Abstract Objectives The incision method operation with a high risk of infection in a clean and clean-contaminated operation requires the use of prophylactic antibiotics to minimize the risk of infection. This study was designed to analyze the effectiveness of prophylactic antibiotics in patients with digestive and oncology surgeries. Methods The statistical method used was chi-square to determine the risk factors for infection at surgical site infections (SSI) in patients with digestive and oncology surgeries. This study had received ethical approval from the Ethics Committee of Dr. H. Slamet Martodirdjo Hospital, Pamekasan. Results There were 67 patients consisted of 48 digestive surgeries (71.6%) and 19 oncology surgeries (28.4%). The criteria of observation on day 30 showed that as 1 (1.5%) SSI patient experienced purulence, inflammation, and erythema around the surgical wound so an analysis of p>0.05 was carried out so that there was no association with the incidence of SSI during hospitalization, but other factors originating from the patient, such as a lack of personal hygiene at home and lack of nutritious food intake was measured in temperature, pulse, respiration, and white blood cells examination before surgery and 24 h after surgery, all within normal ranges. The qualitative analysis of prophylactic antibiotics using the Gyssen method showed that 31 (46.3%) rationales needed an improvement process. Conclusions The widely used prophylactic antibiotics, namely cefazolin and cefuroxime are recommended antibiotics used in incision surgery and rationale used.

Abstrak Penggunaan antibiotik profilaksis bedah yang tidak tepat dapat meningkatkan risiko terjadinya Infeksi Luka Operasi (ILO) dan resistensi obat. Penelitian ini bertujuan untuk menilai ketepatan penggunaan antibiotik profilaksis bedah pada bedah bersih di rumah sakit (RS) di Jakarta. Penelitian ini merupakan studi potong lintang di sebuah RS pemerintah di Jakarta. Data yang diambil berasal dari rekam medis pasien dewasa yang menjalani pembedahan dengan kriteria kelas luka bedah bersih. Data diambil pada periode 1 Januari hingga 31 Desember 2013. Analisis data dilakukan secara deskriptif berdasarkan pedoman penggunaan antibiotik baik nasional maupun internasional. Hasil penelitian menunjukkan 837 pasien menjalani bedah bersih pada periode tersebut. Bedah yang banyak dilakukan adalah bedah abdominal (30%) dan thorak-non kardiak (23,5%) dan lebih dari 96% merupakan bedah elektif. Jenis antibiotik yang paling banyak digunakan adalah siprofloksasin (56,5%). Keseluruhan kesesuaian antibiotik profilaksis adalah 27,7% tepat indikasi prosedur bedah dan hanya 0,5% yang tepat obat. Sebagai kesimpulan, ketepatan penggunaaan antibiotik profilasis di salah satu RS di Jakarta masih rendah. Penggunaan antibiotik yang tepat dapat menurunkan risiko ILO dan mengurangi biaya pengobatan dengan mengurangi peresepan yang tidak perlu dan durasi penggunaan antibiotik. This study aimed to assess the appropriateness use of surgical prophylactic antibiotics in clean surgery wounds in a hospital in Jakarta. This cross-sectional study was conducted at a government hospital in Jakarta. Data were collected from medical records of adult patients undergoing surgery with clean surgical wound criteria from January 1 to December 31, 2013. Data were analyzed descriptively based on guidelines for antibiotics use both nationally and internationally. The result showed 837 patients classified into the clean surgical wound during this period. The most common surgeries were abdominal surgery (39%) and non-cardiac thoracic (23.5%) and more than 96% were elective surgeries. The overall antibiotic prophylactic appropriateness showed 27.7% in surgical procedures and only 0.5% in choice of antibiotics. In conclusion, the appropriateness of surgery antibiotics prophylactic use in one of the hospitals in Jakarta was still low. Appropriate use of antibiotics can reduce the risk of SSI and cost treatment by reducing unnecessary prescribing and duration of antibiotics use. Background: Elective gallbladder surgery is the most common abdominal surgical procedure. Antibiotic prophylaxis is a common conduct in open cholecystectomy, but there is ambiguity about the use of prophylactic antibiotics in laparoscopic cholecystectomy. Some surgeons suggest that the elimination of prophylactic antibiotics in patients undergoing elective laparoscopic cholecystectomy increase the incidence of postoperative infective complications but not to a statistically significant degree. The aim of this study was to evaluate the role of prophylactic antibiotics in laparoscopic cholecystectomy.Methods: A total of 100 patients were included and they were randomized in 2 groups of 50 each. Group A was given prophylactic intravenously (IV) antibiotic within 1 hour before surgery and group B was not given any antibiotics. Results were compared and data analyzed statistically using chi-square and t test. Complications in both the groups were compared.Results: Rate of surgical site infections were 6% (n=3) and 4% (n=2) in group A and B respectively; and the difference between them was not found statistically significant. All infections which occurred in present study were superficial surgical site infection. There was no evidence of deep-seated infections and none of the patients developed distant infection. There was no derangement in any of the biochemical parameter in this study.Conclusions: We were not able to demonstrate any significant benefit from addition of prophylactic antibiotics in elective laparoscopic cholecystectomy.

