Abstract Background: Losartan has been shown to attenuate symptomatic and hormonal responses to hypoglycemia in prior studies. This results predominantly from blocking AT-II receptors blunting the hypoglycemia-induced rise in plasma epinephrine predisposing them to hypoglycemia unawareness. To our knowledge, however there are no case reports describing losartan induced hypoglycemia in a nondiabetic patient. This abstract is the first description of a patient without diabetes mellitus experiencing severe hypoglycemia induced by the ARB, Losartan. Clinical Case: A 51- year old nondiabetic female was found to be somnolent and in acute respiratory failure. Blood sugar was 34 mg/dL. She received D10 by EMS while coming to the hospital. She was intubated and treated for atypical pneumonia based on bilateral interstitial infiltrates. She remained persistently hypoglycemic around 62 mg/dL. Her most recent A1c was 4.4%. Thyroid function was within normal limits. AM cortisol was 16.8 mcg/dL so there was no concern of adrenal insufficiency. She had no history of gastric bypass surgery. She underwent a 72-hour fast and developed symptomatic hypoglycemia. Her venous blood glucose at this time was 49 mg/dL. Her hypoglycemia panel obtained at this time showed C-peptide 0.7 ng/mL, total insulin 2.9 microU/mL, proinsulin 3.1 pmol/L, IGF 45 ng/mL and BHOB 13.5 mmol/L. Her oral hypoglycemic agent screen was negative. Based on levels that were obtained during the hypoglycemic episode, this correlated to a non-diagnostic study for conventionally described causes of hypoglycemia including exogenous insulin, insulinoma, NIPHS, PGBH, oral hypoglycemic agent, insulin autoimmune or IGF-mediated. Her medication list was analyzed again. We learnt that she had recently been started on losartan about a month prior to this admission for her heart failure management. On further review, the patient mentioned that she was noticing these hypoglycemic events in the last month and that seemed to coincide with when she was started on losartan. We subsequently held losartan which resulted in profound improvement in her glycemic control and her blood sugars improved to a range of 110–140 mg/dL consistently. She was discharged from hospital off losartan. Blood sugar was stable on repeat testing outpatient after discharge. Conclusion: This is the first case that demonstrates the role of losartan in causing severe hypoglycemia in patients without a history of diabetes mellitus. Discontinuation of losartan resulted in prompt improvement of hypoglycemia. COVID-19 Associated Mucormycosis (CAM) is an emerging infectious disease that has caused increased mortality & morbidity in India during this second wave of the pandemic. The country has reported more than 30,000 cases and over 2,000 deaths by Mucormycosis so far, according to sources from Union Health Ministry. CAM is now a notifiable disease. At our Tertiary care teaching hospital, which caters for COVID 19 management, we are reporting the first case of Rhino orbital CAM, which was caused by Rhizopus spp. Our patient had a history of contact with a suspected COVID 19 patient and was recently diagnosed with uncontrolled diabetes mellitus. A direct KOH microscopic examination of purulent material aspirated from the sinonasal polyp of the patient revealed fungal elements, and Rhizopus spp was isolated. Due to a shortage of Amphotericin B, the patient was referred to a government higher speciality centre for further management. The patient was followed up & was noted that he was treated with antifungal and discharged following recovery.

Abstract: We reported a case of 56 years old man with critical limb ischemia. Diagnosis was made based on anamnesis, signs and clinical symptoms, as well as investigations that included the presence of rest pain in the left leg. Patient also had a history of diabetes mellitus. Echo- doppler and angiographic investigations supported the diagnosis of critical limb ischemia. In this patient, medicamentous treatment has been administered consisted of antiplatelet and cilostazol, blood sugar control by using insulin. The patient also underwent endovascular revascularization.Keywords: critical limb ischemia, peripheral artery diseaseAbstrak: Telah dilaporkan kasus critical limb ischemia pada seorang laki-laki berusia 56 tahun dengan keluhan nyeri seperti terbakar pada kaki kiri bahkan saat istirahat dengan riwayat diabetes melitus. Hasil pemeriksaan penunjang echo-doppler dan angiografi menyokong terdapatnya critical limb ischemia. Diagnosis ditegakkan berdasarkan anamnesis, tanda dan gejala klinis, serta pemeriksaan penunjang. Pada pasien ini, diberikan penatalaksanaan terapi medikamentosa yaitu antiplatelet dan cilostazol, kontrol gula darah dengan menggunakan insulin, dan dilakukan tindakan revaskularisasi endovaskular.Kata kunci: critical limb ischemia, peripheral artery disease

