Objective — To study the clinical efficacy of miglitol combined with insulin in the treatment of newly diagnosed type 2 diabetes mellitus. Methods — 96 newly diagnosed type 2 diabetes patients admitted to our hospital from January 2021 to September 2021 were selected as the subjects of this study. They were randomly divided into two groups by drawing lots. The control group was treated with acarbose combined with insulin glargine, and the observation group was treated with miglitol combined with insulin glargine. Fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin (HbA1c), blood glucose compliance time, occurrence of adverse reactions and quality of life score of 2 groups were measured before and after treatment. Results — After treatment, fasting blood glucose, 2h postprandial blood glucose and HbA1c in both groups were lower than before (P < 0.05), and there was no statistical significance in the difference between the two groups and the time of blood glucose reaching the standard (P > 0.05). The incidence of adverse reactions in observation group was lower than that in control group, and the quality of life score in observation group was better than that in control group (P < 0.05). Conclusions — Miglitol or acarbose combined with insulin glargine can effectively control blood glucose in patients with newly diagnosed type 2 diabetes, but miglitol combined with insulin glargine has fewer adverse reactions, which can be used as the first choice for clinical treatment of newly diagnosed type 2 diabetes.

Abstract Background and purpose Although participation in cardiac rehabilitation (CR) improves quality of life (QoL), participation in CR, especially among elderly, is limited. We conducted this study to assess whether mobile home-based CR (mCR) increases QoL in elderly (≥65 years old) patients with coronary artery disease (CAD) or a valvular intervention who decline participation in conventional CR. Methods It is designed as a randomised multi-centre study with two parallel arms. Randomisation assigned patients either to mCR or a control group. mCR Consisted of six months of home-based CR with telemonitoring and coaching. Control-group patients did not receive any form of CR throughout the study period. Quality of life was measured with the SF-36v2 questionnaire at 0, 6 and 12 months. Results A total of 179 patients were included in this study (90 control, 89 mCR). A flowchart of the trial is presented in Figure 1. Patients were predominantly male (81.1%). Baseline characteristics can be found in Table 1. Patients using mCR improved on physical QoL after 6 (p=0.026) and 12 (p=0.008) months. There was no difference on mental QoL for both groups (mCR 6 months p=0.563, 12 months p=0.945; control 6 months p=0.589, 12 months p=0.542). No difference existed in QoL between the mCR and control group (physical: 6 months p=0.070, 12 months p=0.150; mental: 6 months p=0.355, 12 months p=0.625). Conclusion Although there is no significant difference in QoL between the control and mCR group, mCR increases physical QoL after 6 and 12 months in elderly patients who decline participation in conventional CR. Therefore E-Health tools should be considered as an alternative for conventional CR when (elderly) patients decline to participate in conventional CR. Figure 1. Introduction: Depression is the most commonly present psychiatric entity in clinical practice, accompanied by significant impairment of both social and professional functioning. In addition, depression frequently develops as complication of other psychiatric disorders and various somatic diseases. Objective: To investigate subjective perception of quality of life and social adjustment, severity of depressive symptoms as well as level of correlation of severity of depressive symptoms and quality of life and social adjustment of patients with recurrent depression in comparison to the group of patients with diabetes and healthy subjects. Method: The study included 45 subjects of both sexes, ranging from 18 to 60 years of age, divided in three groups of 15 subjects each. The experimental group comprised the patients diagnosed with recurrent depression in remission (DSM-IV), one control group was consisted of patients diagnosed with Type 2 Diabetes mellitus and another one comprised healthy subjects. The instruments of assessment were: The Beck Depression Inventory- BDI, The Social Adaptation Self -evaluation scale - SASS, The Psychological General Well-Being Scale - WBQ. Results: Significant difference of both BDI and WBQ scales was found between the experimental and the control group of healthy subjects (ANOVA, Mann Whitney; p?0.01), as well as between two control groups (p?0.02). The level of inverse correlation of mean score values of BDI and SASS scales was significant in the control group of patients with diabetes while such levels of BDI and WBQ scales (Spearman correlation coefficient, p<0.01) were found in all groups of our study. Conclusion: In the group of patients with recurrent depression, significant decline of quality of life and significantly higher severity of depressive symptoms were present in comparison to the group of healthy subjects as well as significant level of inverse correlation of severity of depressive symptoms and quality of life.