Background: Surgical site infection is a significant cause of post operative morbidity. Timing of skin closure following a surgery and its relation to incidence of infection has been studied. The debate whether primary or delayed primary closure have been around for a long time. The aim of this study is to compare the rate of infection in a laparotomy wound in clean contaminated laparotomy cases after primary and delayed primary closure.Methods: 132 patients who underwent laparotomy for clean contaminated were selected. Of this primary closure of the skin was done in 66 cases and delayed primary closure at 48 hours were done in the other 66 patients. Wounds were followed up till post op day 7. The results between two groups were compared using chi square test.Results: two out of 66 cases developed SSI in the delayed primary sutured group whereas 8 out of 66 cases developed SSI in primary suturing group. The results were compared using chi square test and the chi square statistic was 3.8951 and the p value is 0.048428 (<0.05) proving the result significant.Conclusions: The incidence of surgical site infection in laparotomy wound after a primary closure was higher compared to delayed primary closure in clean contaminated laparotomy wounds and the difference is significant based on statistical evaluation.

Background: The infection of a wound is defined as the invasion of organisms through tissues following a breakdown of local and systemic host defences, leading to cellulitis, lymphangitis, abscess and bacteraemia. Southampton criteria and the centers for disease control and prevention criteria for the diagnosis of surgical site infections are used now for severity assessment. There is still controversy existing on the multifactorial causal relationship.Methods: Longitudinal Observational study with nonrandom purposive sampling carried out in the patients in OT, Casuality, ICU and Wards, in our hospital having clean contaminated abdominal operations for one-year period starting from November 2015 determine the factors responsible for surgical site infections following clean contaminated abdominal operations with prophylactic antibiotics(n=150).Results: Diabetes mellitus (odds ratio of 1.9) and emergency procedure (12.6%) were the most important risk factors for development of SSI. E. Coli (45%, n= 9) was the most common organism. Midline incision (n=6/22 = 27.27%) showed highest rate. Other high-risk factors are obesity, malnutrition, anemia, old age and prolonged duration of surgeries.Conclusions: Various host factors like malnutrition, obesity, patients knowledge about hygiene, presence of co-morbidity etc. coupled with environmental factors such as condition of the wounds, delay to initiate operation, duration of operation, prolonged exposure of peritoneal cavity to environment, prophylactic use of antibiotics and factors associated with surgery like type of incision, type of operation and experience of operating surgeon greatly contribute to occurrences of SSI. So, quality of surgical care including immediate assessment of patients, resuscitative measures, adequate preparation of patients and aseptic environment are important for control of SSI. Moreover, in absence of highly advanced surgical amenities, preoperative resuscitative units, modern operation theatre facilities and sophisticated sterilization procedure it is necessary to use prophylactic antibiotics to encounter the various types of micro-organisms responsible for surgical site infection, particularly E. coli.

