MOTOVUN FILM FESTIVAL 2000 Twice as exciting as its first outing, Motovun Film Festival -- "The Second" (1-8 August 2000) showed no signs of that fabled jinx of the follow-up year. Rather, under the anxious gaze of the local populace, the five-day festival prompted a pilgrimage to this picturesque medieval town perched atop a hill on the Istrian peninsula in Croatia (closer to Trieste and Ljubljana than to Zagreb). Indeed, too many cineastes and visitors came, as the open-air arena on the town square comfortably seats only a couple thousand, while the restored town movie theatre can't squeeze in more than 200. Thus, just a day after the box office opened, the circa 50 films on the program were nearly all sold out, so ad hoc screenings had to be arranged on a school playground in the lower town. Ask artistic director Rajko Grlić where all this flamboyant festival flair...

ANY WAY YOU LOOK AT IT, 2008 was a watershed year at the Cannes film festival. Looking ahead, this was Thierry Frémaux’s first year as délégué général, the first time he was listed in the catalogue as the man who put his own signature on the selection. Last year, he was down simply as director artistique, to wit: the festival’s artistic director under the friendly aegis of président Gilles Jacob. Looking back, Gilles Jacob is currently penning his collection of memories as the festival icon over the past 30 years. Knowing Jacob’s unrestrained love for the Festival de Cannes, a spring event he has personally molded into an institution that stands head and shoulders over all other A-category festivals, his memoirs will surely offer insights into how he was able to balance the ideals of auteur cinema with the public’s demand for starlets on the grand staircase and box-office...

Abstract Introduction The Covid-19 pandemic has had a global impact on healthcare. As a result, changes have been made to healthcare provision. In the UK, the focus was changed to urgent and emergency treatment only, and therefore a shift from face-to-face consultations to video and telephone consultations was implemented. Our aim was to look at the telephone consultations in our oral and maxillofacial (OMF) department to identify their effectiveness. Method A search was carried out using the electronic patient record for patients booked for consultations in the OMF clinics from the 23rd of March to the 1st of May 2020. Trauma clinics and ad-hoc clinics were excluded. Results 21 clinic codes were found as suitable for inclusion. 185 consultations were booked during the 6-week period. Of these, 165 were via telephone and 20 face-to-face. A total of 75 consultations resulted in discharge and 110 required further follow up or further investigation. Of the 75 patients discharged, 73 of these were via telephone. The main reason for discharge was delivery of benign biopsy results. Conclusions Covid-19 will have long term impacts on healthcare provision. If implemented correctly, virtual consultations carry the potential in making healthcare more accessible.

KARLOVY VARY INTERNATIONAL FILM FESTIVAL 2004 For the Czech media the 39th Karlovy Vary International Film Festival (2-10 July 2004) was something of a sensation. Václav Havel, the country's ex-president and most popular political figure, was on hand for the official opening to receive a moving standing ovation. Later, Václav Klaus, the current Czech president, also showed for the closing ceremonies. To add to the festivities, Miroslav Ondříček, Miloš Forman's ace cameraman (Oscar Nominations for Amadeus and Ragtime) was honoured with an Award for Outstanding Artistic Contribution to World Cinema (together with Harvey Keitel and Roman Polański). The Prize of the Karlovy Vary Region was given for the first time to Jiří Bartoška, the festival codirector. And the Ecumenical Jury honoured Eva Zaoralová, the festival's artistic director, an Award for Outstanding Contribution to the Karlovy Vary International Film Festival. The results of the study concluded that the ruling of the Constitutional Court against the position of the ad hoc judge is appropriate because it gives the same position on a different matter precisely caused injustice.   In addition to having the authority to check, prosecute, and break the criminal corruption, adhoc judges also have the authority to examine the criminal case of money laundering that the original criminal act is a corruption crime. So here corruption as the original criminal act is often referred to as predicate crimes. This chapter explores Danish director Nicolas Winding Refn. Refn’s first feature Pusher (1996) was a local box-office success that helped usher in the era of the Nordic crime film, which includes his own follow-up Bleeders (1998) and two Pusher sequels (2004 and 2005). His American crime and gangster film, Drive (2011), is set in Los Angeles and is indebted to notable American classics of the genre. Reunited with Hollywood star Ryan Gosling, Refn continued to explore the international pedigree of the crime thriller in Only God Forgives (2013), where Gosling plays an American struggling to stay afloat in the Bangkok underworld. In Neon Demon (2016), Refn returns to Los Angeles, this time the world of fashion, where Hollywood gloss and European film aesthetics meet head-on. His three “American” films thus offer a striking blend of Hollywood genre and European art cinema traditions helping to explain their wildly mixed receptions.

ABSTRACT Introduction: Lower third leg soft tissue defects with anatomical and pathological constraints are posing formidable challenges to reconstructive surgeon. Aim: This retrospective study was conducted to assess the effectiveness of ad hoc posterior tibial vessels perforator-propeller flaps for the reconstruction of small and medium sized soft tissue defects in the lower third leg. Patients and Methods: 22 patients (16 were males and 6 were females) were involved in this study between period of January 2012 and December 2016. We followed the protocol of initial non delineating exploratory incision made to find out single best perforator in all patients. All the defects in leg reconstructed with adhoc posterior tibial vessel propeller flaps. Results: All 22 flaps survived well. All in an average of 13 months follow up period, had pain free walking, with minimal scarring and acceptable aesthesis at the reconstruction sites with no need for any secondary procedure. Conclusion: With inability of preoperatively dopplering the perforators in the lower third leg region, the exploratory posterior nondelineating incision was used in all cases to secure the single best perforator for the propeller flaps. Thus adhoc posterior tibial vessel propeller flaps are dependable, easily adoptable for the reconstruction of soft tissue defects of the lower third leg region.

