Motivational interviewing to increase physical activity in women with gestational diabetes

Regular physical activity improves glycaemic control in pregnant women with gestational diabetes. Motivational interviewing is an effective technique for increasing activity levels. This report evaluates a clinical pathway developed to integrate physical activity motivational interviewing into routine gestational diabetes care. Women attending a single-centre NHS clinic were invited to engage in a physical activity-focused motivational interview. The aerobic physical activity levels of 62 women were evaluated at baseline and at a 2-week telephone follow up, coded into three categories by minutes of moderate intensity physical activity per week: red (<30 minutes), amber (30–149 minutes) and green (≥150 minutes). At baseline, 30.6% of participants were coded red, 41.9% amber and 27.4% green. At follow up, 4.8% women coded red, 38.7% amber and 56.5% green, demonstrating a significant association for increased activity levels after motivational interviewing (P<0.001). This clinical pathway provides encouraging results that physical activity increased significantly in the short term.

Abstract Background: Regular physical activity (PA) improves glycaemic control in women diagnosed with Gestational Diabetes (GDM). However, PA advice competes with other components of care, with many women forgoing the benefits of regular PA. Motivational interviewing (MI) is an effective technique for increasing individual PA levels. A clinical pathway was developed integrating MI on PA into the routine care. This report evaluates the impact of MIs on self-reported PA levels.Methods: Women attending a single centre NHS GDM clinic were invited to engage in an individual PA-focused MI session, delivered by a trained midwife. This included goal setting and activity planning. All women had a confirmed diagnosis of GDM based on a 75g oral glucose tolerance test using the International association of diabetes and pregnancy study group diagnostic thresholds. A modified version of the exercise vital sign was used to evaluate self-reported aerobic PA levels at baseline and two-week telephone follow-up. PA levels were coded into three categories: i) red (<30mins moderate intensity PA (MIPA)/week), ii) amber (30-150mins MIPA/week) and iii) green (150mins MIPA/week). Women with contraindications to PA were excluded. The main outcome was the difference PA levels from baseline to two weeks. The Pearson’s Chi-squared test was used to evaluate statistical difference in self-reported PA levels from baseline to follow-up.Results: Complete follow-up data was obtained from 62 women. Mean gestation was 27+5/40 weeks (9-36+4/40), mean age 31.7y (21-43y) and mean BMI 29.9kg/m2 (18.3-48.2 kg/m2). At baseline, 19 (30.6%) were coded red (<30mins MIPA/week); 26 (41.9%) amber (30-150mins MIPA/week); and 17 (27.4%) green (150mins MIPA/week). Self-reported physical activity levels of these women at two-week follow-up revealed only 3 (4.8%) women coded red, 24 (38.7%) amber and 35 (56.5%) green. This demonstrates a significant association for increased PA levels after MI at two-week follow-up (p=0.0001).Conclusion: This new clinical pathway provides encouraging results, showing that self-reported PA increased significantly at two-week follow-up. Further work is now required to examine the impact on glycaemic control, maternal and fetal outcomes, and maintenance of PA levels. This model of care should be integrated into other high-risk patient groups.

Objectives:To explore whether participation in leisure-time physical activity (LTPA) is associated with participation in occupational, housework, and transport-related physical activity.Methods:Population-based cross-sectional study covering a multistage sample of 972 subjects age 20 to 69 years. Physical activity was measured using the long version of the International Physical Activity Questionnaire. A LTPA score was calculated as follows: min/wk of walking + min/wk of moderate-intensity physical activity + (min/wk of vigorous-intensity physical activity × 2). Similar scores were generated for each domain. For categorical analyses, the scores were divided into 3 categories: 0 min/wk, 10−149 min/wk, and ≥150 min/wk.Results:The proportion of subjects practicing less than 150 min/wk of physical activity in each domain was: leisure-time (69.8%), occupational (58.3%), housework (35.0%), transportation (51.9%). Subjects with a transport-related physical activity score equal to or above 150 min/wk were 40% less likely to be sedentary in leisure-time in comparison with those who did not practice transport-related physical activity. Housework and occupational physical activity were not related to participation in LTPA.Conclusions:Future physical activity campaigns should focus on other domains instead of LTPA alone, particularly supporting transport-related physical activity as a strategy of health promotion.