Background: Surgical site infections (SSIs) are common surgical complications that will increase cost of treatment. The incidence of SSI can be prevented with antibiotic prophylactic. Uncompliance using of prophylactic antibiotics is one of the factors leading to the occurrence a microbes resistance. The data on the using of prophylactic antibiotics in Indonesia is still limited. Objective: This study aims to analyze the using of prophylactic antibiotic guideline on orthopaedic surgery. Methods: The study was conducted retrospectively using data from medical records on patients who had clean and clean-contaminated orthopedic procedures from 2013 to 2016 in the standard operating room of Dr. Soetomo hospital Surabaya. We analyzed the use of prophylactic antibiotics in terms of antibiotic selection, timing of administration, and the compliance to the prophylactic antibiotic local guidelines on orthopaedic surgery. Results: Overall, patient data from 2013 to 2016 was 5246 patients. The compliance rate of prophylactic antibiotics from 2013 to 2016 was 48.3%. This level of compliance uses a selection of antibiotics, dose of administration, delivery mode, delivery time, and route of administration. Conclusion: The results of this study have shown that the prophylactic antibiotic compliance rate on orthophaedic procedures in Soetomo Hospital from 2013 to 2016 was 48,3%. Antibiotic resistance control program quite effective at increasing compliance with the use of the prophylaxis antibiotics.

Infeksi luka operasi (ILO) adalah infeksi pada bagian tubuh yang terpapar oleh ahli bedah saat prosedur invasive. ILO merupakan salah satu infeksi nosokomial yang paling umum dan paling dapat dicegah diantara infeksi nosokomial lainnya, namun kejadian infeksi luka operasi terutama pada bedah abdomen masih menunjukkan beban yang signifikan. Faktor resiko infeksi luka operasi meliputi usia tua, jenis luka operasi, penyakit penyerta seperti diabetes mellitus, pemberian antibiotik profilaksis yang tidak adekuat, status gizi pasien seperti obesitas dan malnutrisi serta durasi operasi yang panjang. Penelitian ini bertujuan untuk mengetahui gambaran kejadian infeksi luka operasi pasca bedah abdomen di RSUD Abdul Wahab Sjahranie Samarinda. Metode penelitian ini adalah penelitian deskriptif laboratorik. Data penelitian diambil dari swab luka operasi dan data rekam medik pada 40 pasien pasca bedah abdomen yang melakukan pergantian perban pertama kali serta memenuhi kriteria inklusi dan eksklusi. Hasil penelitian menunjukkan 30 orang (75%) pasien mendapatkan infeksi luka operasi. Mayoritas pasien terinfeksi berusia diatas 40 tahun, yaitu pada 15 pasien (37,5%). Sebagian besar pasien terinfeksi mendapatkan luka bersih terkontaminasi, yaitu 23 pasien (57,5%). Mayoritas pasien terinfeksi yaitu 27 pasien (67,5%) tidak memiliki penyakit penyerta. Bakteri gram positif, yaitu Staphylococcus epidermidis merupakan bakteri penginfeksi terbanyak yang teridentifikasi pada 10 orang (33,3%) pasien. Seluruh pasien terinfeksi, yaitu 30 pasien (75%) mendapatkan antibiotik profilaksis. Mayoritas pasien terinfeksi memiliki status gizi normal, yaitu 17 orang (42,5%), dan kejadian infeksi lebih banyak terjadi pada pasien dengan durasi <120 menit, yaitu 20 orang (50%) pasien.   Kata Kunci : Infeksi Luka Operasi, ILO, Bedah Abdomen, Faktor Resiko Infeksi Luka Operasi Surgical site infection (SSI) is an infection at the part of the body that was exposed by a surgeon during an invasive procedure. SSI is one of the most common and most preventable nosocomial infection among other nosocomial infections, but the incidence of surgical site infections especially in abdominal surgery still shows a significant burden. Risk factors of surgical site infections include old age, type of surgical wound, comorbidities such as diabetes mellitus, inadequate prophylactic antibiotics, nutritional statuses such as obesity and malnutrition and long duration of surgery. This study aimed to explain an overview of the incidence of postoperative abdominal wound infection in Abdul