, and time variables: nonwear (macro-determined), sedentary behavior (<100 activity counts/minute), and time in low (100−499 activity counts/minute), light (500−2019 activity counts/minute), and moderate-to-vigorous physical activity (MVPA; ≥2020 activity counts/minute) intensities. We describe mean values according to a 24-hour clock. Analysis was repeated in a reduced data set with only those who wore the accelerometer for 60 minutes within each considered hour of the day.Results:Between 12:00 and 17:00, U.S. adults spend approximately 31 minutes each hour in sedentary behaviors, and approximately 14 minutes, 10 minutes, and 2 minutes in low, light, and MVPA intensity activity, respectively. Removing the effect of nonwear time, sedentary behaviors are reduced in the morning hours and increase in the evening hours.Conclusion:At either end of the day, nonwear time appears to distort population estimates of all accelerometer time and physical activity volume indicators, but its effects are particularly clear on population estimates of time spent in sedentary behavior.

Background:This investigation sought to determine how accelerometer wear (1) biased estimates of sedentary behavior (SB) and physical activity (PA), (2) affected misclassifications for meeting the Physical Activity Guidelines for Americans, and (3) impacted the results of regression models examining the association between moderate to vigorous physical activity (MVPA) and a clinically relevant health outcome.Methods:A total of 100 participants [age: 20.6 (7.9) y] wore an ActiGraph GT3X+ accelerometer for 15.9 (1.6) hours per day (reference dataset) on the hip. The BOD POD was used to determine body fat percentage. A data removal technique was applied to the reference dataset to create individual datasets with wear time ranging from 15 to 10 hours per day for SB and each intensity of PA.Results:Underestimations of SB and each intensity of PA increased as accelerometer wear time decreased by up to 167.2 minutes per day. These underestimations resulted in Physical Activity Guidelines for Americans misclassification rates of up to 42.9%. The regression models for the association between MVPA and body fat percentage demonstrated changes in the estimates for each wear-time adherence level when compared to the model using the reference MVPA data.Conclusions:Increasing accelerometer wear improves daily estimates of SB and PA, thereby also improving the precision of statistical inferences that are made from accelerometer data.