  Diabetes mellitus is a metabolic disease that has characteristics of high blood sugar levels (hyperglycemia), this can occur due to abnormalities in insulin secretion. The aim of this study was to determine the relationship of hemoglobin levels with long suffering  Diabetes Mellitus patients.  This study used was correlational design with a cross sectional approach.  The population in this study were 225 people with a sample of 67 respondents with purposive sampling technique.  Data was collected by checking hemoglobin levels and Quesioner. The results showed that almost all respondents had normal hemoglobin levels and most of the respondents had  long history of suffering from Diabetes Mellitus for 5-10 years..  Data analysis used was the Rho Spearman test and can not found the relationship between hemoglobin levels and long suffering Diabetes Mellitus (p value = 0.565). History of suffering from Diabetes Mellitus,did not necessarily have low hemoglobin levels, because it can be noticed from other factors such as routine taking medication, diet, exercise, and lifestyle that can affect the condition of diabetes patients themselves.  Diabetes Mellitus patients need to maintain the blood sugar levels and hemoglobin levels with a healthy lifestyle, take the recommended diet, routinely consume drugs, and check blood sugar levels regularly.

Abstract: Diabetic retinopathy  is a disease that tends to damage retinal microvasculature as chronic progressive, related with the old hyperglycaemia and also connected with diabetes mellitus and hypertension; that can expand into certain level and can turn to serious complication. The purpose of this observation is to know about prevalence of diabetic retinopathy on Polyclinic Opthalmology BLU RSU Prof. Dr. R. D. Kandou in Manado within one year. This method of observation are retrospective descriptive which is utilize the secondary data in notes form that is taken from Department of Opthalmology BLU RSU Prof. Dr. R. D. Kandou in Manado within period June 2012 – May 2013 and October 2013. In this observation that had 498 samples, with 219 people (44%) that is positively diagnosed with diabetic retinopathy. Based from sexes it self, diabetic retinopathy occur more in female patient as much as 116 people (52,96%). However, that disease is dominated with the patient at age 45-64 year old 128 people (58,44%) that also has history of diabetes mellitus. From 219 patients retinopati diabetik, there are 200 people (91,32%) that has vision problem. As we know about the risk of this disease, we are hoping that the patient of retinopati diabetik and diabetes melitus could constantly control their blood pressure and blood sugar, as well as eyes checking to cut down future risk. Tujuan penelitian adalah untuk mengetahui prevalensi retinopati diabetik pada poliklinik ilmu kesehatan mata BLU RSU Prof. Dr. R. D. Kandou Manado selang satu tahun. Metode penelitian bersifat deskriptif retrospektif dengan memanfaatkan data sekunder berupa catatan medik yang terdapat di Bagian Mata BLU RSU Prof. Dr. R. D. Kandou Manado Periode Juni 2012-Mei 2013 dan Oktober 2013. Dalam penelitian ini terdapat 498 sampel, dengan 219 orang (44%) yang terdiagnosa retinopati diabetik. Berdasarkan jenis kelamin, retinopati diabetik lebih banyak dialami pasien berjenis kelamin perempuan sebanyak 116 orang (52,96%). Sedangkan untuk rentang umur, didominasi oleh pasien dengan rentang umur 45-64 tahun sebanyak 148 orang (67,5%), dan terdapat 128 orang (58,44%) yang memiliki riwayat diabetes melitus. Dari 219 orang pasien retinopati diabetik terdapat 200 orang (91,32%) yang visusnya terganggu. Dengan melihat resiko penyakit ini, pasien retinopati diabetik dan diabetes melitus diharapkan dapat mengontrol gula darah dan tekanan darah, serta dengan rutin memeriksakan mata untuk menekan progresifitas penyakit. Abstract Objectives Mucormycosis is a life-threatening condition that can be present in diabetes mellitus. Although the rhino-orbito-cerebral form is the most common, other types may be developed as well. Case presentation The present study reports two children with diabetes who had the cutaneous and pulmonary forms of mucormycosis. The first case was a four-year-old boy without a history of diabetes admitted due to loss of consciousness and a necrotic lesion on his hand. The second was an 11-year-old boy with dyspnea, polyuria, and polydipsia who had developed diabetes two years before, and whose chest X-ray had shown a pulmonary lesion. Both cases were treated with surgical implementation and appropriate antifungal therapy. Conclusions Mucormycosis is a fatal condition that can even be present at the onset of diabetes mellitus. Therefore, any diabetic patient with any presentation of respiratory symptoms such as coughs, or any cutaneous lesion should be examined carefully for mucormycosis after which prompt treatment must begin.