Wahab Sjahranie General Hospital, Samarinda. This research method was a laboratory descriptive study. The research data was taken from surgical wound swabs and medical record data of 40 post-abdominal surgery patients who made the bandage changes for the first time and met the inclusion and exclusion criteria. The results showed 30 patients (75%) had surgical site infections. The highest percentage of infected patients aged over 40 years, i.e. in 15 people (37.5%). Most of the infected patients had clean contaminated wounds, i.e. in 23 patients (57.5%). The majority of infected patients did not have concomitant diseases,  i.e. 27 people (67.5%). Gram-positive bacteria, specifically Staphylococcus epidermidis was the most infectious bacteria identified in 10 patients (33.3%). All infected patients, i.e. 30 patients (75%) received prophylactic antibiotics. The highest frequencies of infected patients had normal nutritional status, i.e.17 patients(42.5%), and the incidence of infection was more common in patients with duration <120 minutes, i.e. 20 patients (50%). Objective:To assess the relative importance of risk factors for surgical-site infections (SSIs) in orthopedic patients and thereby determine which risk factors to monitor in the national surveillance of SSI in The Netherlands.Design:Reanalysis of data on SSI and associated risk factors from two surveillance projects on nosocomial infections, carried out in 1992 and 1993 in The Netherlands: Project Surveillance Nosocomial Infections in the region of Utrecht (PSZU) and the first Project Surveillance Surgical Wound Infections (SWIFT-1). Odds ratios (ORs) were calculated for age, gender, preoperative stay, and the number of operations. In addition, in PSZU, other nosocomial infections, and, in SWIFT-1, prophylactic antibiotics, acute surgery, and wound contamination were studied.Participants:The study was confined to hospitalized orthopedic patients (PSZU, 4,872; SWIFT-1, 6,437).Results:In PSZU, the following ORs were significant in a multivariate model: age 0-44 years, 1.0; 45-64 years, 1.6; 65-74 years, 4.7; and 75-99 years, 6.0. For a preoperative stay over 4 days, the OR was 3.3 (95% confidence interval [CI95], 2.5-4.0), and for multiple surgery, 2.5 (CI95, 1.9-3.0). For females, the OR was 0.8 (not significant). The same model applied to SWIFT-1 gave similar ORs. Adjustment for additional nosocomial infections (PSZU) decreased the ORs for ages over 65 years remarkably. The OR for additional nosocomial infections in patients under 65 years of age was 15.6 (CI95, 4.3-57.4). Adjustment for prophylactic antibiotics, acute surgery, and wound-contamination class (SWIFT-1) did not influence the ORs of the original model, but showed that wound-contamination class was an important risk factor.Conclusions:Age, additional nosocomial infections, wound-contamination class, preoperative stay, and the number of operations were identified as important risk factors for SSI in Dutch orthopedic patients.

Faktor Risiko Kejadian Penyakit Kulit Pada Pekerja Pengangkut Sampah Di Kota Jambi 1Entianopa, 2Ranissa Dwi Imansari, 3Irwandi Rachman       123Program Studi Kesehatan Masyarakat STIKES Harapan Ibu, Jambi   Abstrak Latar Belakang: Kulit merupakan organ terbesar pada tubuh manusia yang membungkus otot-otot dan organ-organ dalam serta merupakan jalinan jaringan pembuluh darah, saraf, dan kelenjar yang tidak berujung, semuanya memiliki potensi untuk terserang penyakit yang salah satunya adalah penyakit kulit. Penyakit kulit merupakan salah satu gangguan kesehatan yang sering dialami oleh pekerja pengangkut sampah. Berdasarkan komposisi sampah yang diangkut serta waktu paparan kerja. Tujuan penelitian ini untuk mengetahui hubungan antara masa kerja, pemakaian Alat Pelindung Diri (APD), dan personal hygiene dengan kejadian penyakit kulit pada pekerja pengangkut sampah di Kota Jambi. