Hyperactivity, a form of sperm motility characterized by vigorous flagellar movements, has been proposed as essential for fertilization in mammals. The objective of the present study was to establish a method for inducing hyperactivityin vitroin boar spermatozoa and to define threshold values to differentiate between hyperactive and non-hyperactive spermatozoa by computer-assisted sperm analysis (CASA) as a prerequisite for analyzing the energy metabolism during hyperactivity. In TALP-HEPES medium, non-frozen boar spermatozoa were stimulated to hyperactivity by 50 μmol l−1Ca2+within 15 min at 37 °C if 5 μmol l−1of the Ca2+ionophore A23187 was present. If 25% seminal plasma was present, boar spermatozoa required higher Ca2+concentrations (about 700 μmol l−1) for hyperactivity. Under both conditions, immobilization and head-to-head agglutination were low so that hyperactive spermatozoa could be analyzed for at least 40 min. The transition from normal to hyperactive movement was characterized by an increase in flagellar beat angle from 49° ± 12° to 200° ± 36° (n= 32) and a decrease in flagellar curvature ratio from 0.89 ± 0.04 to 0.47 ± 0.11 (n= 32). For quantification of hyperactive boar sperm, kinematic parameters of hyperactive and non-hyperactive spermatozoa were measured by CASA and statistically evaluated (receiver operating characteristic (ROC) curve analysis). The threshold values of the following four parameters were well suited for differentiating between hyperactive and non-hyperactive boar spermatozoa (ROC curve analysis: >50% specificity at 100% sensitivity). Hyperactive boar spermatozoa showed mean lateral head displacement >3.5 μm, curvilinear velocity >97 μm s−1, linearity <32% and wobble <71%. According to this multiparametric definition, induction of hyperactivity increased significantly (P< 0.0001) the fraction of hyperactive spermatozoa in semen samples from 5.1 ± 4.3% (n= 13) to 48.3 ± 6.6% (n= 7) in the absence and to 44.2 ± 7.6% (n= 10) in the presence of 25% seminal plasma, while the overall percentage of motile spermatozoa did not change significantly.

The motion ability of mammalian spermatozoa is acquired during their epididymal transit but observed only upon dilution with seminal plasma (SP) at the time of ejaculation (Yanahimachi 1994 in The Physiology of Reproduction, New York: Raven Press). The bicarbonate present in seminal plasma activates multiple sperm functions, some of which are essential for the initiation of motility. Sperm hyperactivity has been observed in vitro in various mammalian species, especially if capacitation of spermatozoa was induced with Ca2+ and bicarbonate media, such as TALP (Harrison et al. 1996 Mol. Reprod. Dev. 45, 378–391). Computer-assisted sperm analysis (CASA) is a tool for the objective assessment of sperm motility. The aim of this study was to determine if there are differences in motility parameters of ejaculated (EJ) and epididymal (EP) boar spermatozoa under different treatments. Ejaculated and epididymal sperm cells from 10 different boars in each group were used. The sperm treatments were: washed in Dulbecco&apos;s PBS supplemented with 0.1&percnt; BSA (PBS-BSA), washed on a Percoll gradient (PG), and unwashed (UW: Control); the sperm samples were incubated in TALP medium at 38.5&deg;C and 5&percnt; CO2 during the analysis. Motion parameters were determined using a computer-assisted sperm analysis (CASA) system. A 7-&micro;L drop of the sample was placed on a warmed (37&deg;C) slide. At least 4 fields per sample were evaluated, with a minimum of 100 spermatozoa counted per sub-sample. The CASA-derived motility characteristics studied were motility (MOT, &percnt;), progressive motility (PM, &percnt;), curvilinear velocity (VCL, &micro;m s&minus;1), straight-line velocity (VSL, &micro;m s&minus;1), average path velocity (VAP, &micro;m s&minus;1), linearity of the curvilinear trajectory (LIN, ratio of VSL/VCL, &percnt;), straightness (STR, ratio of VSL/VAP, &percnt;), amplitude of lateral head displacement (ALH, &micro;m), wobble of the curvilinear trajectory (WOB, ratio of VAP/VCL, &percnt;), and beat cross-frequency (BCF, Hz). Data were analyzed by ANOVA. If we evaluated all of the data together (EJ vs. EP), EP sperm after treatment showed a higher motility (PM: 38.20&percnt;; MOT: 