The pathologic, clinical and radiologic features, diagnosis and treatment of mesenteric cysts in infancy and childhood, based upon a study of 82 cases in the literature, are discussed. An additional case of mesenteric cyst is presented. Mesenteric cysts were usually noted in male children from 2 to 10 years of age, presenting with abdominal enlargement and abdominal pain, and revealing characteristic roentgenographic findings. Excision of the cyst was often complicated by adhesions to the intestinal wall; simultaneous resection of the involved intestine has become a safe procedure, with an operative mortality of 7% in recent years.

Mesenteric cysts are rare and occur in patients of any age. They are asymptomatic and found incidentally or during the management of their complications. They commonly originate from the small bowel mesentery, although a proportion of them have been found to originate from the mesocolon (24%) and the retroperitoneum [1] [2,3,4,5] (14.5%). A mesenteric cyst originating in the sigmoid mesocolon is a very rare nding. They are a rare cause of abdominal pain and are discovered incidentally. If symptomatic, patients with these cysts present with abdominal pain, vomiting and low backache. Performing a thorough physical examination and conducting radiological investigations like ultrasonography (USG), computed tomography (CT) are keys in diagnosing the mesenteric cysts.

Mesenteric cysts are rare, variably-sized intra-abdominal lesions developing during childhood. Their symptoms vary from being asymptomatic and incidentally found to non-specific, presenting as lower abdominal pain, nausea and vomiting, constipation and diarrhea. Clinically, these abdominal masses may be palpable in more than 50% of patients. The diagnosis of these lesions can be made accurately radiologically through abdominal ultrasound and CT. The treatment of choice is complete surgical resection. Hereby we report the clinical course of a 5 year old child with a mesenteric cyst who complained of acute abdominal pain, constipation and vomiting and were surgically treated after being diagnosed with a mesenteric cyst based on radiological examination.

The occurrence of a mesenteric cyst (MC) is common in adults while in children and in infants is rare. In adults mesenteric cysts are often asymptomatic and discovered incidentally; however, in children they commonly present with symptoms of abdominal pain or distension with fever and leucocytosis. We report on a rare case, in our experience, of Mesenteric Chylous cyst (MCC) in an infant with signs and symptoms of intestinal obstruction. Discussion of literature is also reported.

Einleitung: Bei der Differentialdiagnose von intraabdominalen Tumoren ist an die mesenterialen Zysten zu denken. Wir möchten mittels Fallbeispiel an diesen seltenen Befund erinnern. Fallbeispiel: Es handelt sich um eine 35-jährige Patientin mit zweiwöchiger Anamnese von progredienten Abdominalschmerzen. Die Abklärung mittels Sonographie und Computertomographie ergab einen 14 x 12 x 3cm grossen zystischen abdominalen Tumor ohne Beziehung zu Uterus, Adnexen oder Oberbauchorganen, worauf die Patientin laparotomiert und die Zyste reseziert wurde. Diskussion: Mesenteriale Zysten sind selten. Die Pathogenese ist unterschiedlich und die klinische wie auch die radiologische Diagnostik schwierig. Die Symptomatik reicht vom akuten Abdomen über unspezifische Abdominalbeschwerden bis hin zum asymptomatischen Zufallsbefund. Mesenteriale Zysten können entlang des gesamten Gastrointestinaltraktes vom Duodenum bis zum Rektum auftreten. Therapie der Wahl ist die Zystenresektion. Schlussfolgerungen: Mesenteriale Zysten sind seltene intraabdominale Befunde. Die definitive Diagnosesicherung und die Therapie besteht in der Resektion.

MUCH has been written in recent years about the importance of psychogenic disturbances as causes of abdominal pain in childhood. Yet, by no means all recurrent abdominal pain is so caused. A recent article in Pediatrics on nonorganic abdominal pain therefore promised this subsequent paper on pain of organic origin. I would like to caution the reader that his "surgeon's viewpoint" tends to exclude consideration of organic abdominal pain not surgically treated. Perhaps a third paper is indicated. Abdominal pain of whatever origin requires a planned approach to diagnosis. While it is only too easy for the clinician to submit a patient to innumerable investigations of varying degrees of unlikelihood of revealing disorders of differing degrees of rarity, a detailed history and searching interview with the parents, together with a complete physical examination, will go very far to reduce the number of cases submitted to any but quite simple tests. The diagnosis of psychogenic pain, no less than that of organic pain, must rest on positive findings. To make a diagnosis of psychogenic pain, there must be something more than the absence of demonstrable organic disease. There must be significant psychopathology. If there is evidence of neither this nor organic disease, one must resolve to be irresolute and decide to be undecided. One must not make a diagnosis of psychic disease simply because one can find no organic cause. One must also remember that psychic disturbance does not confer immunity from organic disease. A neurotic, psychotic, or brain-damaged child can have appendicitis.

Morris Green's article "Diagnosis and Treatment: Psychogenic, Recurrent, Abdominal Pain" in the July 1967 issue of Pediatrics calls attention to a presenting complaint well-known to pediatric practitioners. Girls in the early months of puberty often come to their physicians with a complaint of recurrent abdominal pain of a somewhat cyclical nature. Physical findings are variable, although there is often some tenderness in one of the lower abdominal quadrants. The general heightened sensitivity to any bodily discomfort which accompanies the pubertal increase in hormone level causes pubescent girls to be quite aware of and frightned by this pain.

The occurrence of adrenal crisis after retroperitoneal laparoscopic unilateral adrenalectomy is usually concealed. If not timely diagnosis and treatment,it may cause shock, and even lead to death. It is very difficult to distinguish the clinical manifestations of adrenal crisis from nausea, vomiting, fatigue,gas separation from the lower diaphragm, abdominal pain, hypotension, hypertension, fever and hypothermia after operation. This makes it very difficult to identify and diagnose adrenal crisis early. This article mainly discusses the early recognition, diagnosis and treatment of adrenal crisis after unilateral adrenalectomy by retroperitoneoscope.

AbstractCongenital hyperinsulinism (CHI), a major cause of persistent and recurrent hypoglycemia in infancy and childhood. Numerous pathogenic genes have been associated with 14 known genetic subtypes of CHI. Adenosine triphosphate-sensitive potassium channel hyperinsulinism (KATP-HI) is the most common and most severe subtype, accounting for 40–50% of CHI cases. Short-chain 3-hydroxyacyl-coenzyme A dehydrogenase hyperinsulinism (SCHAD-HI) is a rare subtype that accounts for less than 1% of all CHI cases that are caused by homozygous mutations in the hydroxyacyl-coenzyme A dehydrogenase (HADH) gene. This review provided a systematic description of the genetic pathogenesis and current progress in the diagnosis and treatment of SCHAD-HI to improve our understanding of this disease.

A city-wide study of intussusception in childhood is reported. The classic signs and symptoms (vomiting, abdominal pain, bloody stools, palpable abdominal mass) are reported in percentages approximating those previously reported by other authors. Intussusception in childhood occurred about once in every 13,000 admissions to general hospitals. Of 26 cases, 3 had surgery without barium enema. In 11 of 23 patients who had roentgenographic examination the intussusception was reduced by enemas (48%). In one patient the lesion recurred after 24 hours. No deaths were encountered in this small series. Provided certain precautions are followed, barium enema is a safe procedure which will save about one-half of the patients a surgical procedure and unnecessary prolonged hospitalization.