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain penelitian cross sectional study. Sampel penelitian yaitu sebanyak 62 pekerja pengangkut sampah yang berada di Kantor Pekerjaan Umum dan Penata Ruang, yang mana seluruh populasi dijadikan sampel. Data dikumpulkan berdasarkan pemeriksaan kesehatan oleh dokter dan dengan kuesioner, kemudian dianalisa menggunakan uji statistik chi-square. Hasil: Hasil menunjukan bahwa pekerja yang mengalami penyakit kulit sebanyak 35 pekerja (56,5%). Berdasarkan hasil analisis chi-square didapatkan bahwa ada hubungan yang bermakna antara masa kerja dengan kejadian penyakit kulit pada pekerja pengangkut sampah dimana nilai (p-value= 0,006), Pemakaian Alat Pelindung Diri (APD) nilai (p-value= 0,008), personal hygiene nilai (p-value= 0,008). Kesimpulan: Untuk meminimalisir risiko terjadinya penyakit kulit pada pekerja pengangkut sampah disarankan perlunya disusun standar operasional prosedur yang aman, penyediaan sarana sanitasi agar dapat mengurangi resiko terkena penyakit kulit. Pentingnya pemakaian APD dan perilaku hidup bersih dan sehat selama bekerja, serta diharapkan pekerja menggunakan APD pada saat bekerja dan lebih memperhatikan personal hygiene.   Kata kunci      : Masa Kerja, APD, Personal Hygiene

Dermatitis merupakan peradangan kulit sebagai respon terhadap pengaruh faktor-faktor lingkungan seperti polutan dan alergen-alergen. Data Dinas Kesehatan Banyumas Tahun 2015 kasus Dermatitis tertinggi Kecamatan Patikraja 1.358 pasien. Bulan Nopember tahun 2015, pasien Dermatitis tertinggi 138 orang di Desa Kedungrandu. Wilayah Desa Kedungrandu merupakan lokasi Tempat Pembuangan Akhir Gunung Tugel dimana tempat pembuangan akhir gunung tugel merupakan yang terbesar di Banyumas. Tujuan penelitian ini untuk menganalisis hubungan jenis sumber air dan personal hygiene dengan kejadian Dermatitis  Metode penelitian yang digunakan adalah observasi dan case control dengan 27 responden kasus dan 27 responden kontrol. Variabel penelitian ini sarana sumber air dan personal hygiene yang terdiri dari perilaku mandi, perilaku berpakaian dan perilaku tidur. Analisis menggunakan analisis SPSS versi 1.7 dengan uji chi-square dengan α 0,05. Hasil penelitian menunjukkan variabel yang memiliki hubungan dengan kejadian penyakit Dermatitis adalah jenis sumber air dengan nilai p value= 0,001, personal hygiene merupakan variabel yang tidak mimiliki hubungan dengan kejadian penyakit Dermatitis di Desa Kedungrandu dengan hasil nilai p value= 1,000. Kesimpulan penelitian yaitu jenis sumber air dapat menjadi salah satu faktor penyebab Dermatitis di Desa Kedungarandu. Peneliti menyarankan dari pihak puskesmas meningkatkan kerja sama dengan pemerintah desa untuk melakukan penyuluhan dan meningkatkan program kesehatan lingkungan.

Keluhan pada organ reproduksi yang sering terjadi adalah Pruritus vulvae yaitu ditandai dengan adanya sensasi gatal parah dari alat kelamin perempuan. Pruritus vulvae disebabkan oleh jamur, bakteri dan virus yang muncul 44% karena buruknya Personal Hygiene dan Hygiene Menstruasi. Penelitian Tahun 2015 di 4 wilayah di Indonesia yaitu di Nusa Tenggara Timur (NTT), Jawa Timur, Papua, dan Sulawesi Selatan terkait kebersihan saat menstruasi menemukan 67% remaja di kota dan 41% remaja di desa masih adanya perilaku negatif. Tujuan penelitian untuk mengetahui beberapa faktor yang berhubungan dengan perilaku hygiene menstruasi melalui Health Belief Model (HBM). Penelitian ini adalah penelitian kuantitatif  dengan menggunakan pendekatan cross sectional dengan sampel sebanyak 101 siswi SMPN 244 di Jakarta Utara, dianalisis secara univariat dan bivariat menggunakan chi square. Perilaku Hygiene Menstruasi baik baru dilakukan 55,4% responden. Perilaku Hygiene Menstruasi berhubungan secara signifikan dengan pengetahuan (OR=5,1), perceived threat (OR=3,9) dan perceived benefit (OR=3,3) dengan P Value < 0.005. Health Belief Model (HBM) bisa dipertimbangkan sebagai suatu pendekatan dalam upaya memperbaiki perilaku hygiene menstruasi pada remaja. Peningkatan pengetahuan direkomendasikan dengan menekankan pada ancaman penyakit yang berkaitan dengan perilaku hygiene menstruasi dan manfaat-manfaat yang langsung dirasakan oleh remaja berkenaan dengan perilaku higiene menstruasi.