74.23&percnt;) than EJ sperm (PM: 29.27&percnt;; MOT: 63.24&percnt;), and all of the motion parameters related to velocities and ALH were higher in EP (VCL: 86.02; VSL: 41; VAP: 57.94; ALH: 3.21) than in EJ (VCL: 69.70; VSL: 34.67; VAP: 48.16; ALH: 2.54). No differences were found for LIN, STR, WOB, and BCF. The treatments significantly affected the VCL and ALH, with lower values for the PG treatment. When VCL was lower and the VSL and VAP were similar, consequently the LIN and WOB were significantly higher for the PG group. STR also was higher for the PG group. In conclusion, when both groups of sperm were incubated in TALP medium, the EJ sperm showed a decrease in the majority of motion parameters when compared with EP sperm. Background: Trochlear dysplasia (TD) is the principle anatomic risk factor for patellar instability (PI). The lateral trochlear inclination (LTI) is a quantitative measure of trochlear morphology on axial magnetic resonance imaging (MRI). Threshold values for patients at risk for PI via LTI measurement have ranged from 11o to 17o. Our group recently described a new technique for characterizing LTI by referencing the fully-formed posterior femoral condyles. This novel measurement technique revealed that the historically-used method was comparable in reliability but significantly underestimated TD. Purpose: The purpose of this study was to define, with high specificity, a LTI value cutoff value consistent with patellar instability using the new LTI measurement technique. Methods: MRI scans of 95 patients aged 9 to 18 years treated for PI at our tertiary referral center were compared with a control cohort of 98 age- and gender- matched patients with knee imaging but no clinical evidence of PI. LTI was measured as the angle formed between a line subtended from the cartilaginous surface of the lateral trochlea at its most proximal extent and a line parallel to the fully-formed posterior condyles. Receiver operator characteristic (ROC) curve analysis was conducted to establish a cutoff value with optimal specificity and sensitivity. Results: Average LTI was significantly lower in the study group (5.9±10.4o) than control (18.9±5.8o) (p<0.001). Area under the ROC curve was 0.86. A proposed 8.9o LTI threshold angle achieves good discrimination between the two groups, with specificity of 0.904 and sensitivity of 0.684. Conclusion: Re-examination of previously described threshold values using ROC curve analysis of LTI measured in reference to the posterior condyles found 8.9o as the optimal threshold value to achieve good discrimination between patients with and without PI. The novel technique has previously been established to have excellent intra- and inter-observer reliability and produced significantly lower LTI values than the historically-used technique. Taken in concert, this new cutoff is valuable to improve future clinical decision making in regards to risk stratification, treatment algorithms and research purposes.

Mutual relationships between selected chemical elements (Na, K, Fe, Cu, Mg, and Zn), basic motility characteristics (motility and progressive motility), and markers of the oxidative balance (superoxide dismutase, catalase, glutathione, albumin, and malondialdehyde) were investigated in bovine seminal plasma and spermatozoa. Computer assisted sperm analysis was used to assess the motility parameters; mineral concentrations were determined by the voltammetric method and flame absorption spectrophotometry; antioxidants and malondialdehyde were evaluated by UV/VIS spectrophotometry. Concentrations of chemical elements in both seminal fractions were in the following descending order: Na > K > Zn > Mg > Fe > Cu. Higher amounts of all minerals and nonenzymatic antioxidants were detected in the seminal plasma (P<0.01; P<0.001), while higher MDA concentration and activity of enzymatic antioxidants were recorded in the cell lysates (P<0.01; P<0.001). Na, Fe, Cu, Mg, and Zn were positively correlated with the motility and antioxidant parameters (P<0.05; P<0.01; P<0.001). Inversely, K exhibited the positive associations with malondialdehyde (P<0.05). This study demonstrates that most chemical elements are integral components of bovine semen and are needed for the protection against oxidative stress development.