Abstract Background the concept of 'patient experience' has become central to how to improve healthcare. Remote communication with patients is today a frequent practice in healthcare services, showing similar outcomes to conventional monitoring at the hospital (CM) while enabling cost reduction in both formal and informal care. The purpose of this study was to analyse the experiences of people with remote monitoring (RM) of pacemakers. Methods Patients were randomly allocated to either the RM or CM. Using the 'Generic Short Patient Experiences Questionnaire' (GS-PEQ), as well as an ad-hoc survey from the 'telehealth patient satisfaction survey' and 'costs survey', patients' experiences were measured six months after the pacemaker implant in a cohort of 50 consecutive patients. The mean age was 74.8 (±11.75) years and 26 (52%) patients were male of which 1 was lost in follow-up. Differences in baseline characteristics between groups were not found. Results Findings showed overall positive and similar experiences in patients in both groups. Significant differences were found in GS-PEQ concerning how RM patients received less information about their diagnosis/ afflictions (p = 0.046). We did not find significant differences in other items such as 'confidence in the clinicians' professional skills', 'treatment perception adapted to their situation', 'involvement in decisions regarding the treatment', 'perception of hospital organisation', waiting before admission', 'satisfaction of help and treatment received', 'benefit received', and 'incorrect treatment'. Conclusions The remote communication of pacemakers was met with positive levels of patients' experiences similarly to patients in the CM group. However, patients included in RM group received less information. Thus, improving the quality and timing of information is required in RM group in the planning and organisation of future remote communication healthcare services for people living with a pacemaker implant. Key messages Patients included in both groups of follow-up showed similar levels in their experiences with the use of the pacemakers. It is necessary improving the quality and timing of information required in remote monitoring of users in the planning of future remote communication healthcare services for people with pacemaker.

Ergonomic job analysis is considered to be a primary step in preventing work disability related to musculoskeletal problems. There has been little work examining what ergonomic changes are actually occurring after an ergonomic job analysis. This paper looks at what types of workplace changes are being implemented and what if any trends exist in these changes. It was found that in the manufacturing setting, workers were more likely to change jobs and workplace changes occurred with line changes rather than as an accommodation for a particular employee. In the office setting, ad hoc individualized changes to workstations were used in an effort to accommodate workers in their current jobs.

AbstractObjectiveThe objective was to investigate whether irregular eating of meals in adolescence predicts the metabolic syndrome and its components in adulthood, and if any specific meal is of particular importance.DesignProspective cohort study with 27 years of follow-up. Information on meals (breakfast, school lunch and dinner with family), lifestyle (alcohol consumption, smoking habits, physical activity, consumption of sweets and pastries) at age 16 years was assessed from questionnaires, and presence or not of the metabolic syndrome and its components were defined at age 43 years in 889 participants (82·1 % of total cohort). Logistic regression was used to calculate odds ratios and confidence intervals.SettingThe Northern Swedish Cohort; all school-leavers of the 9th grade in the town Luleå in 1981.SubjectsAdolescents (age 16 years).ResultsIrregular eating of meals at age 16 years was associated with higher prevalence of the metabolic syndrome at age 43 years (OR=1·74; 95 % CI 1·12, 2·71), but this was explained by concurrent unhealthy lifestyle at age 16 years. Poor breakfast at age 16 years was the only meal associated with the metabolic syndrome at age 43 years, independent of other meals, BMI (kg/m2) and lifestyle at age 16 years (OR=1·67; 95 % CI 1·00, 2·80).ConclusionsIrregular eating of meals in adolescence predicted the metabolic syndrome in adulthood, but not independently of BMI and lifestyle in adolescence. Poor breakfast in adolescence was the only specific meal associated with future metabolic syndrome, even after adjustments. Breakfast eating should be encouraged in adolescence.

578 Background: SB3 was approved in the US and EU as a biosimilar of reference trastuzumab (TRZ). Here, we report 4-year cardiac safety and survival outcomes. Methods: After completing neoadjuvant-adjuvant therapy in patients with HER2 positive early breast cancer, patients from selected countries participated in a 5-year follow-up study of a phase III trial (Pivot et al. Eur J Cancer 2019). The aim was to observe long-term cardiac safety and survival. EFS and OS were analyzed in subgroups by ADCC status within TRZ in ad-hoc analyses. Results: Of 875 patients randomized in the phase III trial, 367 patients (SB3, N=186; TRZ, N=181) were enrolled in the follow-up study. The median follow-up was 53 months. During the follow-up, the incidence of asymptomatic significant left ventricular ejection fraction (LVEF) decrease was low (SB3, n=1; TRZ, n=2), with all patients recovering with LVEF ≥ 50%. No cases of symptomatic congestive heart failure, cardiac death, or other significant cardiac condition were reported. 4-year EFS rates were 83.4% for SB3 and 80.7% for TRZ with a HR of 0.77 [95% CI 0.47, 1.27]. 4-year OS rates were 94.3% for SB3 and 89.6% for TRZ with a HR of 0.53 [95% CI 0.24, 1.16]. From ad-hoc analysis, a difference in EFS and OS was seen between Non-drifted TRZ and Drifted TRZ; Difference between SB3 and Non-drifted TRZ was not statistically significant. Conclusions: In a subset of patients from the phase III trial, comparable long-term cardiac safety and survival at 4-year supports biosimilarity between SB3 and TRZ. Ad-hoc analysis results by ADCC status suggest a possible correlation between ADCC and clinical efficacy. Further follow-up is needed. Clinical trial information: NCT02771795 . [Table: see text]