BACKGROUND Intensive lifestyle modifications have proved effective in preventing type 2 diabetes mellitus (T2DM), yet the efficiency and effectiveness of these modifications need to be improved. Emerging social media interventions are considered useful in promoting these lifestyles; nevertheless, few studies have investigated the effectiveness of combining them with behavior theory. OBJECTIVE This study aims to examine the effectiveness of a 6-month mobile-based intervention (DHealthBar, a WeChat applet) combined with behavioral theory compared with a printed intervention in improving dietary behaviors, physical activity, and intention to change these behaviors among populations at high risk for T2DM. METHODS Participants aged 23 to 67 years were recruited offline in Beijing, China, and were randomized into the intervention group or the control group, which received educational content via DHealthBar or a printed handbook, respectively. Educational materials were culturally tailored recommendations on improving dietary behaviors, physical activity, and intention to change based on the transtheoretical model. Participants in the intervention arm received push notifications twice per week on WeChat and had access to the educational content for the 6-month study period. Participants in the control arm received the same intervention content through printed materials. The outcomes of participants’ behavior change, intention to change behavior, and anthropometric characteristics were collected via online measuring tools at baseline, 3 months, and 6 months. RESULTS In this study, 79 enrolled individuals completed baseline information collection (control: n=38 vs intervention: n=41), and 96% (76/79) completed the 6-month follow-up visit. Attrition rates did not differ significantly between the 2 groups (<i>χ</i><sup>2</sup><sub>1</sub>=0.0, <i>P</i>=.61). Baseline equivalence was found. Participants in both groups reported a statistically significant decrease in energy intake at the 2 follow-up assessments compared with baseline (3 months, control: exp[<strong>β</strong>]=0.83, 95% CI 0.74-0.92 vs intervention: exp[<strong>β</strong>]=0.76, 95% CI 0.68-0.85; 6 months, control: exp[<strong>β</strong>]=0.87, 95% CI 0.78-0.96 vs intervention: exp[<strong>β</strong>]=0.57, 95% CI 0.51-0.64). At 6 months, a significantly larger decrease was observed in the intervention group in energy, fat, and carbohydrate intake, accompanied with a significantly larger increase in moderate-intensity physical activity compared with the control group (energy: exp[<strong>β</strong>]=0.66, 95% CI 0.56-0.77; fat: exp[<strong>β</strong>]=0.71, 95% CI 0.54-0.95; carbohydrates: exp[<strong>β</strong>]=0.83, 95% CI 0.66-1.03; moderate-intensity physical activity: exp[<strong>β</strong>]=2.05, 95% CI 1.23-3.44). After 6 months of the intervention, participants in the intervention group were more likely to be at higher stages of dietary behaviors (exp[<strong>β</strong>]=26.80, 95% CI 3.51-204.91) and physical activity (exp[<strong>β</strong>]=15.60, 95% CI 2.67-91.04) than the control group. CONCLUSIONS DHealthBar was initially effective in improving dietary behavior, physical activity, and intention to change these behaviors among populations who were at high risk of developing T2DM, with significant differences in the changes of outcomes over the 6-month intervention period. Abstract Background Physical activity is increasingly recognized as an important marker of functional recovery following fracture. Objective The objectives of this study were to measure sedentary behavior and physical activity 2 weeks and 6 months following fracture and to determine associated demographic and injury factors. Design This was an observational study. Methods Two weeks and 6 months following fracture, 83 adults who were 18 to 69 years old and had upper limb (UL) or lower limb (LL) fractures wore an accelerometer and an inclinometer for 10 days. We calculated sitting time, steps, moderate-intensity physical activity (MPA), and vigorous-intensity physical activity and conducted linear mixed-effects multivariable regression analyses to determine factors associated with temporal changes in activity. Results At 6 months versus 2 weeks after fracture, participants sat less, took more steps, and engaged in more MPA. Participants with LL fractures sat 2 hours more, took 66% fewer steps, and engaged in 77% less MPA than participants with UL fractures. Greater reductions in sitting time were observed for participants in the youngest age group and with LL fractures, participants with high preinjury activity, and participants who were overweight or obese. For steps, greater improvement was observed for participants in the youngest and middle-aged groups and those with LL fractures. For MPA, greater improvement was observed for middle-aged participants and those with LL fractures. Limitations Although this study was sufficiently powered for the analysis of major categories, a convenience sample that may not be representative of all people with musculoskeletal trauma was used. Conclusions Working-age adults with LL fractures had lower levels of physical activity 6 months after fracture than those with UL fractures. Older adults showed less improvement over time, suggesting that they are an important target group for interventions aimed at regaining preinjury activity levels.