OBJECTIVEInhibition of platelet aggregation is universally used to prevent thromboembolic complications related to stent placement in endovascular neurosurgery, but excessive inhibition potentiates hemorrhagic complications. Previously, the authors demonstrated that two different commercially available measures of adenosine diphosphate (ADP)–dependent platelet inhibition—the VerifyNow P2Y12 clopidogrel assay (measured in platelet reactivity units [PRU]) and maximal amplitude (MA) attributable to ADP activity (MA-ADP) derived from thromboelastography (TEG) with platelet mapping (PM)—yielded wildly different results. This study sought to analyze observed complications to quantify the ideal therapeutic windows for both tests.METHODSNinety-one patients with simultaneous or near-simultaneous PRU and TEG-PM results who underwent craniocervical endovascular stenting at the authors’ institution between September 2015 and November 2017 were identified and retrospectively enrolled. From November 2017 until June 2019, 109 additional patients were prospectively enrolled. For this study, in-hospital thrombotic and hemorrhagic complications (both CNS and non-CNS) were tabulated, and receiver operating characteristic (ROC) curve analysis was used to identify threshold values of the PRU and MA-ADP for predicting each type of complication.RESULTSOf the 200 patients enrolled, 7 were excluded because of anemia or thrombocytopenia outside of the test manufacturer’s specified ranges and 1 was excluded because they did not have a TEG-PM result. Including complications of all severities, there were a total of 15 CNS thrombotic complications, 1 access-site thrombotic complication, 3 CNS hemorrhages, 8 access-site hemorrhagic complications, and 3 hemorrhagic complications not affecting either the CNS or the access site. ROC curve analysis yielded therapeutic threshold values of 118–144 PRU. The results demonstrated PRU has a significant dose-dependent effect on the rates of thrombosis and hemorrhage. Logistic regression models did not demonstrate statistically significant relationships between the MA-ADP and either thrombosis or hemorrhage. ROC analysis based on these models is of little value and did not identify significant threshold values for MA-ADP.CONCLUSIONSThere continues to be poor correlation between the results of TEG-PM and PRU. PRU accurately predicted complications, with a relatively narrow ideal value range of 118–144. The MA-ADP alone does not appear able to accurately predict either hemorrhagic or thrombotic complications in this group.

Background: The alpha angle is the most often used measure to classify cam morphology. There is currently no agreement on which alpha angle threshold value to use. Purpose: To systematically investigate the different alpha angle threshold values used for defining cam morphology in studies aiming to identify this threshold and to determine whether data are consistent enough to suggest an alpha angle threshold to classify cam morphology. Study Design: Systematic review; Level of evidence, 3. Methods: The Embase, Medline (Ovid), Web of Science, Cochrane Central, and Google Scholar databases were searched from database inception to February 28, 2019. Studies aiming at identifying an alpha angle threshold to classify cam morphology were eligible for inclusion. Results: We included 4 case-control studies, 10 cohort studies, and 1 finite-element study from 2437 identified publications. Studies (n = 3) using receiver operating characteristic (ROC) curve analysis to distinguish asymptomatic people from patients with femoroacetabular impingement syndrome consistently observed alpha angle thresholds between 57° and 60°. A 60° threshold was also found to best discriminate between hips with and without cam morphology in a large cohort study based on a bimodal distribution of the alpha angle. Studies (n = 8) using the upper limit of the 95% reference interval as threshold proposed a wide overall threshold range between 58° and 93°. When stratified by sex, thresholds between 63° and 93° in male patients and between 58° and 94° in female patients were reported. Conclusion: Based on the available evidence, mostly based on studies using ROC curve analysis, an alpha angle threshold of ≥60° is currently the most appropriate to classify cam morphology. Further research is required to fully validate this threshold.

