Background. There is growing evidence that cognitive and motor functions after traumatic brain injury (TBI) are actually related. However, we failed to find any concrete evidence proving the interrelationship between balance and cognitive functions therefore the effects of TBI on cognitive and motor functions remain not fully evaluated. Objective. The aim of the study was to evaluate the relationship between the recovery of balance and cognitive functions during physiotherapy in patients with TBI. The methods of the study. The study included 25 individuals who had sustained TBI. The subjects were distributed into two groups: Group 1 consisted of 15 subjects with moderate TBI, and Group 2 – of 10 subjects with severe TBI. The cognitive functions were evaluated using the Mini Mental State Examination, the level of cognitive functioning (consciousness) was evaluated using the Rancho Los Amigos scale, and balance was assessed with the help of the Fullerton Advanced Balance Scale. Results and conclusions. After physiotherapy, improvement was observed in the balance of subjects with moderate and severe TBI. Physiotherapy improved cognitive functions in subjects with moderate and severe TBI. The search for correlations between individual items of the Fullerton Advanced Balance Scale and Mini Mental State Examination revealed relationship between some items in both groups.

Background: The prevalence of senile dementia is increasing worldwide, especially in the developed countries. Nevertheless, drug therapy isn’t often enough to treat this condition. Researchers are evaluating the possible impact of a preventive approach, based on an improvement of lifestyle and the intake of micronutrients. Moreover, there is an increasing interest for combined nutraceuticals that can act as memory and learning enhancers, with a significant and beneficial potential on the cognitive disorders. Objective: To evaluate the effects of a rational assemblage of nutraceuticals on cognitive functions in a sample of 30 elderly subjects. Design: Double bind, cross-over designed trial versus placebo Setting: outpatient clinical practice Participants: 30 elderly subjects with basal Mini-Mental State Examination score between 20 and 27 and self-perceived cognitive decline. Intervention: Treatment with a combination of nutraceuticals based on Bacopa monnieri, L-theanine, Crocus sativus, copper, folate and vitamins of B and D group. After2 months of treatment or placebo. Measurements: Patients were evaluated with Mini-Mental State Examination (MMSE), Perceived Stress Questionnaire (PSQ) and Index and Self-Rating Depression Scale (SRDS). Results: MMSE and PSQ Index significantly improved in the active treatment arm, both versus baseline and versus the parallel arm. Both groups experienced a significant improving in the SRDS scores. Conclusions: We obtained a good and significant improvement of the cognitive functions tested with the MMSE, PSQ-Index and SRDS score, after 2 months of combined therapy of nutraceuticals. Further confirmation will be needed to verify these observations on the middle and long term in a larger number of subjects.

Abstract: stroke is a great health concern because it causes pain, disability, mortality and is expensive in cost. Impaired cognitive function is the inability of decision making, impaired memory, disorientation, perceptual disturbances and thinking difficulties. Objective : to examine cognitive functions of non haemorrhagic stroke patients using the Mini-Mental State Examination in the Medical Rehabilitation Department of RSUP Prof. Dr. R. D Kandou Manado. Methods : This study use a cross section descriptive approach (cross sectional). Subjects were non haemorrhagic stroke patients who come to the medical rehabilitation department of Prof.Dr.R.D Kandou from November to December 2013. Results: 20 people (40%) were found with normal cognitive function, 28 people (56%) had mild cognitive impairment, while the remaining 2 people (4%) had severe cognitive impairment. Conclusion: it is found that there are impaired cognitive functions ranging from mild to severe in non haemorrhagic stroke patients, in which could affect the patients survival. Gangguan fungsi kognitif menunjukkan ketidakmampuan mengambil keputusan, gangguan memori , disorientasi, gangguan persepsi dan kesulitan berpikir. Tujuan: Mengetahui gambaran fungsi kognitif pada pasien stroke non hemoragik menggunakan Mini-Mental State Examination (MMSE) di Instalasi Rehabilitasi Medik BLU RSUP Prof. Dr. R. D Kandou Manado. Metode:  Penelitian ini bersifat deskriptif dengan pendekatan potongan lintang (cross sectional). Subjek penelitian adalah penderita stroke non hemoragik yang datang ke Instalasi Rehabilitasi Medik BLU RSUP Prof. Dr. R.D Kandou Manado dari November-Desember 2013. Hasil: Ditemukan fungsi kognitif normal 20 orang (40%), gangguan fungsi kognitif ringan 28 orang (56%) dan yang mengalami gangguan fungsi kognitif berat 2 orang (4%). Simpulan: Ditemukan adanya gangguan fungsi kognitif ringan-berat pada pasien stroke non hemoragik yang berpengaruh pada kelangsungan hidup penderita. Kata Kunci: Stroke, Fungsi Kognitif

Im folgenden Artikel werden fünf verschiedene Versionen der Mini-Mental-State-Examination dargestellt, die alle auf der Grundlage des Originals von Folstein erstellt wurden, sich jedoch deutlich voneinander unterscheiden und zu unterschiedlichen Ergebnissen kommen, unabhängig davon, ob das Screening von erfahrenen Untersuchern durchgeführt wird oder nicht. Besonders auffällig ist, dass Frauen die Aufgaben «Wort rückwärts» hoch signifikant besser lösten als das «Reihenrechnen». An Hand von Beispielen werden Punkteunterschiede aufgezeigt.

Zusammenfassung. Beim Einsatz der Center for Epidemiological Studies Depression Scale (CES-D) stellt sich das Problem der Dimensionalität des Instruments, dessen Lösung durch die Konfundierung eines Teilkonstruktes (“Wohlbefinden”) mit Besonderheiten der Itemformulierung Schwierigkeiten bereitet, da Antwortartefakte zu erwarten sind. Dimensionsstruktur und Eignung der CES-D zur Erfassung der Depression bei älteren Menschen wurden an einer Stichprobe von 663 über 75-jährigen Teilnehmern der “Leipziger Langzeitstudie in der Altenbevölkerung” untersucht. Da sich die Annahme der Gültigkeit eines partial-credit-Rasch-Modells sowohl für die Gesamtstichprobe als auch für eine Teilpopulation als zu restriktiv erwies, wurde ein 3- bzw. 4-Klassen-latent-class-Modell für geordnete Kategorien berechnet und die 4-Klassen-Lösung als den Daten angemessen interpretiert: Drei Klassen zeigten sich im Sinne des Konstrukts “Depression” geordnet, eine Klasse enthielt jene Respondenten, deren Antwortmuster auf ein Antwortartefakt hinwiesen. In dieser Befragtenklasse wird der Depressionsgrad offensichtlich überschätzt. Zusammenhänge mit Alter und Mini-Mental-State-Examination-Score werden dargestellt. Nach unseren Ergebnissen muß die CES-D in einer Altenbevölkerung mit Vorsicht eingesetzt werden, der Summenscore sollte nicht verwendet werden.