ABSTRACT We analyzed the effect of obesity on daily physical activity (DPA), light-intensity physical activity (LPA), moderate-intensity physical activity (MPA) and vigorous- intensity physical activity (VPA) in children and adolescents. Overall, 462 students from a public school (53.6% boys; aged 7-12 years) were involved in this one-year longitudinal study. Physical activities and sedentary behaviors (SB) were reported in the questionnaire Food Intake and Physical Activity of Students (Web-CAAFE). Obesity was evaluated through Body Mass Index (BMI z score ≥ 2). Obesity was observed in 16.5% of participants, but it did not influence LPA, MPA or DPA. However, VPA were 20% less frequent among obese students (β2=0.80; CI 95%: 0.66-0.98). Obese girls showed less frequency of VPA than non-obese boys (Mean Difference = -0.97; CI 95%: -1.36 to -0.57). Non-obese girls showed less VPA than non-obese boys (Mean Difference = -1.18; CI 95%: -1.40 to -0.95) and obese ones (Mean Difference = -0.57; CI 95%: -0.90 to -0.24). Obese boys showed fewer VPA, if compared to non-obese ones (Mean Difference = -0.61; CI 95%: -0.96 to -0.26). The analysis of the obesity-sex-age interaction showed a greater reduction in VPA among obese girls aged 10 years or more when compared to obese girls aged 7-9 years, and among boys obese and non-obese regardless of age. Amounts of VPA and SB increased simultaneously among non-obese boys and non-obese children aged 7-9 years. Obesity reduced VPA in the sample during the follow up of one year, independently of age and SB.

AbstractObjectivesTo objectively evaluate and describe physical activity levels in children with a stable congenital heart defect and compare those levels with children who do not have a congenital heart defect.MethodsWe matched 21 pairs of children for gender and grade in school and gave them an accelerometer-based motion sensor to wear for 7 consecutive days.ResultsPhysical activity levels did not differ between children with and without a congenital heart defect. During the 7 days of monitoring, children in this study spent most of their time in sedentary behaviours, that is, 6.7 hours of the 13 monitored hours, 54 minutes in moderate-intensity physical activity, and 12 minutes in vigorous-intensity physical activity. Less than one-fifth of all participants, with or without a congenital heart defect, accumulated sufficient physical activity to meet current physical activity recommendations for children and adolescents.ConclusionChildren with a stable congenital heart defect have activity behaviours that are similar to children without a congenital heart defect. Habitual physical activity in children with a congenital heart defect should be encouraged early on in life to develop strong physical activity habits that will hopefully follow them across their lifespan.

The high occurrence of overweight and obesity amongst children is a disturbing health problem worldwide. Possible causes of increasing childhood obesity are inactivity and energy imbalances. The aim of this study was to analyse the total energy expenditure (TEE)and physical activity levels in 9-year-old – 12-year-old overweight and obese children during a weekday and a weekend day, as well as during a weekday morning and afternoon. Twenty-four 9-year-old – 12-year-old children (seven boys and 17 girls), of whom nine were overweight and 15 were obese, were selected from seven public primary schools for this study. Body mass index (BMI) cut-off points were used to distinguish between overweight and obese. Each participant wore an ACTICAL™ monitor to determine their physical activity levels and TEE. It was found that the TEE of the children did not differ between a week day and a weekend day, although the TEE of the week day afternoon differed significantly from that of the weekday morning. Unlike the overweight children, none of the obese children met the requirements of 60 minutes of moderate-intensity physical activity per day. Strategies should be found to increase the activity levels of overweight and, especially, obese children, specifically during the mornings and over weekends.OpsommingDie hoë voorkoms van oorgewig en obesiteit onder kinders is wêreldwyd ’n kommerwekkende gesondheidsprobleem. Die doel van hierdie studie was om die totale energieverbruik (TEV) en fisieke-aktiwiteitsvlakke van 9-jarige – 12-jarige oorgewig- en obese kinders tydens ‘n totale weeksdag en naweekdag te ontleed, asook tydens die oggend en middag van ‘n weeksdag. Vier-en-twintig 9-jarige – 12-jarige kinders (sewe seuns en 17 dogters), waarvan nege oorgewig en 15 obees was, is uit sewe publieke laerskole vir die studie gekies. Afsnypunte volgens die liggaamsmassa-indeks (LMI) is gebruik om tussen oorgewig en obees te onderskei. Elke deelnemer het ‘n ACTICAL™ monitor gedra om hul fisieke-aktiwiteitsvlakke en TEV te bepaal. Die studie het getoon dat die TEV van die kinders op ’n weeksdag en naweekdag nie verskil het nie, alhoewel TEV tydens die middag van die weeksdag betekenisvol verskil het van die oggend s’n. Anders as die oorgewig kinders, het geen van die obese kinders aan die voorgestelde riglyn van 60 minute se matig intensiewe fisieke aktiwiteit per dag voldoen nie. Strategieë moet gevind word om die vlakke van aktiwiteit van oorgewig en veral obese kinders te verhoog, spesifiek gedurende die oggende en oor naweke.