Практической частью репродуктивной технологии является прогноз и оценка фертильности самцов. Существующие методы оценки семени не дают достаточно корректного прогноза результатов осеменения и, следовательно, репродуктивного потенциала того или иного производителя. В работе приведены данные комплексной оценки биологической полноценности семени чистопородных и гибридных хряков-производителей с использованием компьютерной технологии CASA (computer-assisted sperm analysis). Оценивали активность сперматозоидов в образцах классическим способом (микроскопирование) и с помощью компьютерной технологии, программа «Зоосперм 1.0». Исследования проводили в 2014 – 2015 гг. в Лаборатории репродуктивной криобиологии животных (Россия). Объектом исследования служила сперма чистопородных и гибридных хряков-производителей, разводимых в разных регионах Российской Федерации: крупная белая (n = 24), дюрок (n = 28), ландрас (n = 18), пьетрен (n = 8), гибридные (n = 6). Установлено, что высокими репродуктивными качествами отличаются хряки-производители породы ландрас и гибридов, от них в среднем за эякулят получено 290,2 и 290,6 мл спермы соответственно; наименьшими — хряки-производители породы пьетрен (270 мл). Частота встречаемости сперматозоидов с аномальной морфологией высокая у хряков-производителей породы дюрок (12 %) и пьетрен (15 %). Частота встречаемости аномалии в разных сегментах сперматозоидов зависит от индивидуальной особенности хряков-производителей. У одних производителей наиболее часто встречаются морфологические отклонения головки, например, у хряка породы ландрас оно составило 58,60 %, у других часто встречались отклонения от нормы в строении жгутика (12,1 – 29,3 %). Использование компьютерной технологии при оценке биологической полноценности сперматозоидов хряков-производителей является наиболее точным и информативным.

Genetic resource banks (GRB) preserve the genetic material of endangered, valuable individuals or genetically relevant breeds. Semen cryopreservation is a crucial technique to reach these goals. Thus, we aimed to assess the sperm parameters of semen doses from the native pig breed Gochu Asturcelta stored at the GRB of Principado de Asturias (GRB-PA, Gijón, Spain), focusing on intrinsic and extrinsic (boar, season) factors. Two straws per boar (n = 18, 8–71 months of age) were thawed, pooled, and assessed after 30 and 150 min at 37 °C by CASA (computer-assisted sperm analysis system; motility and kinematic parameters) and flow cytometry (viability, acrosomal status, mitochondrial activity, apoptosis, reactive oxygen species, and chromatin status). The effects of age, incubation, and season on post-thawing quality were determined using linear mixed-effects models. Parameters were on the range for commercial boar breeds, with chromatin status (SCSA: fragmentation and immaturity) being excellent. Incubation decreased sperm quality and functionality. The boar age did not have a significant effect (p > 0.05), but the between-boar variability was significant (p < 0.001). The season significantly affected many parameters (motility, kinematics, viability, acrosomal status, mitochondrial activity), especially after 150 min of incubation. In general, samples collected in spring and summer showed higher quality post-thawing, the lowest in winter. In conclusion, the sperm doses from the Gochu Asturcelta breed stored at the GRB-PA showed excellent chromatin status and acceptable characteristics after thawing. Therefore, boar and seasonal variability in this autochthonous breed could be relevant for cryobank management.

Abstract Funding Acknowledgements Type of funding sources: None. Introduction Patients (P) with acute heart failure (AHF) are a heterogeneous population. Risk stratification at admission may help predict in-hospital complications and needs. The Get With The Guidelines Heart Failure score (GWTG-HF) predicts in-hospital mortality (M) of P admitted with AHF. ACTION ICU score is validated to estimate the risk of complications requiring ICU care in non-ST elevation acute coronary syndromes. Objective To validate ACTION-ICU score in AHF and to compare ACTION-ICU to GWTG-HF as predictors of in-hospital M (IHM), early M [1-month mortality (1mM)] and 1-month readmission (1mRA), using real-life data. Methods Based on a single-center retrospective study, data collected from P admitted in the Cardiology department with AHF between 2010 and 2017. P without data on previous cardiovascular history or uncompleted clinical data were excluded. Statistical analysis used chi-square, non-parametric tests, logistic regression analysis and ROC curve analysis. Results Among the 300 P admitted with AHF included, mean age was 67.4 ± 12.6 years old and 72.7% were male. Systolic blood pressure (SBP) was 131.2 ± 37.0mmHg, glomerular filtration rate (GFR) was 57.1 ± 23.5ml/min. 35.3% were admitted in Killip-Kimball class (KKC) 4. ACTION-ICU score was 10.4 ± 2.3 and GWTG-HF was 41.7 ± 9.6. Inotropes’ usage was necessary in 32.7% of the P, 11.3% of the P needed non-invasive ventilation (NIV), 8% needed invasive ventilation (IV). IHM rate was 5% and 1mM was 8%. 6.3% of the P were readmitted 1 month after discharge. Older age (p &lt; 0.001), lower SBP (p = 0,035) and need of inotropes (p &lt; 0.001) were predictors of IHM in our population. As expected, patients presenting in KKC 4 had higher IHM (OR 8.13, p &lt; 0.001). Older age (OR 1.06, p = 0.002, CI 1.02-1.10), lower SBP (OR 1.01, p = 0.05, CI 1.00-1.02) and lower left ventricle ejection fraction (LVEF) (OR 1.06, p &lt; 0.001, CI 1.03-1.09) were predictors of need of NIV. None of the variables were predictive of IV. LVEF (OR 0.924, p &lt; 0.001, CI 0.899-0.949), lower SBP (OR 0.80, p &lt; 0.001, CI 0.971-0.988), higher urea (OR 1.01, p &lt; 0.001, CI 1.005-1.018) and lower sodium (OR 0.92, p = 0.002, CI 0.873-0.971) were predictors of inotropes’ usage. Logistic regression showed that GWTG-HF predicted IHM (OR 1.12, p &lt; 0.001, CI 1.05-1.19), 1mM (OR 1.10, p = 1.10, CI 1.04-1.16) and inotropes’s usage (OR 1.06, p &lt; 0.001, CI 1.03-1.10), however it was not predictive of 1mRA, need of IV or NIV. Similarly, ACTION-ICU predicted IHM (OR 1.51, p = 0.02, CI 1.158-1.977), 1mM (OR 1.45, p = 0.002, CI 1.15-1.81) and inotropes’ usage (OR 1.22, p = 0.002, CI 1.08-1.39), but not 1mRA, the need of IV or NIV. ROC curve analysis revealed that GWTG-HF score performed better than ACTION-ICU regarding IHM (AUC 0.774, CI 0.46-0-90 vs AUC 0.731, CI 0.59-0.88) and 1mM (AUC 0.727, CI 0.60-0.85 vs AUC 0.707, CI 0.58-0.84). Conclusion In our population, both scores were able to predict IHM, 1mM and inotropes’s usage.

Abstract Objective Sepsis is a major cause of mortality for critically ill patients. This study aimed to determine whether presepsin values can predict mortality in patients with sepsis. Results Receiver operating characteristic (ROC) curve analysis, Log-rank test, and multivariate analysis identified presepsin values and Prognostic Nutritional Index as predictors of mortality in sepsis patients. Presepsin value on Day 1 was a predictor of early mortality, i.e., death within 7 days of ICU admission; ROC curve analysis revealed an AUC of 0.84, sensitivity of 89%, and specificity of 77%; and multivariate analysis showed an OR of 1.0007, with a 95%CI of 1.0001–1.0013 (p = 0.0320).
